A Case of Cerebral Aneurysmal Subarachnoid Hemorrhage in Fabry's Disease

  • We report an unusual case of cerebral aneurysmal subarachnoid hemorrage (SAH) with Fabry's disease. A 42-year-old woman presented with aneurysmal SAH originated from a saccular aneurysm of the right posterior communicating artery. The patient was treated by an endovascular coil embolization of aneurysm. Postoperatively the patient recovered favorably without any neurological deficit. During her admission, the patient had a sign of proteinuria in urine analysis. The pathologic findings of kidney needle biopsy implied nephrosialidosis (mucolipidosis of lysosomal stroage disease), which is consistent with a Fabry's disease. It is uncommon that Fabry's disease is presented with aneurysmal SAH, especially in middle-aged patients, but could be a clinical concern. Further investigations are needed to reveal risk factors, vascular anatomy, and causative mechanisms of Fabry's disease with aneurysmal SAH.

A Case of Pineal Ependymoma

  • A 64-year-old man was admitted to our hospital in semicomatous consciousness. Brain computed tomography scans demonstrated $2.6{\times}2.5\;cm$ sized hyperdense mass in the pineal region with multiple punctate calcifications and hydrocephalus. Brain magnetic resonance imaging demonstrated a pineal mass which was heterogeneously enhanced with gadolinium. After external ventricular drainage, the patient regained consciousness. The mass was totally removed via occipital transtentorial approach. No consequent ventricular shunt was needed and the patient recovered without any neurological deficit. Final pathologic report of the tumor was ependymoma.

Facial Nerve Schwannoma Located in Middle Cranial Fossa

  • Facial nerve schwannomas are uncommon tumors. A 40-year-old female presented with left-side facial weakness. Computed tomography[CT] imaging showed a $3\;{\times}\;2cm$ lesion on the posterior portion of the left middle cranial fossa. The mass abutted the anterior aspect of the left petrous bone with a wide erosive change involving the area of the left facial nerve ganglion [geniculate ganglion]. A well-circumscribed extra-axial mass was seen on magnetic resonance imaging[MRI]. The tumor was completely removed through subtemporal approach and the patient was discharged without additional neurological deficit. This rare case is discussed and a review of the relevant literature is presented.

Bilateral Vertebral Artery Dissecting Aneurysms : A Long Term Follow-up Results of Microsurgical Trapping and Proximal Occlusion

  • Regarding the bilateral vertebral artery [VA] dissecting aneurysms, treatment strategy remains controversial because there have not been enough cases to reach a conclusion on the best treatment. We present a patient underwent staged microsurgical trapping and endovascular coiling for each dissecting aneurysm of bilateral VA presenting subarachnoid hemorrhage [SAH]. The ruptured side was managed by VA trapping procedure without any neurological deficit. Postoperative cerebral angiography revealed patent right PICA without filling of previous right dissecting aneurysm and spontaneous occlusion of the left dissecting aneurysm one month after trapping procedure. However, follow-up angiography revealed recanalization and growing of the left VA dissecting aneurysm one year after the operation. The patient underwent endovascular embolization using GDC for the proximal occlusion of the left VA and postoperative course was uneventful.

산조인의 플라보노이드 및 사포닌의 진정작용 (Sedative Action of Flavonoids and Saponin from the Seeds of Zizyphus vulgaris var. spinosus Bunge)

  • The acute toxicity and sedative activity of flavonoids and saponin isolated from Zizyphus seeds have been evaluated in mice. All the compounds elicited potentiation of hexobarbital-induced hypnosis, inhibition of ladder-climbing and caffeine-induced hyperactivity. Swertisin was the most potent of all flavonoids tested. The potency of saponin in these tests was found to be higher than that of swertisin. The activities in rotarod test and electroshock seizure test, on the other hand, were relatively weak or nil. From these findings it was suggested that flavonoids and saponin from Zizyphus seeds have neuroleptic activity without anticonvulsant or muscle relaxant activity accompanied by neurological deficit.

Cervical Osteochondroma Presenting as Brown-Sequard Syndrome in a Child with Hereditary Multiple Exostosis

  • Cervical cord compression due to osteochondroma in hereditary multiple exostosis (HME) is a rare condition, especially in young children. In this report, we discuss a rare case of cervical osteochondroma presenting as Brown-Sequard syndrome (BSS) in a 7-year-old boy with HME. The child was admitted because of hemiparesis involving the right limbs and hypoesthesia on the left side following mild trauma. MR image revealed cord compression by osteochondroma of the C7 lamina. We removed the osteocondroma and the neurological deficit was improved.

상완신경총에서 발생한 신경원성 종양 - 1예 보고 - (Neurogenic Tumor of the Brachial Plexus -A case report -)

  • 상완신경총에 생기는 신경원성 종양은 드문 질환이다. 최근에 저자는 상완신경총에 발생한 한 예의 신경초종을 치험하였다. 환자는 34세 남자로 좌측 쇄골상부에 서서히 자라는 종괴를 주소로 내원하였다. 자기공명영상에서 좌측 쇄골하동맥의 후상방부에 고립성 종괴를 발견하였다. 수술 시 상완신경총 직하부에 피막으로 잘 감싸진 종괴를 발견하였으며, 신경 섬유를 가능한한 다치지 않게 하기 위해 피막을 열고 종양을 적출하였다. 수술 후 신경학적인 결함은 없었다.

Spontaneous Intracranial Hypotension Secondary to Lumbar Disc Herniation

  • Spontaneous intracranial hypotension is often idiopathic. We report on a patient presenting with symptomatic intracranial hypotension and pain radiating to the right leg caused by a transdurallumbar disc herniation. Magnetic resonance (MR) imaging of the brain revealed classic signs of intracranial hypotension, and an additional spinal MR confirmed a lumbar transdural herniated disc as the cause. The patient was treated with a partial hemilaminectomy and discectomy. We were able to find the source of cerebrospinal fluid leak, and packed it with epidural glue and gelfoam. Postoperatively, the patient's headache and log radiating pain resolved and there-was no neurological deficit. Thus, in this case, lumbar disc herniation may have been a cause of spontaneous intracranial hypotension.

  • A rare case of gliosarcoma with neurofibromatosis type I is presented. The patient was a 33-year-old woman who had headache and vomiting for one week. Multiple neurofibromas over her whole body with many cafeau- lait spots were present since childhood. At admission, she had no focal neurological deficit and ophthalmologic examination revealed bilateral Lisch nodules. Brain CT and MRI revealed a heterogeneously enhancing mass in the left fronto-parietal region with marked peritumoral edema and mass effect. The tumor was removed gross totally and a gliosarcoma was diagnosed histopathologically. Post operative course was uneventful with resolution of symptom, followed by radiotherapy with 60 Gy. A brief overview is given of this rare case together with the pertinent literature.

Variability of Platelet Reactivity on Antiplatelet Therapy in Neurointervention Procedure

  • As more intracranial aneurysms and other cerebrovascular pathologies are treated with neurointervention procedure, thromboembolic events that frequently lead to serious neurological deficit or fatal outcomes are increasing. In order to prevent the thromboembolic events, antiplatelet therapy is used in most procedures including coil embolization, stenting, and flow diversion. However, because of variable individual pharmacodynamics responses to antiplatelet drugs, especially clopidogrel, it is difficult for clinicians to select the adequate antiplatelet regimen and its optimal dose. This article reviews the neurointervention literature related to antiplatelet therapy and suggests a strategy for tailoring antiplatelet therapy in individual patients undergoing neurointervention based on the results of platelet function testing.
