A novel classification of anterior alveolar arch forms and alveolar bone thickness: A cone-beam computed tomography study

  • Purpose: This study classified alveolar arch forms and evaluated differences in alveolar bone thickness among arch forms in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods: Axial views of 113 CBCT images were assessed at the level of 3 mm below the cementoenamel junction (CEJ) of the right and left canines. The root center points of teeth in the anterior esthetic region were used as reference points. Arch forms were classified according to their transverse dimensions and the intercanine width-to-depth ratio. The buccolingual alveolar bone thickness of each tooth was measured at 3 mm below the CEJ and at the mid-root level. Differences in the mean thicknesses among arch forms were analyzed. Results: Anterior maxillary arches could be classified as long narrow, short medium, long medium, and long wide arches. Significant differences in buccolingual alveolar bone thickness among the arch groups were found at both levels. The long wide arches presented the greatest bone thickness, followed by the long medium arches, while the long narrow and short medium arches were the thinnest. Conclusion: Arch forms were classified as long narrow, short medium, long medium, and long wide. The buccolingual alveolar bone thickness exhibited significant differences among the arch forms.

Film Image Transfer System (FITS): An Efficient Method for Proper Positioning of Orthodontic Mini-implants

  • Purpose: To describe the newly developed Film image transfer system (FITS) for proper positioning of the orthodontic mini-implant in the narrow interdental space and considerations for better application. Materials and Methods: A patient who was planning to have orthodontic mini-implant treatment on the posterior maxilla was recruited to assess the feasibility of FITS. Dental radiographic film and bite record was taken. And then the film image was transferred on the photographic emulsion coated model using transfer light through film projector (enlarger). After exposing the photo emulsion coating on the model, the image was developed with a working solution for a paper developer and fixed. The surgical guide for the mini-implant was fabricated from the transported FITS data. Results: The completed surgical guide was easily placed intraorally, and allowed a simple and rapid placement of the mini-implant. The site of the implant placement was accurate as planned position. Conclusion: In the reported case, The FITS technique represents an effort to minimize risk to the patient and produce consistently good results based upon accurate information about the anatomy of the implant site.

  • For implant treatment there must be sufficient bone to house the implant body. At least 5mm wide residual bone is needed and usually a 6mm width is preferred by clinicians. However, surgeons sometimes find patients with a narrow ridge, which makes it difficult to place an implant. Therefore, many clinicians perform bone graft or a ridge splitting technique to overcome these poor conditions. The time and cost can be reduced using the ridge splitting technique with immediate implant placement. Recently, many studies reported reliable consequences of ridge splitting technique. This paper reports a successful of implant placement with a ridge splitting technique in a very thin alveolar ridge.

임플란트 일차 안정성에 있어서 고정체 지름에 따른 osteotome 술식의 효과 (The effect of osteotome technique on primary implant stability according to implant fixture diameter)

  • Primary stability is a fundamental criteria of implant success. There has been various trials to increase initial stability and bone to implant contact. The objective of osteotome technique is to preserve all the existing bone by minimizing or even eliminating the drilling sequence of the surgical protocol. The bone layer adjacent to the osteotomy site is progressively compacted with various bone condensers(osteotomes) this will result in a denser bone to implant contact. This improved bone density helps to optimize primary implant stability in low density bone. The use of wide implant is one of methods to increse primary stability. They can be used in special situations in which they can increase the surface area available for implant anchorage and improve their primary stability The aim of this study was to evaluate the influence of the osteotome technique and implant width on primary stability. Osteotome technique was compared with conventional drilling method by resonance frequency measurments according to the implant fixtures diameter. The results were as follows: 1. The average of ISQ value was sightly higher in osteotome technique, but there was not statistically significant in regular and narrow implant(p <0.05). 2. Either osteotome technique or conventional technique. ISQ value was significantly higer as increasing of implant diameter(p <0.05). 3. ISQ value of drilling technique was higer than those of osteotome technique in wide implant. It was assumed to be caused by difference in final preparation diameter.

구개부 치조골 결손을 보이는 상악 전치 상실부의 임플란트 심미보철수복: 증례보고 (Esthetic implant restoration in the maxillary anterior missing area with palatal defect of the alveolar bone: a case report)

  • 치조골 폭이 협소한 상악 전치부의 심미적인 임플란트 수복은 임플란트 식립체의 경사각과 위치가 제한되기 때문에 단순한 임플란트 식립만으로는 심미적인 임플란트 보철물을 제작하기 어려운 일이다. 임플란트 보철물이 변연 치은과 조화로운 출현윤곽을 재현하기 위해서는 임플란트 식립체의 이상적인 치조골 내 위치와 식립각이 설정될 수 있어야 하며, 이를 위해서는 종종 임플란트 식립 전 골증대술을 선행하여야 한다. 본 증례는 구개측의 심한 골 결손을 동반한 얇은 치조골 폭을 보이는 상악 전치 상실부에 골증대술을 시행한 후, 임플란트를 식립하여 심미 보철을 시행한 증례이다. 먼저 협측에 과량의 골증대술을 선행하여 불리한 임플란트 식립 위치를 보상할 수 있는 구개측 식립 경사를 확보하였으며, 임시보철물의 외형을 조정하여 인접 치아와 조화로운 변연 치은 높이와 치간 유두를 형성하였다. 최종적으로 지르코니아 코어를 가지는 도재수복물을 이용하여 보철 수복하였고 심미적, 기능적으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

A two-short-implant-supported molar restoration in atrophic posterior maxilla: A finite element analysis

  • PURPOSE. The aim of this study was to investigate the stress distribution of 2-short implants (2SIs) installed in a severely atrophic maxillary molar site. MATERIALS AND METHODS. Three different diameters of internal connection implants were modeled: narrow platform (NP), regular platform (RP), and wide platform (WP). The maxillary first molars were restored with one implant or two short implants. Three 2SI models (NP-oblique, NP-vertical, and NP-horizontal) and four single implant models (RP and WP in a centered or cantilevered position) were used. Axial and oblique loadings were applied on the occlusal surface of the crown. The von Mises stress values were measured at the bone-implant, peri-implant bone, and implant/abutment complex. RESULTS. The highest stress distribution at the bone-implant interface and the peri-implant bone was noticed in the RP group, and the lowest stress distribution was observed in the 2SI groups. Cantilevered position showed unfavorable stress distribution with axial loading. 2SI types did not affect the stress distribution in oblique loading. The number and installation positions of the implant, rather than the bone level, influenced the stress distribution of 2SIs. The implant/abutment complex of WP presented the highest stress concentration while that of 2SIs showed the lowest stress concentration. CONCLUSION. 2SIs may be useful for achieving stable stress distribution on the surrounding bone and implant-abutment complex in the atrophic posterior maxilla.

근단변위판막술과 함께 유리치은이식술을 사용하여 임플란트 주변 각화치은을 증대시킨 2건의 증례 보고 (Free gingival graft in combination with apically positioned flap for establishment of keratinized gingiva around the implants: Report of two cases)

  • Narrow zone of attached gingiva and shallow vestibule around the implants might contribute to difficulty of cleasing, periimplant mucositis caused by incomplete cleansing and further peri-implantitis. The aim of this case report is to present modification of soft tissue biotype around the implants by free gingival grafts according to timing of surgical intervention and shape of free gingiva. A 44 year-old male patient had a missing area on lower right second molar area with 1 to 2 mm of narrow attached gingiva zone and wanted to be treated by implant placement. In radiographic analysis, there was enough alveolar bone to install an implant, free gingiva from hard palate was grafted following implant placement using double layer flap. The width of attached gingival was increased to 4 to 5mm and well maintained during 5 months of follow up. A 69 year-old female patient also had a missing area on lower right first and second molar area with 1 to 2 mm narrow attached gingiva. Since she had systematically angina pectoris and dental phobia, minimal invasive free gingival graft after implants placement was planned. After 2 months of implant surgery, free gingival graft surgery was performed with healing abutments connection. The grafted gingiva was composed of two strip shaped free gingiva, and they were immobilized by periodontal pack. The width of attached gingival was increased to 4 to 5mm and well maintained during 10 months of follow up. With prosthesis delivery, the patients recovered ideal periodontal environment around implants and masticatory function. In conclusion, periodontal health and masticatory function could be achieved through implant placement and free gingival graft.

단일치아 결손시 $TiUnite^{TM}$ 표면처리한 임플란트의 생존율에 대한 후향적 단기연구 (A 3 year-retrospective study of survival rate in single $Br{\aa}nemark$ $TiUnite^{TM}$ implant)

  • Background: $TiUnite^{TM}$ is a highly crystalline and phosphate enriched titanium oxide surface which has a unique porous surface structure. This improved implant surface enhances bone response and reduces healing period. It also assures early stability of implant. These help to increase the success of implant. The aim of this study is to evaluate the survival rate of $TiUnite^{TM}$ surfaced single implant. Materials and methods: A retrospective analysis of 89 $TiUnite^{TM}$ surfaced implants replacing a single tooth was assessed according to their dental record. The age of the patients ranged from 17 to 82 years (mean age: $45.8{\pm}14.6)$. Data were recorded regarding the survival rate of these implants. Results: Fifty-two implants (57%) were placed in the maxilla, and 37 (43%) in the mandible. Over 75% were placed in the posterior area. Of the placed implants, 67% were the wide type. while 25% were the regular type and only 8% were of the narrow type. The single implants produced an overall clinical survival rate of 96.6% over the observation period (mean 17.9 months). Among 89 implants, only 2 implants were removed and one implant was submerged. Conclusion: According to these data, $TiUnite^{TM}$ surfaced implant in a single tooth restoration showed favorable survival rate although this study was done in a short term period.

Survival rate and clinical evaluation of the implants in implant assisted removable partial dentures: surveyed crown and overdenture

  • PURPOSE. The purpose of this study was to investigate survival rates of the implants used in implant assisted removable partial dentures (IARPDs). MATERIALS AND METHODS. The study was conducted on 21 patients who were treated with IARPDs. The mean follow-up period for IARPD patients ranged from 12 to 185 months (mean 47.9 months). A total of 58 implants were used for IARPDs in two different modalities: 41 for surveyed crowns and 17 for overdentures. The survival of implants was determined by clinical and radiographic evaluations considering relevant factors: location, RPD classification, opposing dentition, splinting, and implant diameter. RESULTS. The survival rate of total 58 implants was 93.1%: 95.1% for implants supporting surveyed crowns and 88.2% for implants used in overdentures. Considering only the implants supporting surveyed crown, regular diameter implants showed a higher survival rate than narrow or wide diameter implants. CONCLUSION. The survival rate of the implants used in IARPDs was 93.1% (surveyed crown: 95.1%, overdenture: 88.2%).

  • Ridge expansion osteotomy(REO) procedure is used to widen the narrow ridge in locations that are too thin to permit the use of standard implant drills. The objective of this technique is to maintain, if possible, all of the existing maxillary bone by pushing the bone aside with minimal trauma. The author has used tis procedure on five patients who had narrow ridge in their soft maxillary bone. Fifteen implants were placed in sites needing ridge expansion using REO. The satisfactory results were obtained. The REO procedure is thought to be a safer and more conservative method of widening a narrow ridge. And this procedure is heatless, offers excellent tactile sensitivity, control, visibility, and takes advantage of available bone.
