Virtual Reality in Cognitive Rehabilitation (인지재활 영역에서 가상현실)

  • Introduction : This study reviews the main areas of cognitive rehabilitation including executive dysfunction, memory dysfunction, perceptual dysfunction, attention deficit, and dysfunctions in activities of daily living in order to apply to the adaptation of occupational therapy. Body : Cognition programs based on the virtual reality are being used not only to evaluate but to train the overall components of human's cognition. Because the cognitive program is concentrating on the real environment, it is known to bring a remarkable transitional effect to the actual environment, compared to the basic computer-based evaluation and training. Applying virtual reality to the rehabilitation program can develop and advance the high technology and can result in a major effect on the innovative treatment technology. Conclusions : In this process, virtual reality is expected to be researched more in the near future. Particularly in the cognitive realm, it is imperative for researchers to pay attention to the improved transitional effect of the virtual reality toward the actual environment, rather than the already existing method of evaluations. Therefore, application of the virtual reality for the cognitive training should be researched for various types of subjects in the diverse aspects of congnitive function. Application of the virtual reality in the cognitive function has its unlimited potential, thus the rehabilitation program integrated with not only evaluation but training and education is expected extensively in the future.

The Effect of Symmetrical and Asymmetric Bilateral Training for Chronic Stroke Patients in Upper Extremity Recovery (만성 뇌졸중 환자의 대칭적, 비대칭적 양측성 상지훈련의 상지기능회복 효과)

  • Objective : To identify the effect of symmetrical and asymmetric bilateral training For stroke patients in upper extremity recovery. Methods : 15 patients with stroke, randomized to an in- phase group(n =7) and anti-phase group(n =8). Each groups received symmetrical and asymmetric bilateral training, 30-min sessions per a day for 5 weeks, total 20 session.Accelerometer was used to evaluate the amount used for both groups. Y-BAT was used to evaluate performance status and satisfaction, ARAT was used to evaluate hand function. Results : the amount used of symmetrical movement training showed significant changes in affected and unaffected side. asymmetric bilateral training. there is a significant difference in affected side before and after receiving asymmetric bilateral training. Also, There was a significant difference between the groups on the affected side. Both training, there was no significant difference in performance, satisfaction, and upper limb function between group but, there was significant differences within-groups, Conclusions : Symmetric training showed higher motor performance than asymmetric training, but, To obtain a clearer difference, it would be necessary to use a neuromuscular assessment tool such as fMRI. Also, need a clearer training protocol and the need for follow-up studies on more stroke patients is suggested.

Surgical Experience of Paraclinoidal Aneurysms (상상돌기 주위 동맥류의 수술적 치험)

  • Objective : Paraclinoidal aneurysms termed that aneurysms arising from proximal internal carotid artery(ICA) between the site of emergence of the carotid artery from roof of the cavernous sinus and origin of the posterior communicating artery(PCoA). These aneurysms pose conceptual and technical surgical problems with regard to acquisition of proximal control and safe intracranial exposure. The efficiency of surgical technique according to the location of paraclinoidal aneurysm was studied for minimal exposure. Materials and Methods : Over the past four years, the authors treated surgically 171 cases of cerebral aneurysm, among them ten patients were paraclinoidal aneurysms with two patients unruptured aneurysms. Mean age was 47 years old, and all patients were female. Three patients were proximal posterior carotid artery wall aneurysms(one large, one giant), four patients carotid-ophthalmic artery aneurysms and three patients superior hypophyseal artery aneurysms. Results : There could be done clip in all cases, there were no deaths and no complication. And no patient developed sustained neurological deficits including visual function except hydrocephalus in one case. Four patients complained of visual disturbance but two patients had recovery after postoperation and two patient were not longer to bad. Conclusion : Our recent experience suggests that preoperative scrutiny of diagnostic angiography allows classification of all paraclinoidal aneurysms regardless of size and surgical technique which this classification has focused on operative approaches unique to each aneurysm projection was helpful to improve the operative outcome with good visual function and to shorten the operative time.

Development and Usability of a Cognitive Rehabilitation System Based on a Tangible Object for the Elderly (고령자를 위한 실감객체기반 인지재활 시스템의 개발과 사용성 연구)

  • Objective: To develop and verify the usability of a cognitive rehabilitation system with diverse cognitive functional levels based on tangible objects for the elderly population. Methods: A study was conducted to investigate the system's strengths and weaknesses by upgrading it with responses from two groups of 15 patients and 4 occupational therapists. After undergoing three forms of training - regarding executive function, memory, and concentration for a total of 20-30 min, the participants were asked to answer a structured questionnaire about contents of the three forms of training, hardware including the tablet PC functioning as a CPU and display media and tangible objects, and satisfaction of experiential usage of the system. Results: Both groups responded that the most interesting training area was executive function while the least interesting was concentration. Six participants reported that the size of the screen of the tablet PC was inappropriate, and five responded that the size of the tool was inappropriate. All therapists and 40% of the patients responded that they were satisfied with this system. Conclusion: This system's features include easy manipulation of tangible tools for performing training tasks, easy selection of and training in cognitive areas based on users' needs, and automatic adjustment of difficulty level based on users' performance. The training environment was designed to be similar to the natural environment by using tangible objects in both hands as input devices for the system, and the system was considered as an alternative to the lack of community cognitive rehabilitation specialists.

Study on the Research Trend of Overseas Elderly Occupational Therapy Using Text Mining (텍스트마이닝을 활용한 국외 노인작업치료의 연구동향 분석)

  • Objective : The purpose of this study was to quantitatively analyze the quantitative changes in, and the status of, overseas occupational therapy using text mining. Methods : Using PubMed, research papers on Elderly, Health and Occupational therapy published between 2009 and 2019 were selected for analysis, Abstracts of the selected papers were analyzed. The number of annual papers, the key words, the key words by year, and the relationship between the words were analyzed. Results : The number of papers published from 2009 to 2019 was 9,941, there was a gradual increase from 2009 to the highest in 2017 or 2018, followed by a decreasing trend in 2019. Within the last five years, the most frequent words were Care, Group, Intervention, Pain, Treatment, and Work. There was a strong relationship between the words based on the average frequency over the last 11 years, function, health, event, and partition. Conclusion : This study is meaningful because it applied a new research method called text mining to the empirical and systematic analysis of trends in occupational therapy and presented macroscopic and comprehensive results. The findings are expected to help establish new research directions at clinical and research sites for occupational therapy related to older adults.

  • Introduction : OMFT is a therapeutic technique based on sensorimotor, motor control and motor learning, and its major goal is to improve oral motor function. The oral motor conceptual hierarchical development is divided into 5 steps: 1) sensorimotor, 2) movement integration, 3) structural movement, 4) functional oral motor, and 5) comprehensive oral motor. Discussion : The OMFT consists of 3 techniques, 10 categories, and 50 sub-item. 1) Warming up technique: 2 categories, 12 sub-item, warming up by sensory awareness and adaptation, therapy situation adaptation, neck movement; 2) Key point technique: 7 categories, 30 sub-item, oral motor facilitation and increasing chewing skill by direct stroke of oral structures such as the face, lips, cheeks, gum, jaws, and tongue; 3) Application technique: 1 category, 8 sub-item, facilitate food intake and swallowing. Conclusion : The goal of this article is to introduce 3 techniques, 50 sub-item of OMFT, as a comprehensive oral motor therapy method, for application to clients. This article provides information that will help oral motor specialists in treating clients with oral motor problems more effectively and professionally.

A Study on Vocational Rehabilitation Evaluation Tool for Mental Disorders (정신장애인 직업재활 평가도구에 대한 고찰)

  • Objective : The purpose of this study is to identify and analyze the vocational rehabilitation evaluation tool for the mentally disabled. Methods : For literature search, the Pubmed database was used, and for the analysis, the development year, evaluation method, number of items, scale, and evaluation items were analyzed. In the analysis method, each evaluation item was divided into four categories: function, internal factor, environment, and mental symptom, and the evaluation elements of each evaluation tool were identified. Results : When searching Pubmed through search terms, 161 documents were retrieved. According to the selection method, Griffiths Work Behavior Scale (GWBS), Occupational Functioning Scale (OFS), Social and Occupational Functioning Assessment Scale (SOFAS), Work Ability Index (WAI), Work Behavior Inventory (WBI), Work Environment Impact Scale (WEIS), and Work and Social Adjustment Scale (WSAS) were screened. The evaluation items of all evaluation tools included job-related functional evaluation. According to the purpose of each evaluation tool, internal factors, environment, and mental symptoms were measured. Conclusion : Occupational skills are skills in which various functions such as physical, cognitive, social skills, and coping skills act in a complex way. Therefore, it is necessary to include the four factors analyzed in this study: function, internal factors, environment, and psychiatric symptoms.

Evaluation of Upper Limb Movement and Function in Stroke Patients Using Electromyography : A Review (근전도를 활용한 뇌졸중 환자의 상지 운동 및 기능 평가에 관한 고찰)

  • Objective : This study aimed to investigate the use of electromyography (EMG) to evaluate upper limb movement or function in stroke patients. Methods : We reviewed papers published in journals between January 2018 and December 2021 using PubMed, EMBASE, Scopus, RISS, and KISS. The main keywords of databases were ('stroke' OR 'hemiplegia') AND ('EMG' OR 'electromyography' OR 'electromyogram' OR 'muscle activity') AND ('Upper limb' OR 'Hand'). Results : Fifteen studies were selected, most of which evaluated muscle activity. Interventions performing tasks related to activities of daily living (ADLs), using assistive technology, and interventions that provide repetitive training were most frequently applied. Conclusions : When evaluating upper limb functions using electromyography, it is meaningful to present an evaluation that can be used according to the purpose of the study and to provide a basis for setting up interventions that can utilize electromyography during evaluation.

Effects of Beat-Keeping Game Through Smartphone Applications on Executive Functions of Children With Developmental Delays (스마트폰 어플리케이션을 이용한 박자 맞추기 게임이 발달 지연 아동의 실행기능에 미치는 효과)

  • Objectives : This study aimed to investigate the effect of beat-keeping games in smartphone applications on improving executive functions in children with developmental delays. Methods : Three children diagnosed with developmental delay were included in this study. The ABA design used a single-subject experimental research design. The independent variable was the beat-keeping game. The game was held three times a week for a total of seven times for 20 minutes, including breaks. The dependent variable, "Visual-motor speed," was measured every session to assess if the beat-keeping game was effective in improving the participant's executive function. Further, before and after the intervention, "Children's Color Trails Test (CCTT)", "Block design," and "Finding hidden picture" were measured. Results : All three participants showed improvement in the performance of the beat-keeping game and the executive functions of "Visual-motor speed" and visual attention. Conclusions : Based on the results of this study, various effective applications for learning and intervention can be developed and applied to children with developmental delays who have difficulty in motivating themselves and lack attention.

A Study on the Evaluation Method of Comprehension Task in Proverbs for the Elderly (노인용 속담 이해력 과제의 평가 방법에 대한 고찰)

  • Objective : This study aimed to establish an analytic methodology that can objectively evaluate the proverb comprehension levels that vary based on the aging process. Method : This study recruited 70 normal elderly aged ≥ 65 years and performed proverb comprehension tasks using an explanation method. A scoring estimation method was designed on 3 and 5-point scales. The difficulty and discrimination levels were calculated based on the two scales, thereby selecting the final questions and evaluation scale. Results : First, the item discrimination index on the 5-point scale was relatively higher than that on the 3-point scale, and 10 items were finally selected. Second, in terms of proverb comprehension ability, the elderly group aged 70 ~ 84 years showed significantly lower performance than those aged 65 ~ 69 years. Additionally, subjects with ≥ 10 years of education showed significantly higher performance than those with 9 ≤ years of education. Third, the performance of the proverbs comprehension task negatively correlated with the reaction time of the executive function task. Conclusion : Proverb comprehension is a task that can distinctively show a deterioration of brain functions in line with the aging process even among the normal elderly group and therefore, can estimate the difference more effectively with an increase in the score of the evaluation scale.
