The Effect of External Pelvic Compression on Shoulder and Lumbopelvic Muscle sEMG and Strength of Trunk Extensor During Push Up Plus and Deadlift Exercise (푸쉬업플러스와 데드리프트 운동 시 골반압박이 견관절과 요골반부 주위근의 근활성도와 체간 신전근 근력에 미치는 영향)

  • Background: Lumbopelvic stability is highly important for exercise therapy for patients with low back pain and shoulder dysfunction. It can be attained using a pelvic compression belt. Previous studies showed that external pelvic compression (EPC) enhances form closure by reducing sacroiliac joint laxity and selectively strengthens force closure and motor control by reducing the compensatory activity of the stabilizer. In addition, when the pelvic compression belt was placed directly on the anterior superior iliac spine, the laxity of the sacroiliac cephalic joint could be significantly reduced. Objects: This study aimed to compare the effects of EPC on lumbopelvic and shoulder muscle surface electromyography (EMG) activities during push-up plus (PUP) and deadlift (DL) exercise, trunk extensor strength during DL exercise. Methods: Thirty-eight subjects (21 men and 17 women) volunteered to participate in this study. The subjects were instructed to perform PUP and DL with and without the EPC. EMG data were collect from serratus anterior (SA), pectoralis major (PM), erector spinae (ES), and multifidus (MF). Trunk extensor strength were tested in DL exercise. The data were collected during 3 repetitions of all exercise and the mean of root mean square was used for analysis. Results: The EMG activities of the SA and PM were significantly increased in PUP with pelvic compression as compared with PUP without pelvic compression (p<.05). In DL exercise, a significant improvement in trunk extensor strength was observed during DL exercise with pelvic compression (p<.05). Conclusion: The results of this study indicate that lumbopelvic stabilization reinforced with external pelvic compression may be propitious to strengthen PUP in more-active SA and PM muscles. Applying EPC can improve the trunk extensor strength during DL exercise. Our study shows that EPC was beneficial to improve the PUP and DL exercise efficiency.

Studies on the Myofibrillar Proteins of Mollusca (연체류의 근원섬유단백질에 관한 연구)

  • In order to compare and examine the general characteristics of myofibrillar proteins which is an important protein source as a food resource and relates directly with muscle contraction, we have extracted the myofibrillar proteins from squid and clam. The ionic strength of myofibrillar proteins connected with Ca-ATPase activity, Mg-ATPase activity and EDTA-ATPase activity showed distinct differences between squid and clam. In the activity-pH curve, actomyosin of the clam had a weak biphasic response. In the low concentration of dioxane, myofibrillar proteins of the squid showed a sudden decrease in activity but myofibrillar proteins of the clam showed in increase in activity. Ethanol and metanol in low concentration caused myosin and HMM from the squid and clam to increase their activities. If we cause modification by NEM, under 10-6M concentration, the activity was increased but above 10-5M concentration, there was a sudden decrease in activity.

Inhibitory Effects of Ssanghwa-tang on Lung Injury and Muscle Loss in a Cigarette Smoke Extract and Lipopolysaccharide-induced Chronic Obstructive Pulmonary Disease Mouse Model (표준담배추출물과 Lipopolysaccharide로 유발한 만성폐쇄성폐질환 동물모델에서 쌍화탕의 폐손상 및 근감소 억제 효과)

  • Objectives: This study evaluated the effects of Ssanghwa-tang (SHT) on lung injury and muscle loss in a COPD mouse model. Methods: C57BL/6 mice were challenged with cigarette smoke extract and lipopolysaccharide, and then treated with two concentrations of SHT (250 and 500 mg/kg). After sacrifice, the bronchoalveolar lavage fluid (BALF) or lung tissue was analyzed by cytospin, ELISA, real-time PCR, flow cytometry analysis, and H&E and Masson's trichrome staining. The grip strength of COPD mice was measured using a grip strength meter. The running time of COPD mice was measured by a treadmill test. Muscle tissue of the quadriceps was stained with H&E and Masson's trichrome staining. Results: SHT significantly inhibited the increase in neutrophil numbers in BALF and significantly decreased immune cell activity in BALF and lung tissue. It also significantly inhibited the increase in TNF-α, IL-17, and MIP2 in BALF. Real-time PCR analysis revealed that the mRNA expression of TNF-α, IL-17, MIP2, and TRPV1 in lung tissue showed a significant decrease compared with the control group. Lung tissue damage was significantly reduced in the histological analysis. The grip strength and running time of the COPD mice showed a significant decrease compared with the control group. In histological staining, SHT was found to reduce the damage to muscle tissue. Conclusions: This study indicates that SHT can be used as a therapeutic agent for COPD patients by inhibiting lung injury and muscle loss.

The Effects of PNF Leg Flexion Patterns according to the Hip Joint Angle on EMG Activity of the Trunk (엉덩관절 각도에 따른 PNF 하지굴곡패턴운동이 체간 근활성도에 미치는 영향)

  • Purpose : The purpose of this study was to analyze the effects of proprioceptive neuromuscular facilitation (PNF) leg flexion patterns according to the hip joint angle on electromyographic activity of the trunk. Methods : Thirty healthy adults volunteered to participate in this study. Subjects were required to complete following the PNF leg flexion patterns on three different hip joint flexion $30^{\circ}$, $60^{\circ}$ and $90^{\circ}$. An surface electromyogram (SEMG) was used to record the electromyographic activities of the trunk muscle in rectus abdominis, internal oblique abdominal, external oblique abdominal, erector spinae. The data were analyzed using the a repeated measures of one-way ANOVA with post-hoc Bonferroni's correction. Result : The results of this study are summarized as follows: The EMG activities of internal abdominal oblique and elector spinae muscle showed a statistically significant difference (p<.05). Conclusion : The result show that electromyographic activity of the trunk muscles significantly changed on PNF leg flexion patterns with difference hip joint angle. Therefore, this study used to basis for the intervention of the trunk muscle strength and stabilization.

Pathophysiology of orthostatic tremor: a multiple case study (길입성 진전의 병태생리: 다증 증례 연구)

  • Introduction : Orthostatic tremor develops in the legs while standing up with no weakness, pain or imbalance in the leg and the tremor is characteristically not observed when walking. However there have been some confusions about orthostatic tremor in several aspects. For the past ten years, we have observed 4 patients with orthostatic tremor. In each case tests were performed to investigate the following three important areas of inquiry about orthostatic tremor. Firstly, whether this disorder is an independent diagnostic entity or a variant of essential tremor. Secondly, whether the progress of this disorder is specifically related with standing posture. Lastly, the nature of the pathophysiologic mechanism behind the appearance of the tremor when standing after the lapse of a certain latent period and its disappearance upon the commencement of walking. Methods : Our 4 cases of orthostatic tremor were studied clinically, electrophysiologically, and pharmacologically. Electrophysiological tests included tremor spectrum test and electromyography. Results : We observed the presence of this tremor in several other tonic postures, as well as its absence, in a vertically lifted position from all our cases. Our cases registered a variable tremor frequency between 5 and 12 Hz according to the tremor spectrum test and EMG. Furthermore all our 4 cases demonstrated patterns of both synchronous EMG activity and alternating EMG activity at various times in homologous muscles of both legs. Orthostatic tremor was improved significantly with propranolol as well as clonazepam. Conclusions : From the results of our study we drew the following conclusions. It is probable that orthostatic tremor is simply a variant of essential tremor rather than being an independent diagnostic entity and that in most cases its development is specifically related with muscle contraction rather than merely with the act of standing. Furthermore we discovered a clue in the previously described neural control mechanism that the nuclear bag fibers in the muscle spindle have lag time of several seconds in their response to muscle strength and that their baseline does not reset fully in rapidly moving muscle. This neural control mechanism could offer sufficient explanation for the phenomena of tremor appearance when standing and disappearance when walking in orthostatic tremor.

  • The purpose of this study was to investigate the lower extremity kinematics and muscle activities of adductor longus(AL), gluteus medius(GME), gluteus maximus(GMA), biceps femoris(BF), rectus femoris(RF), gastrocnemius(GA), and tibialis anterior(TA) using three dimensional and Noraxon 8 channels EMG system during turn 0, 60, 90 and 120 degrees in patients with Parkinson's disease. Seven parkinson's patients and five healthy subjects were participated in the study. Participants with Parkinson's disease demonstrated significant differences in reduction of stride length and stride width. Also, they showed ill difference in muscle activities. The strength and balance of the lower extremity muscles may help to improve cutting movement and to prevent falling in parkinson's patients.

The Effects of Home Visit Healthcare Using a Complex Program on Community-dwelling Frail Elders' Strength, Frailty, and Depression (복합 프로그램을 적용한 방문건강관리가 재가허약노인의 체력, 허약, 및 우울 개선에 미치는 영향)

  • Purpose: The purposes of this study were to examine the effects of home visit healthcare using a complex program on community-dwelling frail elders' physical strength, frailty, and depression and to compare the effects among three regions. Methods: This study was conducted as a quasi-experimental study using one-group pretest-posttest design. A total of 86 subjects were sampled from three regions (metropolis, small city, and rural area). The program was applied for 14 weeks. This program consisted of disease management by a visiting nurse, exercise with a visiting nurse, self exercise, telephone monitoring, health education, counseling, providing handbook, and incentive. Physical strength was measured by balance, upper-limb, and lower-limb muscle strength. Frailty was measured by the Korean Frail Scale and depression was measured by the Geriatric Depression Scale. Paired t-test and ANCOVA were used to analyze the effects. Results: Physical strength, frailty, and depression were significantly improved after applying the complex program. There were not differences among the regions. Conclusion: Home visit healthcare using a complex program was effective in improving frail elders' physical strength, frailty, and depression. There were not differences among the regions. Therefore, home visit healthcare using a complex program needs to be expanded to other regions.

  • The purpose of this study was to examine the effects of physical activity on women's health. I conducted literature reviews for meta-analyses and randomized controlled trials with the target diseases including cardiovascular diseases, diabetes, cancers osteoporosis, and pregnancy outcomes. Women who were active had less total mortality and smaller incidence of hypertension, coronary heart diseases, and stroke. Exercise was better than drug therapies in preventing diabetes and effective in preventing colon and breast cancers. Exercise can reduce the risks of falling injury in elderly women. Walking during pregnancy was not harmful to the mothers and their infants, and desirable to prevent the complications of pregnancy or weight gain after pregnancy. Physical activity at work and leisure-time showed similar effects on women's health. Based on these results, moderate-intensity physical activity should be recommended to all women, Resistive, muscle strength, and balance-training exercise also can be recommended. Doctors' advices are effective to make women exercise, especially using handouts or motivational interviewing techniques. It is desirable to recommend to peform other health promotion measures together such as smoking cessation, weight control and diet control.

The Development and Verification of Balance Insole for Improving the Muscle Imbalance of Left and Right Leg Using based Sound Feedback (청각 피드백이 적용된 좌우 불균형 개선을 위한 밸런스 인솔 개발 및 검증)

  • This study was to develop the balance insole system for detecting and improving the muscle imbalance of left and right side in lower limbs. We were to verify the validation of balance insole system by analyzing the strategy of muscular activities and foot pressure according to sound feedback. We developed the balance insole based FSR sensor modules for estimating the muscle imbalance using detecting foot pressure. The insole system was FPCB have 8-spot FSR sensor with sensitivity range of 64-level. The participants were twenty peoples who have muscle strength differences in left and right legs over 20%. We measured the muscular activity and foot pressure of left and right side of lower limbs in various gait environment for verifying the improvement effect of muscle imbalance according to sound feedback. They performed gait in slope at 0, 5, 10, 15% and velocity at 3, 4, 5km/h. The result showed that the level of muscle imbalance reduced within 30% for sound feedback of balance insole system contrast to high level of muscle imbalance at 169.9~246.8% during normal gait for increasing slope and velocity. This study found the validation of balance insole system with sound feedback stimulus. Also, we thought that it is necessary to research on the sensitivity of foot area, detection of muscle imbalance and processing algorithm of correction threshold spot.

Study of the Muscle Activity of Small Intestine Channel of Hand Taiyang Muscle Using Contact Reflex Analysis (접촉반사분석법을 이용한 수태양경근의 근육 활성도에 대한 연구)

  • This Study was conducted to investigate Muscle Test of Point Selection through CRA(Contact Reflex Analysis) Muscel Diagnosis. So this study used to compare and analyze the effects of Muscel(Deltoid Muscel of Posterior) RMS(Root Mean Squared) and MEF(Median Edge Frequency) Among Groups that do not respond to questionnaire, Tonguibogam Naegyeongpyeon Small Intestine Group, Small Intestine MeridianPathway Primary Symptom and Secondary Symptom Group and Kwanwon(CV4) meridian Principal action Group. The questionnaire is composed of 23 items. The questionnaire was intended to elicit information on assorting into 4 groups. After Survey, Subject had to Muscle test subjects. Muscle experimental methods are as follows: Holding the shoulder without contacting Kwanwon. Holding the shoulder contacting Kwanwon. The first iteration. Group 1,2,3 were increased sEMG RMS compared with First experiment and Second experiment. Group 4(Including Uterus and Intestinal Flora Problem) were decreased sEMG RMS compared with First experiment and Second experiment. This test means that it is similar to diagnosis CRA and Small intestine channel of hand taiyang muscle, not Small Intestine MeridianPathway. It is suggested that subjects with a Small Intestine problem(Uterus and Intestinal Flora Problem) shows the results of decreasing posterior Deltoid Muscular strength. It means that CRA muscle diagnosis is associated with Alarm points diagnosis. But it doesn't consider influence of fat on the surface EMG.
