측방충돌시 휠체어 탑승자의 위험도 분석에 의한 안전성평가 (A safety assessment by Risk Analysis Method on wheelchair occupant in side impact)

  • In this study, for a safety assessment of wheelchair occupant in side impact, we used a dynamic sled impact test results. The test was carried out total 6 times and impact speed was 13g$\pm$0.43/28km/h$\pm$0.95, By using EURO SID-1 dummy, head performance criteria(HPC), abdominal peak force, etc. were measured. We evaluated wheelchair occupant safety by motion criteria(MC) which was measured by head, trunk and side deformation change of wheelchair and Head & Neck injury criteria(HNI) measured by using head and neck deformation angle and time relation. When we assumed that the maximum injury value in side impact was 100%, the results of motion criteria(MC) of wheelchair occupant were max 80.3, mim 32.3 and average 60.3%, Head & Neck injury criteria(HNI) value were max 118.4, min 14.5 and average 59.7%.

119로 내원한 노인교통사고 환자의 손상과 손상중증도계수의 상관관계 분석 (Correlation between injury and the injury severity score in geriatric traffic accident patients transported by 119 rescue services)

  • Purpose: The purpose of the study was to investigate the correlation between injury and injury severity score (ISS) in geriatric traffic accident patients transported by 119 emergency medical technicians (EMTs). Methods: The subjects were 240 traffic accident patients over 65 years old transported by 119 rescue services emergency departments in Busan from January 1, 2014 to August 31, 2015. Results: ISS increased in motorcycle traffic accidents (p=.026), truck injuries (p=.005), and head and neck injury (p<.001). Vital signs were evaluated by 119 rescue EMT. ISS increased in cases of unresponsive unconsciousness (p<.001), hypotension (p=.001), and bradycardia (p<.001). The need for bleeding control and dressing by the EMTs increased ISS significantly (p=.022). Conclusion: In the initial evaluation of geriatric traffic patients, ISS can increase in motor cycle accidents and truck injuries. Due to high ISS, patients with head and neck injury, chest injury, hypotension, or tachycardia should be transferred to advanced level hospitals.

Acute Cerebral Infarction after Head Injury

  • Cerebral infarction rarely occur following head injury. The authors present the case of a 39-year-old man with complete infarction in the middle cerebral artery[MCA] and anterior cerebral artery[ACA] territories ccurred immediately after head injury. He had compound depressed fracture in right frontal bone with no neurological deficit. After the depressed bone elevation, postoperative computed tomography scan showed the right MCA and ACA territory infarction with midline shift. Cerebral angiography obtained on the day after emergent decompressive craneictomy showed the complete occlusion of the internal carotid artery[ICA] at the level of lacerum ICA segment. There was no evidence of neck vessel dissection and basal skull fracture. Cerebral infarction can occur in an ultraearly period after head injury without neck vessel dissection or basal skull fracture. We stress the need for attention to the cerebral infarction as the cause of a rare neurological deterioration of the head trauma.

교통사고로 인한 어지럼 환자의 임상양상 (Clinical Characteristics of the Patients with Dizziness after Car Accidents)

  • Background and Objectives With increasing frequency of car accidents, patients of dizziness caused by car accidents are also increasing. Various types of dizziness or vertigo can occur from car accidents depending on different injury mechanisms. Since accurate diagnosis is important for providing proper treatments, we evaluated clinical characteristics related to vestibular function of patients with dizziness caused car accidents. Subjects and Method In this retrospective case review study that runs from January 2011 to March 2013, a total of 82 patients with dizziness following car accident were enrolled consecutively. We analyzed the final diagnosis of dizziness according to different mechanisms of injury during car accident through clinical record review. Patients who developed dizziness within one month of car accident were included, excluding those who had temporal bone fracture and previous history of dizziness. Results Of the different types observed, 36.6% was head injury, 24.4% whiplash injury, 3.7% complex injury, 2.4% others and the rest was unknown. In the final diagnosis, the different types included 36.6% benign paroxysmal positional vertigo (BPPV), 23.2% unclassifiable dizziness, 18.3% cervical vertigo, 7.3% labyrinthine concussion, 3.7% BPPV with labyrinthine concussion and the rest was others. Of the different types of dizziness symptoms, 58.5% was headache, 45.1% was audiologic symptoms, and others included earfullness, tinnitus and hearing disturbance. Tinitogram and pure tone audiogram results show that 2.9% (27 people) of patients have tinnitus and 7.3% (6 people) have hearing disturbance. Conclusion An accurate diagnosis and timely management would be very important in forming a proper approach for post traumatic vertigo patients.

반회후두신경 손상을 동반하지 않은 갑상선 절제술 환자에서 수술 초기의 음성 및 연하 기능의 변화에 대한 분석 (Analysis of Voice and Swallowing Symptoms after Thyroidectomy in Patients without Recurrent Laryngeal Nerve Injury in Early Postoperative Period)

  • Background and Objectives : After thyroidectomy, many patients experience problems report such things as reduced voice range and vocal fatigue, swallowing problems without superior and recurrent laryngeal nerve injury. The purpose of this study was to evaluate voice and swallowing problems before and after thyroid surgery without laryngeal nerve injury. Materials and Methods : Ninety-three patients who underwent thyroidectomy without laryngeal nerve injury and completed the follow-up evaluations were studied between June 2013 and December 2015. Each evaluation was performed preoperatively, as well as 1 week, 1 month postoperatively. Analysis was performed including voice handicap index (VHI), dysphagia handicap index (DHI), and acoustic voice analysis. Results : Patients show significant variation of parameters in the fundamental frequency (F), maximal phonation time (MPT), shimmer, jitter and soft phonation index (SPI) early after operation, and most of them showed recovery of parameters after 1month of operation. Perceptive complaint of voice and swallowing also showed significant decreased after operation (p<0.005). After 1month of operation, MPT, highest frequency and frequency ranges still showed significant decreased parameters. Comparing acoustic and perceptive parameters of total thyroidectomy and lobectomy, there was no significant changes between them except highest frequency (p=0.042). Conclusion : The results from both subjective and objective evaluations show voice and swallowing disturbance after thyroidectomy even in the absence of laryngeal nerve and provide patients information about the recovery process after surgery. Highest frequency parameter showed most significant changes after operation.

Characteristics and Outcomes of Patients with Bicycle-Related Injuries at a Regional Trauma Center in Korea

  • Purpose: We analyzed the characteristics and outcomes of patients with bicycle-related injuries at a regional trauma center in northern Gyeonggi Province as a first step toward the development of improved prevention measures and treatments. Methods: The records of 239 patients who were injured in different types of bicycle-related accidents and transported to a single regional trauma center between January 2017 and December 2018 were examined. This retrospective single-center study used data from the Korea Trauma Database. Results: In total, 239 patients experienced bicycle-related accidents, most of whom were males (204, 85.4%), and 46.9% of the accidents were on roads for automobiles. Forty patients (16.7%) had an Injury Severity Score (ISS) of 16 or more. There were 125 patients (52.3%) with head/neck/face injuries, 97 patients (40.6%) with injuries to the extremities, 59 patients (24.7%) with chest injuries, and 21 patients (8.8%) with abdominal injuries. Patients who had head/neck/face injuries and an Abbreviated Injury Score (AIS) ≥3 were more likely to experience severe trauma (ISS ≥16). In addition, only 13 of 125 patients (10.4%) with head/neck/face injuries were wearing helmets, and patients with injuries in this region who were not wearing helmets had a 3.9-fold increased odds ratio of severe injury (AIS ≥2). Conclusions: We suggest that comprehensive accident prevention measures, including safety training and expansion of safety facilities, should be implemented at the governmental level, and that helmet wearing should be more strictly enforced to prevent injuries to the head, neck, and face.

  • Laryngeal neoplasm is the second most common malignancy of the upper aerodigestive tract. About 85% to 95% of laryngeal malignancies are squamous cell carcinoma that arises from the epithelial lining of the larynx. The exact cause of laryngeal neoplasm is unknown, but certain risk factors can affect the chances of developing it. Chronic inflammation is a mutagen factor confirmed in the carcinogenesis of various tumor. Inhalation injuries cause histopathologic damage to laryngeal mucosa and inflammation change. This long term inflammation may leads to the development of dysplasia and malignant transformation. Recently, we experienced a case of malignant transformation of laryngeal mucosa after inhalation injury patient 25 years ago. Herein, we reported this rare case and review the relevant literature.

편타 손상 후 소극적 대처 전략이 통증 및 기능에 미치는 영향 (The Effects of Passive Coping Strategies in Pain and Function After Whiplash Injury)

  • PURPOSE: The purpose of this study was to observe the effect of pain and function according to the high and low manual passive coping strategies after whiplash injury. METHODS: The study was tested with 30 patients with neck pain. Coping was measured at after 10 days using the Pain Management Inventory. Group was divided into high passive(experimental) and low passive(control) group. All patients were equally treated with the ordinary therapy. Patients attended physical therapy for 3 times a week, for 6 weeks. Visual analogue scale (VAS) for pain, Pain Disability Index (PDI), neck disability index (NDI), were recorded both before and after the intervention. RESULTS: Each coping strategy group were evaluated after 6 weeks. VAS has been significantly reduced in both groups (p <.05). PDI and NDI was significantly decreased after the experiment before. In the experimental group (p <.001), the control group showed no significant difference. In the comparison between groups VAS was significantly reduced compared with the control group (p <.05), PDI and NDI was significantly reduced compared with the control group (p <.001). CONCLUSION: Low passive coping strategy predicts neck pain and disability recovery. It may be beneficial to assess and improve coping strategy early in whiplash injury.

SUV & 트럭 차종의 USNCAP 통계분석, Part 1: 정면충돌 (Statistical Review for USNCAP on SUV & Pick-up, Part 1: Frontal Crash Test)

  • This paper statistically reviewed for the USNCAP frontal crash test results carried out by NHTSA. Vehicle samples were selected on total 20 vehicles which were included on 15 vehicles for MPV&SUV and 5 Pickup. The results was summarized as followings. The performance for the driver was better than the passenger's in the average sense. There exist distinctions between the driver and the passenger on the USNCAP front test procedure, for example dummy size, seating position and airbag style. Therefore these differences originated in the statistical results. Main effect was Neck injury for crash performance on both dummies on the average value. Root cause of neck injury was different for each dummy, ie, the driver caused from Nte & Ntf, but the passenger did absolutely Nte mode. Reliability evaluated from the standard deviation was highly dependent upon chest injury on the driver and neck injury on the passenger. Restraint system was also summarized.

  • According to the IIHS (Insurance Institute fur Highway Safety), side impacts are made up 30% of all accidents (reported 1998). In the case of auto accidents, head and neck injuries were most common as 58%, injuries to the body's trunk equaled 32%, and injuries to the abdomen were 21%. Therefore in this study, injury of wheelchair occupant in frontal and side impact of wheelchair loaded vehicle was analyzed using computer simulation method. The occupant was restrained at the rear of wheelchair by the lap belt. The detailed fixation and restrain conditions of the wheelchair occupant are referred to SAR J2249's recommendation. We estimated HIC(Head Injury Criteria) and HNIC (Head and Neck Injury Criteria) based on measured data.
