Apocrine carcinoma is a rare form of breast malignancy and is composed of entirely or predominantly of apocrine type epithelial cells. Apocrine metaplastic cells are frequently noted in fine needle aspiration cytology(FNAC) of breast lesions, especially fibrocystic disease. These apocrine cells may occasionally be atypical, to make a diagnostic difficulty. Two cases of apocrine carcinoma of the breast diagnosed by FNAC are described, and differential cytologic points between apocrine metaplasia and apocrine carcinoma are discussed. The first case is a right breast mass of a 37-year-old woman for 20 days. The smears show many single or sheets of large cells on bloody background. Each cell has a large vesicular nuclei with multiple macronucleoli and abundant eosinophilic granular cytoplasm. The second case is a left breast mass of a 35-year-old woman for one month. The smears show similar findings as seen in the first one. Histopathologic findings of both cases are typical of apocrine carcinoma. Electron microscopy demonstrates variable numbers of large osmiophilic granules in diameter of $200\sim600nm$ in both cases.
It is well known that fine needle aspiration biopsy(FNAB) is very useful and has a high accuracy rate in the diagnosis of renal neoplasms. Although there is some indecision to perform the FNAB for a rare possibility of tumor seeding along the biopsy needle tract, it tends to be used increasingly. As in the cytologic diagnosis of metastatic lesion through-out the body, renal cell carcinoma should nearly always be considered in the differential diagnosis, the precise understanding of cytologic features of renal cell carcinoma with various cell types and architectural patterns is necessarily required. In this report, we present three cases of primary renal cell tumors, two of renal cell carcinomas and one of oncocytoma, preponderantly emphasizing the cytologic differential points in the FNAB specimen.
Intracystic papillary carcinoma(IPC) of the breast is a distinctive and very rare variant of intrductal carcinoma. The cytologic features of IPC have been rarely reported, and there are difficulties in distinguishing between benign and malignant papillary breast lesions. Herein we report a IPC of the breast in a 80 year-old female. Fine needle aspiration cytology revealed monotonous cuboidal epithelial cells in small clusters and individually scattered on bloody background. The tumor cells did not show overt cytologic atypia. With the histologic features of this case and review of the literature, the cytologic differential points are discussed.
The production of nonwoven fabrics from natural fibers is already expanding at an industrial level for simple curvature semi-structural part in the automotive industry. To develop their use for technical applications, this paper provides an experimental study of the mechanical behavior of flax-fiber nonwoven preforms. A comparison between different sets of carded needle-punched nonwoven has been used to study the influence of manufacturing parameters such as fibers' directions, the area and the needle punching densities. We have found that the anisotropy observed between both directions can be reduced depending on these parameters. Furthermore, this work investigates the possibility to form double curvature parts such as a hemisphere as well as a more complex shape such as a square box which possesses four triple curvature points. We propose a forming process adapted to the features of the nonwoven structure. The purpose is to determine their behavior under high stress during various forming settings. The preforming tests allowed us to observe in real time the manufacturing defects as well as the high deformability potential of flax nonwoven.
Substances $I{\sim}VII$ are isolated from the root of Glehnia littoralis which has been often used as a subsitute of Bang-Poong in Korea. White needle crystalline substance of $C_{29}H_{50}O$, m.p. $137{\sim}139^{\circ}C$ was confirmed as ${\beta}-sitosterol$ through elemental analysis, derivative synthesis, IR and physicochemical tests. Moreover, it was concluded that white needle crystal of $C_{12}H_{8}O_{4}$, m.p. $188{\sim}189^{\circ}$ was confirmed as bergapten, through a contrast test with standard substance and by UV and IR-spectra. It was found that substance III and VII coincided with the Rf values of imperatorin and umbelliferone and to its colors under the UV-ray, and its similarity to UV-spectrum can be recognized, but their melting points are remarkedly different respectively.
Cytologic features of the fine needle aspiration of metastatic epithelioid sarcoma are presented. The patient was a 48 year-old male who had been sufferring from recurrent and metastatic tumors of the right upper extremity for 6 years. The aspirate from the axillary mass cytologically consisted of isolated large polygonal cells with abundant, cyanophilic, slightly granular cytoplasm. Eccentrically located large round nuclei showed finely dispersed chromatin, small nucleoli, occasional convolution and folding of nuclear membrane, and frequent binucleation. Although both histologic and cytologic similarity of epithelioid sarcoma to squamous cell carcinoma is well known, findings of cyanophilic granular cytoplasm, convoluted nuclei, and binucleation appeared to be the differential points.
Objectives : Hypertension is very prevalent disease, and causes serious cardiovascular complications. Nowadays optimal hypertension treatment is emphasized to reduce the incidence of cardiovascular complications. Auricular acupuncture therapy is economical, safe and effective. Its clinical application is various, and it can be used to control blood pressure. So The effect of auricular acupuncture therapy was investigated. Daily variation of blood pressure during needle-embedded period and variation of blood pressure by blood pressure classification were observed. Methods : The auricular acupuncture points we used were Gangapjeom (降壓點), Sinmun (神門), Gyogam (交感), Sim(心), and Icheom (耳尖). Auricular acupuncture needles were embedded for three days. Blood pressure was checked four times per day and the mean obtained. Results : The following results were obtained : 1. During needle-embedded period, systolic and diastolic blood pressure decreased. Blood pressure decreased most significantly 2 days after treatment. 2. The change of blood pressure in the higher blood pressure group was more remarkable than that of the lower blood pressure group. Conclusions : Through this research, auricular acupuncture therapy is considered as an effective and safe method to lower blood pressure.
Although fine needle aspiration cytology (FNAC) has become one of the primary tools for diagnosing salivary gland lesions, some of these methods continue to confuse pathologists. The most common problems occur so-called basaloid neoplasms. Basal cell adenomas are frequently misdiagnosed as pleomorphic adenomas, and in worse cases, as adenoid cystic carcinomas. The cytologic diagnostic accuracy of basaloid neoplasms could be increased by a better understanding of the histology and the nature of the tumor cells. These are displayed well in aspiration smears. A consideration of differential points on the basis of the epithelial-stromal relationship is offered in this paper.
Objectives : To review the needling depth, therapeutic and adverse effects of intraperitoneal acupuncture and related acupoints. Methods : Studies and reports from the literature, Korean medical health insurance data and databases were searched and selected. Results : The depth of needle insertion for acupoints related to intraperitoneal acupuncture has been documented to range from 0.9 to 10.5 cm depending on the acupoints and literature. In studies of safe needling depth, the justice of safe needling depth varied from study to study, but was generally based on the peritoneum, and the depth varies by patient age, sex, body type, and weight. Clinical studies in China have shown good efficacy and no side effects of abdominal long needle acupuncture, but there have been case reports of adverse events following abdominal acupuncture. Conclusions : To ensure both therapeutic effectiveness and safety of intraperitoneal acupuncture, It is necessary to utilize the ultrasound in clinical practice and conduct related research.
Objectives This study aims to analyze a thermal distribution in biological living tissue during warm needling therapy by using a finite element method. The analysis provides an understanding of warm needling's efficacy and safety. Methods A model which consisted of four-layered tissue and stainless steel needle was adopted to analyze the thermal distribution in living tissue with a bioheat transfer analysis. The governing equation for the analysis was a Pennes' bioheat equation. A heat source characteristic of warm needling therapy was obtained by previous experimental measurements. The first analysis of the time-dependent temperature distribution was conducted through points on a boundary between the needle and the tissue. The second analysis was conducted to visualize the horizontal temperature distribution. Results When heat source's peak temperatures was above $500^{\circ}C$ and temperature rising rates were relatively slow, the peak temperature at skin surface exceeded a threshold of pain and tissue damage ($45^{\circ}C$), whereas when the peak temperature was around $400^{\circ}C$, the peak temperature at the skin surface was within a safe limit. In addition, the conduction of combustion energy from the moxa was limited to the skin layer around the needle. Conclusions The results suggest that the skin layer around the needle can be heated effectively by warm needling therapy, but it appears to have little effect at the deeper tissue. These findings enhance our understanding of the efficacy and the safety of the warm needling therapy.
