위 Helicobacter pylori 감염 및 중성구침윤과 장상피화생의 연관성 (Relationship between Intestinal Metaplasia and Neutrophilic Infiltration of Stomach Caused by Helicobacter pylori Infection)

  • Helicobacter pylori (H. pylori) infection is uncommon in developed countries, yet is common in underdeveloped and developing countries. Infection rate of H. pylori is minimally influenced by economic, environmental, and public health status and genetic factors. Korea is a developing country with a high incidence of H. pylori infection and gastric carcinoma, which is one of the leading causes of death. For this reason, accurate clinical and pathologic data on H. pylori-associated disease are very important. Intestinal metaplasia accompanies chronic gastritis and increases the risk of gastric carcinoma. For this reason, the relationship between H. pylori infection and intestinal metaplasia is very closely linked. Because of this, as the antecedent condition is guessed, it examines the relationship of the H. pylori and the intestinal metaplasia. Intestinal metaplasia is thought to be the basis in the development of intestinal type gastric carcinomas. Recent investigations showed that inflammatory reaction in the gastric fundus affect the development of gastric carcinogenesis. To verify neutrophilic activity in the gastric fundus and development of intestinal metaplasia in both gastric fundus and antral mucosa, their relationship was studied using 159 healthy patients who had undergone gastric endoscopic biopsies without any identifiable pathologic disesaes. When neutrophilic activity accompanied, incidence of intestinal metaplasia was significantly increased (p<0.05). H. pylori infection was statistically and significantly associated with the presence of intestinal metaplasia (p<0.05). These results suggest that H. pylori infection affected the development of intestinal metaplasia in the stomach. These results will help our understanding of H. pylori infection in the pathogenesis of intestinal metaplasia, a preneoplastic condition of the stomach. To reduce the incidence of gastric adenocarcinoma, eradication treatment of H. pylori is recommended when there's a neutrophilic activity in the gastric fundus.

호중구의 Respiratory Burst에 미치는 PLA2 및 PAF와 영향 : In vitro에서의 호중구의 산소기 생성 및 Apoptosis에 관한 연구 (Effect of the Inhibition of PLA2 and PAF on the Neutrophilic Respiratory Burst and Apoptosis)

  • 연구배경 : 급성호흡부전증후군(ARDS)의 병인론을 호중구의 산소기 생성 및 apoptosis 의 관정에서 PLA2 및 PAF의 역할과 연관하여 알아보았다. ARDS의 원인 중 산소기의 역할이 주로 염증성 cytokine 및 지질분자와 관련하여 연구되고 있는 점에 착안하여 내독소에 의한 PLA2, PAF의 작용 및 이에 따른 호중구의 혈관내피세포로의 유착, 산소기에 의한 pulmonary surfactant의 기능에 미치는 영향에 대해서도 알아보았다. 방법 : PMA로 자극된 호중구에서 PLA2 및 PAF의 억제에 따른 산소기 형성의 변화에 대해서도 알아보았으며 내독소에 의한 호중구에서의 PLA2활동도의 변화, lysoPAF remodelling에 미치는 PLA2 및 PAF의 억제의 효과에 대해서도 알아보았다. 또한 내독소 및 PMA에 의해 자극된 상태에서의 PLA2 및 PAF의 억제가 호중구의 apoptosis에 미치는 영향도 알아보았다. 호중구에 의한 조직의 손상은 혈관내피세포로의 호중구의 유착이 선행되어야 하므로 이러한 작용에 PLA2 및 PAF가 미치는 영향을 호중구 유착검사를 통하여 알아보았고 형태학적으로는 산소기와 pulmonary surfactant의 결합을 확인하였다. 결론 : ARDS 시의 호중구의 역할은 PLA2 및 PAF의 작용에 의한 산소기 형성 및 염증성 지질분자의 생성을 통해 조작의 손상을 유발하는 듯하며 이때 PLA2의 억제는 호중구의 apoptosis의 증가 및 산소기의 생성을 감소시키고 또한 호중구의 혈관내피세포로의 유착을 감소시켰다. PAF의 억제는 호중구의 산소기 생성의 감소 및 호중구의 유착을 억제하여 조직의 손상을 감소시키는 것으로 생각되었다.

Effects of Macrolide and Corticosteroid in Neutrophilic Asthma Mouse Model

  • Background: Asthma is a disease of chronic airway inflammation with heterogeneous features. Neutrophilic asthma is corticosteroid-insensitive asthma related to absence or suppression of $T_H2$ process and increased $T_H1$ and/or $T_H17$ process. Macrolides are immunomodulatory drug that reduce airway inflammation, but their role in asthma is not fully known. The purpose of this study was to evaluate the role of macrolides in neutrophilic asthma and compare their effects with those of corticosteroids. Methods: C57BL/6 female mice were sensitized with ovalbumin (OVA) and lipopolysaccharides (LPS). Clarithromycin (CAM) and/or dexamethasone (DXM) were administered at days 14, 15, 21, 22, and 23. At day 24, the mice were sacrificed. Results: Airway resistance in the OVA+LPS exposed mice was elevated but was more attenuated after treatment with CAM+DXM compared with the monotherapy group (p<0.05 and p<0.01). In bronchoalveolar lavage fluid study, total cells and neutrophil counts in OVA+LPS mice were elevated but decreased after CAM+DXM treatment. In hematoxylin and eosin stain, the CAM+DXM-treated group showed less inflammation additively than the monotherapy group. There was less total protein, interleukin 17 (IL-17), interferon ${\gamma}$, and tumor necrosis factor ${\alpha}$ in the CAM+DXM group than in the monotherapy group (p<0.001, p<0.05, and p<0.001). More histone deacetylase 2 (HDAC2) activity was recovered in the DXM and CAM+DXM challenged groups than in the control group (p<0.05). Conclusion: Decreased IL-17 and recovered relative HDAC2 activity correlated with airway resistance and inflammation in a neutrophilic asthma mouse model. This result suggests macrolides as a potential corticosteroid-sparing agent in neutrophilic asthma.

PAF Contributes to Intestinal Ischemia/Reperfusion-Induced Acute Lung Injury through Neutrophilic Oxidative Stress

  • The role of platelet-activating factor (PAF) was investigated in intestinal ischemia/reperfusion (I/R) induced acute lung injury associated with oxidative stress. To induce acute lung injury following intestinal I/R, superior mesenteric arteries were clamped with bulldog clamp for 60 min prior to the 120 min reperfusion in Sprague-Dawley rats. Acute lung injury by intestinal I/R was confirmed by the measurement of lung leak index and protein content in bronchoalveolar lavage (BAL) fluid. Lung leak and protein content in BAL fluid were increased after intestinal I/R, but decreased by WEB 2086, the PAF receptor antagonist. Furthermore, the pulmonary accumulation of neutrophils was evaluated by the measurement of lung myeloperoxidase (MPO) activity and the number of neutrophils in the BAL fluid. Lung MPO activity and the number of neutrophils were increased (p<0.001) by intestinal I/R and decreased by WEB 2086 significantly. To confirm the oxidative stress induced by neutrophilic respiratory burst, gamma glutamyl transferase (GGT) activity was measured. Lung GGT activity was significantly elevated after intestinal I/R (p<0.001) but decreased to the control level by WEB 2086. On the basis of these experimental results, phospholipase $A_2\;(PLA_2),$ lysoPAF acetyltransferase activity and PAF contents were measured to verify whether PAF is the causative humoral factor to cause neutrophilic chemotaxis and oxidative stress in the lung following intestinal I/R. Intestinal I/R greatly elevated $PLA_2$ activity in the lung as well as intestine (p<0.001), whereas WEB 2086 decreased $PLA_2$ activity significantly (p<0.001) in both organs. LysoPAF acetyltransferase activity, the PAF remodelling enzyme, in the lung and intestine was increased significantly (p<0.05) also by intestinal I/R. Accordingly, the productions of PAF in the lung and intestine were increased (p<0.001) after intestinal I/R compared with sham rats. The level of PAF in plasma was also increased (p<0.05) following intestinal I/R. In cytochemical electron microscopy, the generation of hydrogen peroxide was increased after intestinal I/R in the lung and intestine, but decreased by treatment of WEB 2086 in the lung as well as intestine. Collectively, these experimental results indicate that PAF is the humoral mediator to cause acute inflammatory lung injury induced by intestinal I/R.

Effect of the Inhibition of Platelet Activating Factor on Oxidative Lung Injury Induced by Interleukin-$1\;{\alpha}$

  • In order to know the pathogenesis of adult respiratory distress syndrome (ARDS) in association with the oxidative stress by neutrophils, the role of platelet activating factor (1-0-alkyl-2-acetyl-snglycero-3-phosphocholine, PAF) was investigated during acute lung injury induced by interleukin- $1{\alpha}$ (IL-1) in rats. An insufflation of IL-1 into the rat's trachea increased the acetyltransferase activity in the lung and the increase of PAF content was followed. As evidences of acute lung injury by neutrophilic respiratory burst, lung leak index, myeloperoxidase activity, numbers of neutrophils in the bronchoalveolar lavage fluid, neutrophilic adhesions to endothelial cells and NBT positive neutrophils were increased after IL-1 treatment. In addition, a direct instillation of PAF into the trachea caused acute lung leak and the experimental results showed a similar pattern in comparison with IL-1 induced acute lung injury. For the confirmation of oxidative stress during acute lung leak by IL-1 and PAF, a histochemical electron microscopy was performed. In IL-1 and PAF treated lungs of rats, the deposits of cerrous perhydroxide were found. To elucidate the role of PAF, an intravenous injection of PAF receptor antagonist, WEB 2086 was given immediately after IL-1 or PAF treatment. WEB 2086 decreased the production of hydrogen peroxide and the acute lung leak. In ultrastructural study, WEB 2086 mitigated the pathological changes induced by IL-1 or PAF. The nuclear factor kappa B (NFkB) was activated by PAF and this activation was inhibited by WEB 2086 almost completely. Based on these experimental results, it is suggested that the PAF produced in response to IL-1 through the remodeling pathway has the major role for acute lung injury by neutrophilic respiratory burst. In an additional experiment, we can also come to conclude that the activation of the NFkB by PAF is thought to be the fundamental mechanism to initiate the oxidative stress by neutrophils causing release of proinflammatory cytokines and activation of phospholipase $A_2$.

  • 건강한 개의 혈액세포와 골수세포의 세포화학적 특성을 조사하였다. 이 실험에서 혈액과 골수 시료에 항응고제를 일체사용하지 않았으며 시료채취후 즉시 도말표본을 작성하여 30분 이내에 반응을 실시하였다. 이 실험의 결과는 아래와 같다. 1. alkaline phosphatase의 활성은 호산구계통과 간혹 전골수구에서 양성반응을 나타내었다. 2. acid phosphatase의 활성은 대부분의 계통의 세포에서 양성반응을 나타내지만 tartrate로 억제하면 호산구계만 양성반응을 나타내었다. 3. peroxidase 활성은 골수구계통의 모든 세포에서 양성반응을 나타내며 단구에서는 미약한 양성의 미세과립을 나타내었다. 4. naphthyl-AS-D-chloroacetate esterase활성은 호중구계 세포에서만 양성을 나타낸다. 5. $\alpha$-naphthyl acetate esterase활성은 단구와 일부의 임파구에서 양성을 나타낸다 6. Sudan black B 염색은 골수구계 세포와 단구계 세포에서 양성을 나타내었다. 7. $\beta$-glucuronidase 활성은 적혈구계를 제외한 모든 세포에서 양성반응을 나타내었다.

Indoleamine 2,3-Dioxygenase in Hematopoietic Stem Cell-Derived Cells Suppresses Rhinovirus-Induced Neutrophilic Airway Inflammation by Regulating Th1- and Th17-Type Responses

  • Asthma exacerbations are a major cause of intractable morbidity, increases in health care costs, and a greater progressive loss of lung function. Asthma exacerbations are most commonly triggered by respiratory viral infections, particularly with human rhinovirus (hRV). Respiratory viral infections are believed to affect the expression of indoleamine 2,3-dioxygenase (IDO), a limiting enzyme in tryptophan catabolism, which is presumed to alter asthmatic airway inflammation. Here, we explored the detailed role of IDO in the progression of asthma exacerbations using a mouse model for asthma exacerbation caused by hRV infection. Our results reveal that IDO is required to prevent neutrophilic inflammation in the course of asthma exacerbation caused by an hRV infection, as corroborated by markedly enhanced Th17- and Th1-type neutrophilia in the airways of IDO-deficient mice. This neutrophilia was closely associated with disrupted expression of tight junctions and enhanced expression of inflammasome-related molecules and mucin-inducing genes. In addition, IDO ablation enhanced allergen-specific Th17- and Th1-biased CD4+ T-cell responses following hRV infection. The role of IDO in attenuating Th17- and Th1-type neutrophilic airway inflammation became more apparent in chronic asthma exacerbations after repeated allergen exposures and hRV infections. Furthermore, IDO enzymatic induction in leukocytes derived from the hematopoietic stem cell (HSC) lineage appeared to play a dominant role in attenuating Th17- and Th1-type neutrophilic inflammation in the airway following hRV infection. Therefore, IDO activity in HSC-derived leukocytes is required to regulate Th17- and Th1-type neutrophilic inflammation in the airway during asthma exacerbations caused by hRV infections.

A Study on the Mechanism of Immunomodulating Effects of Moxifloxacin in Oleic Acid-Induced Acute Lung Injury

  • Background: It was hypothesized that the immunomodulating effects of moxifloxacin contribute to ameliorate oleic acid (OA)-induced acute lung injury (ALI) by suppression of cytosolic phospholipase A2 (cPLA2). This was based on observations from experiments on rats associated with neutrophilic respiratory burst, cPLA2 activity, and expressions of cPLA2, $TNF{\alpha}$, and COX-II in the lung. Methods: ALI was induced by intravenous injection of OA in male Sprague-Dawley rats. Five hours after OA injection, protein content in bronchoalveolar lavage (BAL), lung myeloperoxidase (MPO) activity, and numbers of BAL neutrophils were measured. As an index of oxidative stress-induced lung injury, the content of malondialdehyde (MDA) in lung tissues was also determined. Lung histology, immunohistochemistry and determination of activity of cPLA2 in lung tissues were carried out. In addition, Western blotting of $TNF{\alpha}$ and COX-II in lung tissues was performed. Results: The accumulation of neutrophils in the lungs was observed after OA injection. BAL protein was increased along with neutrophilic infiltration and migration by OA. Moxifloxacin decreased all of these parameters of ALI and ameliorated ALI histologically. The increased malondialdehyde (MDA) in the lung by OA was also decreased by moxifloxacin. Moxifloxacin not only suppressed cPLA2 expression in the lungs and neutrophils but also decreased cPLA2 activity in lung tissues of rats given OA. The enhanced expressions of $TNF{\alpha}$ and COX-2 in the lung tissues of rats given OA were also suppressed by moxifloxacin. Conclusion: Moxifloxacin inhibited cPLA2 and down-regulated $TNF{\alpha}$ and COX-2 in the lungs of rats given OA, which resulted in the attenuation of inflammatory lung injury.

흰쥐에서 고농도 산소 흡입에 의한 급성 폐손상 시 호중구성 산화성 스트레스의 역할 (Presumptive Role of Neutrophilic Oxidative Stress in Oxygen-induced Acute Lung Injury in Rats)

  • 연구배경: 산소독에 의한 급성 폐손상에 대한 기전은 아직 확실히 알려져 있지 않다. 급성호흡곤란증후군에서 보이는 호중구의 산소기 생성에 따른 조직의 손상이 산소독에 의한 손상에서도 관여하는지를 확인하고자 하였다. 방 법: Plastic cage 내의 기압을 1기압으로 고정하고 흰쥐에게 순수한 산소를 48시간 호흡시킨 후 호중구가 폐장 내로 침윤함으로써 나타나는 급성 폐손상을 생화학적인 지표 및 형태학적인 관찰 등을 통하여 검사하였다. 결 과: 흰쥐에게 순수한 산소를 48시간 호흡하게 한 경우 폐부종, 호중구의 침윤, 폐장 내 MDA 및 $cPLA_2$ 활동도의 증가가 관찰되었고, 형태학적으로도 탐식구 특히 호중구의 침윤에 따른 폐장의 손상이 관찰되었다. 결 론: 산소독에 의한 급성폐손상의 기전은 고농도의 산소에 의한 산소기 생성이 그 원인으로 생각 되지만 산소기의 작용기전은 부분적으로 $cPLA_2$활성도 증가에 의한 호중구의 조직 내 침윤에 따른 이차적 산소기 형성이 그 원인으로 생각되며 시간이 경과할수록 호중구에 폐장 내 침윤에 따른 손상이 더 심해질 것으로 생각된다.

Effect of the Inhibition of Phospholipase $A_2$ in Generation of Free Radicals in Intestinal Ischemia/Reperfusion Induced Acute Lung Injury

  • The role of phospholipase $A_2\;(PLA_2)$ in acute lung leak induced by intestinal ischemia was investigated in association with neutrophilic respiratory burst. To induce lung leak, we generated intestinal ischemia for 60 min prior to the 120 min reperfusion by clamping superior mesenteric artery in Sprague-Dawley rats. Acute lung leak was confirmed by the increased lung leak index and protein content in bronchoalveolar fluid. These changes were inhibited by mepacrine, the non-specific $PLA_2$ inhibitor. The lung myeloperoxidase (MPO) activity denoting the pulmonary recruitment of neutrophils was increased by intestinal I/R, but decreased by mepacrine. Simultaneously, the number of leukocytes in bronchoalveolar fluid was increased by intestinal ischemia/reperfusion (I/R) and decreased by mepacrine. Gamma glutamyl transferase activity, an index of oxidative stress in the lung, was increased after intestinal I/R but decreased by mepacrine, which implicates that $PLA_2$ increases oxidative stress caused by intestinal I/R. The $PLA_2$ activity was increased after intestinal I/R not only in the intestine but also in the lung. These changes were diminished by mepacrine. In the cytochemical electron microscopy to detect hydrogen peroxide, intestinal I/R increased the generation of the hydrogen peroxide in the lung as well as in the intestine. Expression of interleukin-1 (IL-1) in the lung was investigated through RT-PCR. The expression of IL-1 after intestinal I/R was enhanced, and again, the inhibition of $PLA_2$ suppressed the expression of IL-1 in the lung. Taken together, intestinal I/R seems to induce acute lung leak through the activation of $PLA_2$, the increase of IL-1 expression associated with increased oxidative stress by neutrophilic respiratory burst.
