Biological behavior and Treatment of Adenoid Cystic Carcinoma in The Bead and Neck (두경부 선낭암의 생물학적 특성과 치료)

  • Biological behavior and treatment results of 33 patients with Adenoid Cystic Carcinoma (ACC) in the Head and Neck at Yonsei Cancer Confer for 10 years between 1971 and 1980 were retrospectively analysed. Most common, primary site was minor salivary glands such as maxillary sinus, nasal cavity and base of tongue. The typical biological behavior of these tumors was very slowly in growth with long time of duration(mean 19 months) from 1 month to 10 years and more frequent of nerve invasion but rare invasion of neck nodes. Local control and failure pattern in the results of treatment, 16 of 17 patients with irradiation alone were seen complete or partial response but 5 cases of locoregional recurrence, 2 cases of failure of neck node and 4 cases of distant metastasis as lung and brain. On the other hand, among 10 cases of surgery and postoperative irradiation, 2 cases of locoregional failure and 3 cases of distant metastasis as lung and bone. 2 of 4 cases with surgery alone were recurred within primary site. Actuarial overall NED survival at 3 ana 10 years were $52.6\%$ and $42.8\%$, respectively. Survival rate of 10 Patients with surgery and Postoperative irradiation was more high than 17 Patients of radiation alone. Therefore, we have known that surgery with postoperative adjunctive irradiation is most effective treatment modality of adenoid cystic carcinoma in the head and neck. Primary site, treatment modality and with or without nerve ana bone invasion have influenced on prognosis.

  • Branchial cleft cyst is the most common lateral neck cyst; the vast majority are of the second branchial cleft origin. This presumably reflects the greater depth and longer persistence of the second cleft, compared with the first, third, and fourth clefts. We experienced a 49-year-old male whose chief complaint was a abnormal mass of the left parotid gland area and neck. As a result of careful analysis of clinical, radiological, and histopathological findings, we diagnosed it as a second branchial cleft cyst in the neck and obtained results as follows: 1. In clinical examination, there was a 10×15㎝ sized, fluctuant painful mass in the left neck and parotid area. 2. In radiographic examination, a low echogenic mass with internal cystic change in the inferior parotid gland area was noted sonographically. Computed tomograph showed a 3×4㎝ sized, well-defined cystic mass with heterogenous solid component in the anterior border of sternocleidomastoid muscle. MRI revealed 5×6㎝ sized, well-marginated multi separated mass in the same area. 3. In histopathological examination, lining of cyst was stratified squamous epithelium with typical lymph node pattern and inflammatory cell infiltration.

Development of Jacket Bodice Pattern for Elderly Women Using 3D Pattern Design System (3D 패턴 디자인 시스템에 의한 노년 여성의 재킷바디스패턴 연구)

  • This study was conducted to suggest an appropriate jacket bodice pattern for elderly women by comparing the developed pattern with existing ready-made jacket pattern. The developed jacket bodice pattern was produced from the basic sloper in which elderly women's somatotype was fully reflected, by using the newly developed 3D pattern design system. Subjects were 6 elderly women aged from 60 to 69 who have average somatotype in their age group. This study carried out pattern analysis and self-sensory test with three ready-made jackets(national brands) and suggested a developed jacket pattern based on the results. According to the self sensory test results of ready-made jacket, wearing fitness among brands was significantly different even though those brands were same B91 size. The results of ready-made jackets' pattern analysis, size differences among patterns have influenced wearing fitness. The basic numerical formula for the jacket pattern considered elderly women's somatotype were as follows; $B/4+2{\pm}0.2{\sim}0.5$(difference of front and back), $W/4+2{\pm}0.2{\sim}0.5$(difference of front and back), H/4+2~7(ease amount), armhole depth B/4, chest width B/6+1, back width B/6+3 and back neck width B/12+0.5. According to the results of the developed jacket's appearance evaluation, it earned higher scores in silhouette and ease amount, confirming that the developed jacket is appropriate for the elderly women. Also, in the evaluation on space length, the developed jacket pattern was evaluated to allow proper space length in all around of measurement parts.

A Study on the Change of Waist Pattern by Upper Limb Motion -By the Method of Tight Fitting Technique- (상지동작에 따른 길의 변화에 관한 연구 -입체재단법을 중심으로-)

  • In this study, the pattern was copied by the method of Tight Fitting Techinque, which resulted from the changed body by the upper limb motion-front-vertical motion(or vertical motion in front), side-vertical motion, and horizontal motion. And, this study analyzed the change of the pattern and the observed items dimension changed to the pattern. The results are as follows: 1. In the observation of the degree of the pattern change according to the motion of upper limb, the result provides that the motion change in the range of $135^{\circ}$ to $180^{\circ}$ is the largest in front-vertical motion, $45^{circ}~90^{\circ}$ in side-vertical motion, and $0^{circ}~45^{\circ}$ in horizontal motion respectively. 2. The probability test result of the items of the motion is more related with the horizontal width item rather than the vertical length item in the front and back pattern where the back pattern has more effect than the front pattern. And the upper limb-surrounding items are more related than any otheer item. 3. The change of the pattern according to the motion shows the decrese of the neck width and the shoulder legth, the rising of the point of shoulder (or shoulder point) and armpit point, the decrease of the pattern width and the increase of the pattern length. As the angle of the motion grows vertically motion. The change of the shoulder length in the horizontal motion is smaller than that vertical. But as the angle of the motion grows horizontally, it has a tendency of decreas in th width of the front patten and the length of the pattern, whereas the width of the back pattern is noticeably increases.

Unusual Bilateral Impalement Injury with Rusted Iron Bars on Face and Neck

  • Impalement injury is the subset of penetrating trauma, defined as fixed, elongated objects penetrate and remain in the human body cavity or region by relatively low velocity. We report an unusual case of facial and neck impalement where two dirty rusted iron bars penetrated forehead bilaterally and exited neck and ear respectively without causing major organ injuries. After thorough radiologic and physical evaluation, the patient got medical and surgical treatment. The patient was discharged without complication after four day of delayed wound closure. There have been no complications and sequelaes related with trauma, wound infection and scar contracture at 3-year follow-up. According to affected organs and pattern of impalement, individualized and multidisciplinary surgical approach should be considered. Following these guidelines as in this case, it was possible to achieve excellent clinical outcome in impalement injury.

  • This study is aimed for men's jacket draft not only to other style but also helping for men's jacket fits for trend by presenting the course to get a style we want by adding to the partial drafty changes in existing pattern of men's formal jacket selected two kinds of men's jacket style fit for 1997 S/S trend. In this 1997 S/S trend, the trend I of the characteristic formal jacket is a style slim expressed over the basic formal wear, and trend II of the characteristic casual jacket appears to women's wear that roundly expressed by collar line of V-zone returened back by the shoulder line of jacket made of a big diagonal wrinkled line of X-style in front centered around a button of center front line. In this study, it called that the existing traditional formal jacket of men is a A style of jacket and jacket style I, II is a jacket B, C style. Jacket A, B, C style, a distingtion in draft that added to make form an oblique wrinkled line of the X-shaped-making artificially to the front button line sharply distinguished by eyes will be come out by control of front neck width over the back neck width. According to this, we shall be found that place of front neck point is a cardinal point in draft that will be added any changes in a style of jacket.

Types of perception on the body shape of the elderly men

  • The purpose of this study was to provide basic data necessary for the development of clothing that can improve the satisfaction of body shape of elderly men by examining subjective characteristics and characteristics of perception type of older men's body shape. Using the Q methodology, I investigated and analyzed the process of formation and main characteristics of subjectivity of various types of recognition that elderly men express about their body shape. There were four types of perception of body shape in elderly men: flat hip and bird legs skinny body, back bent and abdominal obese body, thick neck and upper body development body, forward cervical neck and protruding abdomen obese body. The actual body shape of elderly men was classified according to the bust, waist circumference, hip circumference, and body part angle. But recognition body shape of elderly men was classified by visual factors like the abdomen, the angle of the neck, the inclination of the back, the slope of the waist, and the degree of obesity. It is necessary to consider the trend of silver fashion which is increasing in demand. In addition, it is necessary to develop clothing patterns of comfortable fit through ergonomic study on the physical characteristics of older men. Consideration should be given to the design and pattern that can reduce the difference between the ideal body shape and the body shape recognized by older men.

A study on the Development on the Basic Pattern for Pet Dog's Wear (애견복 원형 개발에 관한 연구)

  • With the growing interest in pet dogs and pet dog's wear I used this study to compare and analyze the pattern used by each supplier and developed a new pattern for pet dogs' wear which is appropriate for pet dogs' body lines and movement. Test subjects were 91 pet dogs in Busan and after comparing 3 patterns from different suppliers, I developed the pattern reflecting pet dogs' body shapes and movement. Results are as follows; 1) As a result of analyzing measurements, it was found that pet dogs motion in walking on four feet, is different from that of people on two feet. For center measurements, their waist to bust length is longer than their waist to back length. The result of analysis shows that bust circumference impacts most other measurement results and it seems that better results are achieved if the bust line is taken into consideration when measuring bust back length, bust front length, neck base circumference, front leg arc, the length between lateral shoulder etc. 2) As a result of comparing suppliers' patterns, it was discovered that each supplier has different sizes for the same patterns and those sizes are not standardized across the board. Even if the same size of pattern was actually used in the production of pet dogs' wear by the individual companies, there were many variations in size for each item. 3) To develop a more standard pattern of pet dogs' wear, I collected and analyzed 3 patterns from each supplier and designed a new pattern, revising it three times. After test-wearing of the experimental pattern and the studied patterns, and comparing and analyzing the results, the studied pattern had similarly better estimation than the experimental one.

Analysis of Knitwear Preferences and Purchase Behavior of University Students for Pullover Design Development Based upon Baekje Traditional Patterns as Culture Oriented Clothing Products (백제전통문양을 활용한 풀오버 패션문화상품 개발을 위한 대학생의 니트웨어 선호도와 구매행동 연구)

  • The purpose of this study was to investigate the knitwear preferences and purchase behaviors of university students. The subjects were 493 university students in Daejeon and Chungnam Provinces. The method of this study was a survey and measurement instruments were 34 stimuli which were manipulated pullover patterns and shapes and self-administrated questionnaire with knitwear design preference items and knitwear purchase behavior items. Data were analyzed by factor analysis, frequency analysis, Cronbach'${\alpha}$, t-test, ANOVA and $Sch{\acute{e}}ffe$ test using SPSS program. The results of the study were as follows. First, university students most preferred achromatic colors, pastel tones, solid patterns, and 100% cotton. Second, as for knitwear purchase behaviors, university students considered the esthetical factor to be most important among 4 dimensions (comfortable, esthetical, economical, and conspicuous factors) as important purchase criteria, and they used internet web sites for knitwear purchases. Third, there were significant differences in preferred pullover shape depending on neckline shape, neck depth, sleeve shape and clothing length. University students preferred the classic pullover design with V neckline, normal neck depth, set-in sleeves and normal length. Fourth, university students preferred the cloud motif and riding man motif among the 9 Baekje traditional motifs and one point small pattern on the left chest and crosswise bending pattern for pattern arrangement.

Influence of Breathing Patterns on the Thickness of Sternocleidomastoid Muscle and Deep Cervical Flexor Muscles During Craniocervical Flexion Exercise (호흡패턴이 머리목 굽힘 운동시 목빗근과 심부 목굽힘근의 근두께에 미치는 영향)

  • Background: The deep cervical flexor (DCF) muscles have a crucial role in the management of neck pain. For preventing neck pain by activation of the DCF, craniocervical flexion (CCF) is an effective exercise. However, sternocleidomastoid (SCM) muscle is considered to affect negatively the activation of the DCF. SCM muscle which is an accessory muscle for respiration is activated differently depending on types of breathing patterns. It's not certain that breathing patterns affect the SCM and DCF muscles thickness during CCF exercise. Objects: The purpose of this study was to investigate the influence of breathing patterns on the SCM and DCF muscles thickness during CCF exercise. Methods: Forty-five subjects participated in this study, and they were classified according to their breathing pattern, as follows: costodiaphragmatic breathing (CDB) and upper costal breathing (UCB) groups. Ultrasonographic imaging of the SCM and DCF muscles was performed during five incremental levels of CCF during tidal breathing and expiration. Results: There was a significant interaction between the breathing pattern and the phase of CCF for percentage of SCM muscle thickness changes (p<.05). In phase 1 CCF, a percentage of SCM muscle thickness changes was increased in the UCB group than in the CDB group (p<.05). There was an increase in DCF muscles thickness with each additional CCF phase (p<.05). Conclusion: Recruitment of SCM muscle was increased in the UCB group while performing CCF with a low intensity. There were no significant differences on DCF recruitment between the breathing pattern groups. Higher CCF exercise intensities elicited a higher DCF recruitment.
