중립 선 자세에서의 전신진동 주파수에 따른 자세 안정근의 근활성 분석 (Analysis of Muscle Activation related to Postural Stability according to Different Frequency of Whole Body Vibration during Quiet Standing)

  • Purpose: The aimed of this study was to investigate muscle activation related to postural stability according to different frequency of whole body vibration during quiet standing, to identify the most effective training conditions that cause the highest neuromuscular responses, and to evaluate the difference of EMG activation according to the anatomical position of the muscle - proximal or distal from the vibration platform. Methods: Eighteen healthy subjects voluntarily participated in this single-group, repeated-measures study in which EMG data from upper trapezius, rectus abdominalis, external oblique abdominalis, elector spinae, gluteus maximus, rectus femoris, semitendinosus, and gastrocnemius were collected over different frequencies (0-5-10-15-20-25Hz) for each subject during quiet standing. Results: We observed a statistically significant difference in the mean values of %RVC of muscular activation according to different frequencies of whole body vibration during quiet standing in all muscles (p<0.05). Conclusion: Our results indicate that lower frequencies of vibration result in low muscular activation, and higher frequencies elicit high muscular activation. However, the most effective training condition that caused the highest activation was 20 Hz. In addition, the proximally located lower extremity muscles (GCM, RF, ST, GM) showed higher activation than the distally located trunk and neck muscles (ES, EO, RA, UT) together with increasing frequency.

온도, pH 및 빛에 대한 Domoic Acid의 안정성 (Stability of Domoic Acid at Different Temperature, pH and Light)

  • To prevent the food poisoning originated by consumption of shellfish contaminated with domoic acid, the quantitative analysis of domoic acid is to be very important. The stability of domoic acid at different temperature, pH and light was investigated using high performance liquid chromatography (HPLC). The mean recoveries of domoic acid in the methanol extracts from oyster (Crassostrea gigas), blue mussel (Mytilus edulis), short neck clam (Ruditapes philippinarum) and ark shell (Scapharca broughtonii) were 85.4-104.5%, 94.8-101.2%, 91.0-104.6%, and 95.7-109.6%, respectively. The working solutions of domoic acid standard were very stable for one month at $-18^{\circ}C$, $4^{\circ}C$, and room temperature. And domoic acid in the methanol extract from oyster was stable for a day at $4^{\circ}C$ and room temperature, and for a week at $-18^{\circ}C$. Therefore, this implies that quantitative analysis for domoic acid must consider the storage conditions of the standard solutions and the methanol extracts from shellfish. The standard solutions adjusted to pH 3-9 were also stable after heating at $121^{\circ}C$ for 30 min. The effect of light exposure on domoic acid was tested by exposing the methanol extracts to light. Domoic acid degraded slowly when the samples were kept in the dark (brown vial). However, following the light exposure the photodegradation became more rapid; no detectable domoic acid remained in $1.0{\mu}g/mL$ of methanol extract after 5 hours.

  • Quantifying dynamic stability is important to assessment of falling risk or functional recovery for leg injured people. Human locomotion is complex and known to exhibit nonlinear dynamical behaviors. The purpose of this study is to quantify major joints of the body using chaos analysis during walking. Time series of the chaotic signals show how gait patterns change over time. The gait experiments were carried out for ten young males walking on a motorized treadmill. Joint motions were captured using eight video cameras, and then three dimensional kinematics of the neck and the upper and lower extremities were computed by KWON 3D motion analysis software. The correlation dimension and the largest Lyapunov exponent were calculated from the time series to quantify stabilities of the joints. This study presents a data set of nonlinear dynamic characteristics for eleven joints engaged in normal level walking.

  • During orthodontic treatment of impacted teeth, use of appropriate anchorage against the traction force is important. Tooth anchorage with multi-bracket appliances is commonly used but sometimes it causes unwanted movements of adjacent teeth. Skeletal anchorage devices are therefore considered to minimize such side effects. Still their survival rate and positioning are highly limited according to the bone density and the interradicular space. This case report presents a case of two impacted teeth, one of which is dilacerated and horizontally angulated. Using the microplate with short screws and a bendable neck, negative effects on adjacent teeth were minimized and impacted teeth were repositioned with good stability.

  • In this study, a stress analysis is made fur an ankle foot orthosis (AFO) in the view point of structural stability. The investigated AFO is a solid and standard type. To analyze the stress distribution on the neck of AFO, the maximum AFO angular translation data is obtained by gait analysis. The material test of polypropylene is made to obtain the mechanical properties of AFO. The maximum dorsiflexion appears at the midstance in the gait analysis. The experimental angular translation at the top of AFO is about 10.3$^{\circ}$ at mid stance. Three models of AFO with different width of neck are made and analyzed with ABAQUS 6.1. The stress levels and distributions of 3 different width(W$_1$, W$_2$=0.85W$_1$, W$_3$=0.60W$_1$) AFO are investigated. As a result, the standard type(W$_1$) appears to the maximum stresses at the medial edge of cutout area surrounding ankle joint. The maximum stresses of the narrower type(W$_2$) are occured on medial edge and center of ankle. The narrowest type(W$_3$) appears to the maximum stresses at center of ankle. The maximum stresses become smaller as ankle width of AFO is narrower.

이중미세나사산 임플란트의 임상적 및 방사선학적 1년 평가 (Clinical and radiographic evaluation of implants with dual-microthread: 1-year study)

  • Purpose: The stability of periodontal condition and marginal bone level were important to achieve long-term success of dental implant treatment. The aim of this study was to evaluate periodontal conditions and marginal bone loss around 67 GSII(OSSTEM, Seoul, Korea) dental implants with dual-microthread at the neck portion, 1 year after prosthetic loading. Materials and methods: Sixty-seven GS II dental implants in 27 patients(mean age; $47.4{\pm}14.0$ years) who received implant treatments at Pusan National University Hospital, were included in this study. Thirteen US II(OSSTEM, Seoul, Korea) implants with smooth neck design were selected for the control group. Periodontal and radiographic evaluations were carried out at baseline, 6 months and 12 months after prosthetic loading. Results: In the GS II group, plaque index(PI), gingival index(GI) and probing depth(PD) increased as time passed. In the US II group, GI and PD increased. Although marginal bone level was lower in the US II group in all evaluation periods, the changes between the periods were not statistically significant(p>0.05). In each period, periodontal parameters were not statistically significant between groups. Conclusion: One year after prosthetic loading, GS II and US II dental implants showed similar periodontal conditions and marginal bone response, and were within the criteria of success.

Internal Oblique and Transversus Abdominis Muscle Fatigue Induced by Slumped Sitting Posture after 1 Hour of Sitting in Office Workers

  • Background: Prolonged sitting leads to low back discomfort and lumbopelvic muscle fatigue. This study examined the characteristics of body perceived discomfort and trunk muscle fatigue during 1 hour of sitting in three postures in office workers. Methods: Thirty workers sat for 1 hour in one of three sitting postures (i.e., upright, slumped, and forward leaning postures). Body discomfort was assessed using the Body Perceived Discomfort scale at the beginning and after 1 hour of sitting. Electromyographic (EMG) signals were recorded from superficial lumbar multifidus, iliocostalis lumborum pars thoracis, internal oblique (IO)/transversus abdominis (TrA), and rectus abdominis muscles during 1 hour of sitting. The median frequency (MDF) of the EMG power spectrum was calculated. Results: Regardless of the sitting posture, the Body Perceived Discomfort scores in the neck, shoulder, upper back, low back, and buttock significantly increased after 1 hour of sitting compared with baseline values ($t_{(9)}=-11.97$ to -2.69, p < 0.05). The MDF value of the EMG signal of rectus abdominis, iliocostalis lumborum pars thoracis, and multifidus muscles was unchanged over time in all three sitting postures. Only the right and left IO/TrA in the slumped sitting posture was significantly associated with decreased MDF over time (p = 0.019 to 0.041). Conclusion: Prolonged sitting led to increased body discomfort in the neck, shoulder, upper back, low back, and buttock. No sign of trunk muscle fatigue was detected over 1 hour of sitting in the upright and forward leaning postures. Prolonged slumped sitting may relate to IO/TrA muscle fatigue, which may compromise the stability of the spine, making it susceptible to injury.

비탄력성 테이프와 탄력성 테이프의 적용이 앞쪽머리자세 환자의 통증과 머리척추각 및 균형능력에 미치는 영향 (The Effect of Non-elastic and Elastic Tapes on the Pain, Craniovertebral Angle, and Balance of Patients with a Forward Head Posture)

  • PURPOSE: This study was conducted to investigate the effects of non-elastic and elastic tapes on pain, craniovertebral angle, and balance in forward head posture (FHP) patients. METHODS: A total of 44 adults with FHP were randomly assigned to a group that performed a stretching exercise after non-elastic taping (n = 22) and another group that performed the stretching exercise after elastic taping (n = 22), respectively. The stretching exercise was performed five times a week for 30 minutes per session. The visual analogue scale was used to compare neck pain, the craniovertebral angle was measured to compare alignment, and the limit of stability was measured to compare balance. RESULTS: The groups that performed the stretching exercise after both elastic and non-elastic taping showed significant positive changes in pain and the craniovertebral angle of the head in pre- post measurements (p < .05). In the follow-up test for the pain and craniovertebral angle, there was no significant difference from the post-test in the non-elastic group (p > .05), but a significant difference was seen in the elastic group (p < .05). CONCLUSION: Stretching exercises with taping for patients with a FHP are more effective in improving pain and alignment. However, the short-duration tape application did not affect the balancing ability. When the non-elastic tape was used, the effect lasted longer than that of the elastic tape, and pain relief was effective in the case of the elastic tape. Therefore, tape therapy would be more effective if customized according to the patient's condition.

Long-term consistency of clinical sensory testing measures for pain assessment

  • Background: Understanding the stability of quantitative sensory tests (QSTs) over time is important to aid clinicians in selecting a battery of tests for assessing and monitoring patients. This study evaluated the short- and long-term reliability of selected QSTs. Methods: Twenty healthy women participated in three experimental sessions: Baseline, 2 weeks, and 6 months. Measurements included pressure pain thresholds (PPT) in the neck, upper back, and leg; Pressure-cuff pain tolerance around the upper-arm; conditioned pain modulation during a pressure-cuff stimulus; and referred pain following a suprathreshold pressure stimulation. Intraclass correlation coefficients (ICC) and minimum detectable change (MDC) were calculated. Results: Reliability for PPT was excellent for all sites at 2 weeks (ICC, 0.96-0.99; MDC, 22-55 kPa) and from good to excellent at 6 months (ICC, 0.88-0.95; MDC, 47-91 kPa). ICC for pressure-cuff pain tolerance indicated excellent reliability at both times (0.91-0.97). For conditioned pain modulation, reliability was moderate for all sites at 2 weeks (ICC, 0.57-0.74; MDC, 24%-35%), while it was moderate at the neck (ICC, 0.54; MDC, 27%) and poor at the upper back and leg at 6 months. ICC for referred pain areas was excellent at 2 weeks (0.90) and good at 6 months (0.86). Conclusions: PPT, pressure pain tolerance, and pressure-induced referred pain should be considered reliable procedures to assess the pain-sensory profile over time. In contrast, conditioned pain modulation was shown to be unstable. Future studies prospectively analyzing the pain-sensory profile will be able to better calculate appropriate sample sizes.

성대마비와 양성 성대점막질환의 음향학적 특성비교 (Comparative Study on Acoustic Characteristics of Vocal Fold Paralysis and Benign Mucosal Disorders of Vocal Fold)

  • This study aims to analyze the voices of the patients with voice disorders including vocal fold paralysis, vocal fold cyst and vocal nodule/polyp in the aspect of acoustic phonetics. This study intends to collect subsidiary acoustic data in order to make a speech treatment and an standardization of vocal disorders. Subjects and Methods: The subjects of this study were 64 adult patients who underwent indirect laryngoscopy and laryngostroboscopy, and were diagnosed as vocal fold paralysis, vocal fold cyst or vocal nodule/polyp. Experimental group consisted of 20 patients who were diagnosed as vocal fold paralysis, 21 patients who were diagnosed as vocal fold cyst and had the average age of 42.0 $({\pm}10.03)$ ; and 23 patients who were diagnosed as vocal nodule/polyp and had the average age of 40.9 $({\pm}13.75)$. For the methodology of this study, the patients listed above were asked to sit in a comfortable position at intervals of 10cm apart from the patient's mouth and a microphone, and subsequently to phonate a vowel sound /e/ for the maximum phonation time with natural tone and vocal volume then the sound was directly inputted on a computer. During recording, sampling rate was set to 44,100Hz and the 1-second area corresponding to stable zone except the first and the last stage of waveform of the vowel sound /e/ vocalized by the individual patients was analyzed. Results: First, there was no statistically significant difference in jitter and shimmer between vocal fold paralysis and vocal fold cyst, while there was highly statistically significant difference in them between vocal fold paralysis and vocal nodule/polyp. Second, looking into the mean values obtained from NNE, HNR and SNR results associated with noise ratio, the disease showing the most abnormal characteristics was vocal fold paralysis, followed by cyst and nodule/polyp in order. For NNE, there was statistically significant difference between vocal nodule/polyp, and cyst or paralysis. In other words, it was found that the NNE of vocal nodule/polyp was weaker than that of cyst or paralysis. Similarly, HNR and SNR also showed the same characteristics; there was statistically significant difference between vocal fold paralysis and vocal fold cyst or nodule/polyp, and HNR and SNR values of vocal fold paralysis were lower than those of vocal fold cyst or nodule/polyp. Conclusion: For vocal fold paralysis, the abnormal values of acoustic parameters associated with frequency, amplitude and noise ratio were statistically significantly higher than those of vocal fold cyst and nodule/polyp. This finding suggests that the voices of the patients with vocal fold paralysis are the most severely injured due to less stability of vocal fold movement, asymmetry and incomplete glottic closure. In addition, there was no statistically significant difference in the acoustic parameters of tremor among vocal fold paralysis, vocal fold cyst and vocal nodule/polyp. Further studies need to ascertain reasonable acoustic parameters with various vocal disorders as well as to clarify the correlation between acoustics-based objective tools and subjective evaluations.
