The Clinical Effectiveness of Frequently Used Herbal Medicines for Cerebrovascular Disease: A Retrospective Study (뇌혈관질환에 대한 다빈도 처방 한약의 임상적 유효성 : 후향적 연구)

  • Objectives: With the aging of the population, the incidence of a variety of diseases is increasing. Cerebrovascular disease is a major cause of mortality worldwide and therefore requires timely treatment and management. We aimed to evaluate the clinical effectiveness of herbal medicines in treating cerebrovascular disease by screening the most frequently prescribed herbal medicines and analyzing the clinical data of patients with cerebrovascular disease taking these medicines. Methods: In this retrospective study, we analyzed the electronic medical records of patients with cerebrovascular disease who received herbal medication at the Pusan National University Korean Medical Hospital from 10 March 2010 to 31 March 2017. The inclusion criteria were as follows: diagnosis of cerebrovascular disease, use of frequently prescribed herbal medicines, available records of laboratory tests and functional evaluation that was conducted within two weeks before and after admission and discharge, and herbal medicine usage exceeding half the period of hospitalization. Results: Of the total patients with cerebrovascular disease, the number of patients analyzed in this study was 49. There were some changes in the various variables examined after frequently used herbal medicines had been taken, such as complete blood count, liver function test, lipid test, and functional evaluation. No serious adverse events related to herbal medicine were found in this study. Conclusions: We concluded that herbal medicines frequently used for cerebrovascular disease may have some clinical effectiveness. However, because of the retrospective nature of this study and only one hospital involved, the generalizability of our findings is limited.

  • It is often difficult to identify and localize intraorbital foreign bodies despite of modern high-resolution imaging investigation. Especially, posteriorly located foreign bodies have increased risks of morbidity that surgical approach is often complicated. No matter how trivial it seems, retained foreign body, particularly organic in nature, may give rise to severe orbital and cerebral complications. High clinical suspicion, proper diagnostic studies, timely referral to a skilled orbital surgeon are mandatory. We report a case of intraorbital wooden foreign body that required two separate exploration for removal. Initial exploration failed to identify and locate the foreign body completely. After the operation, fistula formation and purulent discharge were developed and the imaging investigation results were equivocal, complicating the management. A second exploration yielded multiple intraorbital wooden foreign body in the apex of orbit. The patient fully recovered without complication. The evaluations and the details of management strategy are discussed.

A Clinical Study of Hip Joint Pain Improved with Gihwangbaekho-tang (지황백호탕(地黃白虎湯)으로 호전된 고관절 통증 환자 치험례)

  • 1. Objectives The purpose of this study is to evaluate the effects of Gihwangbaekho-tang which is based on the Sasang Constitutional Medicine for the patient who has suffered from hip joint pain. 2. Methods The subject is about 86-year-old patient suffering from hip joint pain who was diagnosed as Soyangin interior over heated disease based on his Nature & Emotion, physical characteristics, symptoms, and we have prescribed Gihwangbaekho-tang. We used Visual analogue scale(VAS) for the assessment of Hip joint pain. 3. Results and Conclusions The patient was treated with Gihwangbaekho-tang and hip joint pain, thirst, redness of the face, constipation and general condition were improved. This study describes tile improvement of Hip joint pain and the change of symptoms by using Gihwangbaekho-tang. In this study, we applied Gihwangbaekho-tang to 86-year-old patient for about forty days, and Gihwangbaekho-tang didn't cause any serious side effect on the patient.

  • Miller Fisher syndrome(MFS) has been the focus of conflicting opinions regarding the peripheral versus the central nature of the site of major neural injury. We present our electrophysiological findings in one case of MFS to help clarify the pattern of peripheral nerve injury in this syndrome. A 45-year-old man visited our hospital due to sudden diplopia. Initial examination revealed internuclear opthalmoplegia. The next day, his symptoms rapidly aggravated to complete external ophthalmoplegia, ataxia, and areflexia with hand and foot numbness. Serial electrophysiological studies were performed. The results of brainstem evoked potential(BAEP) and blink reflex were normal in the serial studies. Motor and sensory nerve conduction study(NCS) were normal findings in second hospital day, but ulnar sensory nerve shows no sensory nerve action potential(SNAP) and sural sensory conduction velocity was delayed in 7th hospital day. Our patient's clinical presentation began to improve on 15th hospital day, and his electrophysiologic study showed improvement on 29th hospital day. We believe that all the manifestations of MFS can be explained by the involvement of peripheral nerves without brainstem or cerebellar lesion with the serial electrophysiological studies.

An Analytical Study on Medicine & Medical Affairs Law of Korea (우리나라 의료법규의 문제점과 개정방향)

  • The objectives of this research are (i) to review the legal nature of medicine & medical affairs law, (ii) to identify legal defects in terms of the adequacy and feasibility of its provisions and in terms of the structural order among related laws and acts, and (iii) to find out a rational way for revision of the law. Major findings of this research are as follows: 1) The main defects of the Medicine & Medical Affairs Law of Korea are shortcomings of provisions necessary for securing people's right as well as medical practitioner's responsibility and in adequacy to its post as a mother-law in medical fields. 2) Some provisions of the law are inconsistent with other laws and acts, especially with Health Insurance Law. 3) Medicine & Medical Affairs Law of Korea is required to be totally revised in order to keep up with situational changes and to function as a mother-law in its field. On the basis of these findings this study suggests that all the concerned parties would cooperate to renew the law into very sound feature of the mother-law in health field.

  • Through abdominal diagnoses, deficiency and excess, and cold and heat of the eight principles for differentiating syndromes can be determined, pathogens such as fluid retention, dry stool, retention of undigested food, abdominal mass, blood stagnation, stagnation of Gi, deficiency of Kidney Yang, and, spermatorrhea can be identified, nature and stages of the symptoms can be understood and then pathogenesis analyzed. Abdominal diagnosis can be one of primary factors in deciding treatment, expecting prognosis and treatment effect, and choosing herbal prescriptions. Representative herbs for epigastric stuffiness/fullness are Radix Ginseng, Pericarpium Citri, etc; for fullness of epigastrium, Rhizoma Pinelliae, Pericarpium Citri, Rhizoma Rhei, etc.; for severely rigid epigastrium, Radix Glycyrrhizae, Radix Ginseng, etc.; for epigastric pain, Rhizoma Pinelliae, Pericarpium Citri, etc.; for epigastric fullness and rigidity, Rhizoma Coptidis, Radix Ginseng, etc.; for feeling of obstruction in the epigastirum, Radix Bupleuri, Radix Ginseng, etc.; for palpitation in the epigastrium, Radix Glycyrrhizae, Ramulus Cinnamomi, etc. It is essential to rightly diagnose through comprehensive analysis of the data gained by the four methods of diagnosis, and in doing this, further studies on how to utilize abdomen diagnosis for clinical practice.

Basic Study on the Doin Gigong Exercise for the Reduction of Shoulder-arm Pain (견비통 완화 도인기공체조에 대한 기초적인 연구)

  • Gigong exercise is simple form's game for health care. Gigong exercise is the training method in mind and body, and it prevent or cure the disease by becoming larger the human's nature-healing power This syudy intends to identify the effective processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain. The processing channel of the Doin Gigong Exercise for the reduction of shoulder-arm pain cinsist of warming, main, finishing exercise. The warming exercise start breathing and concentrate upon shoulder-arm exercise. The upper and lower sides join-hands exercise give help smoothly the Gi-circulation of human body in every direction. The finishing exercise consist of experience to Gi with the object of maintaining human comfortable condition. As mentioned above, the Doin Gigong exercise is effective intervention for the reduction of shoulder-arm pain.

A Study on the Figures of Viscera (臟腑圖) in Sancaituhui (《三才圖會》 encyclopaedia illustrations about the all things in nature) by Wang Qi (王圻) of Ming-Dynasty (명대(明代) 왕기(王圻)의 《삼재도회(三才圖會)》 장부도(臟腑圖)에 대한 고찰(考察))

  • This study compared the figures of viscera (臟腑圖) in the seventh volume titled "Body" of Sancaituhui (三才圖會), the illustrated Encyclopedia published in the Ming Dynasty (明代), and the figures of viscera in Leijingtuyi (類經圖翼). One hundred and six volume Sancaituhui was compiled by Wang Qi (王圻) and his son Wang Siyi (王思義) in the Ming Dynasty. It was first published in 1607 and republished in 1609. Sancaituhui is somewhat different from other existing medical books in terms of form and content. Thus, this study examined the difference. Another comprehensive medical book, Leijingtuyi, was written by Zhang Jing-yue (張景岳) in 1624. Both Sancaituhui and Leijingtuyi were published in China before Terrenz's Taixirenshenshuogai (泰西人身說槪), the book which first introduced Western anatomy. Therefore, this study accessed the two medical books to examine the development of figures of viscera before the instruction of Western medicine.

Reference study for concept difinition of 'Seven emotions theory' (칠정학설천석(七情學說淺釋))

  • The theory of seven emotions is a unique theory in oriental medicine which describes the mutual relationship between body and mind of human. Although, the term 'Seven emotions' was not clearly indicated in ${\ulcorner}$The Yellow Emperor's Internal Classic(黃帝內經)${\lrcorner}$, it is appeared in ${\ulcorner}$A Treatise on the Three Catagories of Cause of Diseases(三因方)${\lrcorner}$ written by Chen Yan(陳言) in South-Song Dynasty. It seemed that Chen Yan explained seven emotions as the internal etiologic factor according to the classification of seven emotions of ${\ulcorner}$Ye-Gi(禮記)${\lrcorner}$ under the academic influence during Song Dynasy which emphasized more on the standard of right and wrong rather than individual emotion. Meditation or consideration modulates the function of spleen and stomach and the metabolism of blood and body fluid and it also controls the various emotions and maintains the equilibrium of human body. Human emotions are influenced by the changes of nature and deeply related to time and space including social-environmental factors. The function and strength of seven emotions: joy, anger, anxiety, worry, grief, apprehension and fright are determined by the external stimulation as the causes of illness.

Cytokeratin 20 negative Merkel cell carcinoma consistent with negative Merkel cell polyomavirus

  • Merkel cell carcinoma (MCC) is a rare neuroendocrine tumor that is highly aggressive in nature and indolent in progression. The common risk factors for MCC are senility, prolonged exposure to sunlight, and immune deficient states. Moreover, Merkel cell polyomavirus has recently been characterized to be significantly associated with pathogenesis of MCC, including the expression of Cytokeratin 20 (CK20). Diagnosis is often difficult since histopathological results require a number of differential diagnoses through immunohistochemical (IHC) stains with other cutaneous malignancies. A 67-year-old man presented with a solitary dome-shaped erythematous round mass on the left upper arm for 2 months. Biopsy and IHC studies revealed findings consistent with Merkel Cell Carcinoma of neuroendocrine origin. Common IHC stains usually confirm positive findings for CK20, which is also recognized as the key component in making the diagnosis. We present a CK20 negative MCC in light of expanding the knowledge of unusually stained IHC results in MCC.
