Nasometric and Acoustic Analysis in Experimentally Induced Velopharyngeal Insufficiency in Human (사람에서 유발시킨 구개인두부전증의 비음도와 음향학적 분석)

  • Many tools have been used to evaluate the voice abnormalities of velopharyngeal insufficiency(VPI). The aim of study was to obtain the objective evaluation method of VPI by comparing the acoustic and nasalance data of experimentally induced VPI group and those of normal control group. Ten healthy young men were included in this study Mild and severe VPI were experimentally induced by retracting velopharyngeal movement. Using the nasometer, we obtained the nasalance score of the sustained oral vowels and those of three types of nasometer passages and the slope scores of nasogram of nasal words. And we analysed the change of formant frequencies for the sustained oral vowels and the changes of various parameters of hyper-tnasality by the computerized speech analysis system. The nasalance score of sustained /a/ was increased significantly in VPI conditions. There was no changes in the slope score of nasogram. On the acoustic speech analysis, the second formant frequencies of vowel /e/ and /i/ were decreased significantly in VPI conditions. This results suggested that the measurement of nasalance score and formant frequency might be useful in the evaluation of VPI.

The Effects of Vocal Loudness on Nasalance Scores in Korean Vowels - A Preliminary Study Using NasalView System - (음성강도에 따른 모음의 비음치 연구)

  • This study was designed to examine the effects of vocal loudness on Nasalance scores in Korean vowels. One normal adult without 'hypemasality' produced 3 types of vowels (/i/, /a/, /u/) in 3 levels of loudness (60, 70, 80dB). Nasalance scores (%) and oral & nasal energy intensity (dB) were measured through NasalView system. These data were compared among 3 levels of loudness and among 3 types of vowels. The results were as follow: For all vowels, Nasalance scores significantly reduced when vocal loudness increased. Because the increases of oral energy intensity were larger than that of nasal energy intensity. For all levels of loudness, Nasalance scores significantly differed among vowel types (/i/>/a/>/u/).

  • This study attempted to obtain differences as a function of Korean and Chinese in vowels. The $NasalView^{(R}}$ (Tiger Electronics Inc., Seattle, USA) was used to obtain nasalance values. Ninety eight subjects participated in this study (48 females from Korea and 48 females from China). The stimuli used in this experiment included vowel /i, a, u/. The stimuli were presented in a random order. The entire session was recorded and the average nasalance score was measured via the $NasalView^{(R)}$. The procedure took approximately 10 minutes. The results of this study showed that the difference between Korean and Chinese was significant in vowel /i, a, u/. The average nasalance scores were /i, a, u/ from highest to lowest in order in vowels. The average nasalance was higher in China than Korean in vowels.

A Study of Nasalance Change in Submucosal Type Cleft Palate Patients by Surgery (점막하 구개열 환자의 수술 전후 비음도 변화에 대한 연구)

  • Submucosal type cleft palate is a kind of cleft palate. A submucosal cleft may result in shortening of the anteroposterior dimension of the hard or soft palates or both. The increased distance along with the lack of muscle connection in the soft palate usually accounts for the lack of palatopharyngeal function in patients with submucosal cleft. Resonance disorders which is found in cleft patients show hypernasality or hyponasality. Many cases of submucosal type cleft palate patients visit our clinics due to hypernasality. In this study, resonance disorders was evaluated through nasalance test. Experimental group was composed of submucosal type cleft palate patients. The patients were treated by a so-called combined therapy, i.e., operation and speech training. To observe the changing pattern by surgery, nasalance test was carried out one time before surgery and three times after surgery. Nasometer II was used as a examination. The questionaire was filled with single vowels & diphthongs. The mean nasalance score of the child was significantly lower than that of the adult at every vowel. An early age at operation (under 10 years) was that a better functional result was achieved with patients. The mean nasalance score of /i/ was highest and that of /a/ was the lowest. The result of corrective surgery in selected cases has achieved improvement in all cases. Hypernasality has been consistently diminished. he operation.

  • This study sought to compare the difference of Phonation Threshold Pressure and Nasalance according to vowels. We tested 28 normal females(20~30age) who be in attendance at a school from May to August 2012. We measured Phonation Threshold Pressure through PAS(Phonatory aerodynamic system, model 6600, KAY electronics, Inc) that aero-mechanical instrument called. Phonation Threshold Pressure was measured by using the consonant /p/ and the three vowels /a, i, u/ and was analyzed the three times of Phonation the five times. Also We measured Nasalance through the Nasometer${\Pi}$(model 6450, KAY electronics, Inc). The vowel's phonation was recored by using the three vowels /a, i, u/. the nasalance score was measured via Nasometer. As a Result, Phonation Threshold Pressure according to three vowels show significantly difference. Also, Nasalance according to three vowels show significantly difference. Finally, Correlation coefficient of Phonation Threshold Pressure and Nasalance according to three vowels show positive relation ship. Therefore Phonation Threshold Pressure and Nasalance according to three vowels have closely relationship.

Postoperative Change in Hypertrophic Rhinitis(Study Using Nasometer, CSL and Acoustic Rhinometer) (비후성 비염환자에서 음성검사 및 음향비강통기도검사를 이용한 수술전후 비교)

  • Background and Objectives : With the development of computerized systems, an objective evaluation methods of nasal speech and nasal geometry have become readily available by means of a simple, noninvasive technique. In this study, we assessed the nasality, nasal formant, nasal volume and nasal area in patients with hypertrophic rhinitis before and after turbinate surgery. Material and Method : With the nasometer, we measured nasalance, which reflects the ratio of acoustic energy output of nasal sounds from the nasal and oral cavities. With CSL 4300B, we measured nasal formants. We used acoustic rhinometer to measure nasal area and nasal volume. Postoperative changes of above factors were compared with preoperative values. Paired t-test and Pearson's correlation were used for statistical analysis. Results : The first nasal formant frequency, nasalance scores of three passages(baby, mamma and rabbit passages), minimal cross sectional area(MCA) of narrow side, nasal volume of narrow side and nasal volume of wide side had increased significantly after turbinate surgery (p <0.05). The MCA and nasal volume of narrow side and MCA of wide side showed significant correlation with nasalance score of rabbit passage and baby passage showed significant correlation with nasal volume of narrow side(p<0.05). Conclusion : There were significant increases in nasalance scores, first nasal formant frequency, MCA and nasal volume after turbinate surgery. Thus, we must consider the possibility of voice changes postoperatively in professional voice users.

Gender Differences in Nasalance Scores in Korean Speaking Adults (비음측정기를 이용한 한국어를 사용하는 정상 성인에서 성별에 따른 비음도의 차이에 관한 연구)

  • The purpose of this study was to obtain normative nasalance scores for adult subjects speaking the Korean language and to determine whether significantly different scores exist for female and male speakers. Mean nasalance scores were obtained for normal speaking Korean adults while they are reading vowels, consonants, no nasal sentence, mild nasal sentence, and high nasal sentence. Thirty adults who had lived in Seoul area with normal articulation, resonance, and voice were included. Among the subjects 15 were male aged 24-38 years and 15 were female aged 19-33. Nasometer data were collected and analyzed using the Kay Nasometer 6400. Nasalance scores were evaluated to investigate the effect of gender by using statistical tests. Nasalance data showed that nasalance values varied accroding to speech stimuli, and there was no significant difference in nasalance scores between male and female speakers in most of the language samples.

The Study of Tonsil Affected Voice Quality after Tonsillectomy (편도적출술로 음성변화가 올 수 있는 편도 상태에 관한 연구)

  • Tonsillectomy is the one of operation that is performed the most commonly in otolaryngology field. Many changes that include range of voice, tone, voice quality and resonance were made by tonsillectomy. Sometimes, any patients taken tonsillectomy has suffer from these voice problem after tonsillectomy. However there are less study for these problems until now. Then, we studied to find the anatomical findings that affected the voice quality when tonsillectomy was performed. We evaluated the voice in 2 groups, one is the group showed the normal pharyngeal space by using the transnasal fiberscopy, the other is group showed medially bulging tonsil at pharyngeal cavity by using same method, with perceptual evaluation, nasalance score, nasality, oral formant and nasal formant. We used the computerized speech analysis system, the nasometer and the spectrogram in the CSL program. We could not find any differences in perceptual evaluation between two groups. But objective measures were provided. Nasalance score and nasality on the nasometric analysis were increased significantly and oral formant on the spectrogram was changed singnificantly after tonsillectomy in Group 2. Authors thought medially bulging tonsil in the pharynx is able to affect the voice quality after tonsillectomy when we evaluted through the nasal cavity by the using of fiberscopy and this evaluation would be important especially in singers.

  • The objective evaluation of velopharyngeal closure function is the key to diagnosis and therapy control of velopharyngeal incompetency. The aim of this study is to evaluate the aerodynamic and acoustic characteristics of the velopharyngeal closure function of patients who have developed velopharyngeal incompetency after management with speech aids. The test words were composed of sustained vowels /a/, /i/, /e/, /u/, /ja/, /je/, /wi/ and polysyllabic words /p'ap'i/, /siso/, /mami/ for measuring nasalance, The data was collected before the placement of the speech aids and one to three months after. The results were as follows: The nasalance score of the velopharyngeal incompetency speakers was higher than that of the normal control group, except for nasal sounds, and was decreased after placement of the speech aids, especially in high vowels /i/ (P<.01) and /wi/ (P<.05).

Acoustic Analysis of the Differences of Fricatives and Affricates between Normal Children and Cleft Palate Children (구개파열 아동과 정상 아동의 마찰음과 파찰음의 음향음성학적 특성 비교)

  • The frequency in which noise energy is generated, that is, the point where the preceding vowel ends is the cut-off frequency. Thereupon, this study intends to examine the correlations between, cut-off frequencies, cut-off frequencies changed by the following vowel, and cut-off frequencies and nasalance score, of fricatives and affricates with the subjects of children with the cleft palate and normal children. The subjects of this study are total 12 children residing in Seoul and Gyeonggi area. Six are the children diagnosed to have the cleft palate and whose chronological age are more than six, and another six are the normal children who are also more than six and whose chronological age and sex correspond to those of the former. Each subject was presented with nonsyllable environment and sentence environment(50 environment) of fricatives and affricates. Regarding meaningless syllable environment and sentence environment of fricatives and affricates, children with the cleft palate had lower cut-off frequencies than normal children. As a result of comparative study on correlations between cut-off frequencies and nasalance score of children with the cleft palate and normal children, it doesn't show statistically significant correlations in both meaningless syllable environment and sentence environment of normal children, but it has statistically significant correlations in sentence environment of children with the cleft palate.
