Effects of Nordic Walking Exercise on muscular strength, Flexibility, Balance and Pain in Older Woman with Knee Osteoarthritis (노르딕 워킹이 퇴행성 무릎 관절염 노인여성의 근력과 유연성, 균형 및 통증에 미치는 영향)

  • The purpose of this study is to examine whether the 12-week Nordic walking can improve the physical function and arthritis pain of elderly women with osteoarthritis This study were divided into randomly assigned Nordic Walking Exercise Group (n=9) and Control Group (n=7) for 16 Elderly women diagnosed with Osteoarthritis (age: 73±3.79 year, height: 154.3±4.09 cm). The exercise group used Nordic sticks to carry out 30 minutes of Nordic walking exercise three times a week for 12 weeks, and the kinetic intensity was set at 40-60% of HRR. The control group maintained daily life for the same period. Body composition (weight, percentage body fat, skeletal muscle mass), muscular strength, Flexibility (muscular strength of upper and lower limbs, flexibility of upper and lower limbs), balance ability (static balance, dynamic balance) and pain level were measured as subordinate variables. These indicators were measured twice before and after the exercise program. The study shows that percentage body fat and skeletal muscle mass in the body composition function over 12 weeks of Nordic walking exercise have significant effects after the exercise than before (p=004)(p=.003), and it also shows significant interaction effects between the groups and timings(p=.018)(p=.005). In muscular strength, Flexibility factors, there were significant effects between the groups and timings in the upper limb muscular strength and the lower limb flexibility (p=.009)(p=.036), and a significant difference between the exercise group and the control group(p=.006) in the lower limb muscular strength. In addition, in the upper limb flexibility, there was a more significant difference after the exercise than before(p=.020). There were improvement effects after the exercise than before in the balance ability and the static balance(p=.016), but no difference in the dynamic balance(p>.05). In pain, there was a significant improvement after the exercise than before(p=.022), and a significant difference between the exercise group and the control group(p=.013). In conclusion, the 12-week Nordic walking exercise has positive effects on the body composition functions of the elderly women with Osteoarthritis, and has a positive effect on the improvement of upper limb muscular strength and lower limb flexibility in the health fitness factors. These effects are believed to have contributed effectively to the improvement of the level of pain by contributing to the improvement of physical and motor functions of the elderly women with Osteoarthritis. Therefore, it is considered that Nordic walking exercise, which enhances stability and balance of the patients with Osteoarthritis by using poles, is an effective exercise method for the improvement of the body and motor functions by lowering the pain of the joints and reducing the muscular strength and percentage body fat.

A Study on Comparison of Isokinetic Testing of the Knee Extensors and Flexors for the Woman College Soccer Players and General Waman College Students (여자대학축구선수와 일반여자대학생의 슬관절 신전근 및 굴곡근의 등속성 운동 비교연구)

  • The subjects of this study were 15 woman soccer players and 15 untrained woman students at H-Cheju University. Each subject was measured on the muscle strength and peak torque of knee extensors and knee flexors with isokinetic load ($60^{\circ}$/sec, $180^{\circ}$/sec). The results were as follows: 1. There was no significant difference between right and left leg strength measured isokinetically in each group. 2. The peak torque of knee extensors and flexors was significantly higher in scoccer players than untrainded students (p<0.001). 3. For the angle of knee joint showing the highest peak torque, there was significant difference between soccer players and untrained students in knee extensors(p<0.05) and right flexors (p<0.01). 4. For knee H/Q ratio, there was significant difference between two groups(p<0.05). 5. For the total work and average power at $180^{\circ}$/sec, there was significant difference between two groups in knee extensors(p<0.001) and flexors(p<0.01). 6. In soccer players at $60^{\circ}$/sec, there were significant correlations between peak torque and height and between peak torque and weight.(p<0.001). 7. For the muscular contraction velocity at $60^{\circ}$/sec, there was significant difference between two groups in flexors (p<0.01). 8. For the muscular indurance at $180^{\circ}$/sec in extensors and flexors, there was no significant difference between two groups(p<0.05). In conclusion, there were significantly higher in peak torque, contraction velocity, knee joint's angle with the highest peak torque, H/Q ratio, total work and average power in woman scoccer player than general students. But there was no significant difference in muscular endurance. Soccer performance is based on the various components including muscular endurance that is one of the most important components. So it is necessary that the training method to improve the various components (especially including muscular endurance) should be done.

Effects of 24 weeks of Training program on Aerobic Capacity, Body Composition, Physical Fitness, and Muscular strength in High School Sprinters (24주간 운동이 고등학교 단거리 선수의 심폐능력, 기초체력, 신체조성 및 근력에 미치는 영향)

  • It is impossible for all athletes to be satisfied with a single training program due to the fact that they have different physiological characteristics and needs. However, paucity studies have been introduced a specific training program for high school sprinters. Therefore, this study was designed to help develop and introduce the training program with a physical examination to enhance sprinting performance for experienced high school sprinters. VO2max, muscular strength, basic physical ability, and body composition were measured before and after a 24 week training program. The following are the conclusions based on the results of this study: 1) body fat percentage was increased after 24 weeks of training. Fat free mass and the amount of water in the body increased by 1.5% and 1.1% respectively, 2) $VO_2$maxand anaerobic threshold level showed a significant increase after 24 weeks of training, 3) exercise duration and anaerobic threshold duration were also increased after 24 weeks of training, but not statistically significant, and 4) muscular strength significantly increased due to the execution of combined weight training and circuit training. Development and implementation of the training program in this study made sprinters' physical capacities better in VO2max, anaerobic threshold, and muscular strength and body composition. Also, sprinters were able to increase fat free mass through 24 weeks of training due to increased muscle mass. Therefore, a training program for high school sprinters should include a physical examination to enhance sprinting performance and prevent sports injuries.

Effects of Nitric Oxide Synthase Inhibitor on Hindlimb Muscles in Rats with Neuropathic Pain Induced by Unilateral Peripheral Nerve Injury (산화질소 합성효소 억제제가 일측성 말초신경 손상에 의해 유발된 신경병증성 통증 쥐의 뒷다리근에 미치는 영향)

  • Purpose: The purpose of this study was to examine effects of nitric oxide synthase (NOS) inhibitor on muscle weight and myofibrillar protein content of affected and unaffected hindlimb muscles in rats with neuropathic pain induced by unilateral peripheral nerve injury. Methods: Neuropathic pain was induced by ligation and cutting of the left L5 spinal nerve. Adult male Sprague-Dawley rats were randomly assigned to one of two groups: The NOSI group (n=19) had NOS inhibitor (L-NAME) injections daily for 14 days, and the Vehicle group (n=20) had vehicle injections daily for 14 days. Withdrawal threshold, body weight, food intake and activity were measured every day. At 15 days all rats were anesthetized and soleus, plantaris and gastrocnemius muscles were dissected from hindlimbs. Muscle weight and myofibrillar protein content of the dissected muscles were determined. Results: The NOSI group showed significant increases as compared to the Vehicle group for body weight at 15 days, muscle weight and myofibrillar protein content of the unaffected soleus and gastrocnemius. The NOSI group demonstrated a higher pain threshold than the vehicle group. Conclusion: NOSI for 14 days attenuates unaffected soleus and gastrocnemius muscle atrophy in neuropathic pain model.

Relations among Nutrient Intake, Body Image Satisfaction, and Physical Fitness of College Students in Taegu (대구지역 대학생의 영양섭취상태, 체형만족도, 체력상태 및 그 관련성)

  • The aim of the present study was to evaluate the relations among nutrient intake. self body image and satisfaction, and physical fitness of college students in Taegu. A total of 213 apparently healthy college students living in Taegu were selected for the study. The following anthropometric measurements were made on all participants : weight, height: waist and hip circumferences, and bioimpedence. Using weight and height, the body mass index (kg/$m^2$) was calculated. The 24-hour recall were obtained from subjects. Dietary habits and energy expenditure were examined through questionnaires. Nutrients intake were analyzed by computer aided nutritional analysis program. Variables of Physical fitness were consisted of body fat, muscular strength and endurance, agility. power, flexibility, cardiorespiratory function, physical efficiency index(PEI). Data were analyzed by computer using the SAS 6.12(Statistical Analysis System). The mean heights and weights of these students were similar to those reported for the 50th percentile in a national survey. Nutrient intake of the college students in Taegu was considerably lower than RDA. The male subjects' mean intake of two nutrients, vitamin B$_2$ and calcium, were less than 55% of the RDAs. the female subjects' mean intake of two nutrients, iron and calcium, were less than 55% of the RDAs. A most variables of physical fitness except sit and reach in male students showed a significant higher values than those of female students. There was a significant positive relationship between PEI and the intake of animal protein, fat, Ca. and B$_2$ in male students. Also there was a significant positive relationship between exercise duration of PWC 170 and protein, fat, iron, vitamin A, B$_2$, niacin, and energy intake in female students. The muscular strength showed the significant correlation with weight, height, and waist circumference in male and female students. The cardiorespiratory function and Physical efficiency index showed significantly negative correlation with tricep and subscapular skin thickness in the female students. There were significantly positive correlations between WHR and body efficiency index in male students, and significantly positive correlations between WHR and the cardiorespiratory function in female students. There was no significant level of correlation between the degree of satisfaction of body image and physical efficiency index. However. there was a significant positive correlation between waist circumference (and weight) and physical efficiency index in male and female students and almost all of the surveyed male and female college students wanted less waist circumference or weight even though they were in the criteria of standard waist circumference or weight. In conclusion, a beneficial effect of adequate nutrient intake on physical fitness was found, college students must increase their energy and nutrient intake if it is below than the RDA for improving their physical fitness.

3-Dimension Lumbar Stabilization Exercise has an Influence on Pain of Degenerative Disc Disease Patients and the Spinal Stabilization muscle strength (3차원 척추 안정화 운동이 퇴행성 변성 디스크 환자의 통증과 척추 안정화 근력에 미치는 효과)

  • The purpose of this study lies in finding out the effect that variation of pain and body deflection posture has an influence on the static spinal stabilization after having performed spinal stabilization exercise making degenerative disc disease patients an object over 8 weeks using $CENTAUR^{(R)}$, 3-D spinal stabilization training implement. Subjects : 61 of DDD patients were made as an object of this study (mean age: 45.46 years, SD: ${\pm}12.78$, range: 16-68), their average height was 161.87cm, average weight 60.70kg, 12 males and 49 females were involved. Methods: 8 various investigations were performed and varied values were compared with reinvestigation done after having exercised 8 weeks using 3-D $CENTAUR^{(R)}$. We used VAS(Visual Analog Scale) in order to see the variation of pain intensity, MOS(Modified Oswestry Scale) in order to see activities of daily life. Results VAS was lessened from 7.50 to 2.71, limitation of routine life(MOS) from 20.26 to 9.32, there were remarkable differences statistically(p<0.05). As a result of muscular investigation for static spinal stabilization by 8 variations of body deflection, muscular strength were all increased and there were remarkable differences statistically(p<0.05). Conclusions : It has been turned out that pain and limitation of daily life was lessened as a result of making 61 of degenerative disc disease patients exercised 8 weeks using $CENTAUR^{(R)}$, 3-D spinal stabilization training implement, deep muscular power was increased. Thus it has been turned out that 3-D lumbar stabilization exercise has an effect on the spinal muscles strengthening and alleviation of their pain for degenerative disc disease.

  • This report describes a rare, congenital hypertrophy of the left upper extremity that appeared after compressive bandage of right arm at the age of two. He is eighteen years old, and hypertrophy was aggravated for about 2 years since he started weight training exercise. Recently, skin turgor changed and he visited the Dermatology department. Skin biopsy revealed increased thickness of the dermis. On Orthopaedic examination, the left arm showed non­specific neuro-muscular changes other than easy fatigability a.nd increased skin consistency after exercise, compared to the right arm. The differences of circumference were 2.5 to 4cm according to the level of the upper limb. But the relative proportion of hypertrophy of the limb was balanced., On X-ray examination, bony changes were not shown. Through the MRI, we could find edematous changes of subcutaneous fatty tissue. Muscular structures showed unremarkable changes. Through the endurance test of both arms, we could find a decrease in endurance of the left upper arm musculatures. On histologic examination, infrequent focal necrosis and peri fascicular degeneration of the muscle fiber were present.

Effects of Schisandrae Fructus Supplementation on Apoptosis and Inflammatory Response in Gastrocnemius Muscle of Dexamethasone-Induced Muscle Atrophy Mice (Dexamethasone에 의하여 유발된 근육 위축 생쥐의 비복근 근섬유에서 apoptosis와 염증 반응에 미치는 오미자 추출물의 영향)

  • Dried fruits of Schizandra chinensis Baillon, Fructus Schisandrae, have been widely used for many years to prevent and treat various diseases in Asian countries including Korea and Russia. It has recently been reported that extracts of Fructus Schisandrae are effective for controlling muscle and skeletal diseases. In this study, we investigated the efficacy of ethanol extract of Fructus Schisandrae (EEFS) on apoptosis and inflammatory response in gastrocnemius muscle of dexamethasone-induced catabolic muscle atrophy mice as part of natural substance discovery and functional analysis for improving muscle function. According to the results of this study, EEFS supplementation attenuated body weight gains and suppressed calf thickness loss in dexamethasone-induced muscle atrophic mice. Gastrocnemius muscle immunohistochemistry showed that expression of caspase-3 and poly(ADP-ribose) polymerase, which are representative apoptotic markers, was markedly increased in dexamethasone control mice; however, their expression was effectively reduced in the EEFS-fed mice. EEFS supplementation also prevented dexamethasone-induced increases in immunoreactivity of muscle fibers for myostatin, an important negative regulator of skeletal muscle mass. In addition, EEFS significantly normalized the increased numbers of nitrotyrosine, 4-hydroxynonenal and inducible nitric oxide synthase-positive muscle fibers compared to that found in dexamethasone control mice. These results suggest that EEFS protects dexamethasone-induced muscular atrophy by decreasing apoptosis and inflammatory responses, and EEFS is more likely to be developed as a muscle strengthening agent.

  • Purpose: The life expectancy of patients with spinal muscular atrophy (SMA) type I is generally considered to be less than 2 years. Recently, with the introduction of proactive treatments, a longer survival and an improved survival rate have been reported. In this study, we analyzed the natural courses and survival statistics of SMA type I patients and compared the clinical characteristics of the patients based on their survival periods. Methods: We reviewed the medical records of 14 pediatric patients diagnosed with SMA type I during a 9-year period. We examined the demographic and clinical characteristics of these patients, calculated their survival probabilities, and plotted survival curves as on the censoring date, January 1, 2010. We also compared the characteristics of the patients who died before the age of 24 months (early-death, ED group) and those who survived for 24 months or longer (long-survival, LS group). Results: The mean survival time was $22.8{\pm}2.0$ months. The survival probabilities at 6 months, 12 months, 18 months, 24 months, and 30 months were 92.9%, 92.9%, 76.0%, 76.0%, and 65.1%, respectively. Birth weight was the only factor that showed a statistically significant difference between the ED and LS groups ($P$=0.048). Conclusion: In this study, the survival probabilities at 2 years were far greater than expected. Because of the limited number of patients and information in this study, the contribution of improved supportive care on longer survival could not be clarified; this may be elucidated in larger cohort studies.

The hybrid perventricular closure of apical muscular ventricular septal defect with Amplatzer duct occluder

  • Purpose: Apical muscular ventricular septal defects (MVSDs), especially in small infants, can be difficult to manage using surgical and percutaneous closure. An intraoperative perventricular procedure is a good option for closing apical MVSDs in small children with or without associated cardiac anomalies. We evaluated the results of hybrid perventricular closure of apical MVSDs performed using an Amplatzer duct occluder (ADO). Methods: We retrospectively reviewed the medical records of 5 patients who underwent hybrid perventricular closure of MVSDs with ADOs, from March 2006 to May 2011. The median patient age at the time of the procedure was 12 months (range, 25 days to 25 months), and the median body weight was 9.1 kg (range, 4.3 to 15 kg). Two patients had multiple ventricular septal defects (VSDs; additional perimembranous VSD in 1 patient and multiple MVSDs in the other) and 3 patients had associated cardiac anomalies; complete transposition of the great arteries in 1 patient and an atrial septal defect in 2 patients. All the procedures were performed on beating hearts, exception in 1 case. The ADO selected for the aortic side was at least 1 to 2 mm larger than the largest VSD in the left ventricle side. Results: The procedure was successful in all patients and each device was well positioned. During the median follow-up of 2.4 years, a small residual VSD was noted in 2 patients who had multiple VSDs and no leakage was seen in the other 3 patients. Conclusion: Perventricular closure of MVSD with an ADO is a good option for patients with apical MVSD. However, careful manipulation is important, especially in the case of small infants.
