보건소 근무 한의사의 법.제도적 지위와 역할에 관한 연구 (A Study on the Legal and Institutional Position and Role of Korean Medicine Doctors working at Public Health Center)

  • Objective : We proposed fundmental rules of prospective on legal and institutional position and role of Korean medicine doctors working at public health center. Methods : By the result of this research on the current situation, the grade and allowance given to the Korean medicine doctors working at public health center were different every self-governing body. Results : The reason the Korean Medicine Doctor can't serve as a regular order of 5th grade is that the 'The Enforcement Regulation about Administrative Organization and the Standard of Pixed Number of person of Self-Governing Body(지방자치단체의 행정기구와 정원기준등에 관한 규정 시행규칙)' prescribes the number of regular order of 5th grade is regulated within 7% among the number of regular order officials. But not appointing to office as the regular order of 5th grade infringes on the Constitution, the highest law. The reason the Korean Medicine Doctors can't be appointed to office as the regular order officials by the self-governing body is that 'The Enforcement Order of the Law of Preservation of good health of Local Area(지역보건법시행령)' prescribes the Korean Medicine Doctors are not indispensable to Public Health Center. But in fact, the Korean Medicine Doctors can execute many kinds of work such as medical examination or instructing house nursing. Conclusion : The Korean Medicine Doctors working at Public Health Center serve at low positions as daily use or common use, not receiving a regular order. All laws including the Constitution(헌법), the Medical Services Law(의료법), the Law of Preservation of good health of Local Area(지역보건법), the National Public Service Law(국가공무원법), the Local Public Service Law(지방공무원법) and the Law of Higher Education Law(고등교육법) describe that the Korean Medicine Doctors and the Western Medicine Doctors are equal to their position and right.

우리나라 의사 국가시험 필기시험(2016-2018)의 기초의학 역량 평가 현황의 분석 (Analysis of Basic Medicine-Related Questions in the Korean Medical Licensing Examination (2016-2018))

  • Basic medical education is important for developing the competencies of medical doctors, and it includes basic biomedical sciences, preventive medicine, medical ethics, and clinical science. This study aimed to reveal the current status of the Korean Medical Licensing Examination (KMLE) regarding its evaluation of competencies in basic biomedical sciences. The basic medicine-related questions were screened and selected from the test forms of the KMLE (2016-2018) by personnel conducting basic biomedical science education, and the selected questions were analyzed by three independent groups of undergraduate students at Chonnam National University Medical School in terms of the learning outcomes of basic medical education. The study scope includes the proportion of basic medicine-related questions, which consist of basic medicine questions and basic medicine-related clinical medicine questions, its annual change, discipline distribution, and associated learning outcomes. The average proportions of basic biomedical sciences, preventive medicine and medical law, and clinical sciences were 2.3%, 5.8%, and 91.9% of all questions, respectively. The proportion of basic medicine-related questions, except those on preventive medicine and medical law, was 22.0% of the total, and questions on pharmacology and microbiology accounted for 83.0% of the basic medicine-related questions. The proportion of sub-enabling learning outcomes linked with basic medicine-related questions comprised 14.0% of the total outcomes for basic biomedical sciences and 30.4% for preventive medicine and medical law. It is concluded that the KMLE questions may not sufficiently cover the essential competencies of basic medical education for medical doctors, and the KMLE may need to be improved with regard to competencies in basic biomedical sciences.

공중보건의의 고혈압 진단 및 치료과정 평가 (Quality Evaluation for the Diagnosis and Management of Hypertensives by Pubilc Health Doctors)

  • Background : Little work has been carried out regarding quality assessment research in a primary care setting, comparing with that of hospitals. This study aims to evaluate the process of diagnosis and management of hypertension by public health doctors on the basis of pre-established clinical guideline, and to identify several modifying factors associated with them. Methods : Hypertension was selected as the target disease, because it is a chronic disease which is of great public health importance. Self-administered questionnaires were mailed to public health doctors practicing at health centers and health subcenters across the nation. The response rate was 20.9%. The questionnaire included the diagnosis and management process such as measuring blood pressure, history taking, physical examinations, and treatment approches and potentially modifying factors such as level of training, duration of practice as a public health doctor, and education on management of hypertension. Results : Public health doctors pay little attention in measuring BP, hypertension related history taking, performing physical examination and laboratory examination. But they devoted much effort in diagnosing hypertension exactly and giving nonpharmacological treatment. Among various antihypertensive drugs, calcium-channel blockers were the most preferred agent(50.9%). Level of training, duration of practice ad a public health doctor, and education on management of hypertension made no difference on quality of care(p>0.05). Conclusion : These public health doctors showed poor compliance with the pre-established clinical guidelines, which leaves much to be desired in diagnosing and managing hypertensive patients by public health doctors. This study might be able to contribute to develop some strategies, such as educational programs, which would be able to improve the process of care in hypertensives.

국내 뜸 요법 임상 실태 파악을 위한 전화조사 (Telephone survey for grasping clinical actual stage of moxibustion therapeutics in Korea)

  • Objectives : Moxibustion therapeutics is one of the most popular oriental treatments in Korea. In this study, we operate the Telephone Survey for grasping clinical actual state moxibustion therapeutics in Korea. Methods : Survey questions were developed based on consensus of acupuncture professors. The list of the Korean medical doctors with experiences more than 10 years is provided by the Association of the Korean Oriental Medicine. A stratified random sample of Korean medical doctors is drawn for the telephone interviews. We choose a bound on the error of estimation equal to 6.5 percentage, and the sample size is 260 for the national sample. Telephone interviews with them were conducted by the well-trained interviewers of Korea Institute of Oriental Medicine in Medical researcher from 26th March 2007 to 6th April 2007. Results : Ninty -four percents of Korean oriental medical doctors were male and most commonly, clinical experience of doctors were 20-29 years(47.3%). Sixty-seven percent of Korean oriental medical doctors used moxibustion therapeutics. The most common treatment disease was Musculo-skeletal disorder(38.3%), Digestive disorder(28.6%), Gynecology(14.1%). Indirect moxibustion were as frequent as 65.5% of moxibustion method. The most common reason of unused respondents was 'Lots of smell and smoke'(28.3%), 'The wound left a scar'(20.8%), 'Less effects'(20%), etc. Eighty-three percents Korean oriental medical doctors were against that moxibustion therapy used without doctor's examination Conclusions : This survey provides unique insight into the perception of the Korea medical doctor at moxibustion therapeutics. Future research need to provide more in-depth insight into doctor views of the experience.

북한 보건의료인력개발을 위한 제언 (A Proposal for the Development of Personnel in the DPRK for Public Health and Medicine)

  • The government of South Korea and its medical personnel must make a way by which health professionals who have escaped from the Democratic People's Republic of Korea (DPRK) can play a positive and practical role in unification and south-north medical unification while south-north authority talks on DPRK public health and medicine manpower development are not going smoothly. Medical personnel escaped from the DPRK have to be recruited for the interviewer of the national examination, to improve the accuracy of national examination interviews. For those medical professionals who have escaped from the DPRK with 6 years' medical college education, but failed the interview on the national examination, we propose here a course of 3.6 months for them to have a right to apply the Korean Medical Licensing Examinations (KMLE). We also propose that medical professionals who have escaped from the DPRK who have graduated from a 6-year medical college in the DPRK and who are medical doctors over the fifth grade or with more than 6 years of experience can be qualified as unification medical doctors and be exempted from the KMLE, getting the right to go directly into an internship and residency. They should be permitted to work in manpower development projects for the health professions. They should also be given opportunities such as to become psychiatrists who treat the mental illness of persons escaped from the DPRK and people from North Korea after unification. Medical students in South Korea should earn college credits on the topic of medical unification and not only students, but all South Korean medical personnel, should prepare for north-south medical unification with an open mind. A way for each medical college to participate in DPRK manpower development for the health professions through a memorandum of understanding between the medical colleges of the south and north.

한방 영유아 건강검진 도입 필요성 연구 (Research for Health Examinations for Infants and Children by Korean Medicine)

  • Objectives This study is to analyze current guidelines for health check-ups in infants and children, and to develop better guidelines based on oriental medicine. Methods We analyzed The Manual of 2015's Health Examination for Infants and Children which is a Korean Developmental Screening Test for Infants and Children (K-DST). The statistics from the test was collected from 2010 to 2014 to figure out the problems of Health Examinations for Infants and Children. 20 articles from 2000 to 2014 from RISS, NDSL were also analyzed. Results The current guidelines for health examinations in infants and children didn't include major pediatric diseases such as allergy and asthma. Also, the pediatrics health education materials were mainly focused on hypernutrition but not so much on nutrition deficiency. The number of the centers for Health Examinations for Infants and Children was 52.2% of the number of NIP-Participating medical institutions for infants and children. Conclusions Oriental medicine can be useful to prevent major pediatric diseases and to promote general health in pediatrics. By legislating 'Geon-a-beop', which is a law for infants and children, we can increase the number of medical centers for infants and children to get health check-ups. Currently, there are 9,769 Korean medical institutions, and the rate of traditional medical doctors of public health doctors was 25.5%. Weak Children Questionnaire is developed, considered to be more useful when it is developed with 5-point scale rather than 2.

컴퓨터기반시험에 대한 한의과대학 학생들의 인식 및 만족도 조사 (A Survey on the Perception and Satisfaction of Korean Traditional Medical Students on Computer-Based Test)

  • Objectives : Computer-based test (CBT) will be introduced in the national examination of Korean medicine doctors from 2023. This study was aimed to provide some information, including prior considerations, to those preparing for CBT, by presenting a summary of the survey results from participants in a CBT simulation test. Methods : In December 2020, applicants from the graduating class of the college of Korean medicine from four universities participated in the simulation test for CBT. An online anonymous survey on the applicants' preference for CBT versus paper-based tests (PBT) was conducted. The questionnaire consisted of multiple-choice questions on a 5-point Likert scale and free-form questions. Results : A total of 176 students responded to the survey. Their response to CBT was generally positive. The item with the highest agreement was that it should provide a sufficient opportunity to experience CBT in the undergraduate exam (4.38±0.839). Concerns about the possibility of cheating or poor concentration were also detected. In addition to guaranteeing the stability of the CBT system, there were many comments expressing the need to supplement functions within the CBT system, including the memo or highlighter function. Conclusions : This study confirmed the feasibility of CBT including multimedia questions in the field of Korean medicine education and identified overall positive responses from those who experienced the CBT simulation test. Although time is short and there is so much to prepare, we hope this will be a leap forward for effective educational innovation in line with the changing environment of the health care education field.

인문사회의학의 의사국가시험 도입에 대한 인식도 조사 (A Survey on the Introduction of Medical Humanities and Sociology into the National Medical Licensing Examination)

  • Purpose: This study aimed at investigating the current situation of Medical Humanities and Sociology-related-curriculum in Korean medical schools, and suggesting the courses of study for the contents and methods of the Medical Humanities and Sociology examination, which can be included in the National Examination for Medical Practitioners. Methods: We analyzed Medical Humanities and Sociology-related courses which are offered in Korean medical schools, and a survey was conducted by medical school professors and students and medical journalists. In the survey, the Medical Humanities and Sociology-related courses were divided into 8 parts, and the participants were asked to evaluate the importance of duty, necessity of education, necessity of evaluation and the evaluation method of each part using a seven-point scale. Results: A total of 207 medical school professors and students and 9 medical journalists participated in the survey. The results were similar for the importance of duty and necessity of education of each part, but those for the necessity of evaluation were different. - As a result, there seems to be a gap between the importance of duty and the importance of education of each course. Medical journalists and students group answered differently on the necessity of evaluation of each course was also reserved. Conclusion: It is necessarily recommended to include Medical Humanities and Sociology-related courses such as medical ethics, self-improvement and doctors' social responsibilities in the National Examination for Medical Practitioners.

주관적 체형인식이 체중조절방법 선택에 미치는 영향: 2012년 국민건강영양조사 자료를 이용하여 (How Does Body-Shape Perception Affect the Weight Control Practices?: 2012 Korea National Health and Nutrition Examination Survey)

  • Objectives: This study was to investigate how body-shape perception could influence to weight control practice both in normal and obese group. Methods: We used 2012 Korea National Health and Nutrition Examination Survey to analysis 1) weight control practices of population; 2) consistency between body-shape perception and body mass index; 3) comparison weight control practices between normal group and body mass index (BMI) obese group in perceptional obese group; 4) odds ratio of BMI obese group using herbal drugs for weight control practice in perceptional obese group. Results: We found that study population tends to choose exercise, dietary restriction, meal skip, health functional food, one-food, drug, herbal drug, fasting and self-medication in order of frequency to control weight. The agreement between body-shape perception and BMI within obese group was approximately 64% with 0.40 of Cohen's Kappa coefficient, ranging from 0.384 to 0.423. Within perceptional obese group, choosing each weight control practice methods ratios between normal BMI group and obese BMI group were not significantly different. Within perceptional obese group, obese BMI group showed significant odds ratio (2.58, 95% confidence intervals, 1.38~4.85) than normal BMI group in choosing herbal medication for weight loss when adjusting other variables. Conclusions: We concluded that body-shape perception might be an important factor for choosing weight control program, and roles of Korean medical doctors thought to be enhanced for using herbal medication for weight loss.

검시제도(檢視制度)의 개혁(改革) 방안(方案) - 검시 관여자(檢視 關與者)의 전문성(專門性)을 중심으로 - (Reform of Medicolegal Death Investigation System in Korea - On the Professionalism of involved Personnel -)

  • The reform necessity of medicolegal death investigation system is continuously issued. The problems from the current death investigation system are discussed, specially on the professionalism of the involved personnel such as policemen, doctors, prosecutors. Death investigation exists not only to prosecute the criminals but also primarily to protect the general public's health, safety, and welfare. The reform proposals of death investigation system are followed as below. Statutes require that the prosecutor be notified of certain deaths. All deaths that may reasonably result from anything other than natural disease should be investigated. A death certificate is a legal document which authenticate a death, therefore it is issued by the doctor. The postmortem examination must be rearranged on the university base and performed by the qualified doctors who are trained at least in pathology. The police officer specified in death investigation and forensic identification should be encouraged to keep doing their special work in many ways.
