술후 경막외 Morphine으로 인한 오심 및 구토에 대한 경막외 Droperidol과 Metoclopramide의 효과 비교 (Comparison of Epidural Droperidol and Metoclopramide for Prevention of Postoperative Nausea and Vomiting Associated with Epidural Morphine)

  • Background: There are no controlled studies assessing the effect of metoclopramide and droperidol administered epidurally for the prevention of nausea and vomiting associated with epidural morphine. This study was undertaken to compare the effectiveness of continuous epidural metoclopramide and droperidol in reducing nausea and vomiting associated with epidural morphine. Methods: Ninty patients undergoing elective gynecologic surgery were randomly assigned to one of three study groups; Group A(n=30) patients received continuous infusion of epidural morphine(6.0 mg/day) following a bolus loading dose of 3.0 mg; Group B(n=30), epidural mixture of morphine and droperidol(5.0 mg/day) following a bolus loading dose(morphine 3.0 mg, droperidol 1.5 mg); Group C, (n=30), epidural mixture of morphine and metoclopramide(20 mg/day) following a bolus loading dose(morphine 3.0 mg, metoclopramide 10 mg). For the 24 postoperative hours, the incidence of nausea and vomiting, degree of pain, level of sedation and other adverse effects were evaluated. Results: Incidence of nausea and vomiting, and number of patients who required antiemetic therapy were significantly less in Group B and C than in Group A(P<0.05). Patients in Group A and C were less sedated than those in Group B. Conclusions: We conclude metoclopramide is more effective than droperidol for postoperative nausea and vomiting due to its lower of sedative effect.

지압이 암환아의 오심 구토 정도와 체중변화에 미치는 효과 (Effect of Acupressure on Nausea-vomiting and Weight Change among Pediatric Cancer Patients Receiving Anti-cancer Chemotherapy)

  • Purpose: This study was to confirm the effect of acupressure on the emesis control and the weight change among pediatric cancer patients receiving anti-cancer chemotherapy. Method: Forty pediatric cancer patients, receiving the induction stage of chemotherapy with MTX and vincristine, were divided into control(n=20) and the intervention group(n=20). Both groups received regular anti-emesis medication, but the intervention group was added acupressure maneuver for 5 minutes on P6 point for 3 times a day for 5days: before chemotherapy, lunch and dinner by investigator during the hospitalization and by mother at home. The instruments for this study were Rhode's(1986) Index of nausea, vomiting and retching(INVR), Cas electric scale and pamphlet developed by researcher. Result: Significant differences in the degree of nausea and vomiting were observed between the control and the intervention group as measured by INVR(t=4.73; p=.01). Repeated measures ANOVA also shows that the group effect was significant(F=22.39, P=.01) as was the time effect(F=380.35, P=.01). The group by time interaction was also significant(F=5.27, P=.01). Acupressure maneuver was apparently effective in reducing the degree of chemotherapy-induced nausea and vomiting. There were also statistically significant weight loss noted in the control group than the intervention group(t=5.42, p=.01). Conclusion: Acupressure on P6 point shows an effective adjunct maneuver in reducing the degree of nausea and vomiting and conserving the weight in pediatric cancer patients. Therefore, it is proposed that acupressure should be applied as supportive nursing intervention strategies to relieve chemotherapy induced nausea and vomiting and to prevent weight loss in pediatric cancer patients.

항암화학요법을 받는 암환자의 시기별 오심과 구토, 식욕부진 및 음식섭취에 관한 연구 - 자궁암 환자와 위암 환자의 비교 - (A Study on Nausea & Vomiting, Anorexia and Food intake in Cancer Patients undergoing Chemotherapy Overtime - Comparison between Cervix Cancer and Stomach Cancer -)

  • Purpose: This study was conducted to investigate the level of nausea & vomiting, anorexia and food intake during the periods of chemotherapy and 14 days after discharge. Method: The subjects were 40 cancer patients(cervix cancer : 20 patients, stomach cancer: 20 patients) who had chemotherapy with Cisplatin & 5-Fu. Data were analyzed by mean & standard deviation, ANCOVA. Result: 1. The severity of nausea & vomiting with anorexia in cancer patients receiving chemotherapy was the highest at the 3rd day and then it gradually decreased. At the 14th day after discharge, nausea & vomiting with anorexia still remained with very low levels. 2. The amount of food intake was the lowest on the 3rd day and then gradually increased. However, it was 53.3-72.5% of the ordinary food intake on the 14th day after discharge. Calorie intake was 625.31 Kcal on the 3rd day and 1130.92 Kcal on the 7th day after discharge. 3. There were no significant differences in nausea & vomiting, anorexia, food intake, calory intake between those with cervix cancer and stomach cancer. Conclusion: In cancer patients nausea & vomiting and anorexia were severe and food intake was very poor during chemotherapy but afterwards they were gradually improved, but were not completely recovered on the 14th day after discharge. Therefore the nursing intervention regarding the increase of food intake was necessary during chemotherapy and after discharge.

  • Purpose: The purpose of this study was to examine the effect of Nei-Guan acupressure on nausea, vomiting, and anorexia in gynecological cancer patients receiving chemotherapy. Methods: Subjects consisted of 40 patients with cancer who were hospitalized and had chemotherapy at a general hospital located in Gyeonggi province, Korea. Control subjects (n=20) received only chemotherapy and intervention subjects (n=20) received Nei-Guan acupressure during their chemotherapy. The data were collected from February to October in 2008. Nausea and vomiting were measured with the Rhode's Index of Nausea, Vomiting, and Retching, and anorexia was measured using a Visual Analog Scale. Data were analyzed by X$^2$-test, t-test, repeated measures ANOVA. Results: Severity of nausea and vomiting in the intervention group was significantly less than in the control group. Repeated measure ANOVA revealed significant group effect (F=28.09, p=.000), time effect (F=321.48, p=.000), and interaction effect (F=7.90, p=.000). Severity of anorexia in the intervention group was significantly less than in the control group. Repeated measure ANOVA revealed significant group effect (F=28.68, p=.000), time effect (F=52.32, p=.000), and interaction effect (F=6.35, p=.000). Conclusions: Acupressure can be applied as a supportive nursing intervention to relieve nausea, vomiting, and anorexia in patients receiving chemotherapy.

  • The somatoform disorder are distinguished by physical symptoms suggesting a medical condition, yet the symptoms are not fully explained by the medical condition, by substance use, or by another mental disorder. This is that an unconscious intrapsychic conflict, wish, or need is converted to a somatic symptom and clinically express various symptoms such as headache, dizziness, nausea, vomiting dyspepsia, diarrhea and constipation, etc. We report a case of somatoform disorder patient, who was 9 years old female and complained of nausea, vomiting and dysdipsia. She had her case diagnosed as somatoform disorder in Yong-dong severance hospital and took anti-depressant (chlomipramine) with counseling for 2 months. After treatment, her emotional instability and depression were improved, yet the somatic symptoms remain same. We diagnosed her case as vomiting induced by deficiency of the stomach(胃虛嘔吐) and administered Bihe-yin(比和飮) to her. After administration of Bihe-yin(比和飮) for one month, her somatic symptoms of nausea, vomiting and dysdipsia were almost disappeared and she got acquired her confidence in school life.

Aprepitant in the Prevention of Vomiting Induced by Moderately and Highly Emetogenic Chemotherapy

  • Chemotherapy is a major therapeutic approach for malignant neoplasms; however, due to the most common adverse events of nausea and vomiting, scheduled chemotherapeutic programs may be impeded or even interrupted, which severely impairs the efficacy. Aprepitants, 5-HT3 antagonists and dexamethasone are primary drugs used to prevent chemotherapy-induced nausea and vomiting (CINV). These drugs have excellent efficacy for control of acute vomiting but are relatively ineffective for delayed vomiting. Aprepitant may remedy this deficiency. Substance P was discovered in the 1930s and its association with vomiting was confirmed in the 1950s. This was followed by a period of non-peptide neurokinin-1 (NK-1) receptor antagonist synthesis and investigation in preclinical studies and clinical trials (phases I, II and III). The FDA granted permission for the clinical chemotherapeutic use of aprepitant in 2003. At present, the combined use of aprepitant, 5-HT3 antagonists and dexamethasone satisfactorily controls vomiting but not nausea. Therefore, new therapeutic approaches and drugs are still needed.

  • Background: The aim of our study was to evaluate the antiemetic effects of intravenous dexamethasone in preventing continuously infused epidural morphine-related nausea and vomiting. Methods: Twenty-seven patients requiring general anesthesia for gastrectomy were enrolled in a randomized, double-blinded, and placebo-controlled study. At the end of surgery, all patients received epidural morphine 3 mg and were connected to an epidural morphine infusion pump for 2 days in order to relieve postoperative pain. Before the morphine injection, the dexamethasone group (n = 12) received IV dexamethasone 10 mg, whereas the saline group (n = 15) received IV saline. The incidence of nausea & vomiting, pruritus, back pain and VAS scores were assessed in the recovery room, and at 24 h and 48 h postoperatively. Results: There was no significant difference in the total incidence of nausea and vomiting, pruritus, back pain or in the VAS scores. However, there was no vomiting and no back pain in the dexamethasone group. Conclusions: Intravenous dexamethasone did not significantly decrease the total incidence of nausea or vomiting in patients receiving continuous epidural morphine for postoperative pain control. However, IV dexamethasone appears to decrease the severity of nausea, vomiting and back pain.

Effects of Korean Red Ginseng Extract on Cisplatin-Induced Nausea and Vomiting

  • Ginseng, the root of Panax ginseng CA Meyer, is well known as a tonic medicine for restoring and enhancing human health. In traditional medicine, ginseng is utilized for the alleviation of emesis, which includes nausea and vomiting. However, it has not yet been demonstrated whether ginseng exhibits in vivo anti-nausea and anti-vomiting properties. In this study, we examined the anti-emetic effect of Korean red ginseng total extract (KRGE) on cisplatin-induced nausea and vomiting using ferrets. Intraperitoneal administration (i.p.) of cisplatin (7.5 mg/kg) induced both nausea and vomiting with one-hour latency. The episodes of nausea and vomiting reached a peak after 1.5 h and persisted for 3 h. Treatment with KRGE via oral route significantly reduced the cisplatin-induced nausea and vomiting in a dose-dependent manner. The anti-emetic effect was 12.7 $\pm$ 8.6, 31.8 $\pm$ 6.9, and 67.6 $\pm$ 4.0$\%$ with doses of 0.3, 1.0, and 3.0 g/kg of KRGE, respectively. Pretreatment with KRGE via oral route 1 and 2 h before cisplatin administration also significantly attenuated the cisplatin-induced nausea and vomiting. However this did not occur with a pretreatment 4 h before cisplatin administration. These results are supportive of KRGE being utilized as an anti-emetic agent against nausea and vomiting caused by chemotherapy (i.e. cisplatin).

Olanzapine for Preventing Nausea and Vomiting Induced by Moderately and Highly Emetogenic Chemotherapy

  • Nausea and vomiting are common adverse events in chemotherapy. In spite of the serious effects on the quality of life and further treatment, they remain overlooked by physicians, and no standard treatment has been developed. Neurokinin-1 (NK-1) receptor antagonists and palonosetron are the major agents in the standard regimen for treating moderately and highly emetogenic chemotherapy-induced nausea and vomiting (CINV). However, NK-1 receptor antagonists first became commercially available at the end of 2013 and palonosetron has not been extensively applied in China. Olanzapine was recommended as a therapy for moderate and severe CINV in antiemesis-clinical practice guidelines in oncology in 2014 for the first time. It is an atypical antipsychotic agent, which can block multiple receptors on neurotransmitters. During more than 10 years, olanzapine has demonstrated significant effects in preventing CINV and treating breakthrough and refractor CINV, which was observed in case reports, precise retrospective studies, and phase I, II and III clinical trials, with no grade 3 to 4 adverse events. In particular, it is superior to aprepitant and dexamethasone in delayed nausea and vomiting. Therefore, this compound is worthy of further investigation.

음악요법이 조혈모세포 이식 전 전신 방사선조사 시 불안, 오심과 구토에 미치는 효과 (The Effects of Music Therapy on Anxiety, Nausea and Vomiting in Patients Receiving Total Body Irradiation for Hematopoietic Stem Cell Transplantation)

  • Purpose: The present study was to investigate the effect of music therapy on anxiety, nausea, and vomiting in patients undergoing total body irradiation (TBI) for hematopoietic stem cell transplantation (HSCT). Method: A untreated control group with pretest and posttest was used in this study. A total of 35 patients receiving TBI for HSCT were recruited from a medical center at a university in Korea. The music therapy taking for 15-20 minutes per a time was performed twice a day for three days while the patients in the experimental group were undergoing the TBI. Results: The scores of state anxiety, nausea and vomiting tended to increase in the control group as compare with those in the experimental group. The patients in the control group had been administerd antiemetics more often than those in the experimental group. The levels of systolic and diastolic blood pressure and the pulse rates in the experimental group tended to decrease after the experiment. However, there was no statistically significant differences in anxiety, nausea, vomiting, blood pressure, and pulse between the groups. Conclusion: It is suggested to repeat this study with a enough sample size.
