상지역동학 치료가 수근관 증후군 환자의 통증, 악력, 신경전도속도에 미치는 효과 (The Effects of Upper Limb Neuro Dynamic Treatment on the Pain, Grip Strength and Nerve Conduction Velocity of Patients with Carpal Tunnel Syndrome)

  • Background: When applying the upper limb neurodynamic treatment ULNT1 and upper limb neurodynamic treatment ULNT2, The object of this study evaluates the pain and grip strength, nerve conduction velocity and tries to present the treatment that is effective in the carpal tunnel syndrome through the comparison of effect between tests. Methods: The ULNT1 (n=10) and ULNT2 (n=10) was applied to the carpal tunnel syndrome in patient of 20 subject for 10 time 5 set during 4 weeks. Both groups received conventional physical therapy (hot pack with 30 minutes and ICT with 15 minutes and ultra sound with 15 minutes). Results: The ULNT1 showed the difference which the result of this study significantly in the median nerve motor latency, median nerve motor amplitude, median nerve sensory latency, median nerve sensory amplitude, grip strength and pain. ULNT2 showed the difference significantly in median nerve motor latency, median nerve motor amplitude, median nerve sensory latency, median nerve sensory amplitude and pain. Conclusions: If it implements by applying the traditional physiotherapy and ULNT1 and ULNT2 to the carpal tunnel syndrome subjects according to the subject state and function, the remedial value can be more enhanced.

고양이 치수에서 교감신경에 의한 미세순환조절에 관한 기능적 연구 (ROLE OF SYMPATHETIC NERVE ON THE CONTROL OF MICROCIRCULATION IN THE FELINE DENTAL PULP)

  • The purpose of this study was to investigate the functional involvement of sympathetic nerve in the control of the microcirculation in the dental pulp with the aim of elucidation of the involvement of neuropeptides and sympathetic nerve in neurogenic inflammation. Experiments were done on the 7 cats anesthetised with sodium pentobarbital, and sympathetic nerve to the' dental pulp was stimulated electrically (10 Hz, 4 V, 1.5 ms, 3.5 mins). Ana-adrenoceptor antagonist phentolamine and a neuropeptide Y antagonist D-myo-inositol-1,2,6-trisphosphate (PP56) were injected close intra-arterially into the dental pulp without changing the systemic blood pressure. The probe of laser Doppler flowmeter was placed on the buccal surface of ipsilateral canine teeth to the stimulation, and pulpal blood flow was measured. Stimulation of the sympathetic nerve decreased pulpal blood flow by $55.24{\pm}7.74\;%$ (mean${\pm}$SEM, n = 13). Stimulation of the sympathetic nerve following the injection of the ${\alpha}$-adrenoceptor antagonist phentolamine ($0.1{\mu}g$/kg) caused decrease of pulpal blood flow by $14.35{\pm}3.43%$ (mean${\pm}$SEM, n=5). Phentolamine attenuated the sympathetic nerve-induced pulpal blood flow decrease by $74.02{\pm}9.32%$ (mean${\pm}$SEM) Stimulation of the sympathetic nerve following the injection of the neuropeptide Y antagonist PP56 (2.3 mg/kg) caused decrease of pulpal blood flow by $30.64{\pm}7.92%$ (mean${\pm}$SEM, n=6). PP56 attenuated the sympathetic nerve-induced pulpal blood flow decrease by $44.37{\pm}11.01%$ (mean${\pm}$SEM). These data provide evidences of the co-contribution of nerepinephrine and neuropeptide Y on the sympathetic nerve-induced vasoconstriction in the feline dental pulp. In addition, they show functional evidences that sympathetic nerve plays an active role in controlling the microcirculation of the dental pulp.

안면경련 환자에서 O'Brien법을 이용한 안면신경 차단의 추적조사 (Retrospective Study of Facial Nerve Block with O'Brien Method for Facial Spasm)

  • Background : Hemifacial spasm commonly occurs on muscles about the eye, but may also involve or spread to the entire side of the face. There are many treatment for facial spasm, such as neuro-vascular decompression, local injection of Botulium toxin, facial nerve block at stylomastoid foramen, facial nerve block with O'Brien method. The present study was aimed to investigate the effects of facial nerve block with O'Brien method. Methods : Forty five patients with hemifacial spasm were treated by facial nerve block with O'Brien method from January 1996 to February 1997 We reviewed the charts, retrospectively. Results : Sex ratio was 1:1.7(17 male : 28 female patients). Most patients were 40~60 years old. Most patients well tolerated facial nerve block. Three patients failed to respond to the facial nerve block. We repeated the procedure within one week. Among the 45 patients who received nerve block, 35 received repeated block; 7 patients received second repeat block, 2 patients received third repeat block. After successful nerve block, all patients were free of spasm for 1 to 6 months. Average spasm-free period was 3.5 months. Conclusion : Although the spasm-free period was short, these results suggest facial nerve block with O'Brien method is a safe and comfortable method for treatment of facial spasm.

수근관 증후군 치료결정의 한 요인으로 연령의 중요성 (The Importance of Age as a Factor of Carpal Tunnel Syndrome management)

  • Background : Carpal tunnel syndrome(CTS) is the most common entrapment neuropathy that refers to a group of signs and symptoms resulting from compression of the median nerve at the wrist. The course of CTS in older patients is different from the younger patients. This difference may be the result of different underlying mechanisms. The different nerve conduction studies of CTS may signify different approaches in management. This study was done to assess the differences in nerve conduction study of CTS in younger and older patients. Methods : This study involved 224 patients who visited Gachon Medical School, Gil Medical Center and was diagnosed by nerve conduction study from October 1997 to October 1999. We compared the results of nerve conduction study to age, especially in between those under 60 years and those 60 years or over CTS patients. Nerve conduction study consists of motor studies of both median nerves(terminal latency, compound action potential) and sensory studies(nerve conduction velocity, nerve action potential). And we also evaluated the variables between younger and older patients group. Those variables include sex, symptom period, laterality, abnormal physical findings and radiculopathy. Results : We found that a significant increase of terminal latency(p<0.1), but a decrease in compound motor action potential(p<0.05) in older patient's group. There was no significant differences in sensory nerve conduction velocity and action potential between those under 60 years and those 60 years or even patients. And also there was no significant difference in sex, symptom period, laterality, abnormal physical findings, radiculopathy between older and younger patients. Conclusions : This study showed a significant increase in the terminal latency and a decrease in compound action potential in older patients. The different nerve conduction studies of CTS by age effect may need different approaches in management.

Risk of Encountering Dorsal Scapular and Long Thoracic Nerves during Ultrasound-guided Interscalene Brachial Plexus Block with Nerve Stimulator

  • Background: Recently, ultrasound has been commonly used. Ultrasound-guided interscalene brachial plexus block (IBPB) by posterior approach is more commonly used because anterior approach has been reported to have the risk of phrenic nerve injury. However, posterior approach also has the risk of causing nerve injury because there are risks of encountering dorsal scapular nerve (DSN) and long thoracic nerve (LTN). Therefore, the aim of this study was to evaluate the risk of encountering DSN and LTN during ultrasound-guided IBPB by posterior approach. Methods: A total of 70 patients who were scheduled for shoulder surgery were enrolled in this study. After deciding insertion site with ultrasound, awake ultrasound-guided IBPB with nerve stimulator by posterior approach was performed. Incidence of muscle twitches (rhomboids, levator scapulae, and serratus anterior muscles) and current intensity immediately before muscle twitches disappeared were recorded. Results: Of the total 70 cases, DSN was encountered in 44 cases (62.8%) and LTN was encountered in 15 cases (21.4%). Both nerves were encountered in 10 cases (14.3%). Neither was encountered in 21 cases (30.4%). The average current measured immediately before the disappearance of muscle twitches was 0.44 mA and 0.50 mA at DSN and LTN, respectively. Conclusions: Physicians should be cautious on the risk of injury related to the anatomical structures of nerves, including DSN and LTN, during ultrasound-guided IBPB by posterior approach. Nerve stimulator could be another option for a safer intervention. Moreover, if there is a motor response, it is recommended to select another way to secure better safety.

전기자극의 강도와 측정전극의 간격이 감각신경신호의 파라미터에 미치는 영향 연구 (The Effects of the Stimulation Intensity and Inter-Electrode Distance on the Parameters of the Measured Sensory Nerve Signal)

  • This study was designed to investigate the effects of stimulation intensity and inter-electrode distance on the parameters of the measured sensory nerve signal. 30 healthy subjects participated in this study. Sensory nerve signals were elicited by four different pulse amplitudes, i.e., 3, 6, 9, 12 mA, with the pulse width fixed at $500{\mu}s$. The sensory nerve signals elicited by the four different pulse amplitudes were measured by four different inter-electrode distances (20, 30, 40, and 50 mm). We extracted four parameters (pulse amplitude, pulse width, pulse area, and latency time from stimulation) from the sensory nerve signals. The measured pulse amplitude and pulse width were increased when the measuring inter-electrode distance was increased while the stimulating pulse amplitude was fixed. The measured pulse amplitude was saturated with the stimulating pulse amplitudes of over 6 mA while measuring inter-electrode distance. Under the same condition, measured pulse width was increased, and sensory nerve signal was initiated early. Sensory nerve signals, specially those of pulse amplitude, were distorted by a differential amplification method that commonly measures the human body signal. The experimental results indicate that the differential amplification method is required to be replaced when measuring nerve signals. Our observations suggested that the hyperpolarization of the action potential of the sensory nerve signal for preventing distortion could be used to clarify the correlation between the parameters of the sensory nerve signals and quantification of sensations.

A Morphometric Study of the Obturator Nerve around the Obturator Foramen

  • Objective : Obturator neuropathy is a rare condition. Many neurosurgeons are unfamiliar with the obturator nerve anatomy. The purpose of this study was to define obturator nerve landmarks around the obturator foramen. Methods : Fourteen cadavers were studied bilaterally to measure the distances from the nerve root to relevant anatomical landmarks near the obturator nerve, including the anterior superior iliac spine (ASIS), the pubic tubercle, the inguinal ligament, the femoral artery, and the adductor longus. Results : The obturator nerve exits the obturator foramen and travels infero-medially between the adductors longus and brevis. The median distances from the obturator nerve exit zone (ONEZ) to the ASIS and pubic tubercle were 114 mm and 30 mm, respectively. The median horizontal and vertical distances between the pubic tubercle and the ONEZ were 17 mm and 27 mm, respectively. The shortest median distance from the ONEZ to the inguinal ligament was 19 mm. The median inguinal ligament lengths from the ASIS and the median pubic tubercle to the shortest point were 103 mm and 24 mm, respectively. The median obturator nerve lengths between the ONEZ and the adductor longus and femoral artery were 41 mm and 28 mm, respectively. Conclusion : The obturator nerve exits the foramen 17 mm and 27 mm on the horizontal and sagittal planes, respectively, from the pubic tubercle below the pectineus muscle. The shallowest area is approximately one-fifth medially from the inguinal ligament. This study will help improve the accuracy of obturator nerve surgeries to better establish therapeutic plans and decrease complications.

우안 외전신경마비 회복 후 동측 동안신경마비가 발생한 환자 치험 1례 (A Case of Occurred Oculomotor Nerve Palsy On the Same Side after Recovery of Abducent Nerve Palsy at Right Eye)

  • Recurrence of peripheral nerve palsy is unusual, specially in eye. So there's seldom report about recurrent peripheral nerve palsy in eye. We treated a patient who had consecutive oculomotor nerve palsy after recovery of abducent nerve palsy at right eye. The patient visited our clinic for abducent nerve palsy. When he was hospitalized, ophthalmalgia was VAS 4 but it disappeared when he discharged on 21th of June. Although there was, in the case of abduction of eye, no noticeable change during the hospitalization, it was healed after he received outpatient service twice a week until Nov. 3rd. Oculomotor nerve palsy appeared on 24th of November, 2008 and the patient was hospitalized on Nov. 26th. At that time ophthalmalgia was VAS 4, but disappeared when he discharged. When he entered hospital, the length between upper & lower eyelid and MRD 1 were all 0mm. However, when he discharged, the length between upper & lower eyelid was 11mm, and MRD 1, 4mm which were the same lengths as those of the normal left eye. In the case of eye movement, the motion of supraduction, infraduction, and adduction was entirely inhibited when the patient was hospitalized. By the time of discharge, the inhibition of infraduction was recovered after that of adduction, but the inhibition of supraduction was not recovered. This is a very rare case of peripheral nerve palsy, nevertheless he recovered complete twice by acupuncture and herb medicine therapy.

악관절 수술후 안면신경 손상에 대한 임상적 연구 (CLINICAL STUDY OF FACIAL NERVE INJURY AFTER TMJ SURGERY)

  • Authors have studied retrospectively the facial nerve injury after TMJ surgery through the preauricular approach routine. The study material used was 4 patients of all 113 patients who were diagnosed as internal derangement and have been operated from March 1989 to February 1991 in Youngdong severance hospital, and were induced postoperatived facial nerve injury. The patient group who had the postoperative injured facial nerve was recognized degree of injury using the diagnostic method, Electromyography(EMG) and Nerve conduction test(NCT) which are used widely at present and was treated as conservative care and we identified the recovery time as the same method. The results as follows : 1. The meticulous care and precious surgical technique are needed in both operation and postoperation. During the TMJ surgery, the excessive retraction of the flap and frequent use of nerve stimulator and electric surgical knife should be avoided as possible and postoperative hematoma and swelling should be minimized. 2. The 4 patients were experienced with the postoperative facial nerve injury of all 133 patients who had been operated the TMJ surgery through the routine preauricular approach on our hospital. And the incidence of postoperative facial nerve injury happened was about 0.3% and its incidence was relatively low comparing with any other previous reports. 3. EMG and NCT were considered as useful methods which can diagnose the nerve injury objectively and identified the effect of treatment and recovery time. 4. The faical nerve-injured patients who were induced postoperatively after TMJ surgery, were diagnosed as second-degree nere injury through the EMG and NCT. And the patient group was treated well as conservative physical therapy for about 2 to 4 months.

하악골 신장술 후 하치조신경의 조직학적 변화와 신경성장인자의 발현에 대한 연구 (HISTOLOGICAL CHANGES AND EXPRESSION OF NERVE GROWTH FACTOR IN THE INFERIOR ALVEOLAR NERVE AFTER DISTRACTION OSTEOGENESIS)

  • Distraction osteogenesis (DO) is frequently used technique in reconstruction of bony defects resulted from tumor resection, congenital deformity, and trauma in the maxillofacial region. Although the histologic and ultrastructural changes associated with distraction osteogenesis have been extensively described, the exact changing of the surrounding tissues, such as nerve tissues, were still unclear. This study observed the histological changes and the expression of nerve growth factor (NGF) in the inferior alveolar nerve (IAN) after distraction osteogenesis. Unilateral mandibular distraction (0.5 mm twice per day for 10 days) was performed in eight mongrel dogs. Two animals were sacrificed at 7, 14, 28 and 56 days after completion of distraction, respectively. The distracted IAN and contralateral control nerve were harvested and processed for histological and innunohistochemical examinations. The signs of acute nerve injuries, such as demyelination and partial discontinuation of nerver fiber, were observed in the distracted IAN on 7 and 14 days after distraction. The initial remyelination and regeneration of distracted IAN were showed at 14 days after completion of distraction. At 56 days later, the histologic features of distracted IAN was similar to those of the normal control IAN. The expression of NGF was significantly increased in most distracted nerve tissues on 7, 14 and 28 days after distraction. On 56 days after distraction, the expression of NGF returned to the normal level. This study suggested that the acute IAN injury caused by mandibular distraction were mostly recovered during consolidation period. The NGF was seemed to be induced from Schwann cell and damaged nerve tissues, and it may have important roles in the initial healing of damaged nerves.
