Purpose: Minimally invasive surfactant therapy (MIST) is currently used as a method of surfactant replacement therapy (SRT) for the treatment of respiratory distress syndrome (RDS) in preterm infants with a gestational age of less than 30 weeks. However, few studies have been conducted on MIST in neonates with a gestational age of 30 weeks or more. In this study, we compared MIST with endotracheal intubation as a rescue SRT for spontaneously breathing neonates with a gestational age of 30 weeks or more who were diagnosed with RDS. Methods: We investigated the clinical characteristics of spontaneously breathing neonates admitted to the neonatal intensive care unit of the Inje University Sanggye Paik Hospital from January 1, 2014 to December 31, 2016. These neonates were born at a gestational age of 30 weeks or more and were diagnosed with RDS. The neonates who were administered surfactant by MIST were categorized into the MIST group (n=16) and those who underwent endotracheal intubation were categorized into the control group (n=45). Thereafter, the clinical characteristics between the groups were compared. Results: Compared to the control group, the MIST group was less likely to require mechanical ventilation within 72 hours (P<0.001). The frequency of bradycardia during SRT was also low in the MIST group (P=0.033). Conclusion: MIST is considered relatively feasible and safe for treating RDS for reducing the need for mechanical ventilation and decreasing the occurrence of bradycardia during surfactant administration in neonates with a gestational age of 30 weeks or more.
Background: This study was conducted to investigate the epidemiological features, clinical courses, and diagnoses of neonates who are transferred to neonatal intensive care unit of Yeungnam University Hospital due to tachypnea soon after birth. Methods: Based on medical records, we performed a retrospective study of neonatal intensive care unit admissions due to tachypnea from January 2010 to December 2013. Results: A total of 311 neonates were included in this study. The patient characteristics showed male predominance at 2.65:1. Among the 311 neonates with tachypnea, 127 (40.8%) neonates needed oxygen supply, and 54 (17.4%) neonates needed assisted mechanical ventilation. Transient tachypnea of the newborns (TTN) (158, 50.8%) showed the highest incidence, followed by pneumonia (63, 20.3%), extrapulmonary infection (37, 11.9%), respiratory distress syndrome (21, 6.8%), air leak (16, 5.1%), meconium aspiration syndrome (12, 3.9%), congenital heart disease (5, 1.6%), metabolic acidosis (3, 1%), primary pulmonary hypertension of newborns (2, 0.6%) and anemia (2, 0.6%). Conclusion: Although the neonates with tachypnea showed no other respiratory distress symptom, clinicians should be aware of the possibility of other pulmonary diseases as well as TTN and their extra-pulmonary causes. If tachypnea does not improve within a few hours, the clinician should consider further evaluation and management as soon as possible.
Following the first successful trial of surfactant replacement therapy for preterm infants with respiratory distress syndrome (RDS) by Fujiwara in 1980, several animal-derived natural surfactants and synthetic surfactants have been developed. Synthetic surfactants were designed to overcome limitations of natural surfactants such as cost, immune reactions, and infections elicited by animal proteins contained in natural surfactants. However, first-generation synthetic surfactants that are protein-free have failed to prove their superiority over natural surfactants because they lack surfactant protein (SP). Lucinactant, a second-generation synthetic surfactant containing the SP-B analog, was better or at least as effective as the natural surfactant, suggesting that lucinactant could act an alternative to natural surfactants. Lucinactant was approved by the U. S. Food and Drug Administration in March 2012 as the fifth surfactant to treat neonatal RDS. CHF5633, a second-generation synthetic surfactant containing SP-B and SP-C analogs, was effective and safe in a human multicenter cohort study for preterm infants. Many comparative studies of natural surfactants used worldwide have reported different efficacies for different preparations. However, these differences are believed to due to site variations, not actual differences. The more important thing than the composition of the surfactant in improving outcome is the timing and mode of administration of the surfactant. Novel synthetic surfactants containing synthetic phospholipid incorporated with SP-B and SP-C analogs will potentially represent alternatives to natural surfactants in the future, while improvement of treatment modalities with less-invasive or noninvasive methods of surfactant administration will be the most important task to be resolved.
Neonatal mortality rate (NMR) or infant mortality rate (IMR) are the rate of deaths per 1,000 live births at which babies of either less than four weeks or of one year of age die, respectively. The NMR and IMR are commonly accepted as a measure of the general health and well-being of a population. Korea's NMR and IMR fell significantly between 1993 and 2009 from 6.6 and 9.9 to 1.7 and 3.2, respectively. Common causes of infantile death in 2008 had decreased compared with those in 1996 such as other disorders originating in the perinatal period, congenital malformation of the heart, bacterial sepsis of newborns, disorders related to length of gestation and fetal growth, intra-uterine hypoxia, birth asphyxia. However, some other causes are on the increase, such as respiratory distress of newborn, other respiratory conditions originating in the perinatal period, other congenital malformation, diseases of the blood and blood-forming organs and certain disorders involving the immune mechanism. In this study, we provide basic data about changes of NMR and IMR and the causes of neonatal and infantile death from 1983 to 2009 in Korea.
Purpose: The purpose of this study was to identify the current status and needs in neonatal emergency training for nursing students in Korea and to obtain preliminary information to develop a simulation based educational program on neonatal emergency care for nursing students. Methods: Structured questionnaires were distributed to five hospitals and ten nursing schools during April and May, 2012. Data were collected from 59 nurses who worked in the nursery or Neonatal Intensive Care Units and 13 nursing educators who had specialized in pediatric nursing. Results: Most nurses (86.4%) reported that they had experienced an emergency situation with newborns. Most nursing educators (84.6%) claimed that more intensive training with newborns is needed for nursing students. In particular, training in neonatal resuscitation (72.2%), respiratory distress (59.7%), and neonatal seizures (18.1%) were highly recommended as simulation based training for nursing students. Conclusion: A significant need for neonatal emergency educational programs was found. More efforts should be made to provide nursing students with knowledge and skills for working with neonates. The findings of this survey will ultimately provide a basis for developing a simulation based educational program on neonatal emergency care for nursing students.
Chylothorax in the neonatal period is a rare cause of respiratory distress. Surgical ligation of the thoracic duct is rarely necessary in congenital chylothorax. A 3 day-old newborn delivered by the cesarean section showed signs of respiratory distress suddenly and diagnosed as chylothorax on the right hemithorax. Conservative management such as multiple thoracenteses and tube thoracostomy drainage with nutritional support failed to close the leakage. At age of 60 days, we performed a supradiaphragmatic mass ligature of the thoracic duct visualized after injection of methylene blue into the thigh subcutaneously. Postoperatively, chylous effusion occurred in the left hemithorax and successfully treated with chest tube drainage for several days.
자발성 종격동 기흉은 신생아기에 발생하는 호흡곤란의 드문 원인 중의 하나이며 대부분의 증례에서는 인공호흡, 출산손상, 태변흡입 등이 선행하는 경우가 많으며, 또한 대다수가 미숙아로 알려져있다. 저자들은 신생아 진찰 중 심음이 적게 들리는 소견으로 발견되었던, 위와 같은 위험인자가 없는 건강한 만삭아에서 발생한 자발성 종격동 기흉 1예를 경험하였기에 이를 보고하는 바이다.
목 적 : 신생아 호흡 곤란 증후군의 치료에 우리나라에서 널리 사용되고 있는 국내 제제인 Newfactan$^{(R)}$은 Surfacten$^{(R)}$과의 비교연구는 있었지만 다른 제제와의 비교연구는 그 동안 이루어지지 않았다. 이에 본 연구는 유럽에서 널리 사용되고 있는 돼지 폐에서 추출된 modified natural surfactants인 Curosurf$^{(R)}$와 Newfactan$^{(R)}$과의 치료효과를 비교하고자 본 연구를 실시 하게 되었다. 방 법 : 2004년 4월부터 2006년 12월까지 신생아 중환자실에 신생아 호흡 곤란 증후군으로 진단되어 폐 표면 활성제를 투여받았고 출생체중이 2,500 g 이하이면서 재태연령이 35주 이하인 신생아 중 주요 선천성 기형과 태변흡입 증후군이 있었던 경우를 제외한 65명을 대상으로 하였고 이들을 Curosurf$^{(R)}$를 투여 받은 군 31명과 Newfactan $^{(R)}$을 투여 받은 군 34명으로 나누어 두 제제를 사용한 환아들의 임상적 특성과 초기 투여 반응, 합병증을 의무기록을 토대로 분석하여 비교하였다. 결 과 : Curosurf$^{(R)}$ 투여군과 Newfactan$^{(R)}$ 투여군 사이에 출생체중, 재태 기간, 산모의 병력상 특징에서 두 군간에 차이는 없었고, 폐 표면활성제 투여 후의 호흡지표의 변화에서 두 군 모두 투여 2시간 후부터 의미 있는 호흡지표의 개선을 보였으나 두 군간에는 차이가 없었고, 폐 표면활성제를 재투여한 빈도와 입원기간 중에 사망률도 두 군 사이에 차이가 없었다. 장기 치료효과인 인공호흡기 사용기간, 산소 투여기간, 퇴원당시의 나이를 비교해 봤을 때 산소 투여기간이 Newfactan$^{(R)}$ 투여군에서 31.3${\pm}$21.2일, Curosurf$^{(R)}$ 투여군에서 24.6${\pm}$26.2일로 Newfactan$^{(R)}$ 투여군에서 산소 투여기간이 더 길었지만 통계적으로 유의하지는 않았다. 기흉, 폐출혈, 동맥관 개존, 뇌실내 출혈 등의 급성기 합병증은 두 군간에 차이가 없었으며, 기관지 폐 형성이상의 빈도는 Newfactan$^{(R)}$ 투여군이 35.5%, Curosurf$^{(R)}$ 투여군이 26.9%로 Newfactan $^{(R)}$ 투여군에서 약간 더 높은 경향을 나타내었지만 통계적으로 의미있는 차이는 아니었다. 미숙아 망막증, 뇌실주위 연화증, 괴사성 장염등의 장기 합병증의 발생에서도 두 군 사이에 의미있는 차이는 없었다. 결 론 : Newfactan$^{(R)}$ 과 Curosurf$^{(R)}$ 두 제제의 투여 후의 초기 치료반응 및 장기 투여효과, 급만성 합병증의 발생빈도에 있어서 유의한 차이를 보이지 않았다.

Premature babies of less than 37 weeks gestation might require oxygen therapy as an integral part of treatment and respiratory support. Because of their under-developed lungs, these so-called "preemies" might contract respiratory distress syndrome (RDS). To treat RDS, neonatal oxygen therapy is administered, where controlled oxygen gas is measured as a fraction of inspired oxygen ($FiO_2$). However, exposure to high oxygen content during long treatment could cause oxygen intoxication, which might cause permanent blindness due to retinopathy of prematurity (ROP), whereas insufficient oxygen exposure could cause severe hypoxia. A doctor would use oxygen saturation ($SpO_2$) data and prescribe a dose of $FiO_2$ to maintain $SpO_2$ within a suitable range. One objective is to maintain $SpO_2$ within the acceptable range using $FiO_2$ that is as low as possible. Adjustment of $FiO_2$ would normally be done by nurses every 15 to 30 minutes, which might not be safe in many situations. An error in $FiO_2$ adjustment during a manual procedure could be as large as +/- 2.5%. This paper presents a system that can determine an $FiO_2$ value suitable to the current $SpO_2$ and that automatically adjusts $FiO_2$ with an error clearance of +/- 0.25%.
