마사지요법이 미숙아의 수유불내성과 신체성장에 미치는 효과 (Effects of Massage Therapy on Feeding Intolerance and Physical Growth in Premature Infants)

  • Purpose: The purpose of this study was to identify effects of Massage therapy on feeding intolerance and physical growth in premature infants. Methods: This study was conducted in the NICU of U university hospital, from June to December 2014. A quasi experimental design was used. A total of 60 premature infants were randomly assigned into two groups of 30 infants each. Infants of control group were given conventional treatment, while infants of experimental group given conventional treatment plus massage therapy. Massage therapy was performed for 15 minutes, 60 min before feeding, 3 times per day for 21 times over 7 days. The number of feedings withheld for feeding intolerance, number of gastric residuals, number of fecal excretions and physical growth variables (weight, height) were measured. Results: After the intervention, number of fecal excretions and weight gain in the experimental group were significantly higher than that of the control group. Also, number of gastric residuals in the experimental group was lower than that of the control group. Conclusion: Massage therapy laid the basis for nursing intervention to promote feeding tolerance and physical growth in premature infants.

Catheter-related bloodstream infections in neonatal intensive care units

  • Central venous catheters (CVCs) are regularly used in intensive care units, and catheter-related bloodstream infection (CRBSI) remains a leading cause of healthcare-associated infections, particularly in preterm infants. Increased survival rate of extremely-low-birth-weight infants can be partly attributed to routine practice of CVC placement. The most common types of CVCs used in neonatal intensive care units (NICUs) include umbilical venous catheters, peripherally inserted central catheters, and tunneled catheters. CRBSI is defined as a laboratory-confirmed bloodstream infection (BSI) with either a positive catheter tip culture or a positive blood culture drawn from the CVC. BSIs most frequently result from pathogens such as gram-positive cocci, coagulase-negative staphylococci, and sometimes gram-negative organisms. CRBSIs are usually associated with several risk factors, including prolonged catheter placement, femoral access, low birth weight, and young gestational age. Most NICUs have a strategy for catheter insertion and maintenance designed to decrease CRBSIs. Specific interventions slightly differ between NICUs, particularly with regard to the types of disinfectants used for hand hygiene and appropriate skin care for the infant. In conclusion, infection rates can be reduced by the application of strict protocols for the placement and maintenance of CVCs and the education of NICU physicians and nurses.

수집모유의 미생물오염과 항균제 내성에 관한 연구 (A Study on the Bacterial Contamination and Antimicrobial Resistance in Expressed Human Milk)

  • Purpose: This study is about safety of the expressed human milk by investigating its bacterial contamination and antibiotic resistance of the microbes in the expressed milk. Methods: The data are collected from the 156 mothers and their newborn infants in NICU of U University Hospital from January 2004 to September 2005. Results: 1) The expressed human milk was contaminated by Staphylococcus epidermidis for 66.7%, Two mixed bacterial strain for 11.5%, Acinetobacter species for 8.0%, Klebsiella species for 4.6%, Staphylococcus aureus for 4.6%. 2) The microbes in the human milk had high resistance to the Antimicrobial agents: 77.5% for Penicillin-G, 66.6% for Oxacillin, and 63.7% for Cephalothin. 3) The distribution of microbes showed a significant depending on the place of the milk expression (p=.020). In particular, mixed bacterial strain was found more in the milks expressed at home than the milk expressed at the hospital. Conclusion: This study shows the importance of systematic education of feeding process in expressed milk: poor management of a breast pump, inadequate hand washing and imperfect breast cleaning explain the reasons of contamination 156 cases.

교정나이 40주(만삭)까지 미숙아의 성장패턴 (Growth Patterns of Premature Infants Up to 40th Term Week of Corrected Age)

  • Purposes: Study purpose were to describe growth patterns of premature infants in weight, length and head circumference from birth to 40th week of corrected ages (CA) and to explore factors affecting patterns. Methods: A longitudinal descriptive study was conducted with 267 premature infants. They were categorized into 2 groups; GA group with measurements at birth and the CA group with measurements at CA, which was categorized into 3 groups (group 1-3) by WHO guideline for gestational age (GA) at birth. Results: GA group presented greater measures in all than CA group at same week of life. Among CA groups, group 3 showed the highest measurements, up to 37 weeks of life, though this disappeared at 38-40 weeks. Reversely, group 1 revealed the highest growth rates in all measures, followed by group 2 and group 3. Significant interaction was observed in all measures between week of life and any type of groups. Conclusions: Higher measures in GA group, as well group 3 among CA groups, supported the superiority of intra-uterine environment overriding quality of regimen from NICU. Regardless of growth acceleration, smaller infants remain smaller, indicating that intra-uterine thrifty phenotype may continue at least up to the 40th week of CA.

신생아 기흉의 임상적 고찰 (Clinical Evaluation of Neonatal Pneumothorax)

  • From 1991 to 1994, we experienced 24 cases of neonatal pneumothorax who were admitted to the Neonatal Intensive Care Unit[NICU , Chosun University Hospital. The Following results were obtained.1 The incidence of neonatal pneumothorax was 0.70%, and there were 8 spontaneous pneumothoraces and 16 secondary pneumothoraces. 2 The clinical manifestation of neonatal pneumothorax was as followed. Male infant was dominant[M:F=2:1 , the onset was within 24 hours in the majority[83% , and the right side[62% was more frequent than the left side. The gestation duration and birth weight show no correlation with underlying neonatal pneumothorax. The pulmonary diseases were meconium aspiration syndrome and hyaline membrane disease, and the incidence of those was 58%. Meconium aspiration syndrome occurred earlier than hyaline membrane disease. Symptoms and signs were tachypnea[46% , cyanosis[21% , irritability[13% , chest retraction[8% and apnea[8% .3 The treatments performed were oxygen therapy[17% , thoracentesis[4% and closed thoracostomy with underwater seal drainage[79% . The Mean duration of air leakage was 11.7 hours, and the mean drainage time was 4.35$\pm$1.3day. 4 The overall hospital mortality was 33%, and the rate of complication was 46%. The complications were metabolic acidosis, atelectasis, pleural effusion, pulmonary hemorrhage and pneumonia. We concluded that the prognosis was related to the underlying pulmonary disease.

감각자극이 미숙아의 체중 증가, 행동상태 및 생리적 반응에 미치는 영향 (Effects of a Sensory Stimulation on Weight Gain, Behavioral State, and Physiological Responses in Premature Infants)

  • Purpose: The purpose of this study was to examine the effects of sensory stimulation on premature infants. Method: Thirty three premature infants admitted to NICU of D University Hospital in C city were randomly assigned in two groups (Experimental group: 16, Control group:17). For the experimental group, tactile and kinesthetic stimulation developed by Dr. Field was applied 2 times a day for 10 days. Behavioral state was measured using the Anderson Behavioral State Scale (ABSS). Heart rate, respiration, and oxygen saturation were obtained for each infant before and after sensory stimulation. Hypothesis testing was done using the $\chi$$^2$- test, student t-test, and repeated measures of ANOVA. Result: Hypothesis 1: There was a significant difference in the daily body weight gain between experimental and control group (F= 40.77, p= .0001). Hypothesis 2: There was a significant difference in the frequency of 'inactive awake state' between two groups ($\chi$$^2$= 39.778, p= .001). Hypothesis 3: There were significant differences in the mean of heart rate and $O_2$saturation between two groups (t= -2.174, p= .037; t= 3.080, p= .005). However, there was no significant difference in the mean of respiration rate between two groups (t= -1.966, p= .581). Conclusion: The effectiveness of a sensory stimulation on weight gain and behavioral state in premature infants was supported. Further study is recommended to develop a sensory stimulation method as an independent nursing intervention for premature infant.

모성역할 증진 프로그램이 미숙아어머니의 모성역할 긴장, 양육 자신감, 모성 정체감에 미치는 효과 (Effects of a Maternal Role Promotion Program for Mothers of Premature Infants on Maternal Role Strain, Maternal Role Confidence and Maternal Identity)

  • Purpose: Purpose of this study was to identify the effects on maternal role strain, maternal role confidence and maternal identity of a maternal role promotion program for mothers of premature infants. Method: A quasi-experimental non-equivalent pre-and-post test was used to compare the two groups. The data were collected from 62 mothers from September 10. 2003 to August 30, 2004 at Neonatal Intensive Care Unit(NICU) located in S. city and K. city. The maternal role promotion program was provided three times during the period from two-three days after the baby's admission to 1 month after the baby's discharge. The instruments for measurement were the Perceived Role Difficulty & Steffensmeier Scale, Self Confidence Scale and Semantic Differential Scale. Data were analyzed by means of frequency, mean and SD, $x^2$-test and t-test. Results: There were significant differences in maternal role strain level between the experimental and control group (t=2.163, p=.035), in the maternal role confidence between the two groups(t=-5.645, p=.000) and in maternal identity between the two groups(t=-4.923, p=.000). Conclusion: The results of this study indicate that the maternal role promotion program had positive effects in decreasing maternal role strain levels and increasing maternal role confidence and maternal identity.

신생아중환자실 캥거루 케어 전국 실태조사- 간호행위, 장애요인, 간호사의 지식과 신념 (National Survey of Kangaroo Care Practice, Barriers, Knowledge, and Belief)

  • Purpose: A National survey was conducted to assess neonatal intensive care nurses' practice, barriers, knowledge, and belief regarding Kangaroo Care (KC). Methods: A descriptive survey was conducted. Kangaroo care questionares were sent to nurses in all hospitals in Korea whose were identified as providing neonatal intensive care services(N=263). Descriptive statistics were used to summarized the data. Results: Among 67 neonatal care units, 61.1% adapted KC in their practice. About 60% of nurses in the KC providing hospital actually practiced KC. Major barrier of practicing KC were infant safety concerns, as well as work load of nurses. Respondants who had practiced KC were more knowledgable and were more positive in their belief regarding KC. Conclusion: The findings suggest that in order to overcome barriers of practicing KC, educational programs are recommended designed for nurses. In addition, development of KC practice guideline is necessary to facilicate successful and safe KC.

고위험신생아 아버지가 지각한 스트레스와 간호지지 (Perceived Parental Stress and Nursing Support for Fathers of High Risk Infants)

  • Purpose: The purpose of this study was to examine the paternal stress and nursing support as perceived by fathers of high risk infants admitted to Neonatal Intensive Care Units (NICU). Methods: The study participants included 88 fathers of high risk infants in NICUs. Their parental stress and nursing support were measured using a self-report questionnaire. Results: The parental stress level of the fathers was 3.57 of a possible 5. Among the parental stress items, 'Relationship with infant and parental role' had the highest scores. The perceived nursing support level was a relatively high, 3.90 on a 5-point scale. The lowest score was for the item 'showed concern about my well-being.' The parental stress was lower in fathers with low income, higher in fathers in nuclear families and when the infants' condition was serious. Conclusion: The findings indicate that fathers of high risk infants experience intense parental stress. Thus when designing care to provide support for these fathers and their infants, it is necessary to encourage the fathers' engagement, provide information on how to respond to the baby, and include supportive care to the fathers.

Robust Design of Pulse Oximeter Using Dynamic Control and Motion Artifact Detection Algorithms

  • Arterial oxygen saturation ($SpO_2$) monitoring for newborns requires special attention in neonatal intensive care units (NICUs). Newborns have very low photo-plethysmogram (PPG) amplitudes and their body movements are difficult to contain. Hardware design and its associated signal processing algorithms should be robust enough so that faulty measurements can be avoided. In this study, improved designs were implemented to deal with low perfusion, motion artifact, and the influence of ambient light. Dynamic range was increased by using different LED intensities and a feedback system. To minimize the effects of motion artifact and to discard other unqualified data, four additional algorithms were used, which were based on dual-trace detection, continuity of DC level, morphology of PPG, and simultaneity check of $SpO_2$. Our $SpO_2$ system was tested with newborns with normal respiration in the NICU. Our system provided fast, real-time responses and 100% artifact detection was accomplished under 84% of $SpO_2$.
