한국 저소득층 주민의 보건의료서비스 이용행태와 그 영향요인에 관한 연구 (A Study on Health Service Utilization and it's Determinants in the Low Income Family in Korea)

  • Objectives: The purpose of this study was to analyze health service utilization, and its related factors in low income families who earned half of the average Korean household income. Methods: This was a cross-sectional descriptive survey study in which a nationwide randomization sampling technique was used. The data were collected from July 12 to August 7, 1999, and the total sample size was 5,819 individuals, belonging to 1.753 households. Results: 1) In the utilization of health services for the last 3months, the pharmacy was the type of service that was the most utilized (32.0%), and the health center was the one that was the least utilized (10.3%). About 29% (29.2%) of the respondents could not utilize the health service at all. and 19.8% of the respondents terminated their medical treatments half way to completion because of financial difficulty (89.4%). 2) Analysis of the data using logistic regression showed that living with spouse, level of education, occupation, and income had statistically significant effects on health service utilization. Conclusion: The parameters of health care policies are equity and efficacy for health status, and the health service utilization by low income families. The conclusive resolution for these is the improvement of public health centers for an increased utilization rate of their services.

가습기 살균제 노출 실태와 피해규모 추산 (An Estimation of Population at Risk of Exposure to Humidifier Disinfectant and Associated Health Effects)

  • Objective: This study was designed to estimate the precise nationwide number of those who used humidifier disinfectants (HDs) and experienced specific health effects, including death, due to exposure to HDs in Korea between 1994 and 2011. Methods: A total of 5,000 households (15,472 people) were surveyed by stratifying the sample with a design that is equivalent to nationally approved household systematic sampling and face-to-face interviews from October 16, 2019 to December 30, 2019. Results: The proportion of use of humidifier disinfectants at home was 18.4% (2,844/15,472 people) and the proportion of those who experienced health effects among those who were exposed to HDs was 10.7% (303/2,844 people). Based on these factors, it was estimated that 8.94 million people (95% CI=8.25-9.63 million) were exposed to HDs and 0.95 million people (95% CI=0.87-1.02 million) experienced health effects, which means almost one in five and one in fifty Koreans respectively. The estimated number of deaths from HDs were 20,366 people (95% CI=18,801-21,931). Conclusion: The survey was large and analyzed a representative sample across the country, so this estimation of the nationwide population exposed and at risk to HDs is more accurate and reliable than previous studies. However, it is necessary to confirm whether results similar to those of this study can be reproduced through a survey using a cross-sectional survey at the national level. In particular, a survey focused on death cases is needed.

당뇨병 유병자의 혈당 조절 관련 요인 (Factors Related to Blood Glucose Control in Patients with Diabetes)

  • Objectives: This study was conducted to determine factors related to glycemic control among Korean patients with diabetes. Methods: Data was collected from the Fourth (2007-2009) Korea National Health and Nutrition Examination Survey (KNHANES) which was a nationwide, cross-sectional survey. The study participants who are available for hemoglobin A1c (HbA1c) were 1,441 patients with diabetes aged 30 years and older. Poor glycemic control was defined as $HbA1c{\geq}7.0%$. Data were analysed using logistic regression. Results: The proportion of patients with the poor glycemic control ($HbA1c{\geq}7.0%$) was 49.5%. After adjusting for confounding factors, younger aged persons (30-49 years) were less likely to control the blood glucose than older aged persons (60-69 years) (Adjusted OR=0.533, 95% CI [0.295-0.960]) and longer duration of diabetes was positively related to poor glycemic control (Adjusted OR=2.018, 95% CI [1.418-2.873]). Conclusions: Age and duration of diabetes were significantly related to blood glucose control. Therefore, considering these factors is helpful for developing a strategy to improve blood glucose control in diabetes.

전국 저소득층노인의 보건의료이용과 영향요인 분석 (A Study on Health Service Utilization for the Low Income Elderly in Korea)

  • The purpose of this study was to analyze the relation amongst health status, health care costs. health service utilization among the low income elderly who were 60 years of age or older, earning a half of the average Korean family income. The cross-sectional descriptive survey research we conducted used families randomly sampled nationwide. The data were collected from July 12 to August 7, 1999 and the total sample was 1.259 household members (421 households). These were the major findings. 1. As for the health status. 72.4% of respondents fell ill in the last 1 month; 54% of respondents had chronic disease. 2. As for the health care cost. the cost of hospitalization and the medical treatment were 1.069,000 won and 226.000won. respectively. 59.3% of respondents experienced a burden from the monthly health care expenses. 3. As for the health service utilization for the last 3months. 28.5% of respondents didn't utilize the health service. In addition, 22.2% of respondents gave up a medical treatment because of economic situation (88.8%). 4. The statistically significant determinants of health service utilization are old age, female, living with a spouse, unemployed state, medicare, and more days sick. 5. It is shown conclusively that equity and efficacy of the health care policy are to be considered for lower income older adult.

Association between dietary omega-3 fatty acid intake and depression in postmenopausal women

  • BACKGROUND/OBJECTIVES: This study aimed to analyze the association between dietary omega-3 fatty acid intake and depression in postmenopausal women using data from the Korea National Health and Nutrition Examination Survey (KNHANES) VI. SUBJECTS/METHODS: The KNHANES is a cross-sectional nationwide health and nutrition survey. Dietary data, including omega-3 fatty acids, were assessed using the 24-h recall method. Depression was evaluated using a survey questionnaire. The association between dietary omega-3 fatty acids and depression was evaluated using multivariate logistic regression analysis. Depression, according to the dietary omega-3 fatty acid intake, was expressed as the odds ratio (OR) with a 95% confidence interval (CI). A total of 4,150 postmenopausal women were included in the analysis. RESULTS: In the fully-adjusted model, the group with the highest dietary omega-3 fatty acid intake significantly showed lower prevalence of depression than the group with the lowest intake (OR, 0.52; 95% CI, 0.33-0.83); a significant linear trend was detected (P for trend = 0.04). According to the dose-response analysis using cubic restricted spline regression, this association was linear and monotonic (P for non-linearity = 0.32). CONCLUSIONS: In this study, the dietary omega-3 fatty acid intake in postmenopausal women was inversely proportional to depression in a dose-response manner. Large cohort studies are needed to verify the causality between omega-3 fatty acids and depression in Korean postmenopausal women.

도시 규모에 따른 남·녀 청소년의 흡연실태 비교 -대도시, 중소도시, 군지역 중심으로- (Urban-rural Disparities and Related Factors in Rates of Smoking by Korean Adolescents)

  • Purpose: The study was done to explore relationships between residential areas and smoking rates and to identify related factors contributing to smoking in Korea adolescents. Methods: An analysis was done of smoking rates and socioeconomic position indicators by city size based on a 2012 cross-sectional nationwide online survey conducted with 74,186 Korean middle and high school students aged 12-18 years old. Data were analyzed using x2-test and multiple logistic regression with the SPSS/WIN18.0 program. Results: Analyses revealed that rural boys were more likely to be current smokers compared to metropolitan boys (odds ratio 1.18, 95%-confidence interval 1.01; 1.38) but residential areas and smoking rates among girls were not related. After adjusting for covariates, results showed that city size, Family affluence score, economic status, parents' education level, living with parents, school type, and school achievement were related to increased an proportion of adolescents who smoked. Conclusion: In conclusion, rural living is a determinant of smoking among boys. Tobacco control programs should recognize differences in living conditions between rural and urban areas.

Cancer Screening Status in Korea, 2011: Results from the Korean National Cancer Screening Survey

  • This study was conducted to determine the use of screening for stomach, liver, colorectal, breast, and cervical cancers, which are included in the Korean National Cancer Screening Programme. In 2011 the National Cancer Centre in Korea conducted a nationwide, population-based, cross-sectional interview survey using multi-stage random sampling. Participants included 4,100 cancer-free men 40 years and over of age and women over 30 years of age. The lifetime screening rates for stomach, liver, colorectal, breast, and cervical cancers were 76.2%, 54.3%, 56.1%, 79.0%, and, 74.8%, respectively. The rates of recommended screening for stomach, liver, colorectal, breast, and cervical cancers were 64.6%, 22.9%, 35.3%, 60.4%, and 62.4%, respectively. More than 70% of all screening was attributed to organised cancer screening programmes. The main reason given for non attendance was 'no symptoms'. A greater effort is needed to increase screening rates, especially for liver and colorectal cancers.

  • We investigated the cancer screening rates for five types of cancer (stomach, liver, colorectal, breast, and cervix uteri) using data from the Korean National Cancer Screening Survey (KNCSS), which is a nationwide, annual cross-sectional survey. The eligible study population included cancer-free men 40 years of age and older and women 30 years of age and older. The lifetime screening rate and screening rate with recommendation were calculated. The lifetime screening rates for gastric, liver, colorectal, breast, and cervical cancers were 77.9%, 69.9%, 65.8%, 82.9%, and 77.1%, respectively. The screening rates with recommendation were 70.9%, 21.5%, 44.7%, 70.9%, and 67.9%, respectively. The most common reason for all types of cancer was "no symptoms", followed by "lack of time" and "fear of the examination procedure". Efforts to facilitate participation in liver and colorectal cancer screening among Korean men and women are needed.

여자 대학생의 식생활 실천 지침과 체성분에 관한 연구 (Body Composition of Female College Students through Practice by Standard Guideline for Korean Dietary Life)

  • To study the correlation between body composition values and practice of the Standard Guideline for Korean Dietary Life, a nationwide cross-sectional survey was conducted with 378 female college students living in four Korean cities in December 2005. Among the student subjects, 98 were analyzed for body fat mass, percent body fat, waist-hip ratio(WHR), body mass index(BMI) and obesity degree using Inbody 3.0(Biospace Co, Seoul, Korea). Percent body fat was at a high level of 54.1 %, WHR was at a high level of 58.2% and BMI was at a high level of 41.8%. However, both weight and body fat were below normal at 79.5% and 77.6%, respectively. Percent body fat, WHR, and BMI were slightly negatively correlated with vegetable consumption, but slightly positively correlated with beer intake. WHR > 0.8 was significantly negatively correlated with exercise. Standard Guideline for Korean Dietary Life was practiced to and above normal level by 64.6% of the female college students. However, female college students who are within the normal range of body composition value should notice that obesity will relate to abdominal or visceral fat on their body.

Dietary Risk Factors for Hypertension among Korean Adult Men

  • There have been many studies indicating increased salt intake is related to elevated blood pressure (BP). Hypertension and pre-hypertension are prevalent in Korea. A national survey showed that the Korean average daily salt consumption was 12.5g, which is more than twice the current recommendation in the UK or USA. This study was performed to understand which aspects of the Korean diet contributed to high salt intake and elevated BP in Korea. The subjects consisted of 1,110 Korean adult men aged 30 to 49 years who were selected from the data of 2001 Korean National Health and Nutrition Survey, a nationwide cross sectional survey. The relationship of BP with sodium and high sodium food intakes were analyzed. After categorizing subjects according to their BP, the socio-demographic information and food and nutrients intakes were analyzed. BP increased with a larger amounts of sodium intake and the trend was significant with diastolic BP. However, the trends of BP for sodium densities of the diet were not. People with hypertension significantly tended to be older, larger, and less educated than people with normal BP. And they consumed more foods from the fats, oils & sweets group and larger amounts of discretionary sodium than normal people. The current report advocates that public health intervention, which improves the awareness of the role of salt on elevated blood pressure for the public, should be implemented in Korea.
