중증근무력증에 의한 개의 거대식도증 3례 (Megaoesophagus Seconday to Myasthenia Gravis in Three Dogs)

  • Three dogs (An 8 years-old intact female Poodle, a 7 years-old intact male Schunauzer, and an 8 yearsold Golden Retriever) were presented due to acute vomiting, dyspnea, and generalized weakness. Megaesophagus was confirmed through radiographic examination in all 3 dogs. Relative oesophageal diameter (ROD) was measured and results of ROD measurements showed the possibility of megaesophagus secondary to myasthenia gravis in three dogs. Thus we performed anticholinesterase test as screening test for myasthenia gravis. In all three dogs, esophageal diameter was reduced after neostigmine methylsulfate administration. For definite diagnosis of acquired myasthenia gravis, serum acetylcholine receptor antibody titer was measured, but definite diagnosis was confirmed only in one case. However, based on history, radiographic findings, anticholinesterase test, ROD measurement, other two cases were still suspected as megaesophagus secondary to myasthenia gravis. Treatment with pyridostigmine bromide was initiated in all dogs, and improvement of esophageal diameter was shown in all dogs. One dog was successfully managed for 15 months after initial treatment and, is still alive, but other two dogs were died shortly after initial treatment, because of severe aspiration pneumonia.

안구형 중증 근무력증 치험 1례 (Case Study on Ocular Myasthenia Gravis)

  • Objectives : The purpose of this study is to report the effectiveness of Korean medical treatment on ptosis in myasthenia gravis. Methods : We treated the patient who had suffered from rt. ptosis and was diagnosed with myasthenia gravis with acupuncture, electropuncture, herbal acupuncture, cupping therapy, herbal medicines and western medicine such as corticosteroids and acetylcholinesterase inhibitors. The effectiveness of treatment was evaluated through Relative Interpalpebral Fissure(IPF) and photography. Results : After the treatments, relative Interpalpebral Fissure(IPF) was increased and improvement remained for three months after the treatment was finished. Conclusions : The result indicates that combination therapy of Korean medical treatment and western medicine had an effect on treatment of ptosis with myasthenia gravis.

흉선종의 술후 예후판정요인의 분석 (Postoperative Analysis of Prognostic Factors of Thymoma)

  • In this study, the authors analyzed the prognostic value of four clinical variables[age and sex of patients, association with myasthenia gravis and clinical stage] and histological type in 30 consecutive patients with thymoma, histologically classified as cortical[10],medullary[5] and mixed[15]type according to Marino and Muller-Hermelink classification. There were significant differences between the histological types in the frequency of the different tumor stages and myasthenia gravis and prognosis.Most of the cortical thymomas were at stage III and all of the medullary and most of the mixed tumors at stage I or II.Myasthenia gravis occurred more commonly in patients with cortical[30%] and mixed thymoma[60%] than in patients with medullary thymoma[10%]. Follow-up was conducted in 30 patients,with follow-up range from 3 months to 120 months[mean,47.3months]. 5 year actuarial survival was 100% for medullary thymoma, 73% for mixed thymoma, and 47% for cortical thymoma.The overall survival curve shows that 87.6% of the patients are alive at 2 years and 72.8% at 5 years. And 7 patients was dead during follow-up periods.By Kaplan-Meier technique, we found that the patients who had myasthenia gravis had better prognosis[P<0.05]. Medullary thymoma is a comparatively rare, benign tumor, and usually not associated with myasthenia gravis. Cortical thymoma must be regarded as malignant. Mixed thymoma is intermediate in its behavior between medullary and cortical thymoma. But these tumors should be considered potentially malignant despite of presence as stage I of II disease. Also, the patients with stageI,II had good prognosis and the patients with total resection had good prognosis[P<0.05].

  • Myasthenia gravis with thymoma is associated with an increased risk of second malignancy, but concurrence of myasthenia gravis and thyroid carcinoma is rarely seen. In the treatment, it is emphasized to operate in two stage to avoid myasthenic crisis after surgery. In general, a thyroid cancer surgery is performed after controlling yasthenia gravis by means of thymectomy or medical treatment with steroids and anticholine esterase. We experienced two cases of thyroid carcinoma coexisting with myasthenia gravis, which is thought to be true first report in Korea.

  • Myasthenia gravis is a rare condition, affecting only 1/75000. It is characterized by weakness and fatigue of the voluntary muscles due to impaired neuromuscular transmission at the acetylcholine receptor site. It is probably caused by an autoimmune mechanism leading to reduction of the available nicotinic acetylcholine receptors at the neuromuscular junction. A relationship between the thymus and myasthenia gravis was suggested by Weigert in 1901, and Blalock was the first to report the efficiency of thymectomy in the treatment of myasthnia gravis in 1939. Since that time, thymectomy has become increasingly important in managing patients with myasthenia gravis. Fourteen patients with myasthenia gravis underwent thymectomy between September 1987 and March 1994 and got the following results; 1. The sex distribution was 10 females and 4 males with ages ranging from 15 to 51 years[mean, 34.8 years]. 2. Clinical manifestation of ocular symptoms were seen in all cases, dysphagia in 12[85.7%], extremity weakness in 11 [78.6 %], and dyspnea in 3 [21.4 %]. 3. According to the modified Osserman`s classification, 8 patients were in group IIb, 3 in IIa, 2 in I and 1 in IIc, respectively. 4. Histopathologic examination of the resected thymuses revealed hyperplasia in 8 patients, benign thymoma in 3, and malignant thymoma in 3. 5. There were no perioperative or immediate postoperative deaths but one patient died about two months after the thymectomy due to respiratory failure. 6. During the follow-up period, 11 patients [78.6 %] showed clinical improvement. Among them, 4 cases [28.6 %] showed complete remission, 3 [21.4 %] showed marked improvement, and 4 [28.6 %] showed subjective improvement. 7. Among the 8 patients with thymic hyperplasia, clinical improvement was noticed in 7 patients 5 %]. Among the 6 patients with thymoma, 4 patients [66.7 %] showed improvement.

중증 근무력증치료를 위한 광범위흉선절제술의 효과 (Maximal Thymectomy for Myasthenia Gravis Management)

  • Thymectomy has played a central role in the management of myasthenia gravis. Although both the etiology of myasthenia gravis and the reason for improvement after thymectomy remain incompletely explained, complete removal of the thymus is the logical goal of surgical treatment for this disease.From April 1989 to June 1994, maximal thymectomy was performed for 19 cases of myasthenia gravis at Chonnam National University Hospital. The results were as follows:1.Among the 19 cases, male-to-female ratio was 1:1.4, the age ranged 13 years to 71 years, and a diphasic presentation appeared with a peak in young females and a second peak in elderly males;2.Five cases were classified by modified Osserman`s classification as Group I and Group IIa and 14 cases as Group IIb and Group IIc; 3.Histologic examination of the excised thymus glands revealed normality in 5 cases [26% , thymic hyperplasia in 4 [21% , benign thymoma in 8 [42% , and malignant thymoma in 2 [11% ;4.There was no operative mortalities but two deaths occurred during the follow-up periods due to myasthenic crisis and other causes;5.The clinical improvement and the complete remission rates were 85% and 32%, respectively;6.The clinical improvement and the complete remission rates were not so good in patients with thymomas, beeing 70% and 20%, respectively; and 7.Young women with hyperplasia of the thymic tissue tended to show the best response to thymectomy.

  • Muscle specific tyrosine kinase (MuSK) myasthenia gravis (MG) is a rare subtype of MG, which is immunologically distinct and differential therapeutic response. Though MG is often associated with other autoimmune disorders or malignancy, concurrence of other disease and MuSK MG has been infrequently reported. We present a patient of MuSK MG associated with multicentric Castelman disease.

  • The incidence of myasthenia gravis in patients with thymomas has been reported from 10 to 50 percent, and the controversy ranges from 8-15 percent. This is the report of myasthenia grayis combined with benign thymoma and obtained relatively good result through surgical treatment at the department of thoracic surgery, National Medical center. The thymoma was mixed type and benign nature. Post operatively this patient was developed myasthenia crisis, and treated with mechanical respirator, and recurred the symptoms about 2months later, but well controlled with medical treatment.

흉선질환의 외과적 치료 -중증근무력증 환자에서 약물치료와의 비교관찰- (Surgical treatment of thymic disease -A comparison to medical treatment in myasthenia gravis-)

  • Thymus gland is a kind of endocrine organ which secretes thymosin and thymoprotein. There can be developed variable lesions like thymoma, thymic hyperplasia, thymic cyst, thymolipoma, and carcinoid tumor of the thymus gland. We have experienced 25 patients of thymic disease: thymoma 12, thymic hyperplasia 11, thymic cyst 1, carcinoid tumor 1. The age distribution were ranged from 3 to 66 years and the sex ratio was 1:1.8 [male to female]. Thymectomy was performed in all cases, but 3 cases with deep infiltration to the adjacent structures were not resectable completely. Malignancy [all thymoma] were 5 and the rest were benign. Two cases were died of recurrence after tumor resection. Myasthenia gravis occurred in 10 cases. Among them, 2 were thymoma and 8 were thymic hyperplasia. We could obtained the result that thymectomized cases reached 2 remission and 5 improvement. Myasthenia gravis treated medically [18 cases] had no remission and only 2 clinical improvement. In the light of these results, early radical thymectomy would be most favorable treatment in not only thymic tumor, but generalized myasthenia gravis.

중증근무력증의 수술적 치료 결과 (Surgical Results for Myasthenia Gravis)

  • 중증 근무력증은 자가면역 반응에 의해 유발되는 비교적 드문 질환으로, 여러 가지의 치료 방법이 시행되고 있으나 현재는 완전 관해와 증상의 호전이 탁월하다는 점에서 광범위 흉선 절제술을 주된 치료의 방법으로 선택하고 있다. 본원에서는 지난 8년간 16명의 중증 근무력증 환자에 대해 흉선 절제술을 시행하여 결과를 분석하였다. 5례(31%)에서 완전 관해가 보였으며, 7례(44%)에서는 약 투여량의 감소 또는 증상의 호전을 보여 총75%에서 수술적 호전을 보았다 환자의 나이, 성별, 술전 증상의 정도, 수술적 접근 방법 및 조직학적인 결과 등을 분석하였으나 수술 후 예후와는 통계적 상관관계는 보이지 않았다.
