  • Objective: Flexed posture commonly increases with age in older adults and is characterized by kyphosis and forward head posture. Changes in the posture with age affect both balance and mobility. This study was conducted to examine the effects of a cervical stabilization exercise for community-dwelling older adults to improve balance. Design: Two groups pretest-posttest design. Methods: Fifty older adults were randomly assigned into the cervical stabilization exercise group (n=24) and control group (n=25). The cervical stabilization exercise group (n=24) participated in group exercise for 60 minutes twice a week over 4 weeks. Timed up and go test (TUG), four square step test (FSST), functional reach test (FRT), postural sway, cervical range of motion (CROM), proprioception, craniovertebral angle (CVA) were evaluated before and after the intervention. Results: TUG, FSST, FRT, CROM, Proprioception, CVA showed significantly greater improvement, compared with a control group (p<0.05). Conclusions: Findings of this study demonstrate that cervical stabilization exercise can help improve not only neck functional capacities but also balance. Therefore, it may be used as an effective balance exercise program for community-dwelling older adults.

  • Purpose: To present the procedure and results of dorsal wedge osteotomy fixated by bioabsorbable polyglycolide pins for the treatment of symptomatic Freiberg's disease. Materials and Methods: From January 1997 to December 2002, six patients with Freiberg's disease underwent dorsal wedge osteotomy of the metatarsal neck to bring the healthy plantar part of the metatarsal head into articulation. Bioabsorbable polyglycolide pins were used for the fixation and short-leg walking cast was applied for 4 weeks. Results: All patients returned to full daily activities without pain in three months after the operation. Radiographically, solid healing of the osteotomy was observed at average ten weeks. The active range of motion of the metatarsophalangeal joint increased by a mean gain of 30 degrees, and no complication such as displacement, osteolysis or sinus formation was observed. Conclusion: Dorsal wedge osteotomy fixated by bioabsorbable pins for patients with symptomatic Freiberg's disease is effective procedure that provides relatively early range of motion exercise and avoids second procedure for implant removal.

  • PURPOSE: This study examined the effects of posture improvement exercise using virtual reality programs on the posture and balance of patients with forward head postures. METHODS: Thirty men and women in their 20 s, who had a forward head posture, were divided randomly into a group with posture correction exercise and a group with posture correction exercise combined with virtual reality programs. The posture correction exercise was composed of squats, XCO training, and chin-tuck exercise. In contrast, exercise with virtual reality games involved the Hot Squat, Climbey, and Baskhead programs while wearing a headset. Both groups performed the exercises 15 min a day, three times per week, for four weeks. The balance ability, distance between the acromion and earlobe, and neck joint range of motion were assessed before and after the exercises. RESULTS: Both groups showed significant reductions in the distance between the acromion and the earlobe, along with significant improvements in the range of joint motion. The group that performed the virtual reality exercises showed a significant increase in the limit of stability. Both groups showed a significant decrease in the sway length. In contrast, the group given the virtual reality exercises showed a significant reduction in the sway speed while standing with their eyes closed. CONCLUSION: Exercise applying virtual reality programs can be used in clinical and home programs to correct the postures of individuals with a forward head posture because they can trigger interest in inducing active participation.

  • Purpose: This study was done to assess the effect of changes in forward head posture (FHP), neck mobility and headache clinical parameters on episodic tension-type headache (ETTH) and chronic tension-type headache (CTTH) who did craniocervial exercises. Methods: Twelve CTTH subjects and twelve ETTH subjects were studied. Side-view pictures of subjects were taken in both sitting and standing positions, in order to assess FHP by measuring the craniovertebral angle. Cervical range of motion (CROM) was employed to measure cervical mobility. A headache diary was kept to assess headache intensity, frequency, and duration. All subjects did three types of craniocervical exercise over 8 weeks. Measurements were done at pre-treatment, and at 4 and 8 weeks post-treatment. Results: Forward head posture and headache-related clinical parameters showed a significant improvement after craniocervical exercise (p<0.05). Flexion/extension and left/right rotation of CROM was significantly increased after the intervention (p<0.05), whereas changes in left/right bending did not reach statistical significance. Conclusion: This study indicates that craniocervical exercise may be effective in the management of tension-type headache.

  • This study aims to provide joint angles of comfort for females, based on the psychophysical scaling method. Ten female subjects participated in the experiment for measuring perceived discomfort for varying joint motions. The subjects were instructed to maintain given joint motions for a minute, and to rate their perceived discomfort for the motions during a minute's rest by using the free modulus method of the magnitude estimation. Joint angles of comfort were calculated from the regression equations based on the experimental results, in which levels of joint motions were used as independent variables and perceived discomforts as dependent variables. The results showed that joint angles of comfort for the joint motions investigated were much smaller than full range of motions for corresponding joint motions. The ratios of joint angle of comfort to its range motion for the hip were found to be smallest of all joint motions dealt with in this study, and those for the neck were the largest. In addition, comfortable joint angles for females were much smaller than those for males. It is recommended that when designing or evaluating workplaces ergonomically, different comfortable joint angles should be applied according to workers' or population's gender.

  • Many reaearchers have reproted that poor body postures are associated with pains or symptoms of musculoskeletal dissoders. Therefore, the ergonomic evaluation of postural stresses as well as biomechanical stresses is important when a job such as automobile assembly tasks involves highly repetitive and/or prolonged poor body postures. A macropostural classification shema was developed to characterise various body postures occurring in automobile assembly tasks in the study. To specify a postural code and stress level to each body posture, perceived joint discomforts were subjectively evaluated in the lab experiments for the full range of motion in five human body joints. Based on the reaults, a postural classification scheme was developed where the full range of motion in each body joint was classified into several codes repressenting different stress levels. The automobile tasks were clustered into 12 types based on the result walk-in-surveillance and the possible posture codes for each task type are defined. I was exposed that the poor postural problems in automobile assembly tasks were concerned in most part with arms, trunk and neck. Application of te developed schema to seven operations in automobile assembly tasks showed that the schema can be used as a tool to identify the operations and tasks involving highly stressful body postures. The schema can also be utilised as a basis to prioritise the candidate assembly operations for redesign of work methods.

  • Purpose: This study was conducted to investigate changes in the cervical and lumbar flexion-relaxation ratio, range of motion, pressure pain threshold, and perceived comfort following the wearing of a trunk brace during smartphone watching. Methods: To calculate the number of subjects for this study, an analysis with G*Power was performed at a statistical power of 0.8, an effect size of 0.5, and a significance level of 0.05, based on the results of a preliminary experiment on five subjects. In total, 27 adult men and women were recruited who had been informed of the study's purpose and process and had agreed to participate. All subjects watched content on a smartphone for 20 minutes in the same posture and conditions while wearing and not wearing a trunk brace, and then their cervical and lumbar flexion-relaxation ratio, range of motion, pressure pain threshold, and perceived comfort were measured. Results: Compared to the non-wearing of a trunk brace, the wearing of a trunk brace resulted in a statistically significant smaller decline in cervical extension and right-side cervical rotation (p<0.05). When the subjects wore a trunk brace, their right- and left-side cervical and right-side lumbar pressure pain statistically significantly improved when compared to not wearing a trunk brace (p<0.05). They also perceived a significantly lowered level of comfort 20 minutes after wearing a trunk brace compared to immediately after wearing it (p<0.05). Conclusion: The trunk brace was effective in reducing declines in right-side cervical rotation and the occurrence of left- and right-side cervical and right-side lumbar pressure pain. The findings indicate the need to improve the perceived comfort of trunk braces.
