Incidence of the chlamydia conjunctivitis in the newborn period (신생아에서의 Chlamydia결막염의 빈도에 관한 관찰)

  • After application of silver nitrate on every newborn, the incidence of gonococcal conjunctivitis was markedly decreased. But recently neonatal conjunctivitis due to chlamydia infection is increasing, so clinical observation was made on 26 newborn infants who showed eye discharge from June 1st to August 31st 1989. The results were as follows. 1. The incidence of chlamydia infection among neonatal Conjunctivitis was 34.6%. 2. The most common age at diagnosis was 6-15days of life and there was no sexual prepondrance. 3. There was no significant difference on clinical symptoms between chlamydia conjunctivitis and other Conjunctivitis. 4. Of 26 infants examined. 16 cases revealed no growth on routine bacterial culture. Of the organism cultured, P. aeruginosa was the most common agent(19.2%) and followed by S. aureus(11.5%) and S. epidermidis(7.6%). In one case of chlamydia I conjunctivitis, there was concurrent S. aureus infections. 5. On this study, Giemsa stain did not give significant diagnostic aid of chlamydia conjunctivitis.

Liver Abscess Associated with Maternal Perinatal Infection in a Premature Infant (주산기 모체 감염과 미숙아 간농양 1례)

  • Neonatal liver abscesses are rare, carry a high mortality rate, and are difficult to diagnose. The diagnosis of liver abscesses in the neonate cannot be established from the clinical presentation alone. Risk factors for liver abscesses in neonates are maternal infection, sepsis, umbilical venous catheterization, omphalitis, and necrotizing enterocolitis. In this report, we describe a preterm infant (32 weeks, 1,580 g) who presented with abdominal distension, respiratory difficulties, and a persistent inflammatory response in spite of broad spectrum antibiotic treatment; a large (6${\times}$5 cm) solitary pyogenic liver abscess was identified at 9 days of age. It appeared that the liver abscess had originated in the uterus and umbilical venous catheterization facilitated its spread. Percutaneous drainage under abdominal ultrasound guidance was performed and prolonged antibiotics were treated for 5 weeks, effecting a cure.

An outbreak of neonatal enteritis in buffalo calves associated with astrovirus

  • Background: Enteritis of an infectious origin is a major cause of productivity and economic losses to cattle producers worldwide. Several pathogens are believed to cause or contribute to the development of calf diarrhea. Astroviruses (AstVs) are neglected enteric pathogens in ruminants, but they have recently gained attention because of their possible association with encephalitis in humans and various animal species, including cattle. Objectives: This paper describes a large outbreak of neonatal diarrhea in buffalo calves (Bubalus bubalis), characterized by high mortality, which was associated with an AstV infection. Methods: Following an enteritis outbreak characterized by high morbidity (100%) and mortality (46.2%) in a herd of Mediterranean buffaloes (B. bubalis) in Italy, 16 samples from buffalo calves were tested with the molecular tools for common and uncommon enteric pathogens, including AstV, kobuvirus, and torovirus. Results: The samples tested negative for common enteric viral agents, including Rotavirus A, coronavirus, calicivirus, pestivirus, kobuvirus, and torovirus, while they tested positive for AstV. Overall, 62.5% (10/16) of the samples were positive in a single round reverse transcription polymerase chain reaction (PCR) assay for AstV, and 100% (16/16) were positive when nested PCR was performed. The strains identified in the outbreak showed a clonal origin and shared the closest genetic relationship with bovine AstVs (up to 85% amino acid identity in the capsid). Conclusions: This report indicates that AstVs should be included in a differential diagnosis of infectious diarrhea in buffalo calves.

Caspase-1 Independent Viral Clearance and Adaptive Immunity Against Mucosal Respiratory Syncytial Virus Infection

  • Respiratory syncytial virus (RSV) infection is recognized by the innate immune system through Toll like receptors (TLRs) and retinoic acid inducible gene I. These pathways lead to the activation of type I interferons and resistance to infection. In contrast to TLRs, very few studies have examined the role of NOD-like receptors in viral recognition and induction of adaptive immune responses to RSV. Caspase-1 plays an essential role in the immune response via the maturation of the proinflammatory cytokines IL-$1{\beta}$ and IL-18. However, the role of caspase-1 in RSV infection in vivo is unknown. We demonstrate that RSV infection induces IL-$1{\beta}$ secretion and that caspase-1 deficiency in bone marrow derived dendritic cells leads to defective IL-$1{\beta}$ production, while normal RSV viral clearance and T cell responses are observed in caspase-1 deficient mice following respiratory infection with RSV. The frequencies of IFN-${\gamma}$ producing or RSV specific T cells in lungs from caspase-1 deficient mice are not impaired. In addition, we demonstrate that caspase-1 deficient neonatal or young mice also exhibit normal immune responses. Furthermore, we find that IL-1R deficient mice infected with RSV exhibit normal Th1 and cytotoxic T lymphocytes (CTL) immune responses. Collectively, these results demonstrate that in contrast to TLR pathways, caspase-1 might not play a central role in the induction of Th1 and CTL immune responses to RSV.

The change of route of HIV transmission for the past 11 years in Korea and the projection of the HIV/AIDS cases to the year 2000 (지난 11년간 국내 HIV 감염 전파경로 및 발생추계)

  • Exploring the epidemiological trend of HIV/AIDS is required for making the national AIDS policy. In this study, the trend of HIV/AIDS incidence, rout of transmission and some characteristics of AIDS for the past 11 years in Korea using the reported cases from the national STD screening scheme were reviewed. Based on the results, the trend of main route of transmission according to the year was established by stage and the HIV/AIDS cases in this year was estimated and that to the year 2000 was projected by 'Epimodel' programme. The results were as follows : 1. Of the total infected persons, 76% were in their twenties and thirties, socioeconomically and sexually active age groups. While the transmission by sexual contact overseas was decreasing, the infection through domestic heterosexual and homosexual contact was increasing. 2. In the middle of the 1980's, the infected persons were mainly prostitutes infected through heterosexual contact with the HIV positive foreigner in this county(stage 1). And in the late of the 1980's the main source of infection was the sexual contact overseas and the domestic heterosexual contact(stage 2). Since the early of the 1990's, the infection through the heterosexual contact with non-regular sexual partner in this country has increased rapidly(stage 3), which was the evidence of the possibility of HIV epidemics. After that, it was expected that the infection through the homosexual contacts, the heterosexual contacts with commercial sex workers outside and the non-regular sexual contact inside of this country would increase continuously. In the result, the occurrence of neonatal infection by vertical transmission was expected(stage4). 3. The number of HIV/AIDS was estimated at 572 to 2,313 and the projected number of HIV/AIDS to the you 2000 was around 5,800 including 627 AIDS patients. For the further study on the estimation and projection of HIV/AIDS, it was suggested that the sampling survey on the HIV infection rate in the high risk groups and the sentinel hospital surveillance system should be conducted.

Analysis of Central Line-associated Bloodstream Infection among Infants in the Neonatal Intensive Care Unit: A Single Center Study

  • Purpose: This study aimed to determine the incidence of central line-associated bloodstream infection (CLABSI) in the neonatal intensive care unit (NICU), evaluate the patients' clinical characteristics, and identify the etiologic agents for guidance in prevention and treatment. Methods: A retrospective chart review study of infants classified as having CLABSI was conducted at the NICU of Seoul National University Bundang Hospital from January 2016 to December 2020. Results: Of the 45 infants, 53 had CLABSIs within a follow-up period of 18,622 catheter days. The incidence of CLABSIs was 2.85 per 1,000 catheter days. The most common catheter type was a peripherally inserted central catheter (n=47, 81%). A total of 57 pathogens were isolated, of which 57.9% (n=33) were Gram-positive bacteria, 36.8% (n=21) were Gram-negative bacteria, and 5.3% (n=3) were Candida spp. The most common pathogens were Staphylococcus aureus (n=12, 21%) and coagulase-negative staphylococci (n=12, 21%), followed by Klebsiella aerogenes (n=8, 14%). The median duration of bacteremia was 2 days, and 19 episodes showed bacteremia for 3 days or more. The mortality rate of infants within 14 days of CLABSI was 13.3% (n=6). Conclusions: This study analyzed the incidence of CLABSI and the distribution of pathogens in the NICU. Continuous monitoring of CLABSI based on active surveillance serves as guidance for empiric antibiotic use and also serves as a tool to assess the necessity for implementation of prevention strategies and their impact.

The First Neonatal Case of Panton-Valentine Leukocidin-Positive Staphylococcus aureus Causing Severe Soft Tissue Infection in Korea

  • Staphylococcus aureus (SA) is a common cause of skin and soft tissue infections. Panton-Valentine leukocidin (PVL) toxin-producing strain of SA has been discovered worldwide and is known to cause serious infections. However, reports of neonatal infections caused by PVL-positive SA are rare. Here, we report a case of severe skin and soft tissue infection caused by PVL-positive SA in a 7-day-old neonate. The patient was admitted to the emergency room with a history of fever for one day, tenderness, and sensation of buttocks heating. The infant presented with fever, tachycardia, poor general health, progressive tenderness, and edema of the buttocks on the day of admission. Ultrasonography and magnetic resonance imaging revealed necrotizing fasciitis involving the skin, soft tissue, and muscles. Specimens drained from the buttock lesions confirmed the presence of PVL-positive methicillin-resistant SA (MRSA), and there was no bacteremia. She recovered after one month of intravenous antibiotics and surgical drainages. One month after discharge, she was rehospitalized for otitis externa and was infected with MRSA again. Considering the PVL-positive strain, the patient was treated with intravenous linezolid and dressing. The patient underwent decolonization therapy in a 0.5% chlorhexidine bath and recovered completely without sequelae. This case suggests that aggressive drainage and antibiotic treatment are essential for PVL-producing MRSA infections, and additional decolonization is needed to prevent recurrence and community spread.

A Study about the Relationship between Maternal Attachment and Discharge Against Advice in High Risk Infants (모아 애착과 회복이 어려운 환아의 치료 포기와의 관계)

  • The purpose of this study was to analyze the relationship between maternal attachment and discharge against advice in high risk infants and determine the factors which affect discharge against advice. Data of this study were collected by means of reviewing the medical records of 127 in-patients who were diagnosed as high risk infants in admission and interviewing of the mothers of these patients was done by telephone. The high risk categories were neonatal hyperbilirubinemia, congenital anomaly, congenital heart disease, blood disorder, neonatal infection and birth injury. Maternal attachment was measured by deviding the subjects into 2 groups, the one the continuing treatment group and the other the discharge against advice group. Maternal attachment is determined by an interplay of maternal attitude and specific infant behaviors. Maternal attachment developes through continuous physical and psychological contact between mother and infants. Later it developes into maternal love. The results were as follows: 1. There was a significant association between maternal attachment and discharge against advice, that is, the attachment score was higher in the continuing treatment group. 2. Inspite of controlling medical insurance, severity of disease and the length of stay, it was found that there continued to be either a partially significant or fully significant relationship between maternal attachment and discharge against advice. Stepwise multiple regression revealed that maternal attachment was second in importance as a predictor of discharge against advice, which indicates that maternal attachment was a significant predictor of discharge against advice. 3. Stepwise multiple regression revealed that in 32.3% of these cases the significant predictors of discharge against advice were length of stay, maternal attachment, delivery type, feeding type and income.

Successful Application of Vacuum Assisted Closure in the Case of Neonatal Mediastinitis after Modified Norwood Operation (개선된 노우드 수술 후 신생아에서 발생한 종격동염의 Vacuum Assisted Closure를 이용한 치료)

  • Mediastinitis and sternal wound infection frequently occurred after corrective surgery for complex heart anomalies. Most of the patients are neonate or infant and they have showed low oxygen tension. For that poor condition, application of invasive treatment of mediastinitis is not appropriate and recovery takes a long time. We successfully treated a mediastinitis after Norwood operation using vacuum assisted closure. So we introduce the technique and the modification of neonatal vacuum assisted closure.

Clinical profile of Asian and African strains of Zika virus in immunocompetent mice

  • The mosquito-borne pathogen Zika virus may result in neurological disorders such as Guillain-Barré syndrome and microcephaly. The virus is classified as a member of the Flaviviridae family and its wide spread in multiple continents is a significant threat to public health. So, there is a need to develop animal models to examine the pathogenesis of the disease and to develop vaccines. To examine the clinical profile during Zika virus infection, we infected neonatal and adult wild-type mice (C57BL/6 and Balb/c) and compared the clinical signs of African-lineage strain (MR766) and Asian-lineage strain (PRVABC59, MEX2-81) of Zika virus. Consistent with previous reports, eight-week-old female Balb/c mice infected with these viral strains showed no changes in body weight, survival rate, and neurologic signs, but demonstrated increases in the weights of spleens and hearts. However, one-day-old neonates showed significantly lower survival rate and body weight with the African-lineage strain than the Asian-lineage strain. These results confirmed the pathogenic differences between Zika virus strains. We also evaluated the clinical responses in neonatal and adult mice of different strains. Our findings suggest that these are useful mouse models for characterization of Zika virus for vaccine development.
