Reparability and Surgical Indications of Rotator Cuff Tears (회전근 개 파열의 봉합 가능성 및 수술 적응증)

  • Purpose: The reparability of rotator cuff tears is very important to establish the indication of the surgical treatment of rotator cuff tears. We reviewed the factors that effect to reparability of rotator cuff tears, and considered about the surgical indication of rotator cuff tears. Materials and Methods: The reparability was considered by the surgical reparability and the postoperative healing potentials of rotator cuff. The natural history of rotator cuff tear gives us the information to make decision about the surgical treatment or the conservative treatment. Results: There are 3 general categories of factors that help predict the healing potential of a rotator cuff tear. These include surgical technique, biology, and environmental issues. Surgeon can control the surgical technique by himself. The control of biology comes from careful operative indications by age, acute trauma history, chronicity, and tear size. And the control of environmental factor can give better outcomes by cessation of smoking and nonsteroidal anti-inflammatory drug (NSAID). Conclusion: A better understanding of the natural history and the reparability of rotator cuff tear will help us to estabilish the indication of surgical treatement of rotoator cuff tears, and lead to satisfactory clinical outcomes.

The co-injection of antioxidants with foot-and-mouth disease vaccination altered growth performance and blood parameters of finishing Holstein steers

  • Objective: This study was conducted to evaluate whether the co-injection of antioxidants together with foot-and-mouth disease (FMD) vaccination has the potential to attenuate the negative effects caused by vaccination in Holstein finishing steers. Methods: A total of 36 finishing Holstein steers (body weight [BW]: $608{\pm}45.6kg$, 17 months old) were randomly allocated to one of three treatments: i) control (CON, only FMD vaccination without any co-injection), ii) co-injection of commercial non-steroidal anti-inflammatory drugs (NSAID) with FMD vaccination at a ratio of 10:1 (NSAID vol/FMD vaccine vol) as a positive control (PCON), iii) co-injection of commercial mixture of vitamin E and selenium with FMD vaccination (VITESEL) (1 mL of FMD vaccine+1 mL of antioxidants per 90 kg of BW). Changes in growth performance and blood parameters because of treatments were determined. Results: No significant difference in BW, average daily gain, and dry matter intake of the steers was observed among the treatments. The FMD vaccination significantly increased white blood cells (WBC), neutrophils, platelets, and mean platelet volume (p<0.01) in blood analysis. The count of lymphocyte tended to increase after vaccination (p = 0.08). In blood analysis, steers in VITESEL tended to have higher numbers of WBC, neutrophils, and platelets compared to that of other treatments (p = 0.09, 0.06, and 0.09, respectively). Eosinophils in VITESEL were higher than those in PCON (p<0.01). Among blood metabolites, blood urea nitrogen and aspartate transaminase were significantly increased, but cholesterol, alanine transferase, inorganic phosphorus, Mg, and albumin were decreased after FMD vaccination (p<0.01). Conclusion: The use of antioxidants in FMD vaccination did not attenuate growth disturbance because of FMD vaccination. The metabolic changes induced by vaccination were not controlled by the administration of antioxidants. The protective function of antioxidants was effective mainly on the cell counts of leukocytes.

  • Chronic recurrent multifocal osteomyelitis (CRMO) is an inflammatory bone disorder presenting with sterile osteomyelitis, most often presenting in childhood. Although the etiology is understood incompletely, its association with other auto-inflammatory diseases including inflammatory bowel disease (IBD); psoriasis; Wegener's disease; arthritis; and synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome suggests that dysregulated innate immunity may play an important role in the pathogenesis. We report a case of a 13-year-old boy with CRMO associated with Crohn's disease (CD) successfully treated with infliximab after failure of non-steroidal anti-inflammatory drug (NSAID) treatment. He initially was diagnosed with CRMO based on symmetric and aseptic bone lesions with no fever, lack of response to antibiotic treatment, vertebral involvement, and normal blood cell counts. Despite five months of NSAID treatment, his musculoskeletal symptoms were aggravated, and he developed gastrointestinal symptoms. Finally, he was diagnosed with CRMO associated with CD. Due to the severity of symptoms, infliximab was initiated and produced symptom improvement. This case supports infliximab as another choice for treatment of bowel symptoms in addition to the bone and joint symptoms of CRMO when other first-line treatments are ineffective.

THE EFFECT OF INTRA-ARTICULAR INJECTION OF HYALURONIC ACID AFTER ARTHROCENTESIS IN TREATMENT OF INTERNAL DERANGEMENTS OF THE TMJ (악관절 내장증 치료를 위한 악관절 세정술 후 관절강내 Hyaluronic Acid 주입 효과)

  • This study was designed to investigate the effect of intra-articular injection of hyaluronic acid for the treatment of internal derangements of the temporomandibular joint(TMJ). Sixteen patients with internal derangements of TMJ in 1 male and 15 females aged 68 to 18 years comprised the study material. The patients' complaint was limited mouth opening and pain during function. Patients were divided into 3 groups(articular disc displacement with reduction, articular disc displacement without reduction, osteoarthritis group). The preauricular area was disinfected anesthetized locally with 2% lidocaine hydrochloride. Arthrocentesis was performed. Hyaluronic acid(1.5 ㏄) was then injected into the superior compartment of the TMJ. Active range of motion exercises were instituted at approximately 24 hours postoperatively. Antibiotics and NSAID, three times daily by mouth, was prescribed for 3 days. Clinical evaluation of the patients was done before the procedure, immediately after the procedure, final follow-up visit postoperatively. Pain during function was assessed using visual analog scales(VAS). Maximal mouth opening was recorded as a distance between upper incisal edge and lower incisal edge. Intra-articular injection of hyaluronic acid caused significant reduction of pain during function, significant increase of maximal opening range. These findings suggest that intra-articular injection of hyaluronic acid is useful for decreasing patient reports of pain while increasing functional mobility of the mandible in internal derangements of the TMJ.

Rat Skin Permeation of Diclofenac and its Prodrugs (디클로페낙 프로드럭들의 흰쥐 피부 투과)

  • Various alkyl ester prodrugs of diclofenac were synthesized in order to investigate the relationship between their skin permeation characteristics and physicochemical properties. Solubility in various vehicles was measured at room temperature. 1-Octanol/water partition coefficients (Log P) and capacity factors (k') were measured to determine the lipophilicity of the prodrugs. Stability of prodrugs in the skin extract and homogenate was also investigated before conducting the skin permeation studies. Increases in the Log P and capacity factor values were observed when alkyl esters of diclofenac were prepared. Since the aqueous solubility of the prodrugs was not high enough, they were saturated in propylene glycol (PG) for skin permeation studies. Prodrugs were rapidly metabolized to diclofenac, both in skin homogenate and in dermal extract of skin. The skin permeation rate of alkyl ester prodrugs was significantly higher than diclofenac with shorter lag time. Moreover, a parabolic relationship was observed between the permeation rate and the log P values of prodrugs, and the maximum flux was achieved at a log P value of around 4.0.

  • 현재까지 NSAID 및 SAID의 사용으로 급성염증의 경우는 잘 조절되고 있으나, 류마티스 관절염과 같은 만성염증성 질환은 극복하지 못하였다. 뿐 만 아니라, 상기의 약물들의 장기간 사용으로 인한 부작용이 문제되고 있다. 그러므로, 만성염증성 질환의 치료를 위한 새로운 계열의 항염증제 개발이 시급하며, 많은 연구자들이 여러 가지 식물추출물을 이용하여 신약개발의 가능성을 타진하고 있다. 이의 일환으로, 본 연구에서는 고전문헌에서 사용된 식물들을 대상으로 하여 Rat의 류마티스 관절염 model을 이용하여 그들의 항염증작용을 연구하였다. 여정자 및 등줄나무를 포함한 27종의 식물을 이용하여 각 methanol 추출물을 조제하고, 매일 경구로 투여하였다 (200 mg/kg/day). 류마티스성 관절염은 rat의 족부에 Mycobacterium butyricum (0.6 mg/rat)을 주사하여 유발시켰고, 2차부종의 억제를 추출물의 활성으로 판정하였다. 그 결과, 27종의 식물중 목통, 마황 및 산두근이 2차부종을 유의성있게 억제하였으며, adjuvant 주사 후 16일에 억제율이 각각 22%, 36%, 13%로 나타났다. 산두근을 분획하여 재검정한 결과 50 mg/kg/day의 용량으로 투여시 EtOAc 및 n-butanol 분획에서 억제능이 나타나, 이들 분획을 대상으로 활성물질의 분리를 계속하고 있다.

  • Screening method for NSAIDs (Hon-Steroidal Anti-Inflammatory Drugs) in urine was developed using GC/NCI-MS. Derivatized six fenamates with pentafluoropropionic anhydride showed high sensitivity in NCI-MS. The conditions of the derivatization reaction and chromatographic conditions were established for screening with a trace analysis. Limit of detection was in the range of 4-25 pg/mL. This method may be used to the equine doping analysis for NSAIDs and forensic analysis.

Neurotoxicity of Sodium Salicylate on Spinal Sensory Neurons in Culture (배양 척수감각신경세포에 대한 살리실산 나트륨의 신경독성에 관한 연구)

  • Background: Sodium salicylate (SS) is a nonsteroidal anti-inflammatory drug (NSAID) for the treatment of neuralgia or pain from rheumatoid arthritis. When abused or used in excess, SS can induce cytotoxicity. The present study examined whether SS has a neurotoxic effect. Methods: Cell viability was examined by MTT [3-(4,5-dimethylthiazol-2,5-dipheny ltetrazolium bromide] assay and Sulforhodamine (SRB) assay after cultivating dorsal root ganglion (DRG) neurons derived from neonatal mouse. These cells were treated with various concentrations of SS for 24 hours. In addition, the amount of protein synthesis against SS was measured in these cultures. Results: Cell viability (20, $40{\mu}g/ml$ SS) significantly decreased in a dose-dependent manner. Additionally, SS inhibited protein synthesis after the exposure of cultured mouse DRG neurons to $30{\mu}g/ml$ of SS for 24 hours. Conclusions: The present study suggests that SS is toxic in cultured DRG neurons derived from neonatal mouse by decreasing cell viability and the amount of protein synthesis.

Phonophoretic Delivery of Piroxicam (초음파를 이용한 피록시캄의 경피흡수)

  • Piroxicam is one of the NSAID, which is used in the systemic and topical treatment of a variety of inflammatory conditions. Conventionally, for topical use, the drug is formulated in gel. We designed an phonophoretic drug delivery system to investigate the piroxicam permeability and the influence of ultrasound application (continuous mode, pulsed mode), frequency (1.0 MHz, 3.0 MHz) and intensity $(1.0\;w/cm^2,\;1.5\;w/cm^2,\;2.0\;w/cm^2)$ with 0.5% piroxicam gel. Per cutaneous absorption studies were performed in vitro models to determine the rate of drug absorption via the skin. Permeation study using hairless mouse skin was performed at $37^{\circ}C$ using buffered saline (pH 7.4, 10% propylene glycol solution) as the receptor solution. Anti-inflammatory activity was determined using carrageenan-induced foot edema model in rat. A pronounced effect of ultrasound on the skin absorption of the piroxicam was observed at all ultrasound energy level studied. Ultrasound was carried out for 10 hr. The highest permeation was observed at intensity of $2.0\;w/cm^2$, frequency of 1.0 MHz and continuous output. The inclusion of phonophoresis was found to improve significantly the skin permeation in vitro and the anti-inflammatory activity in vivo.

The Changes in Intestinal Damage and Bacterial Translocation with Time after Administration of Diclofenac (Diclofenac 투여 후 시간경과에 따른 장손상과 장내세균전위의 변화)

  • Non-steroidal anti-inflammatory drug (NSAID)-induced gut damage and bacterial translocation (BT) have not been studies well, especially from the perspective of time after administration of NSAIDs. We therefore examined these changes in animals. The study was performed on 5 groups of rat; a control group (group A) and diclofenac groups (groups B, C, E, and F). Rats in the diclofenac groups were orally administered diclofenac sodium before intestinal permeability (IP) measurement (group B, 1 h before measurement; group C, 10 h before; group D, 22 h before; and group E, 52 h before). The IP, stool pellet number, serum biochemical profile, enteric bacterial number, and BT in the mesenteric lymph nodes (MLNs), liver, spleen, kidney and heart were measured. The administration of diclofenac resulted in significantly increased IP, caused intestinal protein loss, decreased stool pellet number, caused enteric bacterial overgrowth and increased BT in multiple organs in groups A, B, C, and D. IF, intestinal protein loss, and the BT in the liver and the spleen in group E were decreased than those in group D. There were no differences in the other parameters between group D and E. In the recovery phase of the diclofenac-induced gut damage, enteric bacterial overgrowth and BT in the kidneys and the heart did not change while the BT in the reticuloendothelial systems such as in the MLNs and liver was decreased.
