골격성 제III급 부정교합 환자에서 상악골 전방견인장치 사용후 측모 변화에 대한 연구 (A STUDY ON PROFILE CHANGE OF SKELETAL CLASS III MALOCCLUSION PATIENTS AFTER WEARING PROTRACTION HEAD GEAR)

  • 성장기 골격성 제III급 부정교합의 원인은 상악 열성장, 하악 과성장과 두 양상의 조합형태로 나뉠 수 있다. 이중 상악 열성장을 동반한 성장기 골격성 제III급 부정교합의 경우, 상악골 전방견인 장치의 사용이 추천되고 있는데, 이러한 장치의 치료효과는 상악골의 전하방 이동과 하악골의 후하방 회전으로 알려져 있다. 국내외 선학들에 의해 상악골 전방견인 장치의 치료효과에 대한 많은 동물실험과 임상실험 결과가 발표되었으나, 연조직 측모 변화에 대한 연구는 미비한 상태이고, 또한 치료를 받은 환자간의 치료효과에 대한 비교도 부족한 상태이다. 이에 저자는 상악골 열성장으로 인한 성장기 골격성 제III급 부정교합으로 진단받고, 상악골 전방견인 장치로 치료 받은 93명의 환자를 성별, 치료개시 나이별, 구개봉합 opening여부(구강내 장치), 안모성장 형태에 따라 분류하여, 각 유형에 따라 어떠한 경조직과 연조직 측모의 변화가 관찰되는지 여부와, Angle씨 제 I 급 부정교합의 정상군 20명과 상악골 전방견인 장치로 치료받은 환자 사이에 경조직과 연조직에 있어서 성장량과 치료량에 어떠한 관계가 있는지 비교하여 다음의 결론을 얻었다. 1. 상악골 전방견인 장치 사용시 골격적 계측항목과 치아와 연조직 계측항목에서 성장량보다 많은 치료에 의한 변화량을 관찰할 수 있었다. 2. 남녀별 치료개시 나이에 따른 상악골 전방이동량과 하악골의 후하방 회전량에는 유의차가 없었다. 3. R.P.E.에서 La-Li보다 유의성 있는 상악골(A point)의 전방이동을 보였으며, 상악 전치의 순측 돌출이 La-Li를 사용하였을때 보다 감소되었다. 4. 안모성장 형태에 따라서는 상악골 전방이동량에는 유의차가 없었으나, 하악골의 후하방 회전을 나타내는 계측치에서 counterclockwise군이 clockwise군보다 많은 변화를 나타냈다. 5. 상순과 하순의 후경변화는 하부 골조직과 상하악 치아의 위치변화와 높은 상관관계를 보였으며, 상순의 고경과 nasolabial angle이 증가하고, mentolabial angle이 감소되었다.

유치열기 정상교합아동의 측모두부방사선 계측연구 (STUDY ON LATERAL CEPHALOGRAM OF CHILDREN WITH NORMAL OCCLUSION IN THE PRIMARY DENTITION)

  • 본 연구의 목적은 유치열기 부정교합의 진단자료로 사용할 수 있는 정상 계측치를 알아보는 것이다. 4세에서 6세 사이 유치열기 정상교합아동 32명의 측모두부방사선 계측사진 및 안모사진을 이용하여 악골 및 연조직의 전후방적, 수직적 관계를 분석하였다. 수평적인 골격성 계측항목 중 SNA는 $83^{\circ}$, SNB는 $78.72^{\circ}$, ANB는 $5.28^{\circ}$를 나타내어 하악골이 상악골에 비해 후방에 위치하고 있었으며 수직적인 골격성 계측항목 중 Gonial angle은 $127^{\circ}$를 나타내어 성인에 비하여 High angle pattern을 보였다 연조직 분석에서는 성인에 비해 convex한 soft tissue profile이 관찰되었다. 치성 계측항목 중 U1 to SN은 $91.04^{\circ}$, IMPA는 $86.57^{\circ}$를 나타내어 성인에 비해 retroclination된 상하악 유중절치가 관찰되었다. 정상교합아동의 골격성 분석에서 여자가 남자에 비하여 평균 나이가 많음에도 불구하고 linear measurement는 전제적으로 남자가 여자보다 큰 수치를 보였다.

상악전돌의 치료를 위한 상악 후방이동의 안정성 및 고려사항 (Stability and Considerations of Total Maxillary Setback Le Fort I Osteotomy for the Correction of Maxillary Protrusion)

  • Purpose: The purpose of this study was to evaluate the stability of the total setback of maxilla. It also discussed the surgical considerations of the procedure. Methods: The study consisted of 15 patients (mean age, $25.53{\pm}5.71$) who were treated with total setback Le Fort I osteotomy (with or without additional posterior maxillary impaction). The cone beam computed tomography was obtained before surgery (T0), 3~4 days after surgery (T1), and 6 months after surgery (T2). The surgical changes as well as the relapse of reference points in relation to the reference planes were statistically analyzed. Results: The mean maxillary setback was 2.35 mm. There were posterior movements of A-point ($0.73{\pm}0.83$ mm) during T2-T1. An opening of the nasolabial angle (mean $12.58^{\circ}$) was noted. Conclusion: Total maxillary setback allows combining satisfactory functional and cosmetic results for a number of carefully selected patients.

Three-dimensional assessment of nasal changes after maxillary advancement with impaction using stereophotogrammetry

  • Objective: To evaluate the changes in the nose in three dimensions after Le Fort I osteotomy in patients with skeletal Class III malocclusion. Methods: The subjects were 40 adult patients (20 females and 20 males; mean age, 20.3 ± 3.0 years; range, 17.0 to 31.1 years) who underwent one-piece Le Fort I osteotomy with maxillary advancement and impaction treatment for maxillary hypoplasia. The mean maxillary advancement was 4.56 ± 1.34 mm, and the mean maxillary impaction was 2.03 ± 1.04 mm. Stereophotogrammetry was used to acquire three-dimensional images before and at least 6 months after surgery. Results: Alare (Al) and alare curvature (Ac) points had moved vertically and anterolaterally postoperatively. A significant increase was observed in the nasal ala width and alar base width, and no changes were noted in the columellar length, nasolabial angle, and nasal area. There was a significant relationship between maxillary impaction and nasal ala width and horizontal and sagittal positions of the bilateral Al and Ac. The only relationship found was between maxillary advancement and postoperative sagittal location of the subnasale and pronasale. Conclusions: Nasal soft tissues were highly affected by the vertical movement of the maxilla; however, the soft tissue responses were individual-dependent.

한국 성인 여성의 안모에 대한 실물 사진 계측학적 연구 (A PHOTOMETRIC STUDY OF THE FACIAL PROFILES OF KOREAN ADULT WOMAN)

  • The balance of facial sop tissue is important factor for the treatment of maxillofacial deformity patients. There are many cephalometric analysis of facial bone but a few soft tissue analysis of face for the orthognathic surgery, especially in the oriental people. This study aimed at examining the mean value of the soft tissue measurement of 120 Korean adult women with the linear and angular measurement and comparing with Caucasian. The results ere as follows. 1. In the frontal linear measurement, Korean women have larger interocular distant and width of nose, but the width of mouth appeared less than Caucasian. 2. In the frontal angular measurement, Korean women have larger inclination of the eye than Caucasian. 3. In the lateral linear measurement, Korean women have more prominent upper and lower lip position than Caucasian and have longer upper facial height comparing with lower facial height than Caucasian. 4. In the lateral angular measurement, Korean women have more convex facial profile and less nasolabial and geniolabial angle. 5. In the lateral facial ratio, Korean women have larger proportion in the lower department below the stomion.

Le Fort I 상악골이동술 후 상순과 비부의 연조직 변화에 대한 임상통계학적 연구 (A clinico-statistical study of soft tissue changes of upper lip & nose following Le Fort I maxillary movement)

  • The purpose of this study was to assess the soft tissue changes of upper lip & nose using 38 patients who treated with Le Fort I osteotomy for the correction of dentofacial deformities. Patients were devided into three groups. One was advancement group of maxilla(Group I, N=14), another was impaction group of maxilla(Group II, N=12) and the other was combination group(advancement & impaction)(Group III, N=12). Preop. and 1 month postop. (T1), preop. and 6 months postop.(T2) were analyzed and compared. The results obtained were as follows : 1. The upper lip thickness(UL-VP) moved anteriorly approximately 62% of the horizontal maxillary change and this was significant in the advancement group(Group I) 2. The upper lip length(Stm-Sn) and the lower border of upper lip(Stm) moved superiorly 25%, 40% of the maxillary impaction group(Group II) (P<0.05) 3. There was significancy in the upper lip thicness(UL-VP) approximately 56% of the combination group(Group III) (P<0.05) 4. The nasolabial angle decreased in all groups, but there were no significancy.

Clinical evaluation of autologous fat graft for facial deformity: a case series study

  • Objectives: The use of fat grafts in maxillofacial sculpturing is currently a common technique. Unlike fillers, autologous fats unite with facial tissues, but long-term results may still be unsatisfactory. Sharing long-term follow-ups can be helpful in making outcomes more predictable. Materials and Methods: The data from patients who were admitted from 2014 to 2016 for fat augmentation were collected. In all cases, fat grafts were injected by blunt cannula using a tunneling technique in different planes. A fan shape order for the malar, periorbital, nasolabial fold, mandibular angle and body, and perioral area was established. Results: Autologous fat was used for different sites of the maxillofacial regions. Of 15 patients, two patients were not satisfied due to fat graft resorption. For this, further injections were performed six months after the first injection using preserved fat grafts. One patient continued to be dissatisfied. There were no other complications related to fat transplants. Conclusion: Fat transplantation is a safe, reliable, and non-invasive method for facial contour and facial soft tissue defect restoration. Additional methods such as mesenchymal stem cells along with fat injection increase the survival rate of transferred fat.

  • Skin defects of the total nasal columella can significantly impact both nasal respiratory function and aesthetics. The reconstruction of total columella is a complex process and represents a significant challenge for plastic surgeons. Various factors can cause the loss of the columella. Numerous columella reconstruction procedures have been proposed, each with their own set of advantages and disadvantages. The main issues to address include the need for regional flaps from the forehead or nasofacial sulcus, a long pedicle to reach the columella, and the double angular folding that causes a risk of malnutrition or venous congestion. Additionally, using horizontal nasolabial flaps may lead to deformation of the upper lip. In this study, we present a new procedure to reconstruct the nasal columella using "Domino flaps" with two flaps (the horizontal upper lip island flap and nasocheek island flap). This new procedure ensures adequate skin for reconstruction of nasal columella and partial tip, minimizes rotation angle, reduces the angular folding of the pedicle, furthermore limits deformation of the upper lip. "Domino flaps" are a valuable option for surgeons when reconstructing the total nasal columella. However, it is important to consider whether the patient has a beard at the donor sites.

상악 제 2 대구치 발거에 의한 교정치료의 효과 (A STUDY ON TREATMENT EFFECTS OF MAXILLARY SECOND MOLAR EXTRACTION CASES)

  • Orthodontic treatment in conjunction with second-molar extraction has been a controversial issue among orthodontists over many decades. The aim of this study was to investigate the treatment effects of upper second molar extraction cases. The sample included 19 upper second molar extraction orthodontic cases(ten Angle's Class I's and nine Class II's, average age=13Y 6M) cared at Kyung-Hee University Department of Orthodontics. Lateral cephalometric radiographs were taken before and immediately after treatment. Seventy-nine points were digitized on each cephalogram and 38 cephalometric parameters were computed comprising 22 angular measurements, 13 linear measurements, and 3 facial proportions. The data obtained from each malocclusion group were analyzed by paired t-test. The statistical results disclosed that there was no significant change in skeletal pattern after treatment except for that accountable by growth while there was statistically significant change in dentoalveolar and soft tissue patterns. There were no significant changes in Bjork sum, posterior facial height /anterior facial height and lower anterior facial height /anterior facial height. No significant changes in anteroposterior position of maxilla and palatal plane were manifested. Although facial axis and lower facial height was slightly increased and the mandible was rotated backward and downward, there was no remarkable change in the mandibular plane. There were statistically significant changes in distal movement of upper first molar, molar key correction and overjet reduction while there was no change in the occlusal plane. The upper lip was slightly retracted simultaneously with slight increase in nasolabial angle. These results signify that distalization of upper dentition with the second molar extraction does change occlusal relationship without gross modifications in the craniofacial skeletal configurationson. Henceforth the second molar extracted would be recommended to treat severe anterior crowding and protrusion with minor skeletal discrepancy.

Long-Term Evaluation of the Lip and Nose in Bilateral Complete Cleft Lip Patients following Lip Adhesion and Secondary Nose Correction

  • Background Surgical correction of bilateral cleft lip deformities remains one of the most challenging areas in facial plastic surgery. Many surgical techniques and conservative devices have been offered for the early management of bilateral cleft lip in infants. The purpose of this study was to evaluate the effect of lip adhesion on the lip and nose of patients with bilateral cleft lip. Methods A retrospective review of 13 patients with bilateral cleft lip was performed and compared with age-matched noncleft children. Patients underwent lip adhesion at a mean age of 2.8 months, and cheiloplasty at 6.6 months of age using a modification the Mulliken method. Secondary rhinoplasty was performed at the age of 6 in 13 patients. The surgical results were analyzed using photographic records obtained at the age of 1 and 7 years. Twelve length measurements and one angle measurement were obtained. Results All measurements were not statistically different from those of the noncleft age-matched control group at the age of 1. At 7 years of age, upper lip height and vermilion mucosal height were shorter (p < 0.05) than in the control group. Nasal tip protrusion and the nasolabial angle were greater (p < 0.05) than in the control group. Conclusion Lip adhesion followed by secondary rhinoplasty resulted in an acceptable lip and nasal appearance. Although nasoalveolar molding is now widely used, lip adhesion can be an appropriate alternative if an orthodontist is not available due to geographical or economic constraints.
