A Case Report of Neck Pain Patient with Klippel-Feil Syndrome by Cervical Manipulation Treatment (Klippel-Feil 증후군 환자의 경추 도수치료 후 발생한 부작용 치험1례)

  • The Klippel-Feil Syndrome (KFS) is characterized by congenital fusion of two or more vertebrae with hypermobile normal segment. According to this, a patient with KFS can be at risk of severe neurological symptoms after manipulation treatment. We had a KFS patient who suffered from neck pain and limited range of motion at cervical after manipulation treatment. The patient, 49-year-old woman was diagnosed as KFS through Cervical X-ray and MRI. The patient was treated by acupuncture therapy and SCENAR therapy. We measured neck pain by visual analog scale (VAS) and neck disability index (NDI) and checked range of motion at cervical before and after the treatments. After 8 times treatments, the patient's pain decreased and the range of motion increased. From this case, we can recognize the risk of Chuna manipulation treatment for KFS patient. So, we suggest that radiological examination is needed before cervical Chuna manipulation treatment to avoid adverse reactions.

Effect of Mulligan SNAGs on Pain, Neck Dysfunction, and Depression in patients with Cervicogenic headache (멀리건 SNAGs 기법이 경추성 두통 환자의 통증, 경부 기능 장애 및 우울증에 미치는 영향)

  • Background: Cervical headache (CGH) is a common condition that causes serious damage. Mulligan described a sustained natural apophyseal glides (SNAGs) as a manual therapy approach to treat this dysfunction. Although there have been several case studies to evaluate the effectiveness of SNAG, the efficacy of SNAG for cervical headaches is still controversial. Therefore, this study examined the effects of Mulligan's SNAG technique on cervical headache and pain intensity to present basic clinical data. Methods: Thirty-two subjects participated in this study, 16 each in the experimental and control groups. The experimental group applied Mulligan's SNAGs technique and the control group applied the placebo SNAGs technique. Results: Significant differences (p<.05) in the changes in pain, neck disability index (NDI) and Hamilton rating scale for depression (HRSD) were observed between the two groups. Conclusion: The application of Mulligan's SNAG technique to patients with cervical headaches effectively reduced headache, depression, and neck dysfunction. Based on this study, Mulligan's SNAGs technique can be used as an objective research method for additional studies targeting cervical headache patients in the future.

A Case Report on Ossification of Posterior Longitudinal Ligament Treated by Korean Medicine: Focusing on Chuna Therapy (경추간판 장애 및 척수병증을 동반한 후종인대골화증 환자에 대한 한의 복합 치료 1례 증례보고: 추나 요법을 중심으로)

  • Objectives : This study reports the clinical case of a patient diagnosed with ossification of the posterior longitudinal ligament treated with Korean Medicine focusing on Chuna therapy. Methods : The patient was treated by a doctor from July 18th, 2019 to November 28th, 2019. The patient was diagnosed with ossification of the posterior longitudinal ligament based on magnetic resonance imaging results and was recommended to undergo surgery at another hospital. The primary complaint of the patient was left neck pain, left arm sensation, right leg sensation, and right leg loss of strength. The doctor treated the patient with Chuna therapy, acupuncture, cupping, herbal medicine, and pharmacopuncture. The effect of treatment was evaluated using visual analogue scale, neck disability index, pain rating score and Japanese Orthopedic Association score. Spurling, compression, and distraction tests were utilized. Results : Visual analogue scale decreased from 10 at the arm and 10 at the leg to 1 and 2, respectively. Neck disability index decreased from 25 to 11, pain rating score decreased from 66 to 1, and Japanese Orthopedic Association score recovered from 12 to 17 points. In addition, positive spurling test, compression test, and distraction test all improved to negative. Conclusions : Korean medicine treatment can be effective for ossification of the posterior longitudinal ligament patients, even when surgery is recommended. Further clinical studies, such as case series and case-control studies, are required to verify these findings.

Comparative Study of Treatment Effect between Indirect Moxibustion Treatment with General Acupuncture and General Acupuncture Only on Treatment for Neck Pain Caused by Whiplash Injury (편타성 손상으로 인한 경항통 환자를 대상으로 한 일반침 치료와 간접구 병행치료의 임상적 효과비교연구)

  • Objectives : The purpose of this study was to evaluate the clinical effect of indirect moxibustion treatment for patients with neck pain caused by whiplash injury. Methods : From October 2010 to October 2011, 85 patients with neck pain caused by whiplash injury who admitted to Semyung oriental medical hospital were divided into two groups. Group A was treated by general acupuncture, group B by indirect moxibustion and general acupuncture. The efficacy of treatment was measured by visual analog scale(VAS) scores and neck disability index(NDI) scores of before and after 5 days to start treatment. Results : 1. Both group A and B represented effective improvement in VAS and NDI of neck pain caused by whiplash injury. 2. Group B has more statistical effect than group A. Conclusions : Through this research, indirect moxibustion treatment is considered to be effective reducing pain for patients with neck pain caused by whiplash injury.

Effects of Korean Medical Combination Treatment for Herniated Intervertebral Lumbar Disc Patients: An Observational Study (요추 추간판 탈출증 환자에 대한 한의학적 복합치료 효과의 관찰 연구)

  • Objectives : Lumbar disc herniations are common conditions which induce severe pain and disability. We assessed the clinical effectiveness of Korean medical combination treatment. Methods : This is an observational study. We reviewed the medical records of 46 patients with lumbar disc herniations, who attended intensive Korean medical combination treatments at Mokhuri Neck and Back Hospital, Korea during March, 2012 to October, 2012. All the patients took combination treatments of acupuncture, Chuna and herbal medicines during the hospitalization treatments. Zero to ten pain numerating rating scale(NRS) of pain and SF-36 were assessed before and after treatments. Results : During the period, 46 patients with lumbar disc herniation participated in the treatments, respectively. Average admission duration were 20.32 days(SD 9.67) in lumbar disc patients. For lumbar patients, low back pain NRS decreased from $5.84{\pm}2.58$ to $2.12{\pm}1.74$(p<0.001) and radiating pain from $6.25{\pm}2.30$ to $2.59{\pm}1.97$(p<0.001). Total SF-36 score increased significantly after treatment in lumbar disc patients(p<0.001). Conclusions : Korean medical combination treatment might be effective in reducing pain and improving life quality of patients with lumbar disc herniation.

Indicator of Diagnosing Fatigue by Analyzing Questionnaire Data (설문지를 이용한 피로 진단 지표 연구)

  • Objectives This study was designed to development the diagnosis indicators for the fatigue group without disease using analysis of questionnaire data. Methods The subjects were divided into three groups, the fatigue group with disease, the fatigue group without disease, non-fatigue group without disease. Thirteen kinds of questionnaire (Fatigue visual analogue scale, Fatigue severity scale, Pain visual analogue scale, Short-form McGill pain questionnaire score, Headache impact Test-6, Neck disability index, Oswestry low back pain disability questionnaire score, Blood stasis assessment scale, Physical activities questionnaire score, Beck depression inventory, Anger expression inventory score, Mood disability scale, Job stress scale) were carried out for the subjects. Results The results of this study are ; 1) The weight, the systolic and diastolic blood pressure were different among three groups. 2) Among nine questionnaires related to the physical or life conditions, the averages of eight questionnaires were not same one another. 3) Among four questionnaires related to the mental condition or autonomic nervous system, the averages of Beck depression inventory were not same one another. Conclusion The fatigue group without disease can be diagnosed through using the both the questionnaires related to the physical or life condition and the those related to the mental condition or autonomic nervous system.

The Effects of Sitting Questionmark Exercise and Brugger's Relief Exercise on Pain, ROM, Proprioception, NDI in Patients with Chronic Cervical Pain

  • Purpose : This study aimed to evaluate the effects of sitting questionmark exercise (SQE) and brugger's relief exercise (BRE) on pain, ROM, proprioception, NDI in 60 Patients with Chronic Cervical Pain (CCP). Methods : In this study the VAS and NDI were used to investigate changes in pain and disability with SQE and BRE. The pre and post intervention intra group differences were analysed with a paired t-test for mean values, and the inter group differences were analyzed with an independent t-test for mean values. Results : The pain of both of groups was lowered with statistical significance. Pain of SQE group is lower than that of BRE group with statistical significance. ROM in both groups was improved, but there is no significant difference between two groups. NDI in both of groups were significantly decreased after the intervention, but there was no significant difference between groups. there was NDI ratio (%) significant difference between each groups. Conclusion : SQE intervention may be considered a more effective clinical approach for reducing pain and restoring proprioception in patients with CCP.

Effects of a Complex Exercise Program using Dynamic Taping on Patients with Tension-Type Headache and Chronic Neck Pain with Forward Head Posture (다이나믹 테이핑을 적용한 복합 운동프로그램이 긴장성 두통과 전방머리자세를 동반한 만성 목 통증 환자에게 미치는 효과)

  • Purpose : This study examined the effects of pain, neck dysfunction, psychosocial level, headache impact test (HIT), postural alignment, and trapezius muscle tone of the complex exercise program using dynamic taping in patients with tension headache and chronic neck pain with forward head posture. Methods : The design of this is a randomized controlled trial (RCT). Thirty-four patients with chronic neck pain were screened using a randomized assignment program and assigned to experimental group (n=17) and control group (n=17). Both groups underwent a complex exercise program. In addition, the experimental group dynamic taping was applied to the upper trapezius muscle. All interventions were applied three times per week for four weeks. The visual analogue scale (VAS), the neck disability index (NDI), short form-12 health survey questionnaire (SF-12), Headache impact test-6 (HIT-6), Craniovertebral angle (CVA), Cranial rotation angle (CRA), upper trapezius muscle tone were compared to evaluate the effect on intervention. Results : Both groups showed significant differences before and after the intervention in VAS, NDI, SF-12, HIT-6, and CVA, CRA (p<.05). In addition, significant differences in NDI and upper trapezius muscle tone were observed between the experimental group and control group (p<.05). Conclusion : A complex exercise program using dynamic taping for patients with tension headache and chronic neck pain with forward head posture are effective method with clinical significance in improving the function and reducing upper trapezius muscle tone.

Korean Medicine Treatment for Extension Teardrop Fracture of C2 with Post-operative State of Cervical Spine: A Case Report (축추 신전형 소편골절을 동반한 하부경추유합술 시행 환자의 한의학적 치료: 증례보고)

  • The objective of this study was to evaluate the effects of Korean medicine treatment (including acupuncture, herbal medication, and pharmacopuncture) for postoperative pain after cervical surgery in a patient with a cervical (C2) extension teardrop fracture. We measured the patient's cervical range of motion, neck disability index score and numerical rating scale score to evaluate the effects of Korean medicine treatment on postoperative pain after cervical surgery. After 43 days of inpatient treatment, the patient's neck disability index score decreased from 75.5 to 46.67 and the numerical rating scale decreased from 6 to 2. Furthermore, recovery was observed for cervical range of motion. In conclusion, this case suggests that Korean traditional medicine treatment may effectively reduce postoperative pain after cervical surgery for cerviecal extension teardrop fracture.

A Physiotherapy Program for Secondary School Teachers with Forward Head Posture Accompanied by Moderate to Severe Neck Pain: Comparison of the effects of cranio-cervical flexor training and general training (중등도 이상의 목 통증을 동반한 앞쪽 머리 자세를 가진 중·고등학교 교사들을 위한 물리치료적 프로그램: 머리-목뼈 굽힘근 훈련과 일반적 훈련의 효과 비교)

  • Purpose : This study applied general training (control group) or cranio-cervical flexor training (experimental group) using a pressure biofeedback unit along with general training for 4 weeks to secondary school teachers with moderate to severe neck pain and forward head posture. After that, we tried to compare the effects through differences in neck pain intensity (using numberical rating scale), functional performance (using neck disability index), and cranio-vertebral angle change. Methods : All 50 subjects were randomly assigned to either the "experimental group (n= 25)" or the "control group (n= 25)", and the measurements were evaluated in the same way before the intervention (baseline) and after the intervention (4 weeks). During the intervention period, the subject visited the physiotherapy center and made a reservation three times a week at a fixed time as much as possible, and each training session was thoroughly conducted under the 1:1 guidance of the therapist in charge so that the correct movement and number of times could be performed without compensatory action. Results : As a result of the homogeneity analysis on the general characteristics of the subjects, there were no significant differences between the groups in all variables (p>.05). Compared to the "control group", the "experimental group" showed significant improvement after intervention in all measured variables of neck pain intensity, functional performance, and cranial-vertebral angle (p<.05). Conclusion : For secondary school teachers with forward head accompanied by neck pain, cranio-cervical flexor training using a compression biofeedback unit is an excellent method to show superior pain reduction and functional performance improvement compared to general training alone. In addition, it can be presented as a more effective intervention method that can promote recovery of forward head posture, which is an essential element of the solution.
