Purpose : The purpose of this study was to compare the effect of exercise on the neck muscles activities between scapular stability and McKenzie stretch exercise in the forward head posture subjects. Method : After measuring cervical alignment using the Global Posture System, 20 volunteers with forward head posture posture were selected and divided into two groups. The experimental group A(n=15) and Experimental group B(n=15) participated in respectively and McKenzie neck stretching exercise, three times per week for 4 weeks. The scapular stabilization exercise program was comprised middle and lower trapezius strength exercises and the stretching exercise program was comprised levator scapulae and upper trapezius stretching exercise. The activities of the muscles of the posterior neck was then measured using electromyography. Result : After the intervention, there was significant difference of a electromyography activity changes between the pre-test and post-test in the experimental group. Conclusion : This study showed that both scapular stabilization and McKenzie neck stretching exercises are more effective for reducing neck muscles activities.
Compared to previous in-vitro test, FE model showed reliable motion patterns. A finite element model of a 50th percentile male neck was developed to study the mechanics of whiplash injury while the rear impacts. The model was consisted of the whole cervical vertebrae including part of occipital, intervertebral discs. which were modeled using linear viscoelastic materials and posterior elements. The sliding interfaces were defined to simulate contact phenomena in facet joints and in odontoid process. All ligaments and atlanto-occipital membrane were modeled as nonlinear bar elements. Only muscle elements were not considered. Motion of each cervical vertebra was obtained from the dynamic simulation with a MADYMO model for 15 km/h $40\%$ rear end offset impacts. Soft tissue neck injury(STNI) was investigated with a developed FE model. In FE model analysis, the high stress was appeared at C3/C4 disc in offset impact. Further research is still needed in order to improve the developed neck FE model for many different crash patterns.
Hyperfunction of the upper esophageal sphincter (UES) can cause severe dysphagia. This condition referred as cricopharyngeal dysphagia may occur after head and neck surgery due to altered muscle spasm and stenosis of the pharyngo-esophageal segment. Among various treatment options available, Botulinum toxin A (Botox) injection offers a nonsurgical treatment which is useful especially for debilitated patients, and there has been a recent increase in the clinical use of Botox by otolaryngologists for managing such conditions. A 55-year-old male with base of tongue (BOT) cancer suffered from severe dysphagia after total glossectomy and neck dissection treatment. Videofluoroscopic swallow study (VFSS) and flexible endoscopic evaluation of swallowing (FEES) showed inability to pass food through the UES due to cricopharyngeal spasm. After injection of 10 U of Bot ox into each cricopharyngeus muscles (total 20 U) via EMG-guided percutaneous injection, swallowing function had improved and oral nutrition was possible, with food passing through the UES visualized on VFSS and FEES.
Background: Recently, a new method of dysphagia rehabilitation using Kinesio taping (KT) has been attempted and demonstrated an immediate increase in the activation of the suprahyoid muscle during swallowing in healthy adults. Objectives: To investigate the effect of dysphagia rehabilitation using KT on the thickness change of the suprahyoid muscle in patients with dysphagia after stroke. Design: Two-group pre-post design. Methods: In this study, 20 patients with dysphagia after stroke were enrolled and assigned to the experimental and control groups. The experimental group applied KT to the front of the neck and repeatedly swallowed against the tension of the tape. On the other hand, the control group performed repeated swallowing without applying KT. Patients in both groups had swallowed 50 times a day/5 times a week for 4 weeks. For evaluation, the volume of the geniohyoid, mylohyoid, and digastric muscle was measured before and after the intervention using portable ultrasound equipment. Results: As a result of comparing the two groups after the intervention, the experimental group showed more volume increase in mylohyoid (P<.05) and digastric muscle (P<.05) than the control group. Conclusion: This study proved that suprahyoid muscle resistance exercise using KT is effective in increasing the volume of the suprahyoid muscle.
Objective : The Meridian sinew is one of the meridian subsystems, which includes muscles distributed on the twelve meridian. This study was performed to understand which muscle is falling under 'Foot lesser yin meridian sinew'. Methods : We have studied the literatures on meridian sinew theory and searched muscles which correspond to 'Foot lesser yin meridian sinew' in anatomical muscular system. And we researched myofascial pain syndrome about the symptoms of the muscles falling under 'Foot lesser yin meridian sinew'. Lastly we compared 'Foot lesser yin meridian sinew' with 'Deep Frontal Line' - one of the anatomical trains. Results & Conclusion : 1. It is considered that 'Foot lesser yin meridian sinew' includes flexor digitorum brevis muscle, abductor hallucis muscle, medial head of gastrocnemius muscle, flexor digitorum longus muscle, adductor muscle, iliopsoas muscle, erctor spinae muscle. 2. The symptoms of 'Foot lesser yin meridian sinew' are similar to the myofascial pain syndrome with referred pain of the muscles falling under 'Foot lesser yin meridian sinew'. 3. 'Deep frontal line' is similar to 'Foot lesser yin meridian sinew', but not exactly in neck & pelvic muscles.
Purpose: This study aimed to provide objective and basic data for an effective treatment method by examining changes in the mechanical properties of muscles and the degree of tenderness when a combination of stretching exercise and ultrasound therapy is applied to patients with tension headaches. Methods: The participants were classified into two groups based on the intervention received: the therapeutic ultrasound combined with suboccipital stretching (n=15, experimental group) and infrared combined with suboccipital stretching (n=15, control group) groups. The intragroup differences in mechanical muscle properties and modified total tenderness score were compared and analyzed. Results: The comparison and analyses of the changes in muscle tone and stiffness revealed statistically significant intragroup decreases in the upper trapezius and suboccipitalis in the experimental group. Similarly, the comparison and analyses of the changes in muscle stiffness revealed statistically significant intragroup decreases in the upper trapezius in the control group. Conclusion: Therapeutic ultrasound combined with suboccipitalis stretching effectively increased the flexibility of the muscles around the cervical vertebrae and reduced muscle tension and stiffness in tension-type headache patients
Background and Objectives : Adductor type spasmodic dysphonia (ADSD) is neurogenic disorder and focal laryngeal dystonia, while muscle tension dysphonia (MTD) is caused by functional voice disorder. Both ADSD and MTD may be associated with excessive supraglottic contraction and compensation, resulting in a strained voice quality with spastic voice breaks. The aim of this study was to determine the utility of spectrogram analysis in the differentiation of ADSD from MTD. Materials and Methods : From 2015 through 2017, 17 patients of ADSD and 20 of MTD, underwent acoustic recording and phonatory function studies, were enrolled. Jitter (frequency perturbation), Shimmer (amplitude perturbation) were obtained using MDVP (Multi-dimensional Voice Program) and GRBAS scale was used for perceptual evaluation. The two speech therapist evaluated a wide band (11,250 Hz) spectrogram by blind test using 4 scales (0-3 point) for four spectral findings, abrupt voice breaks, irregular wide spaced vertical striations, well defined formants and high frequency spectral noise. Results : Jitter, Shimmer and GRBAS were not found different between two groups with no significant correlation (p>0.05). Abrupt voice breaks and irregular wide spaced vertical striations of ADSD were significantly higher than those of MTD with strong correlation (p<0.01). High frequency spectral noise of MTD were higher than those of ADSD with strong correlation (p<0.01). Well defined formants were not found different between two groups. Conclusion : The wide band spectrograms provided visual perceptual information can differentiate ADSD from MTD. Spectrogram analysis is a useful diagnostic tool for differentiating ADSD from MTD where perceptual analysis and clinical evaluation alone are insufficient.
The role of cervical proprioceptors in the control of body posture was studied in bilaterally labyrinth-ectomized, decerebrate cats. The animals were suspended on hip pins with the neck extended horizontally. With this placement the EMG activities of extensor and flexor muscles of the upper extremities were observed by means of sinusoidal head rotator. The rotator can induce two kinds of neck movement: The one is 'pitch' which describes a rotatory neck motion to transverse axis of the body and mainly occurs at skull-C1 (atlantooccipital) joint and the other is 'roll', side-to-side relation of the neck to longitudinal axis, whose center is C1-C2 (atlanto-axial) joint. The following results were obtained. 1) Responses of EMG activity were closely dependent on the rotatory range of the neck. And the EMG activity was not changed during sustained neck torsion, eliciting a typical tonic neck reflex. 2) On pitching movement, the head-up rotation produced the excitation of bilateral triceps muscles, whereas the head-down rotation produced the inhibition. And the response of bilateral biceps muscles was the opposite to that of triceps. 3) On rolling movement, the side-up rotation of the head produced the excitation of ipsilateral triceps muscles and the inhibition of contralateral ones. And the response of biceps muscles was the opposite to that of triceps. 4) The minimum requirement of motion to evoke EMG activities in the upper extremities was $3.2^{\circ}{\sim}12.5^{\circ}$. These results have shown that the cervical proprioceptors produce tonic discharge on the upper brachial muscles, regulate the EMG activities of those muscles, and are very sensitive to neck rotation. And it can be stated that the cervical proprioceptors may play an important role in the control of body posture and movement.
The purposes of this paper were to investigate the effect of active treatment compared with a conservative treatment and to provide the information for physical therapy in patients suffering from chronic neck pain. Forty female subjects who were diagnosed with cervical radiculopathy participated in this study and were divided into the conservative and active treatment groups. The active treatment group consisted of 20 patients who were treated with therapeutic modalities and active neck exercise program during the admission (15 days) and one month after discharge. The conservative treatment group consisted of 20 patients who were not received with active neck exercise program. The assessment tools were made using visual analogue scale(VAS), neck disability index(NDI) and modified Zung depression scale(MZDS). All subjects were measured three times: before the admission, at discharge, and at one month after discharge. Data were compared by groups using independent t-test. VAS, NDI and MZDS scores measured at admission and discharge were not significantly different between the groups. On the assessment performed one month after discharge, VAS and NDI scores were significantly lower for the active treatment group compared with those of the conservative treatment group(p<0.05), but MZDS score was not significantly different between the groups. In the comparison of two treatment methods for chronic neck pain, active treatment was more effective than conservative treatment. The findings of the study indicate that active treatment in chronic neck pain has a positive effect in relieving pain and restoring neck function.
Objective: This study aimed to investigate the immediate effects of intervention using neuromuscular control, self-stretching (SS), and neck stabilization exercises (NSEs) on neck pain, range of motion (ROM), and proprioception of position sense in adults with neck discomfort. Design: Three-group pretest-posttest design. Methods: Forty-four adults who complained of neck pain participated in the experiment. They were randomly assigned to the following groups: neuromuscular control exercise (NMCE) group (n=15), SS group (n=14) and NSE group (n=15). The NMCE group did rolling with only upper limb pattern on both sides. The SS group performed neck stretching on each side, 3 sets of 30 seconds for each muscle. The NSE group had the pressure biofeedback applied with increases in pressure by 2 mmHg at a time from 20-30 mmHg while in the hook-lying position. All groups performed exercises for 10 minutes. Neck pain, ROM, and proprioception were measured to determine differences between the intervention methods. Results: Intra-group comparisons showed significant improvement after exercise in pain, ROM, and proprioception in the NMC group (p<0.05). In the comparison between groups, the NMC group had a significant decrease in pain compared to the other two groups (p<0.05). There was no difference in ROM between the groups but the NMC group showed significant improvement in left rotation compared to the stabilization exercise group (p<0.05). For proprioception, the NMC group had significantly lower error than the other two groups (p<0.05). Conclusions: NMCEs through upper extremity pattern rolling exercise is effective in improving neck pain, ROM, and proprioception.
