Pancreatic Paraganglioma: a Case Report and Literature Review

  • Paraganglioma is a rare tumor of paraganglia, derived from neural crest cells in sympathetic or parasympathetic ganglions. It can be widely distributed from the skull base to the bottom of the pelvis. The pancreas, however, is a rare location of this neoplasm, and only a limited number of cases have been reported in the English literature, especially with gadoxetic-acid-enhanced magnetic resonance imaging (MRI) and diffusion-weighted images (DWI). We herein report a case of pathologically proven paraganglioma in the pancreas head with a literature review on endoscopic ultrasonography (EUS), computed tomography (CT), gadoxetic-acid-enhanced MRI, and DWI sequence.

Ectopic Intrapulmonary Thyroid: A Case Report

  • An ectopic thyroid is caused by abnormalities in migration of the thyroid during development and rarely occurs in the thoracic cavity or the abdominal cavity. We report the case of a 64-year-old female who had abnormal findings from a thyroid hormone test during follow-up after thyroid cancer surgery. Based on the radioisotope diagnostic test, an ectopic thyroid inside the thoracic cavity was suspected. Through surgical treatment, the patient was diagnosed with ectopic intrapulmonary thyroid. Ectopic intrapulmonary thyroid is reported to be very rare and the case is described along with a literature review.

  • Leiomyosarcoma may occur anywhere in the body but rarely occurs in the heart or great vessels. Leiomyosarcoma may be managed by surgical resection with or without chemotherapy or radiotherapy. Owing to the high rate of metastasis and poor prognosis, a definitive treatment modality for leiomyosarcoma has not yet been suggested. This case study reports the surgical management of the recurrent leiomyosarcoma of the heart and the great vessels in a 63-year-old woman.

  • Although neoplastic involvement of the pericardium is frequently present postmortem, cardiac manifestations before death are uncommon, and cardiac tamponade as the initial presentation of cancer is rare. We are presenting a metastatic pericardial tumor with cardiac tamponade of unknown primary neoplasm. The patient brought to hospital in a state of unconscious. The chest x-ray film showed cardiomegaly with a globular heart shape and right pleural effusion. We underwent an anterior thoracotomy and pericardial window was created. The histopathologic finding of pericardium, pleural and pericardial effusion show a metastatic adenocarcinoma. The patient subsequently received adjuvent radiotherapy and chemotherapy, but he expired on the postoperative 132 day.

Appendiceal Mucinous Neoplasm Detected due to the Protrusion of Mucin, in the Absence of Appendiceal Distension: A Case Report

  • A mucocele is a cystic dilatation of the vermiform appendix that contains mucous material. The symptoms associated with it are not specific and the diagnosis is seldom made prior to surgery. The reported prevalence in appendectomy specimens procured during surgery is 0.2-0.3%. Recently, we experienced a case of patient with appendiceal mucocele detected by colonoscopic examination. This case did not show typical colonoscopic features of a mucocele, demonstrating protrusion of mucin, in the absence of a smooth mound with normal overlying mucosa surrounding the appendiceal orifice. The case involved a 64-year-old woman who underwent a colonoscopy. An abdominopelvic computed tomography scan suggested a normal appendix. Subsequently, we performed an appendectomy. The pathologic finding was a low-grade appendiceal mucinous neoplasm.

  • Sebaceous carcinoma is a malignant neoplasm that usually arises in the sebaceous glands of the eyelids. Its pathogenesis is unknown; however, irradiation history, immunosuppression, and use of diuretics are known risk factors. The mainstay of treatment for sebaceous carcinoma of the eyelid is wide surgical resection with a safety margin of 5 to 6 mm, which often results in full-thickness defects. The reconstruction of a full-thickness defect of the eyelid should be approached using a three-lamella method: a mucosal component replacing the conjunctiva, a cartilage component for the tarsal plate, and a flap or skin graft for the skin of the eyelid. In this case, a full-thickness defect of the upper eyelid was reconstructed after tumor removal using a combination of a nasal septum chondromucosal composite graft and a forehead transposition flap, also known as a "Fricke flap." The flap was designed to include a line of the eyebrow on the lower margin of the flap to replace the eyelash removed during tumor excision. The wound healed completely, without any early or late complications, and the outcome was satisfactory.

원격전이된 미분화성 심장육종의 수술치험 (Surgical Intervention of Undifferentiated Cardiac Sarcoma with Metastases)

  • We report a case of primary undifferentiated cardiac sarcoma. The tumor originated from the left atrial free wall with multi-organ metastases, e.g., lung, and adrenal gland. The patient gradually grew worse with dyspnea and hemoptysis because of the obstructed left atrial outflow. Surgical resection of the left atrial sarcoma was undertaken to save the patient's life, followed by chemotherapy and brain irradiation as adjuvant therapy. The prognosis of cardiac sarcoma with metastases is very poor. However, in patients with hemodynamic instability, surgical intervention could be a therapeutic modality as palliation.

  • 심장에서 발생하는 모셀혈관종은 극히 드문 양성 종양이다. 본원에서는 호흡곤란을 주소로 내원한 13세 남자 환자에서,심포음파상 중등도의 혈류 폐쇄를 초래하는 우심실내 종괴를 확인한 후, 우심방을 통해 완전 절제하여 조직검사상 모세 혈관종을 발견하였기에 문헌고찰과 더불어 증례보고하는 바이다.

  • 횡문근종은 영아에서 발생하는 일차 심종양중 가장 흔한 종양이며, 환자의 약 반수에서 결절성 뇌경화증을 동반한다. 이 종양이 대동맥하 협착을 유발할 경우 그 예후가 좋지 않으므로 수술의 적응증이 된다. 본 단국 대학교 흉부외과 교실에서는 결절성 뇌경화증을 동반한 좌심실내 횡문근종 1례를 수술 치험하였기에 보고하는 바이다.

  • 포천 중문 의과 대학 분당 차 병원 흉부외과에서는 정상 흉선 조직과 성숙된 지방 조직이 혼재되어있는 희귀한 양성 종격동 종양인 흉선지방종을 경험하였다. 환자는 46세의 여자 환자로 하복부 통증을 주소로 내과에 입원하였고 입원 당시 촬영한 단순 흉부 방사선 사진에서 우측 심연에 위치한 지방성 종괴가 발견되었다. 흉부 컴퓨터 단층 촬영을 시행한 결과 흉선지방종이 의심되어 종괴를 절제하였다. 조직 병리 검사상 흉선지방종으로 확인되었다.
