Purpose: This retrospective study compares higher-dose whole-brain radiotherapy (hdWBRT) with reduced-dose WBRT (rdWBRT) in terms of clinical efficacy and toxicity profile in patients treated for primary central nervous system lymphoma (PCNSL). Materials and Methods: Radiotherapy followed by high-dose methotrexate (HD-MTX)-based chemotherapy was administered to immunocompetent patients with histologically confirmed PCNSL between 2000 and 2016. Response to chemotherapy was taken into account when prescribing the radiation dose to the whole brain and primary tumor bed. The whole brain dose was ≤23.4 Gy for rdWBRT (n = 20) and >23.4 Gy for hdWBRT (n = 68). Patients manifesting cognitive disturbance, memory impairment and dysarthria were considered to have neurotoxicity. A median follow-up was 3.62 years. Results: The 3-year overall survival (OS) and progression-free survival (PFS) were 70.0% and 48.9% with rdWBRT, and 63.2% and 43.2% with hdWBRT. The 3-year OS and PFS among patients with partial response (n = 45) after chemotherapy were 77.8% and 53.3% with rdWBRT, and 58.3% and 45.8% with hdWBRT (p > 0.05). Among patients with complete response achieved during follow-up, the 3-year freedom from neurotoxicity (FFNT) rate was 94.1% with rdWBRT and 62.4% with hdWBRT. Among patients aged ≥60 years, the 3-year FFNT rate was 87.5% with rdWBRT and 39.1% with hdWBRT (p = 0.49). Neurotoxicity was not observed after rdWBRT in patients aged below 60 years. Conclusion: rdWBRT with tumor bed boost combined with upfront HD-MTX is less neurotoxic and results in effective survival as higher-dose radiotherapy even in partial response after chemotherapy.

Excessive microglial activation and subsequent neuroinflammation lead to synaptic loss and dysfunction as well as neuronal cell death, which are involved in the pathogenesis and progression of several neurodegenerative diseases. Thus, the regulation of microglial activation has been evaluated as effective therapeutic strategies. Although dieckol (DEK), one of the phlorotannins isolated from marine brown alga Ecklonia cava, has been previously reported to inhibit microglial activation, the molecular mechanism is still unclear. Therefore, we investigated here molecular mechanism of DEK via extracellular signal-regulated kinase (ERK), Akt and nicotinamide adenine dinuclelotide phosphate (NADPH) oxidase-mediated pathways. In addition, the neuroprotective mechanism of DEK was investigated in microglia-mediated neurotoxicity models such as neuron-microglia co-culture and microglial conditioned media system. Our results demonstrated that treatment of anti-oxidant DEK potently suppressed phosphorylation of ERK in lipopolysaccharide (LPS, $1{\mu}g/ml$)-stimulated BV-2 microglia. In addition, DEK markedly attenuated Akt phosphorylation and increased expression of $gp91^{phox}$, which is the catalytic component of NADPH oxidase complex responsible for microglial reactive oxygen species (ROS) generation. Finally, DEK significantly attenuated neuronal cell death that is induced by treatment of microglial conditioned media containing neurotoxic secretary molecules. These neuroprotective effects of DEK were also confirmed in a neuron-microglia co-culture system using enhanced green fluorescent protein (EGFP)-transfected B35 neuroblastoma cell line. Taken together, these results suggest that DEK suppresses excessive microglial activation and microglia-mediated neuronal cell death via downregulation of ERK, Akt and NADPH oxidase-mediated pathways.
유기용제는 현기증, 행동장애, 주의산만, 말초신경증과 같은 신경 독성을 일으키는 물질로 잘 알려져 있다. 그러나, 이러한 신경 독성물질인 유기 용제에 노출된 근로자들이 작업 기억 기능을 수행할 때 인지 부하에 어떻게 영향을 받는지에 관해서 많이 연구가 되어오지 않았다. 따라서, 본 연구에서는 기능적인 자기공명영상을 이용하여 만성적으로 유기용제에 노출된 근로자들이 인지 부하에 따른 작업 기억 기능을 수행할 때 보여지는 신경 변화의 관계를 살펴보았다. 29명의 유기용제에 노출된 근로자들을 대상으로 언어적 작업 기억 기능(1-back and 2-back)을 수행시켰으며 낮은 인지 부하와 높은 인지 부하의 작업 기억 기능을 수행할 때, 인지 부하의 차이에 따라 활성화 되는 뇌 영역의 차이를 구하였다. 1-back의 반응속도가 증가함에 따라 좌측 하위 두정 피질에서의 뇌 활성화가 점점 증가하는 관계를 보였는데, 이러한 증가되는 양상이 더 높은 인지 부하인 2-back에서는 보여지지 않았다. 이를 통해, 인지 부하가 많이 걸릴수록 활성화 되는 뇌 영역이 많아지며, 유기용제에 노출된 근로자들은 어느 정도 낮은 인지 부하가 걸렸을 때는 그만큼의 뇌 활성화가 증가되는데, 높은 인지 부하가 걸리게 되면 더 이상 뇌 활성화가 증가되지 않고 한계에 다다르는 것을 알 수 있었다.
항암, 항염증 및 비만억제 등의 생리활성을 지닌 해조류는 최근 건강기능 식품, 기능성 화장품 그리고 의약품 산업 분야에서 미용과 건강식품 소재로 각광받고 있다. 본 연구에서는 10종의 갈조류 메탄올 추출물을 이용하여 1,1-diphenyl-2-picrylhydrazyl(DPPH) 라디칼 소거능과 아밀로이드 베타 단백질($A{\beta}$)의 신경독성에 대한 HT-22 신경세포 보호효과를 조사함으로써 천연물로부터 안전하고 새로운 신경세포 보호소재를 개발해내고자 한다. DPPH 라디칼 소거능의 경우 미역쇠를 포함한 8종의 갈조류에서는 비교적 낮은 활성산소 소거능을 보인 반면, 감태와 패에서 강력한 활성산소 소거능이 나왔다. $A{\beta}$의 신경독성에 대해 10종의 갈조류 추출물이 갖는 HT-22 신경세포 보호효과를 검증하기 위해 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide(MTT) 분석과 APP, BACE1, iNOS 단백질의 발현양상 및 ERK1/2, p38, JNK1/2 단백질의 활성화 양상을 분석했다. MTT 분석 결과, $A{\beta}$의 신경독성으로부터 미역쇠가 $25{\mu}g/mL$의 농도에서 가장 효과적으로 세포를 보호하고 있는 것으로 나타났고, 알쏭이모자반, 불레기말, 바위수염, 짝잎모자반도 세포 보호효과가 있는 것으로 나타났다. 세포 보호효과가 있는 것으로 밝혀진 5종의 갈조류를 가지고 수행한 단백질 발현분석 결과, 미역쇠는 $A{\beta}$의 신경독성에 의해 HT-22 신경세포에서 발현되는 단백질인 BACE1과 iNOS의 발현을 저해하였다. 이는 미역쇠의 세포보호효과가 $A{\beta}$의 신경독성으로부터 일어난 ERK와 p38의 활성화에 연관된 세포신호전달 경로를 억제하는 것으로 보인다. 그러므로 특히 식용 갈조류인 미역쇠는 $A{\beta}$에 의해 유도된 신경독성에 대해서 신경세포 보호효과를 갖는 건강기능 식품 소재로서의 가치가 충분한 것으로 사료된다.
본 연구에서는 파의 항염증 효과를 밝히고 그 생화학적 기전 해석을 위해 LPS로 활성화된 BV2 microglia를 이용하여 iNOS, COX-2 및 염증성 cytokine의 발현 및 그 산물에 미치는 파 추출물의 영향을 조사하였다. 그 결과 파 추출물은 LPS 처리에 의한 BV2 세포의 iNOS의 발현을 전사 및 번역 수준에서 처리 농도 의존적으로 억제시켰으며, 특히 파 전체 에탄올 추출물(EEWA)의 효과가 가장 탁월하였다. LPS로 유도한 COX-2의 mRNA 및 단백질 발현 역시 파 추출물 처리에 의하여 감소되었으며 뿌리 에탄올 추출물(EERA) 처리군에서 가장 현저한 억제가 관찰되었다. 아울러 염증 반응의 또 다른 주요인자인 염증성 cytokine들(TNF-${\alpha}$, IL-$1{\beta}$ 및 IL-6)의 mRNA 변화를 조사한 결과 파 추출물이 대체적으로 이들 cytokine의 발현을 억제하는 경향을 보였으며, 최종산물인 TNF-${\alpha}$와 IL-6의 생성량 역시 유의한 수준은 아니었으나 감소하는 경향을 보였다. 본 연구의 결과는 파의 추출물은 iNOS, COX-2 및 염증성 cytokine의 발현을 조절함으로서 신경염증 반응을 효과적으로 억제하며, 향후 지속적인 연구가 필요하지만 신경 보호 작용에 탁월한 효능이 있음을 보여주는 것으로 생각된다.
Objectives: The purpose of this study is to analyze the effects of chronic exposure by welders to manganese (Mn) through an analysis of the degree of brain activity in different activities such as cognition and motor activities using the neuroimaging technique of functional magnetic resonance imaging (fMRI). The neurotoxic effect that Mn has on the brain was examined as well as changes in the neuro-network in motor areas, and the usefulness of fMRI was evaluated as a tool to determine changes in brain function from occupational exposure to Mn. Methods: A survey was carried out from July 2010 to October 2010 targeting by means of a questionnaire 160 workers from the shipbuilding and other manufacturing industries. Among them, 14 welders with more than ten years of job-related exposure to Mn were recruited on a voluntary basis as an exposure group, and 13 workers from other manufacturing industries with corresponding gender and age were recruited as a control group. A questionnaire survey, a blood test, and an fMRI test were carried out with the study group as target. Results: Of 27 fMRI targets, blood Mn concentration of the exposure group was significantly higher than that of the control group (p<0.001), and Pallidal Index (PI) of the welder group was also significantly higher than that of the control group (p<0.001). As a result of the survey, the score of the exposure group in self-awareness of abnormal nerve symptoms and abnormal musculoskeletal symptoms was higher than those of the control group, and there was a significant difference between the two groups (p<0.05, respectively). In the correlation between PI and the results of blood tests, the correlation coefficient with blood Mn concentration was 0.893, revealing a significant amount of correlation (p<0.001). As for brain activity area within the control group, the right and the left areas of the superior frontal cortex showed significant activity, and the right area of superior parietal cortex, the left area of occipital cortex and cerebellum showed significant activity. Unlike the control group, the exposure group showed significant activity selectively on the right area of premotor cortex, at the center of supplementary motor area, and on the left side of superior temporal cortex. In the comparison of brain activity areas between the two groups, the exposure group showed a significantly higher activation state than did the control group in such areas as the right and the left superior parietal cortex, superior temporal cortex, and cerebellum including superior frontal cortex and the right area of premotor cortex. However, in nowhere did the control group show a more activated area than did the exposure group. Conclusions: Chronic exposure to Mn increased brain activity during implementation of hand motor tasks. In an identical task, activation increased in the premotor cortex, superior temporal cortex, and supplementary motor area. It was also discovered that brain activity increase in the frontal area and occipital area was more pronounced in the exposure group than in the control group. This result suggests that chronic exposure to Mn in the work environment affects brain activation neuro-networks.
연구목적 자살은 전세계적인 사회적 문제로 자살로 인한 사회적 부담은 점차 증가하고 있다. 자살의 예방을 위해 다양한 시도들이 진행되고 있다. 자살의 위험성이 생활 습관, 그 중 식단의 변화에 의해 증가할 수 있다. 그래서 본 연구는 자살에 대한 보조적 치료로써 식단에 대해 고찰하였다. 방법 본 종설에서는 학술 검색을 통하여 자살의 생화학적 메커니즘을 확인하고 자살에 대한 위험요인으로서 식단, 보호요인으로서 식단에 대해 정리해 보았다. 결과 본 연구에서는 자살의 생화학적 기전을 확인하고, 자살에 대한 위험요인 식단, 보호요인 식단에 대해 조사하였다 신경계통에서 산화스트레스 반응, 염증반응과 같은 신경독성효과가 자살의 위험을 높일 수 있음을 확인하였다. 이들 이전 연구 결과를 바탕으로, 자살과 식단 사이의 이전 연구들을 고찰 해본 결과, 자살에 대한 위험 요인으로 술, 담배, 커피, 탄산음료, 패스트 푸드 등의 과도한 사용 및 섭취가 있었다. 자살에 대한 보호요인으로 비타민 C, 카로틴 등의 항산화 물질 그리고 해산물이 다량 포함하고 있는 오메가 3 지방산 같은 항염증 제재등이 있었다. 결론 자살의 유일한 치료법은 그 예방이다. 그러한 맥락에서 자살의 예방적 치료들의 적용에 있어 효과, 접근성, 안전이 중요할 수 있다. 이런 관점에서 비교적 안전하고, 대중들이 쉽게 섭취할 수 있는 항산화물질 및 항염증 제재들은 자살의 위험을 낮출 수 있는 효과적 보조적 치료가 될 수 있을 것이다. 뿐만 아니라, 자살의 위험을 높일 수 있는 식단을 줄이기 위한 대중들에 대한 교육이 필요할 수 있다
