열공성 뇌경색 환자-대조군에서 신경행동검사의 수행 평가 (A Neurobehavioral Performance Assessment in Lacunar Infarction Case-control Study)

  • 전체 뇌졸중의 $15{\sim}20%$를 차지하고 있는 열공성 뇌경색(lacunar infarction)은 최근 뇌경색형 뇌졸중의 증가와 더불어 그 중요성이 점차 증가하고 있는데, 1년 이내 뇌경색이 재발되거나 노인, 고혈압 및 초기 신경학적 이상과 같은 감수성 인자가 있는 경우 뇌경색 재발과 혈관성 치매 발생 위험이 있어 삼차예방적 측면에서의 적절한 처치가 필요한 질병이다. 그렇지만 열공성 뇌경색은 증상이 없거나 경미한 경우가 상당히 많아 환자 자신이 치료의 중요성을 느끼지 못할 뿐 아니라 의사들도 환자의 경미한 신경학적 이상을 인지하지 못하여 처치의 당위성에 대해 설득력있는 근거를 제시할 수 없어 적절한 처치가 미비한 상황이다. 본 연구는 신경학적 자각 증상은 없지만 MRI상 열공성 뇌경색으로만 진단된 환자군이 MRI상 이상소견이 관찰되지 않는 대조군에 비하여 신경행동 검사상 차이가 관찰될 것이라는 가설을 구명하고자 하였다. 따라서 뇌졸중의 증상 및 증후 없이 두개골 외상 혹은 두통, 경부강직 등과 같은 신경학적 이상으로 MRI를 촬영한 대상자 중에서 다른 이상 없이 열공성 뇌경색으로만 진단될 환자군과 특별한 이상 소견이 없는 것으로 진단된 대조군에서 WHO의 NCTB(neurobehavioral core test battery)검사의 일부와 Perdue pegboard score검사를 시행하였고, 그 결과 대조군에 비해 환자군의 신경행동 이상이 평균 및 가장 빠른 단순반응시간 검사를 제외한 5개 검사에서 관찰되었다. 본 검사의 결과는 자각 증상이 없거나 경미한 증상을 호소하는 열공성 뇌경색 환자들의 차후 필요한 검사의 선별과 삼차 예방적 측면에서 처치의 근거로서 활용될 수 있을 것이다. 또한 자각 증상이 없거나 경미한 증상이 있지만 진찰상 신경학적 이상 증후를 거의 찾을 수 없는 대상자에서 MRI검사 필요성의 근거를 확인하고자 하는 대상자에게 이용될 수 있을 것으로 사료된다.

스트레스 유발 마우스모델에서 뇌염증 및 신경행동 장애 변화 (Neuro-inflammation induced by restraint stress causes impairs neurobehavior in mice)

  • Background : Behavioral stress has been suggested as one of the significant factors that is able to disrupt physiological systems and cause depression as well as changes in various body systems. The stressful events can alter cognition, learning, memory and emotional responses, resulting in mental disorders such as depression and anxiety. Results : We used a restraint stress model to evaluate the alteration of behavior and stress-related blood parameter. The animals were randomly divided into two groups of five animals each group. Furthermore, we assessed the change of body weight to evaluate the locomotor activity as well as status of emotional and anxiety in mice. After 7 days of restraint stress, the body weight had significantly decreased in the restraint stress group compared with the control group. We also observed stress-associated behavioral alterations, as there was a significant decrease in open field and forced swim test, whereas the immobilization time was significantly increased in the stress group compared to the control group. We observed the morphological changes of neuronal death and microglia by immunohistochemistry and western blot. In our study restraint stress did not cause change in neuronal cell density in the frontal cortex and CA1 hippocampus region, but there was a trend for an increased COX-2 and iNOS protein expression and microglia (CD11b) in brain, which is restraint stress. Conclusion : Our study, there were significant alterations observed in the behavioral studies. We found that mice undergoing restraint stress changed behavior, confirming the increased expression of inflammatory factors in the brain.

신생흰쥐의 알콜성 뇌손상 후 환경강화가 BDNF 발현 및 운동수행력에 미치는 영향 (Effects of Enriched Enviroment on Expression of BDNF and Motor Performence after Alcohol-induced Brain Injury in Neonatal Rats)

  • Alcohol exposure during development leads to significant long-term neurobehavior dysfunction and central nervous system alteration. The purpose of this study was to determine the effects of enriched enviroment in developmental period through motor behavior test and expression of BDNF. Neonatal rat exposed to alcohol on postnatal days 4 through 10 were studied. Female Sprague-Dawley pups were assigned to two groups. Experimental group(EG) via 4.5 g kg-1day-1 of ethanol was housed in enriched enviornment for 9 weeks. The main result of this study were as follows: 1. There was significant difference in the mean of weight change between control and experimental group. 2. In motor behavior test, there was significant difference in the mean of weight change between control and experimental group. 3. Regarding the immunoreactivity of BDNF were higher appeared experimental group than control group. In conclusion, the present results reveals that enriched enviroment in developmental period is to be extremely useful in neuronal reprganization and motor behavior improvement after alcohol exposure.

Association of heavy metal complex exposure and neurobehavioral function of children

  • Background: Exposure to heavy metals is a public health concern worldwide. Previous studies on the association between heavy metal exposure and neurobehavioral functions in children have focused on single exposures and clinical manifestations. However, the present study evaluated the effects of heavy metal complex exposure on subclinical neurobehavioral function using a Korean Computerized Neurobehavior Test (KCNT). Methods: Urinary mercury, lead, cadmium analyses as well as symbol digit substitution (SDS) and choice reaction time (CRT) tests of the KCNT were conducted in children aged between 10 and 12 years. Reaction time and urinary heavy metal levels were analyzed using partial correlation, linear regression, Bayesian kernel machine regression (BKMR), the weighted quantile sum (WQS) regression and quantile G-computation analysis. Results: Participants of 203 SDS tests and 198 CRT tests were analyzed, excluding poor cooperation and inappropriate urine sample. Partial correlation analysis revealed no association between neurobehavioral function and exposure to individual heavy metals. The result of multiple linear regression shows significant positive association between urinary lead, mercury, and CRT. BMKR, WQS regression and quantile G-computation analysis showed a statistically significant positive association between complex urinary heavy metal concentrations, especially lead and mercury, and reaction time. Conclusions: Assuming complex exposures, urinary heavy metal concentrations showed a statistically significant positive association with CRT. These results suggest that heavy metal complex exposure during childhood should be evaluated and managed strictly.
