Synthesis and Antimicrobial Activity of N-[2-(aryl/substituted aryl)-4-oxo-1,3-thiazolidin-3-yl]pyridine-4-carboxamide

  • A series of isonicotinyl hydrazones and their 4-thiazolidinones have been synthesized by condensation of isonicotinic acid hydrazide with various aromatic aldehydes to yield Schiff's bases, followed by the cyclocondensation of Schiff's bases with 2-mercaptoacetic acid to yield their 4-thiazolidinones. The synthesized compounds have been characterized by their elemental, analytical and spectral studies. All these compounds were evaluated for their invitro antimicrobial activity against a spectrum of non-resistant and resistant microbial organisms. These studies proved that compounds 5e,i against B. subtilis; 5e,f,h against B. anthracis; 5g,i against S. aureus showed good activity at lower concentrations. Compounds 5d-5i displayed significant activity against resistant strain of K. pneumonia with minimum inhibitory potency in the concentration range of 2-16 ug/ml.

  • Fractures of the styloid process of the temporal bone may occur with or without an obvious relation to trauma. The incidence of either isolated styloid process fracture or in combination with mandibular fractures is rare, and such occurrences are often misdiagnosed or neglected. A fractured styloid when displaced may impinge on adjacent vital structures, leading to neurological or vascular symptoms that vary according to the anatomical structure compressed. Styloid process fractures associated with atlas/C1 fractures have also been rarely reported in the literature. In this review of literature, the majority of patients was treated conservatively, as few demonstrated the necessity of surgical intervention. There is a definitive need for a protocol to recognize and classify styloid fractures to plan for further treatment. The aim of this review was to achieve a comprehensive understanding of all types of styloid fractures, determine the clinical severity of symptoms, and to consider management and prognosis. In addition, a new classification of cervico-stylo-mandibular fractures is proposed based on important evidence in the literature regarding clinical and radiographic factors that might influence the treatment and prognosis of such fractures.

  • Yttrium aluminum garnet, $Y_3Al_5O_{12}$ (YAG) is an extensively used solid-state laser host material. YAG nanocrystals were synthesized using low-temperature glycol method, a modified sol-gel method performed at low temperature that consists of a mixture of salts that are mostly nitrates in an aqueous media. Single-phase nanocrystalline YAG was obtained at $850^{\circ}C$, which is a much lower temperature than with other techniques such as a wet-chemical technique. The structural characterization is done by powder X-ray diffraction, scanning electron microscopy and transmission electron microscopy. A crystallite size range of 20-50 nm was observed for the materials prepared at $850-950^{\circ}C$.

Identification of Functional Site of S-Modulin

  • S-modulin in frog or its bovine homologue, recoverin, is a 26 kDa EF-hand $Ca^{2+}$-binding protein found in rod photoreceptors. The $Ca^{2+}$ -bound form of S-modulin binds to rhodopsin kinase (Rk) and inhibits its activity. Through this regulation, S-modulin is believed to modulate the light-sensitivity of a rod. In the present study, we tried to identify the interaction site of the $Ca^{2+}$ -bound form of S-modulin to Rk. First, we mapped roughly the interaction regions by using partial peptides of S-modulin. The result suggested that a specific region near the amino terminus is the interaction site of S- modulin. We then identified the essential amino acid residues in this region by using S-modulin mutant proteins: four amino acid residues were suggested to interact with Rk. These residues are located in a small closed pocket in the $Ca^{2+}$-free, inactive form of S-modulin, but exposed to the surface of the molecules in the $Ca^{2+}$ -bound, active form of S-modulin. Two additional amino acid residues were found to be crucial for the $Ca^{2+}$ -dependent conformational changes of S-modulin.   • Purpose: This is the descriptive research project of which purpose is to acquire the practice, research, and educational data by establishing the database after confirming, classifying, and relating the nursing diagnosis, nursing intervention, and nursing outcome of Breast cancer patients by using the Yoo Hyung-sook's(2001) related 3N database model as the tool. Method : The Nursing Data occurring on Breast cancer patients nursing process was mapped to nursing diagnosis of NANDA, nursing interventions of NIC, nursing outcomes of NOC the 3N database linkage database which is related with the nursing process that was developed by using Yoo Hyung-sook's(2001). Result : 1. The nursing diagnosis were totally 505, and 26 articles of the nursing diagnosis were applied among 149 nursing diagnosis classification systems. 2. As for the nursing intervention, 250 articles(5l.4%) of nursing intervention were applied among 486 nursing intervention classification systems. 3. Regarding the nursing outcome, 28 articles(1l.2%l of the nursing outcome were applied among 250 nursing outcome classification systems. Conclusion: The result of this research in which the relating among the nursing diagnosis, nursing intervention, and nursing outcome of Breast cancer patients by using 3N nursing database was established is thought to be applied in the research and practice as well as to be utilized in the lecture or practice of the nursing process.

Microwave Assisted Extraction, Optimization using Central Composite Design, Quantitative Estimation of Arjunic Acid and Arjunolic Acid using HPTLC and Evaluation of Radical Scavenging Potential of Stem Bark of Terminalia arjuna

  • The optimization and microwave assisted extraction of stem bark of Terminalia arjuna, quantitative estimation of the marker compounds arjunic acid and arjunolic acid using HPTLC and the evaluation of free radical scavenging activity has been performed in this study. The central composite design was used for optimization and the values of parameters for optimized batch of microwave assisted extraction were 1000 W (Power), 3 minutes (Time) and 1/120 (Solid/solvent ratio). The solvent system to carry out the HPTLC was toluene: acetic acid: ethyl acetate (5: 5: 0.5) and quantitative estimation was done using standard equations obtained from the marker compounds. The in-vitro free radical scavenging activity was performed spectrophotometrically using ascorbic acid as standard. The value of estimated percentage yield of arjunic acid and arjunolic acid was 1.42% and 1.52% which upon experimentation was obtained as 1.38% and 1.51% respectively. The DPPH assay of the different batches of microwave assisted extraction and marker compounds taken suggested that the marker compounds arjunic acid and the arjunolic acid were responsible for the free radical scavenging activity as the batch having the maximum percentage yield of the marker compounds showed best free radical scavenging effect as compared to standard ascorbic acid. The $IC_{50}$ value of the optimized batch was found to be 24.72 while that of the standard ascorbic acid was 29.83. Hence, the yield of arjunic acid and arjunolic acid has direct correlation with the free radical scavenging activity of stem bark extract of Terminalia arjuna and have potential to serve as active lead compounds for free radical scavenging activity.

가정간호 요구에 관한 연구 - 일 지역 종합병원 퇴원환자를 중심으로 - (A Study of Home Care Needs of Patients at Discharge from a General Hospital)

  • This study was conducted to provide basic policy of home care service centered on early discharging patients from general hospitals. This subjects for this study were 291 discharging patients from university hospital in Chon-ju area. The data were collected during the period from July 1, 1993 through July 16, 1993. The measurement tools were developed based on 9 categorized human responses patterns suggested by NANDA and modified by the research team. The collected data were processed with SPSS/PC + frequency, percentage and mean were used for analysis. From the study, the following summerized conclusions have been drown. A. For home care needs, the mean was $19.0\%$ of possible to total 100 and in a range of $6.2\~39.5\%$ 1. Exchanging Pattern: $17.8\%$ 2. Communication Pattern: $8.6\%$ 3. Relating Pattern : $15.4\%$ 4. Valuing Pattern: $13.4\%$ 5. Choosing Pattern : $6.2\%$ 6. Moving Pattern : $22.9\%$ 7. Perceiving Pattern: $16.5\%$ 8. Knowing Pattern : $30.8\%$ 9. Feeling Pattern : $39.5\%$ B. Response to home care services, 1. $85.6\%$ of subjects didn't hear about home care service. 2. Over $90.0\%$ subjects approved home care service. 3. $83.5\%$of subjects were willing to use home care service. 4. $85.9\%$ of subjects will follow to early discharge order. On the basis of this findings, further studies are required to compare home care needs between patients, their family and community health people groups. And also required to develop to information strategies for home care nursing service.

산부인과 간호단위의 간호과정과 SNOMED CT를 이용한 간호진단 온톨로지의 구축 (Construction of the Nursing Diagnosis Ontology in Obstetric and Gynecologic Nursing Unit using Nursing Process and SNOMED CT)

  • Purpose: This study was performed to propose an ontology methodology based on standardized nursing process as framework in obstetric and gynecologic nursing practice. Methods: The instrument used in this study was based on the nursing diagnosis classification established by North American Nursing Diagnosis Association (NANDA) (2009-2011), fifth edition of the Nursing Interventions Classification (NIC) (2008), forth edition of the Nursing Outcomes Classification (NOC) (2008) developed by Iowa State University and systematized nomenclature of medicine clinical terms (SNOMED CT). The nursing records data were collected from electronic medical records of one hospital from August to October 2010. Results: One hundred and forty-one nursing diagnosis statements used in obstetric and gynecologic nursing unit were linked standardized nursing classifications and constructed nursing diagnosis ontology including interoperability. Conclusion: Not only will this result be helpful to complete nurse's lack of knowledge and experience, it will also help to determine nursing diagnosis logically by using standardized nursing process. It will be utilized as the method to construct ontology including interoperability in other nursing units. It will be presented nursing interventions according to nursing diagnosis and thus will be easier to establish nursing planning. This can provide immediate feedback of the nursing process application.

Low doses of amitriptyline, pregabalin, and gabapentin are preferred for management of neuropathic pain in India: is there a need for revisiting dosing recommendations?

  • Background: Current therapy for the treatment of neuropathic pain is often unsatisfactory. Considerable variation in treatment pattern still exists in spite of availability of sufficient literature from various guidelines. Recent Indian market data suggested that the utilization (sale) of drugs such as amitriptyline, pregabalin, and gabapentin was more for low-dose unit packs than that of the high-dose unit packs, raising the belief that these drugs are prescribed at a lower dose than is actually recommended in the guidelines. To test this hypothesis, a survey was conducted across speciality throughout the country to observe the prescription pattern of these drugs amongst the health care providers in India. Methods: Three hundred fifty survey forms were distributed of which 281 forms were included for analysis. Results: It was observed that the commonly used initiation and maintenance dose for amitriptyline, pregabalin, and gabapentin was 5-10 mg/day, 50-75 mg/day, and 100-300 mg/day, respectively. The reason to select the lower dosages was to have a balancing effect to achieve good efficacy with minimum side effects. Care-givers reported no side effects/not many side effects as a reason in 22.2%, 16.88%, and 23.86% patients with amitriptyline, pregabalin, and gabapentin, respectively. Sedation and giddiness were commonly reported with all three drugs. Conclusions: Commonly prescribed drugs for management of neuropathic pain, such as amitriptyline, pregabalin, and gabapentin are preferred at lower doses in Indian clinical settings. Acceptable efficacy and low tolerance to the standard dosage is believed to be the reason behind the prescribed dose.

모바일 응용 기반 간호과정 교육 프로그램 개발 (Development of Education Program for Nursing Process based on Mobile Application)

  • 본 연구는 간호사 및 간호학생을 위한 간호진단, 간호중재, 간호결과 분류체계의 간호과정 프로그램을 모바일 응용 기반으로 개발하였다. 연구재료는 표준화된 분류체계인 북미간호진단협의회의 간호진단 분류체계와 아이오와 대학을 중심으로 개발된 간호중재 분류체계, 간호결과 분류체계를 사용하였다. 기존 연구 방법은 간호과정의 일부분만을 선택하여 개발하므로 환경에 제한적인 프로그램으로 임상에 일반화시켜 환자들에게 적용하기 어려웠다. 그러나 본 연구는 진단-결과-중재의 전체를 연계시킨 프레임워크를 개발하므로 어떠한 임상환경에서도 적용이 가능한 가이드라인으로 개발하였다. 개발된 프로그램은 한글판으로 3월부터 앱 스토어에 등록되었으며 간호교육 도구로 적극적으로 활용되기를 기대한다.
