N-BACK 프로그램이 뇌졸중 환자의 인지, 시지각, 우울, 불안과 일상생활활동에 미치는 효과 (Effect of N-BACK Program for Cognition, Visual-Perception, Depression, Anxiety, Activity of Daily Living in Stroke Patients)

  • 본 연구의 목적은 임상에서 사용하고 있는 뇌 자극 훈련인 N-BACK 프로그램을 뇌졸중 환자들에게 적용하여 인지, 시지각, 우울, 불안과 일상생활활동에 미치는 임상적 효과를 증명하고자 하는 것이다. 이를 증명하기 위해 뇌졸중으로 진단받고 유병기간이 6개월 이상인자로 한국판 정신상태판별검사(MMSE-K)에서 18~23점으로 인지기능 손상이 의심되는 자로써 무작위로 실험군과 대조군 각각 10명씩 선정하였다. 연구자 1인의 진행 하에 임상경력 5년 이상의 숙련된 치료사 8명에 의해 연구가 진행되었으며, 4주간 주 5회 1일 30분, 총 20회를 실시하였다. 그 결과 뇌졸중 환자에게 뇌 자극 훈련인 N-BACK 프로그램을 적용한 실험군이 대조군보다 인지, 시지각, 우울, 불안과 일상생활활동 기능 향상에 긍정적인 결과를 보였다. 이 연구로부터 얻은 결과들은 뇌졸중 환자의 인지, 시지각, 우울 및 불안의 향상을 위해 N-BACK 프로그램을 적용하면 환자의 재활에 긍정적인 효과가 있음을 기대할 수 있고, 그로 인해 일상생활 활동이 보다 독립적으로 향상될 것으로 기대할 수 있다.

Effects of a Postpartum Back Pain Relief Program for Korean Women

  • Purpose. Despite the high prevalence of back pain and its subsequent effects in post-partum women, intervention programs are scarce. The purpose of this study was to test the effects of a back-pain-reducing program on post-partum women who experienced low-back pain during pregnancy. Methods. A non-equivalent control-group pretest-posttest design was used. Pregnant women who attended a hospital for prenatal check-ups and experienced back pain participated in an intervention program (n=27), and the results were compared with women in a control group from another hospital (n=25). Results. At 8 weeks post-partum, the pain intensity, functional limitations were lower in the intervention group than in the control group. However, differences in mean change of the pain intensity and functional limitations between 36 and 39 weeks of gestation and at 8 weeks post-partum were not statistically significant between the groups. Moreover, the flexibility, post-partum functional status, and post-partum depression did not differ significantly between the groups. Conclusions. A back-pain-relief program in this study was not effective to reduce the back-pain intensity in post-partum women and to decrease the associated functional limitations. The implications for nursing practice and directions for future research are discussed.

  • Purpose: This study aimed to evaluate the effects of exercise program on chronic low back pain, daily living disability and depression in chronic low back pain patients treated with epidural injections. Methods: The design of this study was a nonequivalent control group pretest-posttest experiment. The sample was recruited among low back pain patients treated with epidural injections from an orthopedic specialty hospital. Participants were randomly assigned to a treatment group (n=25) or a comparison group (n=27). The treatment was a six week exercise program for low back pain. Data were collected from September to November 2011, and were analyzed using descriptive statistics, $x^2$-test, one-tailed t-test for independent samples, and Mann-Whitney U test using the SPSS/WIN 12.0 program. Results: Patients in the treatment group reported statistically significantly lower levels of back pain on flexion and extension, less daily living disability, and less depression than those in the comparison group. Conclusion: The back pain relief exercise program could be an effective adjunct nursing intervention for low back pain patients treated with epidural injections.

Effect of Neuromuscular Stabilization Exercise Program Using Whole Body Vibration on Patients with Low Back Pain

  • Objective: The purpose of this study is to investigate the effect of stabilization exercise on whole-body vibration on pain, dysfunction, psychosocial factors, balance ability, and abdominal contraction with patients with low back pain. Design: A randomized controlled trial Methods: A total of 34 patients with low back pain were assigned randomly to experimental group (n=17) and control group (n=17). Both groups underwent a neuromuscular stabilization exercise program. In addition, the experimental group implemented the neuromuscular stabilization exercise program using whole-body vibration. All interventions were applied 60 min per session, 3 times per week for total 4 weeks. Numeric Rating Scale (NRS), Korean version of Oswestry Disability Index (K-ODI), Fear-Avoidance Beliefs Questionnaire (FABQ), balance ability, muscle thickness and contraction ratio were compared to evaluate the effect on intervention. Results: Both groups showed significant differences in NRS, balance ability, and muscle thickness in contraction, contraction ratio before and after intervention (p<0.05). In addition, the experimental group showed significant difference in the amount of change in NRS, balance ability and muscle thickness in contraction, contraction ratio values than the control group (p<0.05). Conclusions: Neuromuscular stabilization exercise program combined with whole-body vibration stimulation has been proven to be an effective and clinically useful method to decrease pain, dysfunction, increase balance ablilty, and transverse abdominis muscle thickness in contraction and contraction ratio for patients with low back pain.

Effect of High-Intensity Complex Exercise Program Using Whole-Body Vibration and Respiratory Resistance for Low Back Pain Patients with High Obesity

  • Objective: The purpose of this study was to investigate the effect of high-intensity complex exercise program using whole-body vibration (WBV) and respiratory resistance on pain and dysfunction, psychosocial level, balance ability, and pulmonary function in low back pain (LBP) patients with high obesity. Design: A randomized controlled trial Methods: A total of 44 LBP patients withhigh obesity (body mass index, BMI≥30kg/m2) were randomly assigned to an experimental group (n=22) and a control group (n=22). Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the high-intensity complex exercise program combined with WBV and respiratory resistance. In order to compare the effects depending on the intervention methods, numeric pain rating scale (NRPS), Roland-Morris disability questionnaire (RMDQ), fear-avoidance beliefs questionnaire (FABQ), balance ability, and pulmonary function were used for measurement. Results: Both groups showed significant differences in NRPS, RMDQ, FABQ, balance ability before and after intervention (p<0.05). In addition, the experimental groupshowed significant difference in the amount of change in RMDQ, balance ability and pulmonary function values than the control group (p<0.05). Conclusions: High-intensity complex exercise program using WBV and respiratory resistance has been proven to be an effective and clinically useful method to decrease dysfunction, increase balance ablilty, and pulmonary function for LBP patients with high obesity.

사무직 근로자의 허리통증 및 기능 개선에 대한 3가지 중재의 효과 비교 (Comparison of the Effects of Three Interventions on Back Pain and Functional Improvement in Office Workers)

  • Purpose : In this study, rectus abdominis relaxation intervention was administered to office workers who were experiencing low back pain due to sitting for extended periods of time in an incorrect posture. This study aimed to develop an effective treatment program for individuals who experience low back pain. This was done by verifying changes using the Korean Oswestry Disability Index (K-ODI) and considering kyphosis. Both factors are related to low back pain. Methods : This study included 39 office workers with low back pain. The participants were randomly assigned to three groups. 1) The functional massage and self-stretching (FAS) group (n=14). 2) The diaphragmatic breathing and self-stretching (DAS) group (n=13). 3) The self-stretching (S) group (n=12). All groups applied the intervention for 30 minutes a day, thrice a week for four weeks. All participants were evaluated using the K-ODI and thoracic kyphosis measurements before and after the intervention. Results : The findings of this study are as follows. All three groups had improved K-ODI scores after the intervention. The FAS and DAS groups showed a greater effect than the S group. However, there was no difference in effect between the FAS and DAS groups. Kyphosis was not improved in any of groups after the intervention, and there was no difference between the three groups. Conclusion : This study showed that the FAS, DAS, and S programs were effective relaxation interventions that improved the K-ODI for office workers with low back pain. The FAS and DAS programs were more effective than the S program. Therefore, it is recommended to combine relaxation and stretching of the rectus abdominis muscle for office workers who experience low back pain. Clinically, a relaxation intervention that is most appropriate for the patient, depending on his or her condition, should be.

3D 가상착의 프로그램을 활용한 청소년 남학생의 보디스 원형 비교 (Comparison of bodice prototypes of adolescent male students using 3D virtual simulation program)

  • In this study, we used the 3D simulation program to create bodice patterns for adolescents boy students and analyze the fitting and air gap through 3D simulation. The purpose of this study was to select the bodice pattern method that best suited the body shapes of the adolescents male students. The subjects of this study were the French E pattern, Japanese N pattern, and Korean industrial L and J patterns. The applied size was the sixth Korean human body size survey data of 2010 Korea. The age range of the subjects was 13-18 years. DC Suite Program Ver. 5.1 was used, and SPSS 23.0 program was used for data analysis. As a result of comparing the shapes of the bodice patterns in the 3D simulation program, the E pattern had two waist darts on the front and one shoulder dart and one waist dart on the back, and the J and L patterns only had one dart on the back. The N pattern had no darts on either the front or back. As a result of examining the appearance evaluation of the pattern, air gap, color distribution, and clothing cross-section, the E pattern was evaluated as the closest to the body, and the N pattern was evaluated as the pattern with the largest allowance. The J pattern was evaluated as the best fit for body shape of the male adolescents. However, it is necessary to develop a pattern system by adding extra ease in setting the center front length.

Effects of Thoracic Mobility Exercise Program on Pain, Proprioception, and Static Balance Ability in Patients with non-Specific Chronic Low Back Pain

  • Objective: The objective of this study was to investigate the effect of the thoracic mobility exercise program on pain, proprioception, and static balance ability in patients with non-specific chronic low back pain. Design: Randomized controlled trial design. Methods: Thirty patients with non-specific chronic low back pain participated in this study. The participants were randomized into the thoracic mobility exercise group (n=15) and the lumbar stabilization exercise group (n=15). Both groups received traditional physical therapy for 30 minutes per session. In addition, the thoracic mobility exercise group and the lumbar stabilization exercise group each exercise 3 times a week for 6 weeks. All participants were measured visual analog scale, proprioception test, and static balance ability before and after the intervention. Results: After 6 weeks of interventions, the thoracic mobility exercise group showed greater improvement in visual analog scale, proprioception test, and static balance ability than the LSE group (p<0.05). Further, the thoracic mobility exercise group had significant Enhancements in all measured variables compared to the baselinetest (p<0.05). However, the lumbar stabilization exercise group had significant improvement only visual analog scale, and static balance ability compared to the baselinetest (p<0.05). Conclusions: Our investigation demonstrates that the thoracic mobility exercise is an effective intervention method for improving pain, proprioception, and static balance ability in patients with non-specific chronic low back pain.

허리 불안정성이 있는 허리통증 환자에게 실시한 자가-복합 운동프로그램이 통증과 기능, 심리사회적, 균형 능력 그리고 배가로근에 미치는 효과 (Effect of Self-Complex Exercise Program on Pain, Function, Psychosocial, Balance Ability, and TrA Muscle in Patients with Lumbar Instability: A Randomized Controlled Trial)

  • Purpose : Low back pain (LBP) is reported as a risk of experiencing musculoskeletal disorders due to muscle stiffness and hypokinetics. The lumbar spine in an unstable state causes imbalance and lumbar instability. Therefore, This study examined the effects of lumbar stabilization exercise and self-complex exercise program on pain, function, psychosocial level, static balance ability, and transverse abdominal muscle (TrA) thickness and contraction ratio in patients with lumbar instability. Methods : The design of this is a randomized controlled trial (RCT). Twenty-six LBP patients participated in this study. Screening tests were performed and assigned to the experimental group (n=13) and control group (n=13) using a random allocation program. Both groups underwent a lumbar stabilization exercise program. In addition, the experimental group implemented the self-complex exercise program. All interventions were applied three times per week for four weeks. The quadruple visual analog (QVAS), the Korean version of the Oswestry disability index (K-ODI), Korean version of fear-avoidance belief questionnaire (FABQ), static balance ability, TrA thickness, and contraction ratio were compared to evaluate the effect on intervention. Statistical significance was set at 𝛼=.05. Results : Both groups showed significant differences before and after the intervention in QVAS, K-ODI, FABQ, static balance ability, and TrA thickness in contraction (p<.05). In addition, significant differences in K-ODI and FABQ were observed between the experimental group and control group (p<.05). Conclusion : A lumbar stabilization exercise and self-complex exercise program resulted in reduced dysfunctions, psychosocial stability in patients with lumbar instability. Therefore, Lumbar stabilization exercise and self-complex exercise program for patients with lumbar instability are effective method with clinical significance in improving the function and psychosocial stability.

키네시오 테이핑 자가 적용 교육 프로그램의 참여를 통한 만성요통환자들의 통증관리 인식과 건강관련 삶의 질의 변화 (The Changes of the Pain Care Awareness and Health Related Quality of Life of the People with the Chronic Low Back Pain through the Participation in the Kinesio-Taping Self Application Education Program)

  • 본 연구는 키네시오 테이핑 자가 적용 교육 프로그램의 참여를 통해 만성요통환자들의 통증관리 인식과 건강관련 삶의 질의 변화를 알아보는 데 목적이 있다. 본 연구의 대상은 만성요통환자 30명이며, 실험군(n=15)과 대조군(n=15)으로 구분하였다. 실험군은 키네시오 테이핑 자가적용 교육을 적용하였으며, 대조군은 치료사에 의한 테이핑 처리를 적용하였다. 이에 따라, 통증관리 인식의 변화를 분석하기 위해 캐나다작업수행측정 도구를 활용하여 대상자들의 수행도와 만족도의 변화를 측정하였다. 건강관련 삶의 질의 변화를 분석하기 위해서 간이 건강상태 조사 설문지 Short term-8 health survey(SF-8)을 활용하여, 전반적 건강상태, 신체적 기능, 신체적 역할, 신체 통증, 활력, 사회적 기능, 정신건강, 정서적 역할의 변화를 측정하였다. 통증관리 인식의 분석결과 실험군의 수행도 만족도가 통계적으로 유의미하게 향상되었으며, 사후 검사에서 집단간 차이가 나타났다. 건강관련 삶의 질의 분석결과 실험군의 전반적 건강상태, 신체적 기능, 신체적 역할, 신체 통증, 활력, 사회적 기능, 정신건강, 정서적 역할이 통계적으로 유의미하게 향상되었으며, 사후 검사에서 집단간 차이가 나타났다. 키네시오 테이핑 자가적용 교육 프로그램은 일상생활에서 통증을 관리하여 건가관련 삶의 질을 향상시키는데 의미 있는 방법이다. 따라서, 카네시오 테이핑 자가적용 교육 프로그램을 만성요통환자들에게 고려해야 할 것이다.
