노인의 근육뼈대계 통증이 횡단보도 보행에 미치는 영향 (Impact of Musculoskeletal Pain on Pedestrian Crossing among the Aged)

  • 본 연구는 282명의 노인을 대상으로 근육뼈대계 통증이 횡단보도 보행에 미치는 영향을 알아보고자 실시되었다. 연구방법은 근골격계 증상 조사표를 이용하여 노인의 근육뼈대계 통증을 조사한 후 4, 6차선 횡단보도를 보행하도록 하여 보행속도, 보행시간, 걸음수를 측정하였다. 연구결과 신체부위별 근육뼈대계 통증은 허리통증의 호소율이 가장 높았고(33.3%), 허리와 다리/발에 근육뼈대계 통증이 있을 경우 보행속도는 감소하고 걸음수와 보행시간이 증가하는 특징을 보였지만, 지팡이 사용이 횡단보도 보행에 도움을 줄 수 있는 것으로 나타났다. 하지만 허리와 다리/발의 통증정도는 횡단보도 보행특성 사이에 통계적으로 유의한 연관성을 확인할 수 없었다. 이후 지속적인 연구를 통해 노인의 다양한 신체적 문제들과 횡단보도 보행에 관한 연구들이 이루어지길 바란다.

Study of Work-Related Musculoskeletal Pain and Health-related Quality of Life among Hospital Workers

  • Purpose: This study examined the work-related musculoskeletal pain and quality of life of hospital workers. Methods: Self-reported questionnaires were sent to 350 hospital workers at Seoul and Gyeonggi-do, of which 341 were returned. The questionnaire had four items that covered the demographic information, areas of musculoskeletal problems, pain rating scale, and quality of life. The analysis was completed using descriptive statistic, and the differences between pain and demographic variables were identified using a chi-square test. The differences between the type of occupation and quality of life were analyzed by one-way analysis of variance and a Kruskal-Wallis test. Results: The 12-month prevalence of work-related musculoskeletal pain was 86.1% of physical therapists, 86.5% of occupational therapists, 77.1% of dental hygienists, and 75.8% of nurses. A significant difference in the general and work related variables was observed between the subject with a physical burden and type of occupation. The most affected pain sites of the physical therapist included low back, hand and wrist, shoulder, and neck. The occupational therapists included the hand and wrist, shoulder, neck, but the nurses and dental hygienists reported the shoulder, back, hand and wrist. A significant difference in the quality of life was noted between the subjects in physical therapists and dental hygienists and the subjects in the nurses and occupational therapists (p=0.00). Conclusion: These findings show that hospital workers appear to be high risk for work-related musculoskeletal pain, and the quality of life of physical therapists and nurses was higher than that of dental hygienists and occupational therapists.

Severity of Musculoskeletal Pain and Its Effect on Psychosocial Factors in Veterans With Posttraumatic Stress Disorder

  • PURPOSE: The aim of this study is to investigate the relationship between psychosocial factors and the severity of musculoskeletal pain in veterans with posttraumatic stress disorder (PTSD). METHODS: A total of 60 subjects were recruited from among the veterans with musculoskeletal pain at D Veterans Hospital. PTSD was evaluated by using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition; severity of pain was measured by using the short-form McGill Pain Questionnaire (SF-MPQ); depression and anxiety were measured by using the Symptom Checklist-90-Revision; and the quality of sleep was measured by using the Pittsburgh Sleep Quality Index. All data were analyzed using SPSS 18.0 software for Windows. RESULTS: The averages cores of pain intensity ($7.48{\pm}1.67$), SF-MPQ-sensory ($13.84{\pm}7.52$), SF-MPQ-affective ($4.41{\pm}3.79$), depression ($19.30{\pm}11.37$), anxiety ($13.39{\pm}7.99$), and quality of sleep ($10.05{\pm}5.89$) were obtained in veterans with PTSD. SF-MPQ-sensory measures sleep quality (r=0.346, p<0.01), SF-MPQ-affective measures depression (r=0.318, p<0.01) and anxiety (r=0.404, p<0.01), and these showed a statistically significant positive correlation in veterans with PTSD. Pain levels were observed to be higher in veterans with PTSD. Moreover, in these subjects, physical pain had a significant influence on the anxiety variable among the psychosocial factors. CONCLUSION: These findings suggest that musculoskeletal pain provides meaningful information about depression, anxiety, and sleep disorder in veterans with PTSD. Our data suggest that musculoskeletal pain may need to be addressed as part of the health management process of veterans.

근골격계 만성통증 환자가 지각한 통증, 가족지지 및 삶의 질과의 관계 (The Relationship between Pain Level and Perceived Family Support and Quality of Life in Musculoskeletal Patients with Chronic Pain)

  • The purpose of study is to identify the relation between pain level and perceived family support and quality of life in musculoskeletal patient with chronic pain. The subjects for the study consist of 155 patients with musculoskeletal pain that received medical treatment in hospital or by attending hospital in Chonju. The data were collected during the period from August 5 to August 14, 1998 by means of interviews with structured questionnaire. Data analysis was done by descriptive statistics. t-test, ANOVA, Pearson's correlation, Regression. Cronbach alpha using the SAS program. The result of this study were as follows : 1. The mean score of pain was 8.02, family support was 3.88 and quality of life was 3.07. 2. Hypothesis : The first hypothesis that 'The lower pain level is, the higher quality of life is' was accepted (r=-.2178, p= .0065). In addition, pain level of musculoskeletal patient with chronic pain provided predicted 4.7%(F=7.619, P= .0065) of quality of life. The second hypothesis that 'The higher perceived family support is, the lower pain level is' was rejected (r=-.0376, p= .6425). The third hypothesis that 'The higher perceived family support is, is higher quality of life is' was accepted (r= .3212, p= .0001). In addition, perceived family support of musculoskeletal patient with chronic pain provided predicted 10.31% (F=17.597, p= .0001) of quality of life. 3. General characteristics related pain were age(F=6.85, p= .0001),educational-level(F=9.29, p= .0001), occupation(F=5.81, p= .0037), marriage status(F=8.09, p= .0005), family numbers(F=5.73, p= .001), benefits of medical care(F=4.09, p= .0019), pain period(F=9.52, p= .0001), part of pain(F=2.33, p= .0352), pain period(F=3.08, p= .0181). 4. General characteristics related pain were sex(t=3.20, p= .0017), support sources(t=3.26, p= .0014), pain period(F-4.52, p= .0018). 5. General characteristics related pain were religion(t=3.11. p= .0022), benefits of medical care(F=3.61, p= .0293), pain duration(F=3.03, p= .0195). In conclusion, perceived family support in musculoskeletal patient with chronic pain is an important factor that can improve their quality of life. Therefore, nurses must establish nursing plan included patient's family when nurses carry out nursing intervention and education for patient so that a patient promote quality of life by maintaining optimal wellbeing.

간호사의 감정노동, 피로 및 근골격계 통증의 관계 (Relationships among Emotional Labor, Fatigue, and Musculoskeletal Pain in Nurses)

  • 본 연구는 간호사의 감정노동, 피로 및 근골격계 통증수준을 파악하고 그 관계를 분석하기 위해 시도한 서술적 조사연구이다. 종합병원에서 근무하며 연구참여에 동의한 총 235명의 간호사를 연구대상으로 하였다. 자료는 간호업무 수행 중 경험하는 감정노동, 피로 및 근골격계 통증수준을 구조화된 설문지를 이용하여 조사하였으며 수집된 자료는 실수와 백분율, 평균과 표준편차, t-test, ANOVA, Scheffe test, Pearson 상관계수 및 다중회귀(multiple regression)로 분석하였다. 연구결과에서 간호사들의 평균 감정노동수준이 3.27점, 평균 피로수준은 2.04점, 및 평균 근골격계 통증수준은 4.43점으로 조사되었다. 간호사들의 감정노동과 피로수준은 근골격계 통증수준과 유의한 양의 상관관계가 있었으며(r=.180, p=.006; r=.278, p<.001) 감정노동은 피로수준과도 유의한 양의 상관관계를 나타내었다(r=.353, p<.001). 간호사의 근골격계 통증에 영향을 미치는 요인은 신체적 피로(${\beta}=.324$, p<.001)와 성별(여성)(${\beta}=.171$, p=.009)이었으며 이들 변수는 근골격계 통증수준을 13.7% 설명하는 것으로 나타났다(F=16.271, p<.001). 이상의 결과를 통해 간호사가 병원에서 간호업무수행 중 경험하는 감정노동은 피로와 근골격계 통증에 상호 관련됨에 따라 간호사의 감정노동과 피로를 감소시키고 근골격계 통증을 예방하기 위한 방안모색이 필요함을 알 수 있었다.

Experiences of the Specified Symptoms from the Selected Work-Related Musculoskeletal Syndrome and Posture and Action of the Dental Hygienists When Work-Related Musculoskeletal Diseases of Dental Hygienists Patient Care Procedure

  • Objective: The purpose of this study is to examine about dental hygienists' myofascial pain syndrome, lower back pain, carpal tunnel syndrome (CTS), medial and lateral epicondylitis, hand-arm vibration syndrom and work-related musculoskeletal syndrome (WMSD) experience and hygienists' posture, motion. Methods: The self-administered questionnaire was surveyed from June 1 to September 30 of 2018 targeting 280 dental hygienists in Gyeongnam province and 266 dental hygienists' answers were analyzed. Results: The average daily working hours of a dental hygienist was more than eight hours 59.0%, with an average of 33 patients per day. The average number of patients who receive treatment for more than 30 minutes is 15. The angle of motion of the subjective evaluation was above 60%. Medical position and form of movement were more than 50% above the standard level. Symptoms of posture and motion that cause WMSD were hand-arm vibration syndrome 68.1%, myofascial pain syndrome 58.6%, lower back pain 51.1%, CTS 50.4% in order. Experience WMSD related symptoms which dental hygienists experience were myofascial pain syndrome 92.9%, CTS 57.9%, lower back pain 56.4%, medial and lateral epicondylitis 37.2%, hand-arm vibration syndrome 24.4%. Conclusion: The above results showed the posture and motion of dental hygienists and were found that the experience rate which dental hygienists experience WMSD of myofascial pain syndrome, Lower back pain, and CTS was significantly more than 56%.

호텔종사원의 근무환경에 따른 근골격계 통증이 이직의도에 미치는 영향 (Effect on Turnover Intention in Hotel Employees with Musculoskeletal Pains by Working Environment)

  • 본 연구의 목적은 호텔종사원의 근무환경에 따른 근골격계 통증이 이직의도에 미치는 영향에 대해 알아보기 위함이다. 근골격계 통증을 호소하는 호텔종사원 200명을 대상으로 근골격계 통증, 근무환경, 이직의도와 일반적인 특성을 면대면 및 자기기입식 설문조사를 통하여 확인하였다. 설문지 중 응답오류와 무응답의 4부를 제외한 196부(98%)를 최종 분석한 결과는 다음과 같다. 근무환경은 근골격계 통증(허리통증, 어깨통증, 손목통증)과 통계적으로 유의미한 부(-)적 상관관계가 있는 것으로 나타났고(r=-.339~-.448, p<.01), 이직의도와도 통계적으로 유의미한 부(-)적 상관관계가 있는 것으로 나타났다(r=-.536, p<.01). 또한 근골격계 통증은 이직의도와 통계적으로 유의미한 정(+)적 상관관계가 있는 것으로 나타났다(r=.358~.413, p<.01). 근무환경이 근골격계 통증에 영향을 미치는 것으로 조사되었으며, 이직의도는 근무환경이 적절하게 이루어지지 않을수록 근골격계 통증이 심할수록 상승하는 것으로 조사되었다. 따라서 적절한 근무환경의 개발 및 근골격계 질환의 예방을 위한 프로그램이 필요하다.

생산직 여성근로자의 직무스트레스와 근골격계증상이 건강관련 삶의 질에 미치는 영향 (The Effects of Occupational Stress and Musculoskeletal Symptoms on Health-Related Quality of Life in Female Labor Workers)

  • Objectives: The objectives of this study were to investigate female labor workers' occupational stress and musculoskeletal symptoms and to identify the effects of their occupational stress and musculoskeletal symptoms on their health-related quality of life. Methods: A survey was conducted through direct interviews using a musculoskeletal symptoms questionnaire, the Korean Occupational Stress Scale(KOSS), and the Short-Form 36-Item Health Survey(SF-36). Subjects were 112 female labor workers in three factories in D city who were selected by convenience sampling. Results: Factors significantly affecting health-related quality of life were found to be: occupational stress(${\beta}$=-.36); degree of pain, with medium pain(${\beta}$=-.31) and extremely severe pain(${\beta}$=-.24); duration of pain, with more than 1 week-less than 1 month(${\beta}$=-.25) and more than 6 months(${\beta}$=-.16); frequency of pain, with once per 2-3 months(${\beta}$=-.22); responses to pain such as medical leave, use of worker's compensation insurance, task change, etc.(${\beta}$=-.16), and Slightly difficult(${\beta}$=-.16) versus Not hard at all. These variables demonstrated that health-related quality of life is 48%(F=11.72, p<.001) in female workers. Conclusions: To improve female labor workers' health-related quality of life based on the above results, occupational health managers should reduce the workers' occupational stress, develop and apply health interventions regarding musculoskeletal symptoms, prevent the early onset of musculoskeletal symptoms, and protect and promote the workers' health.

반도체 제조회사의 근골격계부담작업 유해요인조사 실태와 개선방안 (A Study on the Risk Assessment and Improvement of Musculoskeletal Burden Works in the Semiconductor Manufacturer)

  • In Korea, companies which have work-related musculoskeletal burden works should have conducted legal risk assessments every three years from 2004 onwards. However, due to problems with the legal definition of work-related musculoskeletal burden works, some companies may have been exempted from the risk assessment even though their workers still experience work-related musculoskeletal pain. For example, the manufacturing process used by a particular semiconductor manufacturing company involved a great deal of legal musculoskeletal bueden works. However, this company eliminated the musculoskeletal burden works by continuously introducing automated processes, and finally, in 2016, all work which was legally defined as musculoskeletal burden work were removed from the company's manufacturing process. Nevertheless, in a 2016 survey, 9.6% of the company's workers still complained of musculoskeletal pain, and in a 2019 survey this proportion actually increased to 15.7%. This incident demonstrates the limitations and problems of the current legal risk assessment of work-related musculoskeletal burden work. Therefore, this study proposes two improvements to solve these problems. Firstly, it is necessary to broaden the current legal definition of work-related musculoskeletal burden works. For example, vibration risk factors and push/pull tasks that are currently missing from the definition should be included. Secondly, it is proposed that a survey on musculoskeletal pain should be conducted for all workers, regardless of whether they are engaged in musculoskeletal burden works. The results of this study could be used to improve the legal risk assessment of work-related musculoskeletal burden works.

만성 근골격계 통증환자에 대한 태극침법의 임상적 유효성: 후향적 의무기록 분석 연구 (Taegeuk Acupuncture for Patients with Chronic Musculoskeletal Pain: A Retrospective Analysis of Medical Records)

  • Objectives : This study aimed to assess the effectiveness and safety of Taegeuk acupuncture for patients with chronic musculoskeletal pain. Methods : A retrospective analysis of the electronic medical records of Pusan National University Korean Medicine Hospital from March 2012 to March 2013 was performed. Eligible cases were outpatients who had received at least six sessions of Taegeuk acupuncture for their pain and had agreed to the use of their medical records for research purposes. Pain levels, heart rate variability, and patients' perceptions of the safety of acupuncture were investigated through the medical records and follow-up telephone interviews. Results : Fifteen cases were selected for analysis. All had suffered from musculoskeletal pain for at least one year. After six sessions of Taegeuk acupuncture, the average score on the pain numeric rating scale decreased from $6.7{\pm}2.0$ to $2.9{\pm}2.2$(a 43 % reduction). Follow-up telephone interviews revealed that patients experienced no adverse event after Taegeuk acupuncture. Conclusions : Patients with chronic musculoskeletal pain showed a reduction of more than half of the baseline pain after six sessions of Taegeuk acupuncture. However, the beneficial effects observed in this retrospective analysis should be interpreted with caution due to selection and recall bias. Further randomized controlled trials of Taegeuk acupuncture for chronic musculoskeletal pain are warranted.
