강제유도 운동치료는 2주간 건측의 사용을 억제하기 위해 깨어 있는 시간의 90%를 제한하는 동시에 환측에 매일 6시간 이상 강도 높은 훈련을 실시하는 것이다. 하지만 강제유도 운동치료가 뇌졸중 환자의 임상 적용에 있어서 문제점을 보였고, 이를 최소화하기 위해 수정된 강제유도 운동치료가 고안되었다. 뇌졸중 환자를 대상으로 수정된 강제유도 운동치료의 적용이 다양한 방법으로 연구되어져 왔고, Motor Activity Log(MAL), Fugl-Meyer Assessment(FMA), Wolf Motor Function Test(WMFT), Action Research Arm Test(ARAT), Functional Independence Measure(FIM), Stroke Impact Scale(SIS)을 통해 뇌졸중 환자의 기능 향상에 대한 효과를 증명하였다. 수정된 강제유도 운동치료가 뇌졸중 환자의 상지기능, 일상생활활동과 삶의 질 향상을 위한 치료 방법으로써 작업치료 임상에서 유용하게 적용될 수 있을 것이라 사료된다.
Background: The presence of visuospatial impairment can make patients slow functional recovery and impede the rehabilitation process in TBI patients. Objective: The aim of this study is to investigate effects of prism adaptation treatment for functional outcomes in patients following traumatic brain injury. Methods: The subject received prism adaptation treatment for 2 weeks additionally during traditional rehabilitation for 4 weeks. The Patient has prism adaptation treatment while wearing wedge prisms that shift the external environment about $12^{\circ}$ leftward. The patient received 10 sessions, 15-20min each session. Outcome measures were visuospatial deficit(line bisection, latter cancellation), Visual and spatial perception(LOTCA-visual perception and spatial perception), motor function of upper extremity(FMA U/E; Fugl-Meyer motor assessment upper extremity, ARAT; Action research arm test), balance(BBS; Berg Balance Scale), mobility(FAC; Functional ambulation classification) and functional level(FIM; Functional independent measure). All Assessments took place on study entry and post-treatment assessments were performed at discharge from the hospital. Results: After prism adaptation, the visuospatial impairment scores improved as indicated in the line bisection(-15.2 to -6.02), latter cancellation(2 to 0) and LOTCA- spatial perception scores(7 to 9). The upper motor function improved as indicated in the scores of affected FMA U/E(21 to 40) and ARAT(4 to 22). Ambulation and balance improved as indicated in the BBS scores(25 to 38) and FAC scores(0 to 4). ADL function improved as indicated in the FIM total scores 54 to 70(motor 34 to 61, cognition 20 to 29). Conclusion: Prism adaptation did improve functional level such as motor functions and ADL abilities in TBI patient. Further research is recommended.
objective to ascertain whether the concept of the therapeutic side is associated with changes in the blind sport mapping that represents the brain function. design Physiological blind spot maps were used as an integer of brain activity before and after acupuncture needling on the meridian point Hapkok(合谷) and Techung(太衝) in the unilateral side decided by double-blind controlled study(20 subjects). setting outpatient clinic participants: adult volunteers intervention twenty subjects were divided into two comparative groups and underwent specific acupuncture therapy on the unilateral side. Blinded examiners obtained reproducible pre and post-acupuncture cortical maps, which were subjected to statistical analysis. main outcome measures Brain activity was demonstrated by reproducible circumferential measurements of cortical hemispheric blind spot maps before and after acupuncture on the unilateral side. in case of acupuncture needling on the ipsilateral side of an enlarged side of bilnd spot, there were reduction of blind spot in 7 cases of 10 subjects, and enlargement in 3 cases. in case of acupuncture needling on the contralateral side of the enlarged side of blind spot, there were enlargement of blind spot in 6 cases of 10 subjects, and reduction in 4 cases. results the significant changes in the blind spots before and after acupuncture were observed Acupuncture needlings on the ipsilateral or contralateral side of an enlarged cortical map were associated with the concept of the therapeutic side traditionally accepted in the oriental medical society. Acupuncture needling on the ipsilateral side of an enlarged blind spot map is associated with the reduction of map, and increaed contralateral cortical activity. Acupuncture needling on the side opposite an enlarged blind spot map is associated with the enlargement of map, and decreased cortical activity. conclusion Reproducible maps of cortical responses can be used to measure the neurological consequences of acupuncture needling. Acupuncture can affect the somatic sensory informations that reach to the contralateral thalamus, and so affect thalamic integration. we found that acupuncture therapy may be associated with an increase or a decrease in brain function depending on the side of acupuncture needling. thus, the traditional concept of the contralateral therapeutic theory in acupuncture approach has the clinical significance in the view of brain function.

본 연구는 척수 앞뿔의 세포손상을 준 흰쥐를 대상으로 수영운동과 미노싸이클린의 치료적 중재를 통해 운동기능과 신경계의 회복을 알아보고 운동에 의한 척수손상의 신경학적 회복기전을 밝히는 것의 목적이 있다. 본 연구의 진행은 Sprague-Dawley계 흰쥐 28마리의 허리뼈 1-2번 사이에 6-OHDA로 척수손상을 일으킨 다음 3일 후에 치료를 시작하였다. 수영은 주 5회 15분간 운동을 실시하고 미노싸이클린은 척수손상을 유발한 후 12시간마다 복강내 주입하였다. 척수손상을 유발한 후 치료를 시작하기 전에 운동기능의 평가를 시행하며 2주 동안 4번의 검사가 이루어졌으며, Bcl-2 발현에 관한 측정은 2주간 치료 후 검사하였다. BBB 척도에서 대조군에 비해 실험군에서 7일에 차이가 있었으며, 14일에는 II, III군에 비해 실험군 Ⅳ군에서 증가하였다. Bcl-2의 면역학적 소견에서 실험군 모두 대조군에 비해 척수 앞뿔에서 Bcl-2의 발현이 증가되었으며, 실험군 Ⅳ에서 가장 많은 Bcl-2의 발현이 관찰되었다. 척수손상 후 미노싸이클린과 수영은 행동학적, 면역학적 소견을 긍정적으로 개선시키는 것으로 나타났으며, 수영과 미노싸이클린을 함께 실시한 경우 세포사멸의 감소와 운동기능의 회복에 효과가 있음을 알 수 있었다.
This study is intended to examine the tDCS and Montoya stair task(MST) on sensorimotor recovery and glial scar expression in MCAo induced stroke model of rat. To achieve this goal, this study selected 80 SD rats of 8 weeks. The experiment groups were divided them into four groups, and assigned 20 rats to each group. Group I was a experimental control group; GroupII was a tDCS application group after MCAo; Group III was a MST application group after MCAo; Group IV was a tDCS and MST application group after MCAo. In each group, neurological function test measurement, motor behavior test, montoya stair task test, immunohistochemistric finding of GFAP expression finding were analyzed. In motor behavior test, the outcome of group I was significantly difference than the other group, especially from 14days. In montoya stair task test, the outcome of group I was significantly lower than the other group especially, group II were significantly different on 14days and group IV was most significantly difference than the other group. In immunohistochemistric finding, group II, III, IV were decrease GFAP expression on depend on time stream. These results throughout the MCAo due to focal ischemic brain injury rat model four weeks tDCS and MST was applied, when the neurobehavioural, upper extremity function and ability, histopathologic data suggest that sensorimotor function recovery and a positive influence on glial scar decrease and confirmed that.
목적: 본 연구는 국외 뇌졸중 환자에게 적용한 한 말초감각신경자극의 효과에 대하여 고찰하기 위한 것으로 국외 학술지를 대상으로 체계적 문헌고찰 연구방법을 시행하였다. 연구방법: 2015년 10월 이전까지 국외 학술지에 게재된 논문을 Pubmed를 통하여 검색하였다. 주요 검색 용어로는 'peripheral nerve stimulation', 'electrical stimulation', 'sensory stimulation', 'somatosensory stimulation', 'stroke', 'hemiplegia', 'hemiparesis' 와 'hand', 'arm', 'upper limb'를 사용하였다. 최초 검색된 논문은 501편이었으나 선정 및 배제기준을 거쳐 10편의 연구가 선정되었다. 결과: 임상적으로 널리 사용되고 있는 말초감각신경자극은 뇌졸중 환자들에게 적용이 될 때에 보다 다양한 중재방법으로 연구에 적용이 되고 있었다. 말초감각신경자극치료는 뇌졸중 환자의 상지 기능에 효과를 나타냈으며, 대뇌피질의 활성화에도 긍정적인 영향을 나타냈다. 결론: 본 연구는 말초감각신경자극치료의 적용에 대한 근거를 제시하며, 향후 국내연구에서는 다양한 중재방법을 적용하여 말초감각신경자극의 효과를 더 정확하게 측정을 할 수 있는 방법에 대한 연구가 필요하다.
서론 : 본 연구는 대상자의 신체기능과 활동 및 참여를 위해 다양한 치료적 중재를 실시하는 작업치료 분야에 몰입경험 이론의 소개를 목적으로 한다. 본론 : 작업치료 영역에서의 몰입경험 이해를 위해 국내·외 문헌고찰을 통한 몰입경험의 개념과 특징, 단계와 뇌과학적 특성, 측정방법 및 관련 연구들을 정리하였다. 작업치료에서 대상자의 몰입경험은 '시간이 흐르는 것을 잊으면서 하고 있는 치료 활동에 완전하게 빠져드는 심리적 상태'로 정의할 수 있다. 대상자는 과제에 대한 명확한 목표를 인식하며, 도전과 기술 수준이 높은 상태로 균형을 이룰 때에 몰입을 경험하게 된다. 선행 연구에서 몰입을 경험한 대상자는 주의 및 집중, 집행 기능, 상지기능, 일상생활활동 수행 증진과 긍정적인 경험에 긍정적 변화가 있음을 확인하였다. 결론 : 국내 작업치료 영역에서 다양한 치료적 중재에 대상자의 몰입경험이 고려된다면 치료 결과의 효과는 보다 촉진될 것이라 생각되어진다.
Objectives: Cognitive impairments, ranging from mild to severe, adversely affect daily functioning, quality of life, and work capacity. Despite significant efforts in the past decade, more than 200 promising drug candidates have failed in clinical trials. Herbal remedies are gaining interest as potential treatments for dementia due to their long history and safety, making them valuable for drug development. This review aimed to examine the mechanisms behind the effect of Polygonum multiflorum on cognitive function. Methods: This study focused primarily on the effects of Polygonum multiflorum and its chemical constituents on cognitive behavioral outcomes including the Morris water maze, the passive avoidance test, and the Y maze, as well as pathogenic targets of cognitive impairment and Alzheimer's disease (AD) like amyloid deposition, amyloid precursor protein, tau hyperphosphorylation, and cognitive decline. Additionally, a thorough evaluation of the mechanisms behind Polygonum multiflorum's impact on cognitive function was conducted. We reviewed the most recent data from preclinical research done on experimental models, particularly looking at Polygonum multiflorum's effects on cognitive decline and AD. Results: According to recent research, Poligonum multiflorum and its bioactive components, stilbene, and emodin, influence cognitive behavioral results and regulate the pathological target of cognitive impairment and AD. Their mechanisms of action include reducing oxidative and mitochondrial damage, regulating neuroinflammation, halting apoptosis, and promoting increased neurogenesis and synaptogenesis. Conclusion: This review serves as a comprehensive compilation of current experiments on AD and other cognitive impairment models related to the therapeutic effects of Polygonum multiflorum. We believe that these findings can serve as a basis for future clinical trials and have potential applications in the treatment of human neurological disorders.
목적 : 본 연구는 뇌졸중 환자를 대상으로 한 이중과제 훈련(Dual-task training)의 효과를 알아본 연구들을 분석하는 것을 목적으로 한다. 연구방법 : NDSL, RISS, PubMed, CoChrane, EMBASE를 통해 지난 10년 간 국내외 학술지에 게재된 논문들을 검색하였고, 최종적으로 10개의 논문을 선정하였다. 이를 전통적 단일계층 근거 모형에 따른 질적평가를 시행하고, Comprehensive Meta Analysis 3.0 프로그램을 통해 메타분석을 시행하였다. 결과 : 선정된 10편의 논문의 질적 수준은 모두 전통적 단일계층 근거 모형의 I과 II에 해당되어 높은 편에 속했다. 이중과제 훈련을 구성하고 있는 운동과제로는 7편의 문헌에서 보행 또는 균형 과제를 선택했으며, 3편의 연구에서 상지운동 관련 과제를 선정하였다. 메타분석을 실시한 결과 일상생활활동 수행능력이 0.65, 인지기능이 0.64로 보통(medium) 효과크기로, 하지운동 기능과 상지운동 기능은 각각 0.34와 0.22로 작은(small) 효과크기를 보였다. 일상생활활동 수행능력과 인지기능의 효과크기만 통계학적으로 유의미하였다(p<0.05). 결론 : 본 연구를 통해 이중과제 훈련이 뇌졸중 환자의 일상생활활동 수행능력 및 인지기능 회복에 유용한 중재기법이 될 수 있음을 확인하였다. 이는 국내 임상 환경에서 뇌졸중 환자에게 적절한 중재방법을 선정할 때 도움이 될 만한 자료로 활용될 수 있을 것이다.
