The Effects of Self-Administered Hand Reflexology on Fatigue in Student Nurses in Clinical Practice (자가 손 반사요법이 간호대학생의 임상실습에서의 피로에 미치는 효과)

  • Purpose: This study was to identify the effects of self-administered hand reflexology on level of fatigue in college student nurses during clinical practice. Method: A non-equivalent control group, quasi-experimental research with pre and post-test design was used. Forty-seven student nurses in clinical practice were assigned to either an experimental or control group. The experimental group performed self-hand reflexology for 10 minutes once a day during the 5 days of clinical practice. Results: 1. While there was a significant difference in degree of fatigue and fatigue scores in the experimental group over time, there was no significant difference between the two groups. 2. Vigor score for the experimental group was significantly higher than those of the control group during the 5 days except for day 5, but there was no significant difference between the two groups. 3, Levels of fatigue for the experimental group increased after 1 hour in the 1st post time, but there was no significant difference between the groups. Conclusion: Self-administered hand reflexology was not found to be an effective method for relieving fatigue for student nurses in clinical practice. Also physiological effects were not identified in this study, and so further research is needed.

Postoperative analgesic effects of the quadratus lumborum block in pediatric patients: a systematic review and meta-analysis

  • Background: This study assessed the postoperative analgesic efficacy and safety of the quadratus lumborum block (QLB) in pediatric patients. Methods: Electronic databases were searched for studies comparing the QLB to conventional analgesic techniques in pediatric patients. The primary outcome was the need for rescue analgesia 12 and 24 hours after surgery. Secondary outcomes covered the Face-Legs-Activity-Cry-Consolability Scale (FLACC) scores at various time points; parental satisfaction; time to the first rescue analgesia; hospitalization time; block execution time; block failure rates, and adverse events. Results: Sixteen randomized controlled trials were analyzed involving 1,061 patients. The QLB significantly reduced the need for rescue analgesia both at 12 and 24 hours after surgery (12 hours, relative risk [RR]: 0.45; 95% confidence interval [CI]: 0.01, 0.88; 24 hours, RR: 0.51; 95% CI: 0.31, 0.70). In case of 24 hours after surgery, type 1 QLB significantly reduced the need for rescue analgesia (RR: 0.56; 95% CI: 0.36, 0.76). The QLB also exhibited lower FLACC scores at 1 hour (standardized mean difference [SMD]: -0.87; 95% CI: -1.56, -0.18) and 6 hours (SMD: -1.27; 95% CI: -2.33, -0.21) following surgery when compared to non-QLB. Among QLBs, type 2 QLB significantly extended the time until the first rescue analgesia (SMD: 1.25; 95% CI: 0.84, 1.67). No significant differences were observed in terms of parental satisfaction, hospitalization time, block execution time, block failure, or adverse events between QLB and non-QLB groups. Conclusions: The QLB provides non-inferior analgesic efficacy and safety to conventional methods in pediatric patients.

The Outcomes of Open Ankle Fractures in Patients Managed by Early or Delayed Definitive Fixation: A Comparative Analysis of 73 Patients

  • Purpose: Ankle fracture fixation is the gold standard of treatment but it does have its own complications. There is inadequate data regarding the comparative effectiveness of early vs. delayed fixation for open ankle fracture outcomes. This study compares the clinical and functional outcomes of open ankle fractures treated by early or delayed definitive fixation and identifies the limitations of both methods. Materials and Methods: All 73 patients enrolled in the study underwent surgical intervention within 24 hours of injury. The early fixation group (group A) consisting of 39 patients underwent definitive fixation as an index procedure, while the delayed fixation group (group B) consisted of 34 patients who underwent debridement and external fixator application as an index procedure and definitive fixation when soft tissue condition was conducive. All patients were evaluated at 2, 6, and 12 weeks postoperatively and then three monthly for a year. Results: Enneking and American Orthopaedic Foot and Ankle Society scores were markedly higher in the early fixation group at 6 months postoperatively (p-values <0.001 and 0.011, respectively). However, no discernible intergroup difference was evident at 12 months postoperatively. Between 6 and 12 months, group functional outcome scores were significantly different. At 6 months, there was a substantial difference in dorsiflexion between the two groups (p-values 0.001 and <0.001, respectively), but no difference was observed at 12 months postoperatively. At 6 and 12 months, group average plantar flexions were non-significantly different. Conclusion: Early definitive fixation of complex ankle fractures using a targeted approach produced promising results for lower grade open fractures (grades 1 and 2), and delayed definitive fixation, after initial external fixation to allow for soft tissue stabilization, produced promising results for higher grade open fractures (grades 3A and 3B). At 12-month follow-ups, clinical and functional outcomes achieved using these strategies were equivalent.

A Comparative Study on the Dietary Attitudes, Dietary Behaviors and Diet Qualities of Food and Nutrition Major and Non-major Female University Students (식품영양 전공 및 비전공 여대생의 식태도, 식행동 및 영양평가에 관한 비교 연구)

  • The purpose of this study was to compare dietary attitudes, dietary behaviors and diet qualities of Food and Nutrition major and non-major female student at a university located in Inchon. The subjects included 74 female Food and Nutrition and majors and 45 non-Food and Nutrition major female university students. This cross-sectional survey was conducted using a self-administered questionnaire, and the data were analyzed by a SPSS 10.0 program. The nutrient intake data collected from Three-day recalls were analysed by the Computer Aided Nutritional Analysis Program and then the diet quality was estimated using the Mean Adequacy Ratio (MAR), the Nutrient Adequacy patio (NAR) and the Index of Nutritional Quality (INQ). There was no significant difference in dietary behaviors between Food and Nutrition majors and non-majors. However, most of the female university students had poor eating habits, such as overeating, unbalanced meals, and skipping of meals. In particular, more than 50% of the female university students skipped breakfast. As for weighing themselves once a week, changing dietary lifestyles, calculating food calories and having interest in information on nutrition on health, the ratio of Food and Nutrition majors was significantly higher as compared to that of the non-majors. There was a significant difference in food consumption frequency except for mick and milk products, and iron-rich foods between the Food and Nutrition majors and non-majors. However, there was a significant difference in the frequency scores of iron-rich floods between the Food and Nutrition majors and non-majors. The MAR of Food and Nutrition majors and non-majors were 0.80$\pm$0.13, 0.79$\pm$0.13, respectively. However, in the Food and Nutrition majors and non-majors, the NARs of calcium were 0.61$\pm$0.21 and 0.59 $\pm$ 0.20, and that of iron were 0.61$\pm$0.21 and 0.59$\pm$0.16, respectively, which was extremely low when compared to that of the other nutrients. This result was similar to that of the INQ. Therefore, nutritional education is necessary if female university students are to practice optimal nutrition, including well-balanced diets and eating foods of high nutritional quality.

Effectiveness of herbal ointment Biyeom-go according to cold-heat pattern identification: a subgroup analysis on patients with rhinitis (한열(寒熱) 변증에 따른 한의 외용 치료제 비염고의 치료 효과 : 하위 그룹 분석)

  • Objectives : Hwanglyeonhaedok-tang(黃連解毒湯) is a representative herbal formula with "clear heat(淸熱)" effects. The aim of this study was to evaluate the effects of Hwanglyeonhaedok-tang-based intranasal herbal ointment Biyeom-go based on the cold and heat pattern identification questionnaire(CHPIQ). Methods : We performed a subgroup analysis of the previously published prospective observational study. A total of 58 patients with rhinitis were administered Biyeom-go for 4 weeks, and its effects on the Total Nasal Symptom Score(TNSS), Mini Rhinoconjunctivitis Quality of Life Questionnaire(Mini-RQLQ) score, and nasal endoscopy index score were analyzed based on CHPIQ. Results : Among the 58 patients, the heat and non-heat patterns were shown by 39 and 19 patients, respectively, while the cold and non-cold patterns were shown by 46 and 12 patients, respectively. The change in TNSS from baseline negatively correlated with the heat pattern score(p=0.011). Improvement in TNSS was greater in the heat pattern group than in the non-heat pattern group, with a borderline significant difference(p=0.07). Mini-RQLQ and nasal endoscopy index scores tended to be lower in the heat pattern group than in the non-heat pattern group, but without a statistically significant difference. Conclusion : The findings indicate that CHPIQ is a useful tool for the diagnostic and prognostic evaluation of patients with rhinitis. This study provides fundamental evidence of the close association between the cold-heat pattern in patients with rhinitis and the treatment effects of Biyeom-go.

  • Objectives : The purposes of this study were to examine the reliability and validity of the Korean version of Social Communication Questionnaire (K-SCQ) and to determine cut-off scores for diagnosis of autism spectrum disorder (ASD). Methods : A total of 166 subjects with ASD and their 186 unaffected siblings were recruited through child psychiatry clinics of university hospitals. Board certified child psychiatrists screened all probands suspected to have ASD based on the Diagnostic and Statistical Manual of Mental Disorders, fourth edition. To confirm the diagnoses, the Korean versions of the Autism Diagnostic Observation Schedule and the Autism Diagnostic Interview-Revised (K-ADI-R) were administered to all the subjects. All parents completed the K-SCQ and Social Responsiveness Scale (SRS). The non-ASD siblings were evaluated with the same instruments as the probands with ASD. We performed a factor analysis to examine the structure of K-SCQ. For testing the validity of K-SCQ, we compared the difference in Lifetime and Current scores of probands with ASD and their non-ASD siblings using t-test and analysis of covariance. Correlations between the K-SCQ and other measurements of ASD symptomatology, including K-ADI-R totals and domain scores and SRS, were examined. Receiver operation characteristic curve analysis was performed to extract cutoff scores discriminating affection status. Results : Four factors were extracted through factor analysis of K-SCQ ; 1) social relation and play, 2) stereotyped behavior, 3) social behavior, and 4) abnormal language. Cronbach's internal consistency was .95 in K-SCQ Lifetime, and .93 in K-SCQ Current. There were significant differences in total score of K-SCQ, both in Lifetime and Current between the ASD group and non-ASD siblings group (p<.05). K-SCQ scores were significantly correlated with K-ADI-R subdomain scores and SRS total scores (p<.05). The best-estimate cut-off scores of K-SCQ for diagnosis of ASD were 12 for 48 months and over, and 10 for below 47 months. Conclusion : Our findings suggest that the K-SCQ is a reliable and valid instrument for screening autistic symptoms in the Korean population. Lower cut-off scores than the original English version might be considered when using it as a screening instrument of ASD.

Clinical Efficacy of Endoscopic Pancreatic Drainage for Pain Relief with Malignant Pancreatic Duct Obstruction

  • Objective: This study was conducted to investigate the efficacy of pancreatic drainage for pain relief in advanced pancreatic cancer. Method: Seventy-one patients with pancreatic carcinoma were divided into two groups: dilated and non-dilated pancreatic ducts. All patients underwent endoscopic retrograde cholangiopancreatography (ERCP), endoscopic biliary stenting and pancreatic stenting. Visual Analog Scale (VAS) scores, pain remission rates and survival time were evaluated during follow-up. Results: The post-ERCP VAS score of the dilated group was lower than that of the non-dilated group at 1 and 3 months post-ERCP. There was no difference at 6 months. The pain remission rate in the dilated duct group was significantly higher than that in non-dilated duct group in 1 and 3 months post-ERCP. The median survival times were 8.17 and 8.22 months respectively. Conclusion: Endoscopic pancreatic drainage can relieve pain of advanced pancreatic cancer accompanied by safe dilation of the pancreatic duct.

Growth patterns and nutritional status of small for gestational age infants in Malaysia during the first year of life

  • Purpose: This study aimed to identify small for gestational age (SGA) infants' growth patterns, nutritional status, and associated factors. Methods: This prospective cohort study was conducted at primary-care child health clinics in Greater Kuala Lumpur, Malaysia. The sample consisted of infants who fulfilled the criteria and were born in 2019. The anthropometric data of infants were assessed at birth and at 1, 3, 6, 9, and 12 months. Results: A total of 328 infants were analysed. In total, 27.7%(n=91) of the subjects were SGA infants, and 237 of them were not. Significant differences in the median weight-for-age and length-for-age z-scores were observed between SGA and non-SGA infants at birth, 1 month, 6 months, and 12 months. There was a significant difference between the growth patterns of SGA and non-SGA infants. Birth weight and sex significantly predicted the nutritional status(stunting and underweight) of SGA infants during their first year of life. Conclusion: SGA infants can catch up to achieve normal growth during their first year of life. Even though the nutritional status of SGA infants trends worse than non-SGA infants, adequate infant birth weight monitoring and an emphasis on nutritional advice are crucial for maintaining well-being.

Difference of Pain Depending on the Bed Rest Period after Lumbar Puncture in Patients with Leukemia (백혈병환자의 요추천자 후 침상안정시간에 따른 두통과 요통의 차이)

  • Purpose: This study was conducted for an evidence-based nursing intervention on bed rest period after puncture, by confirming whether there was a difference in scores of headache and back pain between the 1-hour bed rest group and the 6-hour bed rest group following lumbar puncture in patients with leukemia. Method: The subjects were 45 inpatients of the department of hematology at a general hospital in Seoul, from January 1 to March 31, 2005. There were 21 patients in the 1-hour bed rest group and 24 patients in the 6-hour bed rest group. A graphic rating scale was used to measure headaches and back pains. Collected data were analyzed with SPSS 12.0. Non- parametric statistics were used to analyze the data. Results: There was no significant difference between the 1-hour bed rest group and the 6-hour bed rest group in the headache level and back pain level. Conclusion: Therefore, it is considered that a long period of bed rest over 6 hours after lumbar puncture is not a necessary nursing intervention, and repeated studies are required to confirm the result.

The Research of Job Stress and MSDs Symptoms of Small Plants with Agricultural Products (소규모 농산물 가공사업장 작업자의 직무스트레스 평가 및 근골격계부담작업 유해요인 조사)

  • The purpose of this study is to survey job stress and Musculoskeletal Disorder(MSDs) of workers in small plants with agricultural products. The subjects were comprised of 69 workers participated in "Helping the rural women to get the small businesses" project. The questionnaire surveys were Korean Occupational Stress Scale(KOSS), and Legal Risk Assessment of Musculoskeletal Disorders(KOSHA CODE H-30-2003). As a result of the survey, scores of KOSS 8 sub-items except for job demand were similar to that of Korea workers. Prevalence of MSDs symptoms of small plants workers was high percent (44.9%). And the rate according to kinds of product was significant difference at neck (p<0.01), lumbar (p<0.01), and leg (p<0.01). There was no significant difference between view person sign of MSDs and non MSDs according to sex, year, the number of workers, working years, working time per one day, rest time per one day, and subjective working intensity, but there were significant difference between high stress group and low stress group about MSDs at interpersonal conflict(p<0.05). Job stress and MSDs were important factor to workers of small plants producted agricultural products. Therefore, starting a business with small plats, workers consider interpersonal conflict and body part showing MSDs.
