Epidemiology of Oral Cancer in Asia in the Past Decade- An Update (2000-2012)

  • The prevalence of oral cancers (OC) is high in Asian countries, especially in South and Southeast Asia. Asian distinct cultural practices such as betel-quid chewing, and varying patterns of tobacco and alcohol use are important risk factors that predispose to cancer of the oral cavity. The aim of this review is to provide an update on epidemiology of OC between 2000 and 2012. A literature search for this review was conducted on Medline for articles on OC from Asian countries. Some of the articles were also hand searched using Google. High incidence rates were reported from developing nations like India, Pakistan, Bangladesh, Taiwan and Sri Lanka. While an increasing trend has been observed in Pakistan, Taiwan and Thailand, a decreasing trend is seen in Philippines and Sri Lanka. The mean age of occurrence of cancer in different parts of oral cavity is usually between 51-55 years in most countries. The tongue is the leading site among oral cancers in India. The next most common sites in Asian countries include the buccal mucosa and gingiva. The 5 year survival rate has been low for OC, despite improvements in diagnosis and treatment. Tobacco chewing, smoking and alcohol are the main reasons for the increasing incidence rates. Low socioeconomic status and diet low in nutritional value lacking vegetables and fruits contribute towards the risk. In addition, viral infections, such as HPV and poor oral hygiene, are other important risk factors. Hence, it is important to control OC by screening for early diagnosis and controlling tobacco and alcohol use. It is also necessary to have cancer surveillance at the national-level to collect and utilise data for cancer prevention and control programs.

  • Oral, cervical and breast cancers, which are either preventable and/or amenable to early detection and treatment, are the leading causes of cancer-related morbidity and mortality in India. In this paper, we describe implementation science research priorities to catalyze the prevention and control of these cancers in India. Research priorities were organized using a framework based on the implementation science literature and the World Health Organization's definition of health systems. They addressed both community-level as well as health systems-level issues. Community-level or "pull" priorities included the need to identify effective strategies to raise public awareness and understanding of cancer prevention, monitor knowledge levels, and address fear and stigma. Health systems-level or "push" and "infrastructure" priorities included dissemination of evidence-based practices, testing of point-of-care technologies for screening and diagnosis, identification of appropriate service delivery and financing models, and assessment of strategies to enhance the health workforce. Given the extent of available evidence, it is critical that cancer prevention and treatment efforts in India are accelerated. Implementation science research can generate critical insights and evidence to inform this acceleration.

중소 사업장 근로자의 치석제거 경험 관련요인 (The Associated Factors with Scaling Experience among Some Workers in Small and Medium-Sized Companies)

  • 본 연구는 중소사업장에 종사하는 생산직 근로자를 대상으로 치석제거 경험 관련요인을 알아보기 위해 수행되었다. 조사대상은 광주광역시 188개 중소기업 중 편의추출법으로 5개 사업장을 선정하여 사업장에 근무하는 근로자 455명을 대상으로 자가 보고형 설문조사를 통해 일반적 특성과 근무관련특성, 치석제거 경험 등을 조사하였다. 빈도분석, t-검정, 교차분석, 다중로지스틱 회귀분석을 이용하여 분석하였다. 대상자의 최근 1년 이내 치석제거 경험은 47.0%였으며, 근로자의 연령이 낮은 경우(aOR, 3.09; 95% CI, 1.60~5.96), 직위가 높은 경우(aOR, 2.68; 95% CI, 1.55~4.63), 구강건강에 대한 관심도가 높은 경우(aOR, 2.15; 95% CI, 1.02~4.52), 최근 1년간 정기적인 구강 검진 경험이 있는 경우(aOR, 2.76; 95% CI, 1.50~5.11), 치석제거 건강보험급여화를 인지하고 있는 경우(aOR, 2.91; 95% CI, 1.80~4.72)가 치석제거 경험의 가능성이 높았다. 결론적으로 치석제거 경험은 47.0%로 비교적 낮았으며, 치석제거 경험에 관련된 요인들은 연령, 직위, 사용 중인 구강관리보조용품, 정기적인 구강검진, 구강질환으로 인한 조퇴 경험, 치석제거 급여화 인지여부였다. 이러한 요인들을 고려하여 근로자의 근무환경 개선 및 치석제거의 인식도 향상을 통해 치석제거 경험률을 높인다면 향후 근로자들의 치주관리에 긍정적인 영향을 미칠 것으로 판단된다.

구순열의 초음파 진단 (Prenatal Sonographic Diagnosis of Cleft Lip)

  • The frequency of fetal malformations accounts for around 3-5% and evaluation of the health of the fetus and screening for fetal malformations has become an important part of prenatal care. Improvements in prenatal diagnosis have allowed identification of malformation in fetuses during first and second trimesters of pregnancy. Prenatal ultrasonography has become routine part of antenatal examination. For development of imaging, the accuracy of diagnosis is getting higher and earlier diagnosis of congenital malformation, such as cleft lip and palate, can provide to parent counseling, and opportunity to prepare the further treatment. For the better understanding of congenital cleft lip diagnosis to the oral and maxillofacial surgeons, as healthcare providers, we reviewed around 19 english-written articles and summarized some knowledges of ultrasound findings in the prenatal cleft lip fetus.

영유아 건강검진 시행 초기 1년의 결과 분석 (One-year evaluation of the national health screening program for infants and children in Korea)

  • 목 적 : 2007년 11월 15일부터 시작된 영유아 건강검진의 시행 일년간의 검진 결과를 분석하고, 설문조사를 통하여 수검자와 검진의사의 의견을 조사하여, 향후 영유아 건강검진의 개선 및 발전 방향을 알아보고자 하였다. 본 연구는 국민건강보험공단과 질병관리본부의 지원 하에 수행되었다. 방 법 : 2007년 11월 15일부터 2008년 10월 16일까지 4개월, 9개월, 18개월, 30개월, 5세 검진 대상자 총 2,729,340명의 검진 자료를 분석하였다. 수검자는 1,035명을 대상으로 전화 설문조사를 시행하였고, 262명의 검진의사를 대상으로 설문조사를 시행하였다. 결 과 : 전체 수검률은 35.3%이었다. 수검률은 월령이 증가하면서 감소하는 경향을 보였으며, 성별에 따른 수검률의 차이는 없었다. 구강검진은 6.9%의 수검률을 보였다. 소득이 가장 높은 그룹의 수검률이 가장 낮았고, 의료급여 수급자에서도 낮았다. 검진은 의원에서 82.6%의 수검이 이루어졌다. 4개월 수검자 중 9개월 재 수검자로 계산한 재 수검률은 57.3%이었다. 전체 수검자의 3.1%가 종합판정에서 유소견율을 보였으며, 소득분위가 낮은 그룹에서 유소견율이 높았다. 수검자 설문조사에서는 문진표가 어렵지 않다고 하였고(94%), 검진과정도 73%에서 대체로 만족할 만한 수준이라 응답하였다. 상담 시간 5-10분간 상담을 받은 수검자의 84.2%, 10분 이상 상담을 받은 수검자의 94-95%에서 상담시간에 만족한다고 답하였다. 응답자의 73.2%에서 상담과 교육이 도움이 되었다고 응답하였다. 건강교육은 77.1%에서 수검되었고 설명서도 73.7%에서 수취되었다. 검진의사는 결과 통보서를 작성하는 것에 어려움이 없었고(80%), 현 수가의 적절성은 51.1%가 매우 낮거나 낮다고 응답하였다. 검진의의 98.5%에서 현 검진이 어린이 건강증진에 도움이 된다고 하였으며, 94%가 앞으로도 지속적으로 참여하겠다고 응답하였다. 결 론 : 영유아 건강검진은 시행 첫 일년 동안 성공적으로 시작되었다. 향후 수검률의 향상과 질 관리가 필요하며, 소득분위가 낮은 계층에 대한 지원이 요청된다. 검진의 완전 전산화와 최소 5년 주기의 정기적인 가이드라인의 개정이 필요하다.

신생아-학령전기 대상자의 맞춤형 방문건강관리 기록지 및 모형 개발 (Development of Health Assessment Tools and Tailored Home Visiting Nursing Service Model for Children in Poverty)

  • Purpose: The purpose of this study was to develop child's health assessment tools and tailored home visiting nursing service model in a community. Methods: Based on the literature review and several types of workshops participated with the child health nursing professors and visiting nurses in public health centers from May to December 2009, the standards of child health assessment tools, service model and education materials for visiting nurses were developed. Results: Some record forms were newly developed, including neonatal assessment, breast feeding, mother-infant interaction, oral care, vaccination and safety, and appropriate developmental screening tests in the community were selected. For systematic health care management in the community, problem list, problem criteria, health care plan, outcome criteria were also developed. Conclusion: On the demand of growing need for health promotion and early intervention for children and their association with parenting and socioeconomic status, assessment and control measures are indispensable to the promotion of child health for vulnerable population. Children's health and developmental problems, and safe circumstances can be assessed using this assessment tools, and can be used for tailored home visiting nursing care for children.

수면호흡장애의 진단과 수면다원검사 (The diagnosis of sleep related breathing disorders and polysomnography)

  • Sleep related breathing disorders(SRBDs) are a group of diseases accompanied by difficulties in respiration and ventilation during sleep. Central sleep apnea, obstructive sleep apnea(OSA), sleep-related hypoventilation, and hypoxemia disorder are included in this disease entity. OSA is known to be the most common SRBDs and studies show its significant correlation with general health problems including hypertension, arrhythmia, diabetes, and metabolic syndrome. The diagnostic process of OSA is composed of physical examinations of the head and neck area and also the oral cavity. Radiologic studies including cephalography, CT, MRI, and fluoroscopy assist in identifying the site of obstruction. However, polysomnography(PSG) is still considered the gold standard for the diagnosis of OSA since it offers both qualitative and quantitative recording of the events during a whole night's sleep. The dentist who is trained in sleep medicine can easily identify patients with the risk of OSA starting from simple questions and screening questionnaires. Diagnosis is the first step to treatment and considering the high rate of under-diagnosis for OSA the dentist may play a substantial role in the diagnosis and treatment of OSA which will eventually lead to the well-being of the patient as a whole person. So the objective of this article is to assist dental professionals in gaining knowledge and insight of the diagnostic measures for OSA including PSG.

The Effect of Metabolic Syndrome on Periodontal Disease in Korean Adults: Based on the Data from the 6th Korea National Health and Nutrition Examination Survey (2013∼2015)

  • Background: The purpose of this study was to evaluate the association between metabolic syndrome and periodontal disease in Korean adults. Methods: This study analyzed the screening data of 12,686 adults aged ≥19 years, including demographic characteristics, lifestyle habits, and each component of metabolic syndrome, obtained from the sixth Korea National Health and Nutrition Examination Survey database. Periodontal health status was measured by the community periodontal index. Subjects with three or more risk factors were considered as having metabolic syndrome. The Rao-Scott chi-square test was performed to assess the relationships between demographic characteristics, lifestyle habits, components of metabolic syndrome, and periodontal disease. Logistic regression analysis was performed based on the complex sample to evaluate the relationship between metabolic syndrome and periodontal disease. Results: The prevalence of periodontal disease was higher among the subjects with advancing age, lower average household income and education level, those working in production, residents of eup-myeon areas, in past or current smoker, those with excessive alcohol consumption habit in a week, and reduced brushing frequency and the use of oral care products (p<0.001). Each component of metabolic syndrome was associated with higher prevalence of periodontitis in the subjects with abnormal than in those with normal levels (p<0.001). The prevalence of periodontal disease in subjects with metabolic syndrome was approximately 1.443 times higher than that in normal subjects (odds ratio,1.443; p<0.001). Conclusion: This study confirmed the association between metabolic syndrome and periodontitis in Korean adults, and further studies will be needed to determine the causal relationship between the two conditions.

Associated factors of depression in pregnant women in Korea based on the 2019 Korean Community Health Survey: a cross-sectional study

  • Purpose: Various individual and social factors influence depression in pregnant women. The purpose of this study was to identify the influence of socioeconomic status, health behaviors, and health status on depression of pregnant women in Korea. Methods: This study analyzed data from the 2019 Korean Community Health Survey conducted from August to October 2019. A structural questionnaire with Patient Health Quetsionnaire-9 (PHQ-9), health behavior, health status, and psychological characteristics was used. The data of 1,096 pregnant women between the ages of 19 and 55 years were analyzed using descriptive statistics, independent-test and chi-square tests, and multiple regression. Results: The mean score of prenatal depression as measured by the PHQ-9 during pregnancy was 2.35 points out of 0 to 27 points. Low income (B=0.69, p<.001), low-education level (B=0.70, p<.001), skipping breakfast (B=0.34, p=.001), less than 8 hours of sleeping (B=0.26, p=.009), binge drinking during pregnancy (B=0.46, p=.001), and stress (B=1.89, p<.001) were significantly associated with increased depression scores. In contrast, depression scores significantly decreased as subjective health status (B=-0.59, p<.001) and subjective oral health status (B=-.17, p=.003) increased. Conclusion: Findings support the need for healthcare policies and clinical screening to alleviate prenatal depression, especially for pregnant women with low socioeconomic status, poor health behavior, poor health status, and high stress.

치아 외과적 발거 전 혈액검사의 필요성과 이의 정책적 제시 (Needs of Preoperative Blood Sample Test in Surgical Extraction: Suggestion of New Policy)

  • Purpose: The third molar extraction is one of the mostly performed procedures in the department of oral and maxillofacial surgery. In most of dental clinic or hospital, the third molar extraction used to be frequently performed in an office-based surgery, and most patients did not have specific medical history with young ages. Medical history taking are dependent on the only way by asking to the patients about their individual conditions. Therefore, as the specialists of the oral and maxillofacial surgery in the field of dentistry, we suggest a new policy that the preoperative lab must be performed routinely before extraction of the third molar. Methods: This study is based on 1,096 patients who have been managed with third molar extractions, from March 2008 to September 2011 by a single surgeon. The preoperative lab, including complete blood count, coagulation panel, chemistry and serology, was performed before any surgical procedures. The results were informed to the patients regardless of their abnormalities, and any abnormalities related to the surgical procedures, such as platelet count and coagulation factors, were checked and corrected safely. Results: Through the preoperative blood test, systemic diseases that the patients had not recognized before, such as anemia, leukopenia, fatty liver and chronic renal disease, were identified. Patients with acute or chronic leukemia, Hepatitis B, and HIV positive, were also detected as a small number. Also, the possibilities of the cross-infection between dentists and patients or between patients and patients, and any other emergency situations can be prevented; as well as the public health condition can be improved, too. The patients were satisfied with low cost preventive blood test and high quality of medical services. Conclusion: Therefore, routine medical lab testing, including history taking are needed before an office-based minor surgery, such as third molar extractions, and these results were suggested as a new policy in the field of dentistry.
