Korean Medicine Treatment Including Capsaicin-containing (CP) Pharmacopuncture for Acute Low Back and Hip Pain: A Case Report of 3 Patients (급성기 허리 및 고관절 통증에 대한 CP 약침을 포함한 한의 치료 3례 증례보고)

  • This case study reports the clinical response to Korean Medicine (KM) treatments including capsaicin-containing (CP) pharmacopuncture in three patients with acute low back and hip pain. Three patients were diagnosed with acute lumbar sprain, and were treated with CP pharmacopuncture, and other KM treatments including acupuncture, cupping, and herbal medicine. Numeric Rating Scale (NRS), Oswestry Disablility Index (ODI), and Clinical Evaluation Grade (CEG) were used to evaluate symptom changes. After one or two treatments, acute pain with limited range of motion in the low back and thigh region improved with a decrease in NRS, ODI, and CEG. These results suggest that further studies on KM treatments including CP pharmacopuncture for acute low back and hip pain are warranted.

Intradermal Acupuncture for Acute Neck Pain with Limited Range of Motion in the Cervical Spine: Case Reports (과립식 피내침 치료로 호전된 경추 관절가동범위 제한을 동반한 급성 경항통: 증례보고)

  • We report on 4 patients suffering from acute neck pain and limited range of motion (ROM) in the cervical spine who were treated with intradermal acupuncture. The treatment effect was evaluated with numeric rating scale (NRS), ROM, and Neck Disability Index (NDI). After treatment, the NRS and NDI scores decreased and the ROM of the cervical spine increased in all patients. This report shows that intradermal acupuncture treatment can be effective for patients with acute neck pain with limited ROM. However, further controlled studies are needed to confirm the effect of intradermal acupuncture on such patients.

  • Objective: In this case report, we aimed to determine the effect of Kyejigadahuang-tang on central post-stroke pain (CPSP). Methods: A 42-year-old woman presented with whole-body neuralgia due to CPSP. According to the diagnostic system based on Shanghanlun provisions (DPIDS), the patient was treated with Kyejigadahuang-tang. The results were evaluated using the Numeric Rating Scale (NRS) and Korean Neuropathic Pain Questionnaire (KNPQ). Results: After administration of Kyejigadahuang-tang for 30 days, the NRS score decreased from 8.5 to 3 and the KNPQ score decreased from 151 to 51. Conclusions: The patient completely recovered from CPSP following treatment with Kyejigadahuang-tang according to DPIDS.

  • This case study aims to report Korean medicine treatment's response to weakness and numbness in chronic inflammatory demyelinating polyneuropathy (CIDP) patient. The patient received Korean medicine treatment during hospitalization, including Duhuo Jisheng Tang and pharmacoacupuncture. The assessment was performed using the Functional Independence Measure (FIM), Numeric Rating Scale (NRS) for numbness, and Oswestry Disability Index (ODI). After 21 days of treatment, the FIM improved from 82 to 126, NRS improved from 6 to 2, and ODI improved from 37 to 8. There were no side effects after receiving Korean medicine. This case suggests that Korean medicine treatment can induce treatment response for lower extremity weakness and numbness in CIDP patients.

Case Report of a Patient Who Experienced Symptomatic Improvement Using Combined TMJ Balancing Therapy to Chronic Patellofemoral Pain (만성 슬개대퇴통증 환자에 턱관절균형요법을 병용하여 증상 개선을 보인 치험 1례)

  • The purpose of this case report is to introduce a case of immediate results with TMJ balancing therapy (TBT) in the treatment of chronic patellofemoral pain (PFP). A 17-year-old female patient with 4-month-old chronic knee pain that was unresponsive to conventional treatment was treated with TBT for a total of 5 sessions. The primary outcomes were evaluated using the Numeric Rating Scale (NRS) and the Korean version of the Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Following the first treatment, the NRS decreased rapidly, and the KWOMAC improved after a total of five treatments. These findings suggest that TBT can be an effective pain management option for chronic PFP patients with temporomandibular joint malposition. However, as this study is a report of a single case, further research is necessary.

The Effect of Sweet Bee Venom Pharmacopuncture(SBVP) on Cancer-Related Pain : A Randomized Controlled Trial and Double Blinded - Pilot study (암성통증에 대한 SBVP 효과 - 무작위대조시험, 이중맹검)

  • Objectives : To investigate the therapeutic effects of SBVP in the treatment of patients with cancer-related pain. Design : A prospective randomized, double-blind, placebo-controlled study of SBVP. Setting : The study was conducted at the East West Cancer Center of Daejeon University Dunsan Oriental Hospital from March 1, 2007 to June 20, 2007. Patients : 11 patients diagnosed with cancer-related pain of over 3rd degree on the Numeric Rating Scale(NRS)(0, no pain at all, 10, worst pain imaginable) were entered into a double-blind, placebo-controlled trial of SBVP. They were randomized into Groups A and B(SBVP and control group, respectively) using the table of random sampling numbers and never informed of their affiliation by the coordinator. 5 of 6 patients in Group A and 4 of 5 patients in Group B completed the clinical trial. Intervention : SBVP(1ml/day) for group A and Normal Saline Placebo(1ml/day) for group B was injected into the abdomen acupoint, Zhong Wan(CV 12). The treatment was administered daily for five days. Outcome Measures : Degree of cancer-related pain was measured using the Numeric Rating Scale(NRS) before and after each treatment for "Pain right now" and "Average pain in last 24 hours". Statistical Analysis : Analysis regarding variations in NRS was carried out by applying t-tests(independent sample t-test and paired sample t-test) and Wilcoxon signed rank test with level of significance at 5%. Results : Differences in NRS of "Pain right now" for the two groups were statistically significant. The mean improvement point of SBVP was significantly higher than the control group($2.48^{\circ}{\pm}1.52$ vs $0.97^{\circ}{\pm}1.88$, p<0.05). Differences in average pain score before and after treatment in SBVP group were also significant($5.13^{\circ}{\pm}1.77$ vs $2.65^{\circ}{\pm}0.67$, p<0.05) compared with control group. The two groups showed no significant differences for long term effects in "Average pain in last 24 hours." Conclusion : Although further study will be needed on the large scale, SBVP shows potential as an effective treatment for immediate relief of cancer-related pain.

  • Objectives: The Purpose of this study is to report the effects of Korean medical treatment on patient of peroneal neuropathy caused by sitting cross-legged. Methods: The patient diagnosed as peroneal neuropathy was treated by Acupuncture, Bee Venom therapy, Herbal medicine. To evaluate the effects of Korean treatment, we used Range of Motion(ROM) of Ankle, Visual Analog Scale(VAS), Numeric Rating Scale(NRS), Ankle-Hind-foot Scale(AHS), Digital Infrared Thermal Imaging(DITI). Results: After treatment, ROM of Ankle, VAS, NRS, AHS were improved. Temperature Difference(${\Delta}T$) was decreased on DITI. Conclusions: This study suggests that Korean medical treatment might be effective on sitting cross-legged patient diagnosed as Peroneal Neuropathy.

A Case Report of a Patient with Lateropulsion in Lateral Medullary Infarction Improved by Korean Medicine Treatment Including Scalp Acupuncture (가쪽쏠림보행을 동반한 외측 연수경색 환자의 두침요법을 병행한 한의치료: 증례보고)

  • The aim of this study was to report the effect of Korean medicine treatment with scalp acupuncture of patient with lateropulsion, dizziness, headache caused by lateral medullary infarction. We treated the patient with scalp acupuncture, acupuncture, herbal medicine. Numeric rating scale (NRS), Korean version of berg balance scale (K-BBS), gait balance evaluation, dizziness handicap inventory (DHI), activities-specific balance confidence scale (ABC) and vestibular disorders activities of daily living scale (VADL) were used to evaluate symptoms. NRS of Lateropulsion, dizziness, headache were decreased. K-BBS score increased from 4 to 56, and in the gait balance evaluation, it returned to normal. DHI, VADL score were decreased and ABC score increased from 0 to 1,300. This case report shows that Korean medicine treatment with scalp acupuncture can be effective in lateral medullary infarction. However, further controlled studies are needed to confirm the effect of scalp acupuncture on such patients.

The Effectiveness of Korean Medicine Treatment Including Facial Chuna Manual Therapy for the Sequelae of Bell's Palsy: Four Case Series (특발성 안면마비 후유증에 대한 안면추나를 포함한 한의한 치료: 4예 보고)

  • This study aims to demonstrate the effectiveness of facial chuna manual therapy in treating the sequelae of Bell's palsy. Four patients were treated with facial chuna manual therapy once a week for 4 weeks, consisting of facial muscle massage, acupoint pressure, contracture chuna, and synkinesis chuna. The changes in symptoms (contracture and sysnkinesis) were measured using the Sunnybrook Facial Grading Scale (SFGS), Synkinesis Assessment Questionnaire (SAQ), Facial Disability Index (FDI), Contracture/Synkinesis scale using a facial scanning system, Numeric Rating scale (NRS) for synkinesis or contracture, and Was It Worth It questionnaire. After treatments, SFGS, Contracture/Synkinesis scale, and NRS for synkinesis or contracture showed significant improvements. SFGS increased in three cases from 39~76 to 52~85 score. SAQ decreased in two cases from 53.33~57.78 to 40.00~55.56. FDI increased in three cases from 120~128 to 138~145. These results suggest that Korean medicine treatment, including facial chuna manual therapy can be effective in improving the sequelae of Bell's palsy.

A Case Report of Ataxia with Cerebellar Atrophy (소뇌위축으로 인한 실조 증례 보고)

  • Objectives: This study is a report of the clinical application of traditional Korean medicine on ataxia with cerebellar atrophy. Methods: The ataxia in this case occurred 5 years ago. The symptoms had been getting worse until the patient came to hospital. The MRI of this patient showed atrophy of the cerebellum. We measured the state of this case with the Korean version of the Berg Balance Scale (K-BBS), before and after treatment and Numeric Rating Scale-11 (NRS-11) of the symptoms, every week. We treated the case with traditional Korean medicine such as acupuncture and Gamiyukmijihwang-tang. Results: After treatment, the score of K-BBS increased, while the score of NRS-11 decreased. Conclusions: According to this study, traditional Korean medicine is effective for controlling ataxia caused by cerebellar atrophy.
