Hair Growth Activity and Melanogenic Activity of Oriental Medical Prescription (한방복합처방단의 발모효과 및 멜라닌 생성 촉진효과)

  • To develop a new natural cosmetic hair care product for scalp treatment, we investigated the effect of enzyme modified (Viscozyme, Pectinex) oriental medical prescriptions on hair growth and melanogenesis. Enzyme modified oriental medical prescriptions showed the antioxidative effect, inhibitory effects on lipoxygenase activity and $5{\alpha}$-reductase activity and melanogenic activity. In human irritation test, they did not show any adverse effect. Based on these results, we suggest that enzyme modified oriental medical prescriptions may potentially be used in a cosmeceutical hair care product for scalp treatment.

Study on the Anti-angiogenic Activity of Ethanol Extract of Bojungbangam-tang (보정방암탕 에타놀층의 혈관형성 저해작용에 관한 연구)

  • The anti-angiogenic activity of ethanol extract of Bojungbangam-tang, a new herbal prescription composed of nine crude drugs, was evaluated in human umbilical vein endothelial cells (HUVECs). HPLC profile revealed that five major compunds such as apioliquiritin, narirutin, hesperidin, liquiritin and glycyrrhizin. Ethanol extract of Bojungbangam-tang (EBJT) did not showed any significant cytotoxicity against HUVECs up to 200 ug/ml. EBJT significantly inhibited basic fibroblast growth factor (bFGF)-induced HUVECs proliferation to 69% at 200 ${\mu}g/ml$. Migration using window scraping method and tube formation in bFGF stimulated HUVECs were also significantly suppressed by EBJT in a dose-dependent manner. Taken together, these results suggest that Bojungbangam-tang can be a potent prescription for angiogenesis related disease.

A Review of Clinical Studies with Herbal Medicine for Depression - Based on Randomized Controlled Clinical Trial - (우울증에 대한 한약물 치료 문헌적 고찰 - 무작위 대조군 임상연구를 중심으로 -)

  • Objectives : This study aimed to investigate frequently used herbal materials among herbal prescription for depression focusing on randomized controlled trial. Methods : Every article relevant to depression was initially obtained from China National Infrastructure(CNKI), Korean database and book hand-searching. Searching keywords were 'depression', 'herbal medicine' and 'randomized controllled trial(RCT)'. Results : Among comorbidity with depression, the most accompanied disease was that of circulatory system. Among sixty-five articles, depression with cerebral vascular disease was twenty-eight. Article about mood disorder was twenty-four. High frequently used herbal materials were Bupleuri Radix(41times), Curcumae Radix(34 times), Acori Graminei Rhizoma and Cnidii Rhizoma(24 times). Conclusions : According to this study, we could know select frequent-used herbal medicine. In a clinical treatment, herbal materials can be added herbal prescription related to depression. As these results, it can be helpful to develop new drugs.

Quality Evaluation of Medication Counseling in Korean Community Pharmacies (지역약국에서 시행하는 복약지도의 질적 수준 평가)

  • Background: Medication counseling is important to improve qualify of pharmaceutical care, but there are lack of studies to investigate satisfied counseling practice in community pharmacies. Purpose: This study was to investigate current medication counseling provided by community pharmacists. Method: Questionnaires to assess medication counseling practices were mailed to nationwide 1,269 community pharmacists who were Sookmyung Women's University alumni. Result: One hundred sixty five pharmacists were responded to the questionnaires (response rate 13.63%). 16.8% of them all respondents had counseling room in their pharmacies. 75% and 29.9% of respondents provided patient counseling always for new and refilled prescriptions, respectively. Counseling was provided primarily by verbal, but especially for drug name, usage and storage, it was provided in written concurrently. 31.1% of respondents was satisfied with counseling practices by themselves, and a major barrier of unsatisfied counseling was lack of time in 64.6% of respondents. Majorities of respondents (66.3% and 76.3%) spent 1-3 minutes and <1 minute, for new prescription and refilled prescription, respectively. Approximate 99% of respondents presented necessity of standardized patient counseling information for drugs and 97.6% responded that quality of patient counseling would be improved by using software containing counseling information. Old respondent group had higher accumulative patient counseling practice scores and pharmacy owner group took much time for counseling, resulting in higher scores too. Conclusion: This study showed that qualified patient counseling practice has not been reached in community pharmacies. Much more discussion to seek ways to improve the quality would be necessary.

Prescriptions Utilizing Vegetable Oils in 『Donguibogam』(『東醫寶鑑』) (『동의보감(東醫寶鑑)』중 식물성 기름(油)을 활용한 방제)

  • Object : To investigate which diseases the utilization of various vegetable oils were used and how they were used for treatment, and to change and develop vegetable oil combined prescriptions so that the Korean medical treatment effect in clinical practice can be enhanced. Methods : All words containing oil(油) were searched and selected oils used for simple prescription and oil combined prescription. Aii of animal oils such as butter(酥油), Suis oil(猪油), Anceris oil(鴈油), and Actonis caro oil(猯油) were excludIt is suggested to develop a new formulation by succeeding the utilization methods of various effects of sesame oil and other vegetable oils.ed. All selected oils were classified as sesame oil(麻油)(inclusive, 脂麻油, 胡麻油, 香油, 淸油) or other vegetable oils. Result : 1. In 『Donguibogam』(『東醫寶鑑』), 14 other vegetable oils were used to 23 prescriptions. 2. Sesame oil was used to 187 prescriptions over 41 disease categories as of 20 effects described in the 『Zhenghebencao』(『政和本草』). 3. In 28 prescriptions, sesame oil was also used for hygiene and prevention, emetic, preparating herbs and so on. Conclusion : The 101 prescriptions used for medicinal auxiliary materials are due to the efficacy of treatments of malignant furuncles, sores, scabies, promoting regeneration of tissue and flesh, reliving pain, dispersing abscess and swelling and supplementing fissured skin. In 『Donguibogam』(『東醫寶鑑』), the unique efficacy of each vegetable oil and sesame oil was used in various ways and preparation forms to treat various diseases. It is suggested to develop a new formulation by succeeding the utilization methods of various effects of sesame oil and other vegetable oils.

A Comparative Analysis of the Systems Related to the Production, Authorization, and Listing for Insurance of Herbal Medicine Products in South Korea and Taiwan (국내와 대만의 전통약제제 생산.허가.보험등재 관련 제도)

  • Objectives: Systems related to the production, authorization, and listing for insurance of herbal medicine products were compared between South Korea and Taiwan to illuminate herbal medicine products system issues in South Korea. Methods: Papers, and laws and policies related to the production, authorization, and listing for insurance of herbal medicine products in South Korea and Taiwan are analyzed to create the primary documents. The documents from South Korea were screened with the advice of a specialist, while those from Taiwan have been verified through local investigation and with the help of a related specialist. The screened documents were then compared and analyzed in the order of the systems related to the production, authorization, and listing for insurance of herbal medicine products. Results: The systems related to the production of herbal medicine products satisfy GMP requirements in both countries, while Taiwan has more specialized systems related to the production of herbal medicine products and a more strict authorization program as compared to South Korea. While South Korea has most of the herbal medicine products classified as non-prescription drugs, Taiwan has them as prescription drugs. And while South Korea does not allow new herbal medicine products to be listed for insurance, Taiwan allows for once-a-year application toward listing for insurance. Conclusions: In order to ensure the safe and effective use of herbal medicine products, systems related to the production, authorization, and listing for insurance of herbal medicine products are to be established, while the categorization of medicine products principally used by Korean medicine doctors should be prepared. Furthermore, prescription by a Korean medicine doctor for new drugs made with natural products and their listing for insurance need to be encouraged.

  • Through this research, it was discovered that Wang Sa-wung studied classics such as "Nai Kyung", and brought about new theries and prescription by combining it with his own experience. He asserted 'Ki Hwa Chu Ki' theory which differentiated 'Shin Kam' and 'Bok Sa' within 'On Byung', and this was a new theory which was not in existence before. The main reason for the recognition received by the future generation is, not only because of the substantial theoretical background but, due to the fact that his ideas were based on making practical use easier within rich near a bed experience.

Translation study on the Gageum's Sanghanbuik (가금(柯琴) "상한부익(傷寒附翼)" 번역(飜譯) 연구(硏究))

  • 'Sanghallonju'(傷寒論注) reorganized the formation according to method of 'the classification of similar symptoms' and annotated the text of Sanghallon, introducing his new methodology and 'Sanghallonik'(傷寒論翼) proclaimed his new finding of the science of the Sanghan. Meanwhile, 'Sanghanbuik' (傷寒附翼) explains various prescriptions in the 'Sanghallon'. It categorizes prescriptions according to the six Meridians and sum up Gageum's research by commenting on the target symtoms and the use of medicine on each prescriptions. Gageum's study is consistent in desire for embodying the universality of the differentiation of syndromes in accordance with the theory of the six Meridians.(六經辨證) in the medical scene. From his work, the substantiality of the six 'Sanghandbuik' is a publication that shows the essence of Gageum's medical science from his inclination, conclusion and concrete methodology.

Compilation of 104 Experimental Theses on the Antitumor and Immuno-activating therapies of Oriental Medicine (한의학의 항종양 면역치료에 관한 연구 -1990년 이후 발표된 실험논문을 중심으로-)

  • This study was done to compile 104 experimental theses which are related to the antitumor and immuno-activating therapies between February 1990 through February 2002. Master's and doctoral theses were dassified by schools, degrees, materials, effects, experimental methods of antitumor and immunoactivity, and results. The following results were obtained from this study : 1. Classifying the theses by the school, 34.6% were presented by Daejeon University, 29.8% by Kyung-hee University and 11.5% by Won-kwang University. Of all theses, 51.0% were aimed for the doctoral degree and 43.3% were for the master's degree. All of three universities have their own cancer centers. 2. Classifying the theses by herb materials, complex prescription accounted for 60.3%, single herb accounted for 24.8% and herbal acupuncture accounted for 14.2%. Considering the key principles of the traditional medicine, complex prescription was much more thoroughly studied than single herb prescription. The results showed that the complex prescription had both antitumor activity and immuno-activating activity, which might reflects on multi-activation mechanisms by complex components. 3. Classifying the theses by the efficacy of herbs examined, in single herb, invigorating spleen and supplementing was 35.5%, expelling toxin and cooling was 29.0%, activating blood flow and removing blood stasis was 12.9%. In herbal acupuncture, invigorating spleen and supplementing was 52.9%, expelling toxin and cooling was 29.4%. In complex prescription, pathogen-free status was 41.9%, strengthening healthy qi to eliminate pathogen was 35.5%, strengthening healthy qi was 22.6%. It is presumed that the antitumor and immunoactivating therapy based on syndrome differentiation is the best way to develop oriental oncology. 4. Classifying the theses by antitumor experiments, cytotoxic effect was 48.1 %, survival time was 48.1 % and change of tumor size was 42.3%. Survival rate was not necessarily correlated with cytotoxicity. These data reflect the characteristic, wholistic nature of the oriental medicine which is based on BRM (biological response modifier). 5. Classifying the theses by immunoactivating experiments, hemolysin titer was 51.0%, hemagglutinin titer was 46.2% and NK cell's activity was 44.2%. In the future studies, an effort to elucidate specific molecular and cellular mechanisms of cytokine production in the body would be crucial. 6. Classifying the theses according to the data in terms of antitumor activity, 50% was evaluated good, 24.0% was excellent, and 15.5% have no effect. In an evaluation of immuno-activating activity, 35.9% was excellent and 18.0% showed a little effect. The index point, as described here, may helps to use experimental data for clinical trials. Changes in index points by varying dosage implicate the importance of oriental medical theory for prescription. 7. In 167 materials, IIP (immunoactivating index point, mean : 3.12±0.07) was significantly higher than AIP(antitumor index point, mean : 2.83±0.07). These data demonstrate that the effect of herb medicine on tumor activity depends more on immunoactivating activity than antitumor activity. This further implies that the development of herbal antitumor drugs must be preceded by the mechanistic understanding of immunoactivating effect. 8. After medline-searching tumor and herb-related articles from NCBI web site, we conclude that most of the studies are primarily focused on biomolecular mechanisms and/or pathways. Henceforth, we need to define the biomolecular mechanisms and/or pathways affected by herbs or complicated prescriptions. 9. Therefore, the most important point of oriental medical oncology is to conned between experimental results and clinical trials. For the public application of herbal therapy to cancer, it is critical to present the data to mass media. 10. To develop the relationship of experimental results and clinical trials, university's cancer clinic must have a long-range plan related to the university laboratories and, at the same time, a regular consortium for this relationship is imperative. 11. After all these efforts, a new type herbal medicine for cancer therapy which is to take care of the long-term administering and safety problem must be developed. Then, it would be expected that anti-tumor herbal acupuncture can improve clinical symptoms and quality of life (QOL) for cancer patients. 12. Finally, oriental medical cancer center must be constructed in NCC (National Cancer Center) or government agency for the development of oriental medical oncology which has international competitive power.

A Study on the Nursing Profession as Stipulated by Health & Medical Laws of Korea (우리나라 보건의료법령에 명시된 간호에 관한 연구)

  • The purpose of the study is to find out how laws related to the nursing profession can be improved by analyzing the rules and regulations concerning nursing. Furthermore, to help settle legal matters in the process of doing nursing work. The data used for the study are the Health and Medical Act, the Maternal and Child Health Act, the School Health Act, the Special Act for Health and Medical Service in Rural Areas, the Industrial Health & Safety Act and the Notice on Nursing Professional Courses analyzed by age and content. The results of the study are as follows : First, basic nursing practice includes 'nursing care for recuperation and assistance in medical treatment and in special areas including the pre-vention of disease, maintenance of health, control of environment, and other therapeutic activities. It is suggested that the phrase 'assistance in medical treatment' should be eliminated as it limits the basic nursing practice to the assistance of the medical treatment. Second, Article 56 of the Health & Medical Act prescribes a special nurse but it does not prescribe a specific job. Accordingly, the new provison concerning the specific jobs of a special nurse should be added or a job guide should be inseated. Third, it is prescribed that those who have completed the training course after obtaining a license are qualified to be a midwife, a special nurse and a nurse practitioner working in special areas. However, school nurses, occupational health nurses and maternal and health workers are required to obtain a nurse license, but not to take an additional training course. Nurses working in special areas should be legally recognized as nurse specialists. The regulations to control various qualification standards consistently should be established. Fourth, the qualifications and types of nurses by area prescribed by Article 54 of the Health and Medical Act are not consistent with those of special nurses as recognized by affiliated organizations of the Korean Nurse Association and some hospitals. Accordingly, the qualifications and types of special nurses should be adjusted in consideration of special nurses. Fifth, as Article 16, Paragraph 2 of the Health and Medical Act does not prescribe the type and scope of first - aid treatment that nurses can provide, the first-aid treatment of nurses might be considered as an unlicensed practice. The specific regulations regarding these matters should be established. Sixth, the contents of the nursing record, which are prescribed by Article 21 of the Health and Medical Act as a duty, include 1) matters concerning body temperature, pulse, breath and blood pressure 2) matters concerning drug prescription 3) matters concerning input and output 4) matters concerning the treatment and nursing care (Article 17 of the Enforcement Regulations, Health and Medical Act). However, these matters are limited to basic nursing care and assistance in medical treatment. The new recording methods on nursing process are suggested to be adopted legally. Seventh, the prescription right entrusted to nurses which are prescribed by the School Health Act, the Special Act on Health and Medical Service for Rural Areas, and the Industrial Health and Safety Act are not consistent with the rights of nurses as prescribed by the Health and Medical Act. New regulations prescribing the partial right for medical treatment entrusted to nurses in consideration of the restraint of time and place in emergency situations should be established.
