Study on inhibition effects on metastasis and recurrence of Traditional Herb Medicine(THM) (한약의 전이재발억제 효과에 관한 연구)

  • Recently, the occurrence and death rates of cancer have increased rapidly. Metastasis and recurrence is the most important factor of death from cancer. So inhibition of recurrence and metastasis can increase the cure rate of cancer disease. At the basic chinese traditional medicine(TCM), there are a lot of theory related to neoplasm treatment. The metastasis and recurrence of neoplasm was the basis of yudu(餘毒) on remained neoplasm cell and stagnation of blood, thermotoxo, phlegm, asthenia of healthy enerngy and stagnation of vital energy. The principles therapy of neoplasm on metastasis and recurrence was based on knowledge of the method of support the healthy energy and strengthen the body resistance, promote blood circulation to remove blood stasis, clear away heat and toxic materials, dissipate phlegm and disperse the accumulation of evils. But the major clinical features of neoplasm was to be considered in developing a treatment plan include (1) distinguish between clinical and pathologic staging - acute and chronic, (2) classification of pathologic pattern, and (3) distingction of body situation : for examples asthenia - sthenia etc. It was most important to distinguish between supporting the healthy and eliminating the evil factors and to treat differently at the root and the branch cause of a neoplasm. This paper's results indicate that identification and effective use of THM can inhibite netastasis and recurrence and then it will help increasing survival rate. Also as BioTechnology(BT) has developed rapidly nowadays, as genes related to cancer have revealed. So it will give rise to the development of searching the mechanism of herb medicines which have inhibition effects of metastasis and recurrence.

$H\"{u}rthle$ Cell Tumor of the Thyroid (갑상선의 $H\"{u}rthle$씨 세포 종양)

  • H$\"{u}$rthle cell neoplasm of the thyroid gland is an uncommon, but potentially malignant lesion. However, in many instances, the malignant potential of the H$\"{u}$rthle cell neoplasm is very difficult to judge histologically. For this reason, the biologic behavior of this tumor and its optimal treatment have come under considerable debate in recent years. In order to review the clinicopathologic features of the H$\"{u}$rthle cell neoplasm and to determine its optimal treatment modalities, we studied 26 patients with path logical proof of H$\"{u}$rthle cell tumor from January 1987 to September 1997. We also performed an immunohistochemical study using the monoclonal antibodies against antigen CD34 for the angiogenic activity of this tumor and evaluated the differences of microvessel density(MVD) between benign and malignant tumors. The age of the patients ranged from 1 to 71 years with a mean of 44.2 years. There were 6 males and 20 females(M : F= 1 : 3.3). The accuracies of fine needle aspiration biopsy and frozen section were very low; 6.3% and 34.8%, respectively. There were 20 benign tumors and 6 malignant tumors(23.1%). All the malignant tumors were microinvasive(intermediate) type which had minimal capsular invasion and most of them(5 cases) were diagnosed postoperatively. Any specific clinicopathologic differences were not seen between benign and intermediate groups. Most of the cases had conservative surgeries(15 ipsilateral lobectomy-isthmusectomy, 7 subtotal thyroidectomy) while total thyroidectomy was performed in 4 cases. Of the cases with malignant tumor, 2 had ipsilateral lobectomy-isthmusectomy, 3 had subtotal thyroidectomy and the remaining 1 had total thyroidectomy. Mean size of the tumors was 3.0 cm(0.1- 8.5 cm) in the greatest diameter and multiple tumors were seen in 6 cases(23.1 %). During the follow-up period, only one recurrence(3.8%) of benign tumor occurred but distant metastasis or cause-specific death was seen in the benign or intermediate groups. Mean MVDs of the benign(n=13) and intermediate(n=6) groups were $121.7{\pm}35.3$ and $114.3{\pm}31.7$, respectively and there was no statistical significance between them. In conclusion, because of the low accuracies of fine needle aspiration biopsy and frozen section for the H$\"{u}$rthle cell neoplasm, the extent of surgery could be individualized based on permanent pathologic examination; Conservative surgery would be adequate for patients with benign or intermediate H$\"{u}$rthle cell neoplasm and total or near-total thyroidectomy for those with definite malignancy.

  • Multiple primary lung cancer is classified into a synchronous primary lung cancer or a metachronous primary lung cancer. Both are rarely encountered disease entities. We report our surgical experience of each one case of synchronous and metachronous primary lung cancer.

  • The recommended approach for dumbell trmor is a one-stage removal by a team work of both neurosurgeons & thoracic surgeons. Recently we removed a case of dumbell tumor in a 69 years old woman with standard posterolateral thoracotomy and concomitant laminectomy as single stage operation. Postoperative course was uneventful and there were no postoperative neurologic signs.

  • We report a case of primary cardiac fibroma which is a rare cardiac neoplasm and arising most frequently from right ventricular free wall and interventricular septum. A 2-month-old male infant with cyanosis was admitted for evaluation and treatment. Echocar-diographic lEnding was a cardiac mass. Successful nearly total removal of the tumor was performed. The pathologic result was compatible with fibroma. The postoperative course was not eventful, and have been in good condition upto now.

  • As reported in the literature, extraskeletal mesenchymal chondrosarcoma is a rare malignant tumor of soft tissue, and it has a unique, distinctive histologic picture and poor prognosis.The common sites are the orbit, the cranial dura mater, the neck, the thigh, the leg, the chest wall, and the retroperitoneum. Radical excision of the tumor seems to be the primary treatment.We report experience with a very rare case of extraskeletal mesenchymal chondrosarcoma in the posterior mediastinum.

  • We report two cases of giant soft tissue sarcomas metastatic to the lung from lower extremities. The lung metastasis occurred 2 years later from original diagnosis in 27-year-old woman with hemangiopericytoma and 8 years later in 54-year-old woman with synovial sarcomtt. We had performed pleuropneumonectomies with partial resection of pericardium involved. The postoperative courses were uneventful and postoperative adjuvant therapy was begun.

  • Posterior mediastinal goiter extending to carotid sheath posteriorly is rare. Recently we experienced two cases of posterior mediastinal goiter presenting dyspnea due to tracheal compression. The one was a 48-year-old female with mediastinal tumor shadow on chest roentgenogram . The other was a 54-year-old female with palpable mass on neck and huge mediastinal mass. These masses were resected completely through the right posterolateral thoracotomy and median sternotomy respectively. The postoperative courses were uneventful.

Carcinosarcoma of the Esophagus : 2 Case Report (식도에서 발생한 상피육종암 2례보고)

  • Carcinosarcoma of the esophagus have an interesting pathologic feature of admixture of carcinomatous and sarcomatous lesion and accounts for 0.5∼1.5% of all esophageal neoplasm. Generally, it has been reported that these have better prognosis than the squamous cell carcinoma. We have experienced two cases of carcinosarcoma occurring at the mid-esophagus. In both two cases, Ivor Lewis operation was performed and lymph node metastasis was absent. The pathologic diagnosis was confirmed as carcinosarcoma. Postoperative course was uneventful and the patients have been followed up for 3 months and 3 years, respectively, without any problems. We report these cases with a brief review of the literatures.

A Case of Lipoma of Palatine Tonsil (구개 편도 지방종 1례)

  • Lipomas is one of the most common benign neoplasm in the body, but only rarely occurs in the oral cavity We report here a case of a 49-year-old female with a lipoma of palatine tonsil. Microscopical examination revealed a stromal neoplasm composed of mature fat with the presence of a few thin and anastomosing fibrous septa. The surface epithelium was of nonkeratinizing stratified squamous type. There was the dense fibrous connective tissue between mature fat and surface epithelium. The histopathologic findings and literature on tonsillar lipoma is reviewed.
