간호의 질 평가에 관한 국내 간호연구 분석 (The Analysis of Nursing Research in Korea in relation to the Evaluation of Quality of Nursing)

  • Purpose: The purpose of this study is to analyse research trends related to the evaluation of quality of nursing, thereby obtaining basic data relating to the identification of current situation of instrument development of quality of nursing, and of future research orientation and to the development of performance index of nursing organization. Method: The data were collected from June to December, 2002 through the review of total of 32 research papers which had evaluated quality of nursing among published papers in Korea from 1976 to August 2002. The analysis was done in terms of research period, the periodicals in which research papers were published, domains and the approaches of evaluation of nursing quality. The content analysis of lowest-level items amounting was performed using NIC developed by McCloskey & Bulecheck(1998) and consequential indices of quality of nursing developed by Chi(1995). Results: Twenty-three of 32 papers(71.8%) turned out to be published after 1995, indicating surging interest in the evaluation of quality of nursing from the mid 1990s. Also, ten of 12 research papers dealing with subjects' diseases were published after 1995. In content analysis of lowest-level items of the process-oriented evaluation of quality of nursing, the highest nursing intervention was patient education about procedure and treatment; followed in descending order by patient education about disease process, strengthening of communication, managing environment, infection control, admission care, defecation and urination care. In content analysis of lowest-level items of the outcome-oriented evaluation of quality of nursing, items of physical and psychological state, of patient and family satisfaction, of knowledge and home care, of change of patients' state, of addressing nursing issues, and of patient recovery were the outcome indicators in more than 60 percent research papers. Conclusion: The findings provided the foundation for their effective use in nursing practice with comparing and presenting various core evaluation items representing process and outcome domains.

인체 혈구암세포 U937에서 해양해면동물에서 추출된 Pectenotoxin-2에 의한 Apoptosis의 유발에 관한 연구 (Induction of Apoptosis by Pectenotoxin-2 Isolated from Marine Sponges in U937 Human Leukemic Cells)

  • 본 연구에서는 U937 인체 백혈병 세포의 증식에 미치는 PTX-2의 영향을 조사한 결과, PTX-2의 처리에 따라 U937 세포는 처리 농도 및 처리 시간 의존적으로 심한 형태적 변형과 함께 증식이 억제되었다. 이러한 PTX-2 처리에 의한 U937 세포의 증식억제는 apoptosis 유발과 관련이 있었으며, 이를 DAPI staining에 의한 apoptotic body 형성, flow cytometry를 이용한 sub-G1 세포 빈도의 정량적 분석을 통하여 확인하였다. 이러한 PTX-2 처리에 의한 U937 세포의 apoptosis 유발은 Bcl-2 family에 속하는 anti-apoptotic 인자인 Bcl-$X_L$의 발현 감소 및 IAPs family에 속하는 유전자들의 선택적 발현 감소와 연관성이 있음을 알 수 있었다. 이상의 결과들은 인체 암세포에서 PTX-2의 항암작용을 이해하는데 중요한 자료가 될 것이고 나아가 PTX-2을 포함한 그와 유사한 항암제 후보물질들의 연구에 있어서 기초 자료로서 사용될 수 있을 것으로 생각된다.

암 환자가 느끼는 경제적 도움에 대한 필요와 이에 영향을 미치는 요인 (Cancer patients' need for financial assistance and its related factors)

  • Background : Cancer is a disease that not only places a significant burden on patients clinically but also requires significant expense for diagnosis and treatment. Although the cancer coverage of health insurance has recently been expended, the need for financial assistance among cancer patients and their families is still expected to be significant. In this study, cancer patients' need for financial assistance in Korea was examined and its influence factors were analyzed. Methods : Target study subjects were those who are over 18 years of age and were diagnosed with cancer more than four months prior at the National Cancer Center and 9 Regional Cancer Centers in Korea during the period from July to August of 2008. Quarter sampling was conducted according to the ratio of the type of each cancer. A face to face interview survey was conducted. A total of 2,661 cancer patients finished the survey. Medical charts were reviewed in order to obtain the cancer type and SEER stage of cancer patients. An ordered logistic regression model was used to examine the level of need for financial assistance according to the demographical, clinical, and socio-economic variables of cancer patients. Result : The percentage of cancer patients who needed financial assistance was 69.0%, and 36.9% needed significant financial assistance. The need for financial assistance was perceived to be greater in males, younger age group, low income group, low education group, medical aid recipients, those who were diagnosed recently, those with a low level of quality of life measured through EQ5D, and those with decreased income after cancer diagnosis. Conclusion : In spite of the current policy to increase health insurance coverage, the majority of cancer patients and their families in Korea still need financial assistance due to cancer. In particular, there were more vulnerable groups, such as the low income, or low education group. In the future, policies that focus on the disadvantaged, which strengthen social security, should be considered for achievement of a substantially better quality of life for cancer patients and their families.

  • 식물 바이러스는 작물 수확량에 상당한 손실을 일으키고 작물 생산을 지속적으로 위협하여 세계 식량 안보에 심각한 위협이 된다. 그 중 tomato spotted wilt virus (TSWV)는 주로 원예작물을 감염시키는 가장 위협적인 식물 바이러스로 넓은 기주 범위를 가진다. Reverse-transcription quantitative real-time PCR (RT-qPCR)은 TSWV의 민감한 검출을 위해 널리 사용되고 있지만 표준화의 어려움으로 인해 유용성이 감소한다. 따라서 본 연구에서는 TSWV 검출을 위해 민감하고 정확한 reverse transcription droplet digital polymerase chain reaction (RT-ddPCR)을 확립하였다. TSWV 검출에 대한 RT-qPCR 및 RT-ddPCR의 민감도를 비교하였고, TSWV에 대한 RT-ddPCR의 특이성 분석은 고추에서 주로 발생하는 바이러스 및 음성 대조군에서 특이성을 확인한 결과 증폭되지 않았다. RT-ddPCR 및 RTqPCR에 의해 측정된 TSWV의 선형회귀곡선은 모두 높은 선형성을 나타냈지만, RT-ddPCR 분석이 10배 이상 더 민감하고 더 낮은 TSWV의 copy 수를 검출할 수 있었다. 종합적으로, 우리의 연구 결과는 RT-ddPCR이 TSWV 검출에 대해 높은 민감도와 특이성을 제공하고 낮은 농도의 현장 시료에서 TSWV 검출하는 데 적합하다는 것을 보여준다.

Effect of ciglitazone on adipogenic transdifferentiation of bovine skeletal muscle satellite cells

  • Ciglitazone is a member of the thiazolidinedione family, and specifically binds to peroxisome proliferator-activated receptor-γ (PPARγ), thereby promoting adipocyte differentiation. We hypothesized that ciglitazone as a PPARγ ligand in the absence of an adipocyte differentiation cocktail would increase adiponectin and adipogenic gene expression in bovine satellite cells (BSC). Muscle-derived BSCs were isolated from six, 18-month-old Yanbian Yellow Cattle. The BSC were cultured for 96 h in differentiation medium containing 5 µM ciglitazone (CL), 10 µM ciglitazone (CM), or 20 µM ciglitazone (CH). Control (CON) BSC were cultured only in a differentiation medium (containing 2% horse serum). The presence of myogenin, desmin, and paired box 7 (Pax7) proteins was confirmed in the BSC by immunofluorescence staining. The CL, CM, and CH treatments produced higher concentrations of triacylglycerol and lipid droplet accumulation in myotubes than those of the CON treatment. Ciglitazone treatments significantly increased the relative expression of PPARγ, CCAAT/enhancer-binding protein alpha (C/EBPα), C/EBPβ, fatty acid synthase, stearoyl-CoA desaturase, and perilipin 2. Ciglitazone treatments increased gene expression of Pax3 and Pax7 and decreased expression of myogenic differentiation-1, myogenin, myogenic regulatory factor-5, and myogenin-4 (p < 0.01). Adiponectin concentration caused by ciglitazone treatments was significantly greater than CON (p < 0.01). RNA sequencing showed that 281 differentially expressed genes (DEGs) were found in the treatments of ciglitazone. DEGs gene ontology (GO) analysis showed that the top 10 GO enrichment significantly changed the biological processes such as protein trimerization, negative regulation of cell proliferation, adipocytes differentiation, and cellular response to external stimulus. Kyoto Encyclopedia of Genes and Genomes pathway analysis showed that DEGs were involved in the p53 signaling pathway, PPAR signaling pathway, biosynthesis of amino acids, tumor necrosis factor signaling pathway, non-alcoholic fatty liver disease, PI3K-Akt signaling pathway, and Wnt signaling pathway. These results indicate that ciglitazone acts as PPARγ agonist, effectively increases the adiponectin concentration and adipogenic gene expression, and stimulates the conversion of BSC to adipocyte-like cells in the absence of adipocyte differentiation cocktail.

대도시·중소도시·읍면지역 및 동일한 지역내에서의 소득수준에 따른 노인의 영양소 섭취 현황: 국민건강영양조사 2016-2018년 자료 활용 (Nutrient Intake Status of the Elderly in Metropolitan, Middle & Small Cities, and Rural Areas according to Income Level within the Same Region: Korea National Health and Nutrition Examination Survey (2016-2018))

  • There is little information on the nutrient intake according to the city size and small town in Korean elderly. This study analyzed the nutritional consumption of older people in metropolitan, middle and small cities, and rural areas according to four income levels. The recent data from the 2016~2018 Korean National Health and Nutrition Survey, Centers for Disease Control and Prevention were used. The final analysis included 4,325 individuals (Male: 1,856, Female: 2,469) over 65 years old. Multivariable regression with a complex sample design was conducted to compare the nutrient intake among the groups. In a comparison within regions, the nutrition status of the elderly in small towns was more vulnerable than metropolitan and middle & small cities. The energy intakes were similar between the groups. The carbohydrate intake of middle & small cities was significantly higher than the other regions. The intake of other nutrients in metropolitan and middle & small cities appeared to be higher than in rural areas. The number of nutrients with statistical significance between low and high-income levels were 19 in metropolitan, 11 in middle & small cities, and 5 in rural areas. Each contribution of carbohydrate, fat, and protein to the total energy intake was lower in the low-income level than the high-income level in metropolitan and middle & small cities. On the other hand, in rural areas, only the contribution of protein to energy intake was lower in the low-income level than the high-income level. Cities with higher levels of urbanization had more severe nutritional inequality in relation to the income level. There was also nutritional inequality present in rural areas but it was to a lesser extent. Moreover, the generally low level of nutrient intake was problematic in rural areas. These findings could be used as fundamental evidence for developing community nutritional policies for the elderly.

엽산: 2020 영양소 섭취기준과 한국인의 영양상태 (Folate: 2020 Dietary reference intakes and nutritional status of Koreans)

  • 엽산은 비타민 B군에 속하는 수용성 비타민으로, 핵산 합성과 아미노산 대사에서 단일탄소를 전달해 주는 반응의 조효소 역할을 한다. 엽산은 새로운 세포가 형성되어 성장하는 임신기와 성장기에 매우 중요한 영양소이며, 여성의 임신 전 적절한 엽산 영양상태는 신경관 결손증을 예방한다고 알려져 있다. 이 외에도 엽산 섭취 부족은 빈혈, 고호모시스테인혈증, 심혈관질환, 암, 인지 장애, 우울증 등 다양한 질병과도 관련이 있다고 보고되어, 엽산은 전 생애주기 동안 건강을 유지하기 위해 충분히 섭취해야 하는 영양소이다. 본 연구에서는 2020 한국인 엽산 섭취기준의 개정 근거를 살펴보고, 국민건강영양조사로부터 엽산 섭취량과 혈청 엽산 농도를 성별, 연령대별로 분석하였으며, 향후 엽산 섭취기준 개정에 참고할 만한 내용을 제언하였다. 표준체중의 변경에 따라 영아 후기의 충분섭취량과 15-18세의 평균필요량이 2015년과 달리 변경되었으나, 권장섭취량과 상한섭취량에는 변화가 없었다. 2016-2018년 국민건강영양조사 결과에서 대부분의 연령에서 엽산 섭취량은 권장섭취량에 미치지 못하였으며 특히 15-29세 여성의 섭취량이 권장섭취량 대비 매우 낮았다. 임신부와 수유부의 엽산 섭취량도 권장섭취량 대비 60% 이하로 낮았으나, 혈액수준은 다른 연령층에 비해 높아 보충제를 섭취한 결과로 보인다. 앞으로 국민건강영양조사에서 보충제 섭취량도 조사해야 할 것이며, 엽산의 섭취량 평가를 위해서는 생식품, 조리된 식품, 강화식품 중의 엽산 함량에 대한 DB가 구축되어야 할 것이다. 또한 혈청 엽산 뿐 아니라 적혈구 엽산 농도와 혈장 호모시스테인 농도도 분석할 필요가 있으며, 분석방법에 대한 질 관리가 필요하다.

한국성인의 제2형 당뇨병 유무에 따른 체액 조성 차이 및 영양소 섭취량 분석 (Analysis of difference in body fluid composition and dietary intake between Korean adults with and without type 2 diabetes mellitus)

  • 본 연구는 한국인을 대상으로 제2형 당뇨병 환자와 질병이 없는 정상대조군을 비교하여 성별에 따른 체액 불균형 정도와 영양소 섭취량을 평가하고자 하였다. 조사대상자는 총 57명으로, T2DM이 있는 T2DM군 (n = 36)과 T2DM이 없는 정상대조군 (n = 21)으로 나누어 분석하고, 성별에 따른 체액량의 차이를 고려하여 남성 T2DM군 (n = 24), 남성 정상대조군 (n = 9), 여성 T2DM군 (n = 12), 여성 정상대조군 (n = 12)으로 세분화하여 지표들의 특징을 분석하였다. BIA를 통하여 연구대상자의 체액 조성을 분석한 결과, ECW/ICW는 T2DM군이 정상대조군보다 유의적으로 높았고, 부종지수를 나타내는 ECW/TBW는 T2DM군이 정상대조군보다 유의적으로 높게 나타났다. 성별에 따른 체액 조성을 분석한 결과, 남성과 여성 모두 T2DM군은 정상대조군보다 높은 ECW/ICW, ECW/TBW를 보이며 동일한 체수분율 양상이 나타났다. 식사일기를 통해 연구대상자의 영양소 섭취량을 분석한 결과, 1,000 kcal 당 탄수화물, 식이섬유, 비타민 A, 비타민 C, 나트륨, 칼륨 섭취량은 T2DM군이 정상대조군보다 많았고, 1,000 kcal 당 지방, 콜레스테롤 섭취량은 T2DM군이 정상대조군보다 적었다. 본 연구를 통해 T2DM 환자에게서 체액 불균형이 나타날 가능성이 높음을 알 수 있었으며 추후 당뇨 합병증의 평가, 진단의 기초 자료로 널리 활용될 것으로 사료된다.

소양호, 남양호, 안동호 및 춘천호에 서식하는 잉어과 어류의 간흡충 Clonorchis sinensis 감염 조사 (Infestation Status of Clonorchis sinensis Metacercariae in Cyprinid Fish from four Artificial Lakes in South Korea)

  • 간흡충(Clonorchis sinensis)은 우리나라에서 인체 감염율이 가장 높은 기생충이며, 이는 담수어류를 중간숙주로 삼는다. 따라서 강 유역에 서식하는 담수어류를 대상으로 간흡충의 메타세르카리아(Clonorchis sinensis metacercariae, CsMc)가 감염되어 있는지를 조사한 연구가 많이 수행되었으나 호수에 서식하는 물고기를 대상으로 한 연구는 거의 없다. 본 연구에서는 주요 인공호수인 소양호, 남양호, 안동호, 춘천호에서 2016년부터 2017년까지 잉어과어류를 채집하여 CsMc의 감염 여부를 조사하였다. 이생흡충류의 메타세르카리아는 남양호에서 채집한 치리 및 붕어와, 소양호에서 채집한 피라미와 끄리에서 관찰되었다. 간흡충에 특이적인 프라이머를 사용하여 PCR을 수행한 결과, 남양호의 치리에서 관찰된 것이 CsMc 이었음을 확인할 수 있었다. 본 연구는 주요 인공호수에 서식하는 잉어과어류에서 CsMc의 감염 실태에 대한 정보를 제공하고 있으며, 남양호의 치리에서 CsMc 감염을 처음으로 보고하고 있다는 것에 의의가 있다.

Integration and Reanalysis of Four RNA-Seq Datasets Including BALF, Nasopharyngeal Swabs, Lung Biopsy, and Mouse Models Reveals Common Immune Features of COVID-19

  • Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndromecoronavirus-2 (SARS-CoV-2), has spread over the world causing a pandemic which is still ongoing since its emergence in late 2019. A great amount of effort has been devoted to understanding the pathogenesis of COVID-19 with the hope of developing better therapeutic strategies. Transcriptome analysis using technologies such as RNA sequencing became a commonly used approach in study of host immune responses to SARS-CoV-2. Although substantial amount of information can be gathered from transcriptome analysis, different analysis tools used in these studies may lead to conclusions that differ dramatically from each other. Here, we re-analyzed four RNA-sequencing datasets of COVID-19 samples including human bronchoalveolar lavage fluid, nasopharyngeal swabs, lung biopsy and hACE2 transgenic mice using the same standardized method. The results showed that common features of COVID-19 include upregulation of chemokines including CCL2, CXCL1, and CXCL10, inflammatory cytokine IL-1β and alarmin S100A8/S100A9, which are associated with dysregulated innate immunity marked by abundant neutrophil and mast cell accumulation. Downregulation of chemokine receptor genes that are associated with impaired adaptive immunity such as lymphopenia is another common feather of COVID-19 observed. In addition, a few interferon-stimulated genes but no type I IFN genes were identified to be enriched in COVID-19 samples compared to their respective control in these datasets. These features are in line with results from single-cell RNA sequencing studies in the field. Therefore, our re-analysis of the RNA-seq datasets revealed common features of dysregulated immune responses to SARS-CoV-2 and shed light to the pathogenesis of COVID-19.
