Cancer Screening in Korea, 2012: Results from the Korean National Cancer Screening Survey

  • We investigated the cancer screening rates for five types of cancer (stomach, liver, colorectal, breast, and cervix uteri) using data from the Korean National Cancer Screening Survey (KNCSS), which is a nationwide, annual cross-sectional survey. The eligible study population included cancer-free men 40 years of age and older and women 30 years of age and older. The lifetime screening rate and screening rate with recommendation were calculated. The lifetime screening rates for gastric, liver, colorectal, breast, and cervical cancers were 77.9%, 69.9%, 65.8%, 82.9%, and 77.1%, respectively. The screening rates with recommendation were 70.9%, 21.5%, 44.7%, 70.9%, and 67.9%, respectively. The most common reason for all types of cancer was "no symptoms", followed by "lack of time" and "fear of the examination procedure". Efforts to facilitate participation in liver and colorectal cancer screening among Korean men and women are needed.

한 대학병원 위암 환자의 위험요인과 조기검진 효과 (Risk factors and effect of screening for gastric cancer in a university hospital)

  • 진행성위암의 위험요인을 파악하고 조기검진의 효과를 평가하고자 2007년 6월부터 2011년 12월까지 대전 소재 1개 대학교병원에서 위암으로 최초 진단과 수술을 받은 조기위암 171명과 진행성위암 환자 342명, 총 580명을 대상으로 설문조사와 의무기록 조사를 실시하였다. 두 군간에 카이제곱 검정과 로지스틱 회귀분석으로 교차비를 계산한 결과 진행성위암은 조기위암에 비하여 70세 이상에서(OR 2.393; 95%CI 1.329-4.310), 과거 흡연자(OR 1.612; 95%CI 0.970-2.680), 혈액형 A형(OR 1.784; 95%CI 1.035-3.075) 및 H. pylori균 감염(OR 1.699; 95%CI 0.905-3.191)에서 상대적으로 높았다. 또한 체중감소(OR 2.752; 95%CI 1.333-5.684)와 소화불량(OR 1.574; 95%CI 1.069-2.319)이 중요한 증상과 징후였다. 조기진단의 효과는 파악할 수 없었지만 조기진단이 성공적인 치료의 기회를 높이고 생존율을 향상시킨다는 것을 감안할 때 집단을 대상으로 조기검진을 고취시키는 교육과 선별검사가 요구된다.

혈청 펩시노겐치를 이용한 위암 및 위선종의 집단검진 (Serum Pepsinogen Levels as a Screening Test of Gastric Cancer and Adenoma in Korea)

  • To evaluate the validity of serum pepsinogen levels as a screening tool for gastric cancer and adenoma, immunoradiometric assays of serum pepsinogen I level (PG I), II level (PG II) and esphagogastroduodenal endoscopies were done in 757 health examinees. Serum PG I level was higher in subjects with active duodenal ulcer (n=45, $75.2{\pm}34.3{\mu}g/l(mean{\pm}standard\;deviation)$, p<0.01) and gastroduodenal ulcers (n=8, $75.6{\pm}19.8{\mu}g/l$, p<0.05), and was lower in those with gastric adenoma(n=4, $37.7{\pm}37.2{\mu}g/l$, p<0.2) than those with normal, mild gastritis findings or ulcer scars (n=378, $56.6{\pm}24.9{\mu}g/l$. Serum PG II level was higher in subjects with active duodenal ulcer($17.2{\pm}13.8{\mu}g/l$, p<0.2), active gastro-duodenal ulcers ($18.3{\pm}7.4{\mu}g/l$, p<0.2) and gastric carcinoma (n=3, $23.8{\pm}10.9{\mu}g/l$, p<0.05) than those with normal, mild gastritis findings or ulcer scars $(14.5{\pm}7.9{\mu}g/l)$. Serum PG I/PG II ratio was higher in subjects with active duodenal ulcer($5.1{\pm}1.6$, p<0.05) and was lower in those with chronic gastritis(n=107, $4.1{\pm}1.7$, p<0.05), gastric polyp(n=19, $3.9{\pm}1.4$, p<0.2), gastric adenoma(n=4, $2.1{\pm}1.9$, p<0.01) and gastric carcinoma(n=3, $2.7{\pm}1.2$, p<0.1) than those with normal, mild gastritis findings or ulcer scars ($4.5{\pm}1.7$). Serum PG II level increased with age until 6th decade, whereas serum PG I/PG II ratio decreased with age in 378 subjects with normal, mild gastritis findings or ulcer scars. The screening criteria of serum PG I<$70{\mu}g/l$ and PG I/PG II ratio<3.0 for detecting gastric cancer and adenoma gave a positive rate of 15.7%, sensitivity of 57.1% and specificity of 84.7%.

부산시 일개구 지역주민의 위암선별검사 수검 및 반복수검 실태 및 관련요인조사 (Stomach Cancer Screening and Repeat Screening Behaviors among Adults in an Urban Area)

  • The importance of repeat screening for stomach cancer is well known to decrease deaths from stomach cancer. This study was aimed at assessing practice behaviors and to identify related factors in the aspects of demographic factors, health status and cancer risk recognition, attitude to cancer screening, health behaviors, and inhibiting or facilitating factors to stomach cancer screening in an urban area. Data was collected through self-administered questionnaires from 403 people aged 40 to 69 years from April 23th to May 15th, 2002. Practice behaviors were classified as : "ever" or "never", and "repeat" or "not repeat" grouped based on a recent 5 year screening history. The results were as follows: 1. Among the 403 subjects: "the ever group" was 23.8% and "the repeat group" was 4.0%, The rates of screening were 20.5% for men, 27.8% for those women, and 27.3% for aged 40-49, 24.5% for those aged 50-59, 19.0% for those aged 60-69. The rates of repeat screening were 4.0% for men, 4.1% for women, and 2.9% for those aged 40-49, 6.3% for aged 50-59, 2.5% for those aged 60-69. 2. The main factors associated with adherence to gastric cancer screening were education(post high school vs below : OR=2.44), previous cancer screening(yes vs no : OR=2.61), belief in personal health(no vs yes : OR=2.72), health status(unhealthy vs healthy : OR=3.40), possibility of cancer compared to others(low vs not low : OR=2.56), and regular exercise(yes vs no : OR=2.94). The main factor associated with adherence to gastric cancer repeat screening was other cancer screening(yes vs no : OR=6.33). Consequently, there is a need to change the recognition of the importance and necessity of stomach cancer screening in healthy conditions through health education, and to perform multiple screening tests each visit.

일 광역단위에서의 사회경제적 수준과 위암 수검률과의 관련성 (The Relationship between Socioeconomic Status and Gastric Cancer Screening in the Population of a Metropolitan Area)

  • 이 연구에서는 위암검진 참여 현황을 파악하고, 수검률에 영향을 미치는 요인 중 사회경제상태 지표인 소득, 교육수준과 위암 수검률의 관련성을 알아보고자 하였다. 이 연구는 2009년과 2010년 전라남도 지역사회 건강조사 자료를 이용하였으며 40세부터 69세까지 21,220명을 대상으로 분석하였다. 사회경제상태와 위암 수검률과의 관련성은 로지스틱 회귀분석을 이용하여 분석하였다. 연령별 암 수검률은 40대 52.1%, 50대 63.7%, 60대 67.3%로 연령에 따라 증가하였다. 교육 수준이 높을수록 위암 수검률에 대한 교차비는 증가하였다(무학을 포함한 초등학교 졸업을 기준으로 중학교 졸업 OR=1.21; 95% CI=1.07-1.37, 고등학교 졸업 이상 OR=1.41; 95% CI=1.26-1.58). 또한 소득이 높을수록 위암 수검률에 대한 교차비도 증가하였다(2사분위 OR=1.14; 95% CI=1.02-1.28, 3사분위 OR=1.30; 95% CI=1.13-1.48, 4사분위 OR=1.62; 95% CI=1.44-1.84). 소득수준에 따른 위암 검진율은 연령대에 따라 다른 양상을 보였다. 이러한 차이는 40대에서 가장 높았다(40대 2사분위 OR=1.26; 95% CI=1.03-1.54, 3사분위 OR=1.49; 95% CI=1.20-1.86, 4사분위 OR=2.03; 95% CI=1.65-2.50, p for interaction=0.05). 낮은 사회경제상태는 낮은 위암 수검률과 관련이 있었으며, 특히 40대에서 사회경제상태에 따른 위암 수검률 차이가 크게 나타났다. 사회경제상태에 따른 위암 수검률 격차를 줄이기 위해서는 연령대별로 검진 형평성 달성을 위한 정책적, 환경적 지원이 요구될 것으로 보인다.

Single-incision Laparoscopic Gastrectomy for Gastric Cancer

  • The implementation of national cancer screening has increased the detection rates of early gastric cancer (EGC) in Korea. Since the successful introduction of laparoscopic gastrectomy for gastric cancer in the early 1990s, this technique has demonstrated improved short-term outcomes without compromising long-term oncologic results. It is associated with reduced pain, shorter hospitalization, reduced morbidity rates, better cosmetic outcomes, and equivalent mortality rates as those for open surgery. Laparoscopic gastrectomy improves patients' quality of life (QOL) and provides favorable prognosis. Single-incision laparoscopic gastrectomy (SILG) is one extremely minimally invasive method, theoretically offering improved cosmetic results, less postoperative pain, and earlier recovery after surgery than conventional multiport laparoscopic gastrectomy. In this context, SILG is thought to be an optimal method to promote and maximize patients' QOL in the acute postoperative phase. However, the technical difficulties of this procedure have limited its use. Since the first report describing single-incision distal gastrectomy in 2011, only 16 studies to date have evaluated SILG. Most of these studies have focused on the technical feasibility and safety of SILG because its long-term outcomes have not been reported. This article reviews the advantages and limitations of SILG.

Endoscopic Findings in a Mass Screening Program for Gastric Cancer in a High Risk Region - Guilan Province of Iran

  • Background & Objectives: Gastric cancer is a leading cause of cancer-related deaths in both sexes in Iran. This study was designed to assess upper GI endoscopic findings among people > 50 years targeted in a mass screening program in a hot-point region. Methods: Based on the pilot results in Guilan Cancer Registry study (GCRS), one of the high point regions for GC-Lashtenesha- was selected. The target population was called mainly using two methods: in rural regions, by house-house direct referral and in urban areas using public media. Upper GI endoscopy was performed by trained endoscopists. All participants underwent biopsies for rapid urea test (RUT) from the antrum and also further biopsies from five defined points of stomach for detection of precancerous lesions. In cases of visible gross lesions, more diagnostic biopsies were taken and submitted for histopathologic evaluation. Results: Of 1,394 initial participants, finally 1,382 persons (702 women, 680 men) with a mean age of $61.7{\pm}9.0$ years (range: 50-87 years) underwent upper GI endoscopy. H. pylori infection based on the RUT was positive in 66.6%. Gastric adenocarcinoma and squamous cell carcinoma of esophagus were detected in seven (0.5%) and one (0.07%) persons, respectively. A remarkable proportion of studied participants were found to have esophageal hiatal hernia (38.4%). Asymptomatic gastric masses found in 1.1% (15) of cases which were mostly located in antrum (33.3%), cardia (20.0%) and prepyloric area (20.0%). Gastric and duodenal ulcers were found in 5.9% (82) and 6.9% (96) of the screened population. Conclusion: Upper endoscopy screening is an effective technique for early detection of GC especially in high risk populations. Further studies are required to evaluate cost effectiveness, cost benefit and mortality and morbidity of this method among high and moderate risk population before recommending this method for the GC surveillance program at the national level.

[논문철회]위암 수술 후 외래환자의 영양상태 평가: 5가지 영양검색도구의 비교연구 ([Retracted]Assessing Nutritional Status in Outpatients after Gastric Cancer Surgery: A Comparative Study of Five Nutritional Screening Tools)

  • Objectives: This study aimed to examine the characteristics of patients according to their nutritional status as assessed by five nutritional screening tools: Patient-Generated Subjective Global Assessment (PG-SGA), NUTRISCORE, Nutritional Risk Index (NRI), Prognostic Nutritional Index (PNI), and Controlling Nutritional Status (CONUT) and to compare the agreement, sensitivity, and specificity of these tools. Methods: A total of 952 gastric cancer patients who underwent gastrectomy and chemotherapy from January 2009 to December 2012 at the Samsung Medical Center were included. We categorized patients into malnourished and normal according to the five nutritional screening tools 1 month after surgery and compared their characteristics. We also calculated the Spearman partial correlation, Cohen's Kappa coefficient, the area under the curve (AUC), sensitivity, and specificity of each pair of screening tools. Results: We observed 86.24% malnutrition based on the PG-SGA and 85.82% based on the NUTRISCORE among gastric cancer patients in our study. When we applied NRI or CONUT, however, the malnutrition levels were less than 30%. Patients with malnutrition as assessed by the PG-SGA, NUTRISCORE, or NRI had lower intakes of energy and protein compared to normal patients. When NRI, PNI, or CONUT were used to identify malnutrition, lower levels of albumin, hemoglobin, total lymphocyte count, total cholesterol, and longer postoperative hospital stays were observed among patients with malnutrition compared to those without malnutrition. We found relatively high agreement between PG-SGA and NUTRISCORE; sensitivity was 90.86% and AUC was 0.78. When we compared NRI and PNI, sensitivity was 99.64% and AUC was 0.97. AUC ranged from 0.50 to 0.67 for comparisons between CONUT and each of the other nutritional screening tools. Conclusions: Our study suggests that PG-SGA and NRI have a relatively high agreement with the NUTRISCORE and PNI, respectively. Further cohort studies are needed to examine whether the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

A Registry Program for Familial Gastric Cancer Patients Referred to Cancer Institute of Iran

  • Background: Gastric cancer is the second most common cause of cancer death. It has a poor prognosis with only 5-10% of hereditary etiology. If it is diagnosed, it could be helpful for screening the other susceptible members of a family for preventive procedures. Usually it is identified by symptoms such as presence of cancer in different members of family, some special type of pathology such as diffused adenocarcinoma, having younger age and multiple cancer syndromes. Hence, designing a registry program can be a more practical way to screen high risk families for a preventive program. Materials and Methods: Based on the inclusion criteria, a questionnaire was prepared. After pilot on a small number of patients, the actual data was collected from 197 patients and processed in SPSS 16.0. Results: Totally, 11.8% of the patients were younger than 45 years old. Blood type 'A' was dominant and males had a higher risk behavior with higher consumption of unhealthy food. Adenocarcinoma was reported in majority of cases. 21.8% of the patients had the including criteria for familial gastric cancer (FGC). Conclusions: The high percentage of FGC population compared to the other studies have revealed a need to design an infrastructural diagnostic protocol and screening program for patients with FGC, plus preventive program for family members at risk which could be done by a precise survey related to frequency and founder mutations of FGC in a national registry program.
