무심폐기 관상동맥 우회술에 있어서 이식편의 대동맥부 근위부 문합 방법에 따른 수술 후 신경학적 합병증 발생의 비교: 하트스트링을 이용한 방법 대 고식적인 수기 문합술 (Comparison of the Neurologic Outcome according to the Method of Proximal Graft Anastomosis at the Aortic Side during Off-pump Coronary Artery Bypass Grafting: The Heartstring Sealing System versus Conventional Manual Anastomosis)

  • 배경: 관상동맥 우회술의 대동맥부 근위부 문합을 위한 상행 대동맥 부분 겸자는 대동맥 내벽에 존재하는 죽상반의 색전에 의한 뇌경색의 위험성을 높일 뿐 아니라, 겸자에 따른 직접적 대동맥 손상의 원인이 될 수 있다. Heartstring proximal sealing system (Guidant Corporation, Santa Clara, Calif)을 이용한 근위부 문합은 대동맥 부분 겸자를 피할 수 있어, 이러한 잠재적 위험을 방지할 수 있는 것으로 제시되어 왔다. 이 연구는 무심폐기 관상동맥 우회술의 근위부 문합에 있어서 대동맥 부분 겸자를 통한 수기 문합과 Heartstring system을 이용한 문합간의 수술 후 신경학적 합병증을 비교하고자 하였다. 대상 및 방법: 2003년 1월부터 2008년 8월까지 499명의 환자가 무심폐기 관상동맥 우회술을 시행받았다. 그 중 182명의 환자가 Heartstring system을 이용한 이식편 대동맥 근위부 문합술을 시행받았고(I군), 317명의 환자가 고식적 수기 문합술을 시행 받았다(II군). 이 두 군간의 수술 후 신경 손상을 비롯한 주요 합병증 및 사망율을 비교하였다. 결과: 나이, 성별, 좌심실 구출율, 신장 기능, 당뇨, 고혈압, 흡연력 등의 수술 전 인자들에 있어서 두 군간에 유의한 차이는 없었다. 수술 전 뇌허혈의 과거력은 환자는 두 그룹간에 유의한 차이가 없었으나(I군 31.3%, II군 34.4%, p=0.48), 뇌혈관 MRI상에서의 내경동맥의 협착이 75%가 넘는 환자는 I군이 81명(44.5%), II군이 95명으로(30.0%) 두 군간의 유의한 차이를 나타냈다(p=0.003). 수술 후 새로 발생한 신경 이상의 경우 I군에서 3명, II군에서 2명으로, 두 군간에 유의한 차이를 보이지 않았으나(p=0.258), I군의 경우 모두 일과성 허혈이었던 것에 반해 II군의 경우 모두 색전에 의한 다발성 뇌경색으로 나타났다. 그 외 수술 후 발생한 주요 합병증(p=0.47)및 사망(p=0.40)에 있어서 두 군간에 유의한 차이를 보이지 않았다. II군에서 한 명의 환자에서 수술 중 근위부 분합에 따른 대동맥 박리증이 발생하여 상행대동맥 치환술을 시행하였다. 결론: 비록 수술 후 신경학적 합병증의 빈도가 낮아 통계학적 유의성에 다다르진 못했으나, Heartstring system을 이용한 근위부 문합술은 고식적 문합에 비해 색전에 의한 수술 후 뇌졸중의 합병 및 대동맥 손상의 위험도를 낮출 것으로 기대해 본다.

Unstable Pathologic Vertebral Fractures in Multiple Myeloma : Propensity Score Matched Cohort Study between Reconstructive Surgery with Adjuvant Radiotherapy and Radiotherapy Alone

  • Objective : Although radiotherapy (RT) is recommended for multiple myeloma (MM) involving spine, the treatment of choice between reconstructive surgery with RT and RT alone for pathologic vertebral fractures (PVFs) associated with structural instability or neurologic compromises remains controversial. The purpose of this study was to evaluate the clinical efficacies of reconstructive surgery with adjuvant RT for treatment of MM with PVFs by comparing with matched cohorts treated with RT alone. Methods : Twenty-eight patients underwent reconstructive surgery followed by RT between 2008 and 2015 in a single institution, for management of PVFs associated with structural instability of the spine and/or neurologic compromises (group I). Twenty-eight patients were treated with RT alone (group II) after propensity score matching in a 1-to-1 format based on instability of the spine, as well as age and performance. Clinical outcomes including the overall survival rates, duration of independent ambulation, neurological status, and numeric rating scale (NRS) for back pain were compared. Results : Clinical and radiological features before treatment were similar in both groups. The median survival period was similar between the two groups. However, the mean duration of independent ambulation was significantly longer in group I (88.8 months; 95% confidence interval [CI], 66.0-111.5) than in group II (39.4 months; 95% CI, 25.2-53.6) (log rank test; p=0.022). Deterioration of Frankel grade (21.4% vs. 60.7%, p=0.024) and NRS for back pain (2.7±2.2 vs. 5.0±2.7, p=0.000) at the last follow-up were higher in the group II. Treatment-related complications were similar in both groups. Conclusion : In patients with unstable PVFs due to MM, reconstructive surgery may yield superior clinical outcomes compared with RT alone in maintaining independent ambulation and neurological status, as well as pain control despite similar median survival and complications.

Comparative Analysis of Surgical Outcomes of C1-2 Fusion Spine Surgery between Intraoperative Computed Tomography Image Based Navigation-Guided Operation and Fluoroscopy-Guided Operation

  • Objective : Fixation of the C1-2 segment is challenging because of the complex anatomy in the region and the need for a high degree of accuracy to avoid complications. Preoperative 3D-computed tomography (CT) scans can help reduce the risk of complications in the vertebral artery, spinal cord, and nerve roots. However, the patient may be susceptible to injury if the patient's anatomy does not match the preoperative CT scans. The intraoperative 3D image-based navigation systems have reduced complications in instrument-assisted techniques due to greater accuracy. This study aimed to compare the radiologic outcomes of C1-2 fusion surgery between intraoperative CT image-guided operation and fluoroscopy-guided operation. Methods : We retrospectively reviewed the radiologic images of 34 patients who underwent C1-2 fusion spine surgery from January 2009 to November 2018 at our hospital. We assessed 17 cases each of degenerative cervical disease and trauma in a study population of 18 males and 16 females. The mean age was 54.8 years. A total of 139 screws were used and the surgical procedures included 68 screws in the C1 lateral mass, 58 screws in C2 pedicle, nine screws in C2 lamina and C2 pars screws, four lateral mass screws in sub-axial level. Of the 34 patients, 19 patients underwent screw insertion using intraoperative mobile CT. Other patients underwent atlantoaxial fusion with a standard fluoroscopy-guided device. Results : A total of 139 screws were correctly positioned. We analyzed the positions of 135 screws except for the four screws that performed the lateral mass screws in C3 vertebra. Minor screw penetration was observed in seven cases (5.2%), and major pedicle screw penetration was observed in three cases (2.2%). In one case, the malposition of a C2 pedicle screw was confirmed, which was subsequently corrected. There were no complications regarding vertebral artery injury or onset of new neurologic deficits. The screw malposition rate was lower (5.3%) in patients who underwent intraoperative CT-based navigation than that for fluoroscopy-guided cases (10.2%). And we confirmed that the operation time can be significantly reduced by surgery using intraoperative O-arm device. Conclusion : Spinal navigation using intraoperative cone-beam CT scans is reliable for posterior fixation in unstable C1-2 pathologies and can be reduced the operative time.

Fundoplication in Neonates and Infants with Primary Gastroesophageal Reflux

  • Purpose: Gastroesophageal reflux in infant is a physiological process. However, surgery is performed in high risk infants with severe gastroesophageal reflux disease (GERD) when medical management fails. This study focuses on efficacy and safety of Nissen fundoplication for GERD in infants under age 12 months. Methods: This study was a retrospective case analysis of 11 neonates and infants under 12 months of age who underwent Nissen fundoplication following a failure of medical treatment between June 2010 and June 2013 at Pusan National University Children's Hospital. The records were reviewed to determine the effect of fundoplication on symptoms and post-operative complications. Results: A total of 11 infants consist of four males and seven females. Mean birth weight was $2,305.5{\pm}558.6g$ (1,390-3,130 g). They had some underlying disease, which are not related with GERD such as congenital heart disease (54.5%), prematurity (45.5%), neurologic disease (18.2%), respiratory disease (18.2%), and other gastrointestinal disease. Mean body weight at surgery was $3,803.6{\pm}1,864.9g$ (1,938.7-5,668.5 g). Mean age at operation was $99.9{\pm}107.6days$ (17-276 days). Duration from operation to full enteral feeding was 10.9 days. Symptoms related GERD disappeared in all patients including one who got reoperation. One infant died of congenital heart disease unrelated to surgery. There were no complications related to fundoplication. Conclusion: Fundoplication is effective and safe treatment in the neonates and infants with severe GERD.

Radiofrequency Neurotomy of the Gray Ramus Communicans for Lumbar Osteoporotic Compression Fracture

  • Objective : The aim of this study was to determine the efficacy of percutaneous radiofrequency neurotomy[RFN] of ramus communicans nerve in patients suffering from severe low back pain due to osteoporotic compression fracture. Methods : Twenty two patients with lumbar osteoporotic compression fracture who had intractable back pain for less than two weeks and were performed with RFN at L1-L4 from May 2004 to December 2005 were retrospectively analyzed. Clinical outcome using visual analogue scale[VAS] pain scores and modified MacNab's grade was tabulated. Complications related to the procedure were assessed. Results : Twenty-two female patients [age from 63 to 81 years old] were included in this study. The mean VAS score prior to RFN was 7.8, it improved to 2.6 within postoperative time of 48 hours, and the mean VAS score after 3 months was 2.8, which was significantly decreased. Eighteen of 22 patients were graded as excellent and good according to modified MacNab's criteria at final follow up. All patients recovered uneventfully, and the neurologic examination revealed no deficits. Two patients showing poor results worsened in symptom. Percutaneous was performed eventually resulting in symptom improvement. There were no significant complications related to the procedure such as sensory dysesthesia, numbness or permanent motor weakness. Conclusion : RFN is safe and effective in treating the painful osteoporotic compression fracture. in patients with intractable back pain due to lumbar osteoporotic compression fracture, RFN of gray ramus communicans nerve should be considered as a treatment option prior to vertebroplasty.

안면골 골절에 관한 임상적 연구 (CLINICAL STUDY ON FACIAL BONE FRACTURES)

  • To investigate epidermiologic trend in maxillofacial fractures. We retrospectively studied 934 patients with maxillofacial fractures between $1981{\sim}1987$ and $1995{\sim}1999$. The results were compared in the previous group (Group A, patients treated between $1981{\sim}1987$) with those in the recent group (Group B, $1995{\sim}1999$). Also, we studied 516 patient between $1995{\sim}1999$ for occupation, associated injuries, treatment and complications. The sex ratio of men to women decreased (5.6 : 1 in Group A vs. 3.5 : 1 in Group B), and the largest age group was 20 to 29 years. There was the highest incidence in September and Fall. The most frequent cause of maxillofacial fracture in both Group A and B was traffic accidents. The distribution of fracture site did not change appreciably, but the frequency of midfacial fracture increased. In the recent group, the largest occupational group was salaried men, and the largest associated injuries of maxillofacial fracture was head and neck injury. Open reduction was used in 91% of the cases, and post-operation complications were infection, neurologic problem, malocclusion, and mouth opening limitation etc. Our results suggest that the clinical features of patients with maxillofacial fractures have changed during the past decade.

Anterior Choroidal Artery Aneurysm Surgery : Ischemic Complications and Clinical Outcomes Revisited

  • Objective : Surgical results for anterior choroidal artery (AChA) aneurysms have previously been reported as unsatisfactory due to inadvertent occlusion of the AChA, while the low incidence of AChA aneurysms hampers the accumulation of surgical experience. The authors reviewed their related surgical experience to document the ischemic complications and surgical outcomes. Methods : Identification of the AChA at its origin by rigorous visual scrutiny, careful microdissection, and meticulous clip placement to avoid the AChA origin are all crucial surgical maneuvers. A retrospective review of a surgical series of 62 consecutive cases of an AChA aneurysm between 2004 and 2012 was performed. Results : All patients, except for five (8.1%) with a small residual neck, showed complete aneurysm obliteration in postoperative angiographic evaluations. There was no incidence of procedure-related permanent AChA syndrome or oculomotor nerve palsy, while three (4.8%) patients suffered from transient AChA syndrome. The clinical outcomes [the 3-month modified Rankin Scale (mRS)] of the patients were related to their preoperative World Federation of Neurologic Surgeons (WFNS) grade. As regards the 3-month mRS, significant differences were found between patients with an unruptured aneurysm (WFNS grade 0; n=20), good-grade subarachnoid hemorrhage (WFNS grade 1-3; n=30), and poor-grade subarachnoid hemorrhage (WFNS grade 4-5; n=4). Conclusion : In surgical treatment of AChA aneurysms, a risk of AChA insufficiency can be minimized by taking every precaution to preserve the AChA patency and intraoperative monitoring. In addition, the surgical outcome is primarily determined by the preoperative clinical state.

하악골 골절의 임상통계학적 연구 (ANALYSIS OF 334 CASE REPORTS OF MANDIBULAR FRACTURE)

  • We retrospectively reviewed 334 inpatients who sustained a total of 518 mandibular fractures and who ewer treated in our department between l980-1990. This results were obtained as follows : 1. In respect of incidence, there were the highest frequency in July, and the lowest frequency in May. The number of patients has not been increased year after year due to competition with other department in our hospital. 2. The age frequency was the highest in the 2nd decade(38.9%) and the ratio of man to women was 4.9 : 1. 3. The most frequent cause of mandibular fracture was traffic accident(43.4%), and the next was fall down(24.3%), fist blow(71%), industrial accident(21%) and others in order. In the traffic accident, autobicycle accident was 14.1%. 4. The most common location of mandibular fracture was symphysis(38.8%), condyle(20.7% ), angle(19.9%) and body(15.1%) were next in order of frequency. The classification by location of fracture, the frequency of single fracture was 54.8%. 5. In 334 patients of mandibular fracture, the frequency of associated injuries was facial laceration(58.4%), teeth injuries(37.7%), extremity injuries(13.2%) were next in order of frequency. 6. The patients arrived in hospital immediately within 24 hours after accident wee 61.4% of all. In respect of treatment, open reduction was 68.7% of all. 7. Complications including infection were present 11.1% of patient. Other complications inclued delayed healing malocclusion, malocclusion and neurologic problem.

가정간호 대상자의 의료 기구사용에 관한 조사연구 (Personal Use of Medical Equipment in Home Care Patients)

  • This study was conducted to investigate the use of medical equipment in patients receiving home care service. The subjects of this study were 88 patients cared by seven home care nursed who were registered in the Seoul Nurses Association. Data was collected from Aug. 1, 1998 to Dec. 30, 1998. The findings are as follows. 1. The sample was found to be 55.7% female : 51.7% over 65 years old. 75% with neurologic disease including CVA, brain tumor, ICH, Parkinsonism & Spinal stenosis and 78.4% living in Seoul. The Clinical experience of the home care nurses was greater than five years. 2. Medical equipment which the patients possessed were foley catheters(61.4%), L-tubes(59.1%) and tracheostomy tubes(51.1%). 3. Technical difficulties in use of medical equipment were related to home care ventilators(60.0%), L-tubes(3.8%) and tracheostomy tubes(2.2%). 4. Most of the medical equipment were obtained from the hospital where they had been admitted previously or from medical equipment companies. 5. Complications from the use of this equipment were infection through invasive techniques including wound drainage tubes(50%), and IV injections(22.2%), The complications were resolved through referral to the doctor of the hospital where they were previously admitted or through community health centers. 6. Most of the equipment was disposable, and equipment was disinfected by using various methods including boiling and soaking in antiseptic solutions. These findings suggest that consistant policy on the management of medical equipment is necessary for the safety of home care patients.

척수종양 수술 후 발생한 척추 불안정 : 추궁성형술과 추궁제거술의 비교 연구 (Spinal Instability Following for Spinal Cord Tumors : Laminoplasty vs. Laminectomy)

  • Objectives : To demonstrate the preventive effect on post-operative spinal deformity of the laminoplasty compared with laminectomy for spinal cord tumor. Methods : Sixteen patients who underwent laminoplasty and 89 patients who underwent lamenectomy were reviewed retrospectively after removal of spinal cord tumors between February 1981 and December 1999. The mean follow-up period after laminoplasty and laminectomy were 29 months(range, 12-49 months) and 66 months(range, 6-120 months) respectively. Patients were observed neurologically and radiologically. Results : Postoperative kyphosis was observed 17 patients(19%) in the laminectomy group, but was not observed in the laminoplasty group. Postoperative slipping was observed 16 patients(18%) in the laminectomy group, but was not observed in the laminoplasty group. Complications such as postoperative scoliosis, rotation, and swan-neck deformity were observed in the laminectomy group, but were not observed in the laminoplasty group. Conclusions : Laminoplasty prevented postoperative spinal deformities after removal of spinal cord tumors, and afforded anatomic reconstruction of the vertebral arch. Additionally, appropriate patient selection maximizes the chance of optimal neurologic outcomes and minimizes complications. Laminoplasty appears to warrant further evaluation as an alternative to wide laminectomies for exposure of intraspinal tumors.
