Surgical Treatment of Metastatic Bone Tumor in Extremity (사지에서 전이성 골종양의 수술적 치료)

  • The incidence of metastatic bony lesion has been increased recently, with the advanced therapeutic modalities of malignant tumors. The purpose of this study was to evaluate the effectiveness of surgical treatment in impending or established pathologic fractures due to metastasis in extremity. From 1981 to 1992, thirty two patients who had established or impending pathologic fractures in extremity had been treated surgically. The locations of metastasis, were lower extremity in 24 cases(femsral head and neck;5, intertrochanteric;7, subtrochanteric;8, femoral shaft;4) and upper extremity in 8 cases(proximal humerus;2, humoral shaft;6). The method of surgery was excision of tumor mass and rigid internal fixation to allow immediate mobilization, except 2 cases of amputation. The results were analysed by two criteria. The first was the grade of performance status that was made by modification of Functional Classification of New York Heart Association. The second was the degree of pain relief. The performance status was improved at least one grade in all patients and the mean improvement was 1.7 grades. The satisfactory pain relief was achieved in 81% of patients. Postoperatively, most of the patients had only slight to moderate limitation of physical activities and they became comfortable at rest and ordinary activities. The solitary metastatic lesions showed better pain relief(92%) than multiple metastasis(75%). The curative surgery was done in 3 cases, and they showed more than 24 months of disease-free survival.

Intravenous Pyogenic Granuloma in Sentinel Vein: A Case Report (전초정맥에 발생한 정맥관 내 화농성 육아종: 증례보고)

  • Purpose: Lobular capillary hemangioma is a vascular tumor that commonly occurs as a cutaneous lesion. Intravenous pyogenic granuloma is a rare form of lobular capillary hemangioma that usually occurs in the veins of the neck and the upper extremities. Methods: A 41-year-old man presented with 3 months history of nodules on the left temporal area and these lesions were clinically mistaken for a typical lipoma or epidermal cyst, but the nodules appeared unusually vascular with sentinel veins on excision. Results: On the histopathologic examination, the excised tissue was observed as an intraluminal polypoid mass, which was attached to the vein via a fibrovascular stalk and capillaries in a loose edematous fibromyxoid stroma, and so the lesions were confirmed to be intravenous pyogenic granuloma. At 6 months after excision, there has been no recurrence and the patient is asymptomatic. Conclusion: Intravenous pyogenic granuloma is a rare variant of lobular capillary hemangioma. This article describes the surgical and histopathological findings of treating pyrogenic granuloma.

The Effect of Nutrient Intake and Energy Expenditure on Bone Mineral density of Korean Women in Taegu (한국여성의 연령별 골밀도와 그에 미치는 영향인자에 관한 연구(I) 골밀도와 영양소 섭취 및 에너지 소비량의 관계 -대구지역을 중심으로-)

  • The purpose of present study was to assess the change of bone mineral density (BMD) by age and the dietary factors influcencing on BMD in Korean women in Taegu. The subjects were 242 healthy female in the range of 7-67 years old, and were divided into 4 age groups. BMD of lumbar of lumbar spine, femur(neck, ward's triangle, trochanter)and total body was measured by dual energy X-ray absorptionmetry. The nutrient intake measured by convenient method was similar to or more than the level of RDA. The significant relationship between nutrient intake and BMD was observed. Particularly for lumbar spine and total body in group 4 (50-60yr), such as energy, carbohyrate, protein, fat, Ca, animal Ca, meat Ca, Fe, thiamin and niacin were found significantly positive correlations. By analysis of multiple regression, significant relationships were shown between protein and lumbar spine and total body BMD in group 4, between ascorbic acid and total body BMD in group 2(17-34 yr). Energy expenditure showed better corrlations with BMD rather than nutrient intake. BMD was significantly greater in subjects consuming a mean dietary Ca intake more than 125% of RDA compared with less than 75% of RDA. In high Ca intake group ($\geq$125% of RDA), there were the significantly negative correlations between animal (meat) Ca and BMD in each bone of 7-9 years. The excessive intake of animal protein in this age group was found. And the correlations between BMD and past milk consumptin were significant in all skeletal sites of group 1(7-16 yr). This study confirms that the most effective way of preventing osteoporosis and the fractures is to maximize peak bone mass in early life and to minimize bone loss through the balanced adequate intake of Ca and other nutrients and regular physical activity.

Dermoid cysts: Epidemiology and diagnostic approach based on clinical experiences

  • Background Dermoid cysts are congenital tumors that are benign. Dermoid cysts with intracranial extension can cause serious neurological complications. It is important, therefore, to determine whether a patient has a dermoid cyst when their chief concern at a doctor's visit is a mass in the head or neck area. In this study, we present a literature review of dermoid cysts and an analysis of the authors' experiences, with the goal of providing guidance useful for the diagnosis and treatment of dermoid cysts. Methods This study retrospectively analyzed the medical records of 62 patients who visited the two medical clinics with which the authors are affiliated. The patients were enrolled between October 2003 and January 2017. Results Of the 62 patients analyzed in this study, 32 were 0 to 5 years of age (52%) and 23 were 17 years of age or older (37%). Forty-seven patients underwent 1 or more imaging study during the process of diagnosis. Thirty-two patients were suspected to have a dermoid cyst. Forty-nine patients were analyzed to determine the depth of the cyst. Bone was seen in 43 patients through imaging tests or during actual surgery, and nine of the 43 had bony problems (21%). Conclusions This study found that dermoid cysts were present in many adults, and that a high rate of deep lesions was observed, as well as many cases in which even the bone was affected. These results suggest, therefore, that dermoid cysts should be considered, and medical professionals should actively conduct imaging studies.

Treatment of Beign Multiple Symmetrical Lipomatosis with Liposuction (지방흡입술을 이용한 양성 대칭성 지방종의 치험례)

  • Purpose: Benign symmetrical lipomatosis(Madelung's disease) is a disease of uncertain etiology that manifests as massive lipomatous deposits in specific area of the body. It is usually located on the neck, shoulder, proximal regions of the extremities and the abdomen. Madelung's disease is found many in middle-aged, Mediterranean man. Imaging using either computed tomography or magnetic resonance imaging is often recommended. Dietary restriction provides no relief of disease. Abstinence with alcohol may delay further progression but does not regression tumor mass. The objective of this study is to prove effectiveness of liposuction for treatment of benign lipomatosis. Methods: We conducted liposuction as a treatment for benign lipomatosis. After general anesthesia, We conducted power assisted liposuction and amount of suctioned volume was about 2500 cc. After surgery, we did compressive dressing and then followed by pressure garment. Results: Six months after surgery, patient was examined for follow up at outpatient department. Recurred lipoma was not observated. Patient was satisfied with result.Conclusion: The only effective therapy for Madelung's disease is surgical removal but recurrence could be occurring. Also multiple surgical scars will be visible. We report a patient with multiple large lipomatosis successfully treated with liposuction.

Cystic Lymphangioma in the Chest Wall of the Child - A case report - (소아의 흉벽에 발생된 림프관종 - 1예 보고 -)

  • Lymphangioma is a developmental anomaly that is known to occur in the neck and axilla, and only rarely in the mediastinum, retroperitoneum, groin and pelvis. An isolated chest wall lymphangioma is a rare benign neoplasm. In case of large sized lymphangioma, surgical excision is preferably recommended as the treatment of choice. We operated on a three-year old female for excision of chest wall. In pathologic diagnosis, it diagnosed the mass as chest wall lymphangioma.

The Study of Pytoestrogen Intake and Bone Mineral Density of Vegetarian and Nonvegetarian Postmenopausal Women (채식과 일반식 폐경 후 여성의 식물성 에스트로겐 섭취와 골밀도와의 관련성)

  • There is some evidence that phytostrogen plays an important role in bone metabolism in postmenopausal women. In this study, we investigated the phytoestrogen intake levels and the relation between dietary phytoestrogens intake and bone mineral density of vegetarian (n = 77) and omnivore postmenopausal women (n = 122 . Vegetarian women, all of them were seven day adventists, who had been on vegetarian diet (almost lacto-ovo vegetarians;a few vegans) oyer 20 yrs. The average age of vegetarians and omnivores were 62.3 yrs and 60.2 yrs, respectively and, there was no significant difference. However, body weight (p < 0.001), body mass index (p < 0.001) of vegetarians were significantly lower than those of omnivores. The mean daily energy intake of vegetarians and omnivores were 1386.1 kcal (76.3% of RDA) and 1424.5 kcal (76.9% of RDA), respectively. The mean calcium intake of vegetarians (456.7 mg, 66.3 % of RDA) was not significantly different from that of omnivores (453.5 mg, 65.2 % of RDA). The mean daily isoflavones (daidzein +genistein) intake of vegetarians and omnivore were 33.9 mg and 23.9 mg, respectively. The vegetarians consumed significantly greater quantities of isoflavones (p < 0.05) and lignans precursor (p < 0.05). In the vegetarians, intake of isoflavones was significantly positively correlated with BMD of femoral neck, after adjusted for age and BMI. Also lignan precursor intake of vegetarians was significantly positively correlated with BMD of spine, after adjusted for age and BMI. In conclusion, in omnivore post menopausal women, intake of phytoestrogen such as isoflavones and lignans was little low, and it is not clear that positive association with bone mineral density. But in vegetarian postmenopausal women, phytoestrogen intakes be important factors related to bone mineral density.

Factors Affecting Bone Mineral Density in Korean Women by Menopause (폐경 전 . 후 중년 여성들의 골밀도에 영향을 미치는 요인)

  • Bone mineral density (BMD) focus one's attention on prevention effects of osteoporosis. This study was conducted to investigate BMD (lumbar spin : L2- L4, femur neck : m, femur trochanter TR, femur ward's triangle : WT, wrist) and look into the factors that affect BMD by menopause in 89 nonsmoking healthy Korean women (40-60 yr). Anthropometric index and body composition, nutrient intakes, osteocalcin and total protein, albumin, total cholesterol, triacylglycerol and calcium in serum were determined. Body fat mass, diastolic blood pressure and serum cholesterol concentration of postmenopausal women were significantly higher than those of premenopausal women. FN, TR, WT, wrist BMD of postmenopausal women were not different from those of premenopausal women. However L2- L4 BMD of postmenopausal women was lower than that of premenopausal women. Significant positive correlations were found between L2-L4, FN, TR, WT and weight (r = 0.44, r = 0.64, r = 0.58, r = 0.57) and significant positive correlations were found between FN, TR, WT and BMI (r =: 0.54, r = 0.41, r = 0.54) of premenopausal women. Whereas significant positive correlation was only found between TR BMD and weight. BMI (r = 0.38, r = 0.29) of postmenopausal women. FN BMD and WC (waist circumstance) of premenopausal women were found significant positive correlation (r = 0.35) whereas L2-L4 BMD and WC of postmenopausal women was found significant negative correlation (r = -0.31) . In premenopausal women. differences of bone BMD were not shown by exercise and alcohol drinking, but in postmenopausal women, significant difference of FN BMD was shown by exercise and alcohol drinking. These results suggested that by menopause, effects of weight, BMI, WC, exercise and alcohol drinking on bone BMD were different. Therefore, by menopause, we should consider the different ways to increase the bone BMD according to different factors.

A Case of Giant Lymph Node Hyperplasia in the Spermatic Cord (정색을 침범한 거대 림프양 증식증 1례)

  • Giant lymph node hyperplasia(Castleman's disease) is a rare disease, which represents a peculiar form of lymph node hyperplasia. Generally, it has been considered as benign and localized disease but recently, revealed malignant transformation in some cases of multicenteric form. It usually occurs on the mediastinum and occasionaly neck, lung, axilla, mesentery, broad ligament, retroperitoneum or soft tissue of extremities. Histopathologically, it is divided into hyaline vascular or plasma cell type and the former is characterized with prominent vascular proliferation and hyalinization in the central portion and tight concenteric layering of lymphocytes at the periphery of the fillicles(mantle zone) and the latter is characterized by a diffuse plasma cell proliferation in the interfollicular area. From the point of view of clinical presentation, it has been divided into solitary form, which presents as a localized mass located most commonly in the mediastinum, and multicenteric form, which occurs multiple location and has systemic manifestation arid transformation into malignancy. Herein we report a case of Giant lymph node hyperplasia occuring in the left spermatic cord in a 58-year old male with brief review of literatures.

A Study of Factors Affecting Bone Mineral Density in Children: Anthropometric Measurements, Socioeconomic Factors, Family History, and Other Environmental Factors (성장기 아동의 골밀도에 영향을 주는 요인에 관한 연구: 신체계측치, 사회경제적 요인, 가족력 및 기타 환경요인)

  • This study was conducted to investigate the bone mineral density (BMD) and the anthropometric measurements, socioeconomic factors, family history of osteoporosis and other environmental factors affecting BMD in children. One hundred sixty children (80 males, 80 females) in second grade of elementary school in Seoul were recruited; the mean age was 7.7 $\pm$ 0.47 years. Bone mineral density was measured in the lumbar spine (LS), femoral neck (FN), femoral trochanter (FT), and Ward's triangle (WT) by Dual Energy X-ray Absorptiometry (DEXA). Socioeconomic factors, family history of osteoporosis, and other environmental factors were assessed by questionnaire. Bone mineral densities of LS, FN, FT, and WT were 0.677, 0.637, 0.618, and 0.658 g/$\textrm{cm}^2$. BMD of boys was higher than that of girls (LS: 0.685 vs. 0.669, FN: 0.660 vs. 0.614, FT: 0.632 vs. 0.604, WT: 0.678 vs. 0.639 g/$\textrm{cm}^2$). Anthrometric measurements (height, weight relative body weight, circumference of waist and hip) were positively correlated with BMD. With increasing family income, BMD of LS was significantly lowered, and negative correlation was observed between mother's age at birth of subject and the BMD of FN. And the BMD of children with a family history of osteoporosis was significantly lower. However BMDs were not different by educational level of parents, family pattern, delivery term, birth weight, and type of feeding. Further studies are needed to clarify the factors affecting BMD of children and earlier age, included infants and even prenatal life. If any association is revealed and persist until the attainment of peak bone mass, osteoporosis prevention programs are needed to be start very early in the life cycle.
