Establishment of a library of fragments for the rapid and reliable determination of anabolic steroids by liquid chromatography-quadrupole time of flight-mass spectrometry

  • Anabolic steroids have similar structures to testosterone, both of which promote the growth of muscle mass and increase strength. However, the side effects of anabolic steroid use may lead to heart attacks or strokes. Additionally, the excessive use of steroids inhibits the production of the sex hormones in the body via a negative feedback loop, which results in testicular atrophy in males and amenorrhea in females. Currently, the method of choice used to test for the presence of anabolic steroids is GC-MS. However, GC-MS methods require chemical derivatization of the steroid sample to ensure compatibility with the analytical method; therefore, analysis of many different samples is difficult and time consuming. Unlike GC-MS, the liquid chromatography-quadrupole-time of flight mass spectrometry (LC-Q-TOF-MS) method is suitable for many samples. Twenty-two different anabolic steroids were analyzed by LC-Q-TOF-MS with various collision energies (CE). Accurate mass spectral data were obtained using a Q-TOF-MS equipped with an electro-spray ionization source and operated in the positive MS/MS mode for several classes of steroids that are often the targets of testing. Based on the collected data, fragmentation pathways were carefully elucidated. The high selectivity and sensitivity of the LC-Q-TOF-MS instrument combined with these fragmentation pathways offers a new approach for the rapid and accurate screening of anabolic steroids. The obtained data from the 22 different anabolic steroids will be shared with the scientific community in order to establish a library to aid in the screening of illegal anabolic steroids.

Effects of Nutrition Education and Exercise Program on Obesity Index and Behavioral Modification in Moderate Obese Women (영양교육과 운동을 병행한 프로그램이 중등도비만여성의 비만도와 행동변화에 미치는 효과)

  • Objectives: The purpose of this study was to investigate the behavioral modification of obese adults who underwent nutritional and physical activity education. Twenty obese females, aged 20-60 years old, with BMIs (Body Mass Index) >30 or body fat (%) >40 were subjected to this study. Methods: The physical activity education program consisted of doing exercise in a gymnasium together or home exercise. Dietary attitudes and dietary intakes were assessed using weight control, physical activity, and eating habits. The nutrition-exercise educational period was 12 weeks. Results: After the study period, there was significant improvement in physical activity and eating habits score. Furthermore, there was a significant increase in the dietary intakes of fiber, iron, potassium, vitamin A, vitamin $B_6$, and niacin. Blood pressure, blood glucose, and total cholesterol levels showed a tendency to decrease, but there was no significant difference. BMI, fat mass, abdominal circumference, and visceral fat levels were significantly reduced while muscle mass significantly increased. Conclusions: This study suggests that behavioral modification by nutrition and physical activity education with feedback has positive effects on dietary intake and anthropometric biomarkers in obese adults. Therefore, lifestyle interventions of this kind could be recommended as a method for obesity management.

The Effects of Hand Function on the Angle of Holding a Smartphone (스마트폰 사용 시 팔의 지지 각도가 손의 기능에 미치는 영향)

  • The purpose of the study is to prevent pain and malfunction of the hand by correcting the angle of holding a smartphone. Subjects comprised of 21 healthy students. This research was conducted from April 21 2014 to April 27 2014. The three groups were as follows: first group included 7 students who held the phone at an angle of 90 degrees without a sling, and second group included 7 students who held the smartphone at an angle of 90 degrees with a sling; and the third group included 7 students who held the smartphone at an angle of 120 degrees with a sling. VAS was measured through the Finkelstein Test. Also, pain rating and muscle strength were assessed four times 30 minutes before and after the experiment for one and a half hour with pinch grip, dynamometer, and visual analogue scale. Smartphone was used. In all of the measurements, period showed a significant difference (p<.05) between both sides and a significant difference was not found between the group. In the time ${\times}$ group, pain score and grip strength were significantly different in the right side only (p<.05). According to the results of this experiment, providing support to the hand helps to increase the hand function by reducing the stress.

The effect of aroma foot spa on stress (아로마풋스파가 스트레스에 미치는 영향)

  • To identify the effect of the aroma foot spa on the stress, 12 university students with stress were divided into control group (6 subjects) and aroma foot spa group (6 subjects). Control group was not subjected to any kind of intervention while aroma foot spa group was subjected to 10 minutes of foot bath using 5% Juniper, lavender and lemon essential oil and 10 minutes of foot massage using massage cream during four weeks, two times per week. As for the evaluation method, brain wave was studied to measure the amount of change in stress. After measuring the change in the brain wave before and after the experiment, it was proven that the aroma foot spa group s Alpha wave (Z=-2.364, p<.05) and SMR wave (Z=-1.981, p<.05) were higher than those of the control group. Moreover, when the pre and post experiment results of the aroma foot spa group were measured, it was proven that the Theta wave (Z=-2.366, p<.05) decreased while Alpha wave (Z=-2.371, p<.05) increased. In other words, aroma foot spa that included foot bath using essential oil and foot massage increased Alpha wave which in turn influenced the brain wave due to increased blood circulation resulting from muscle relaxation. Moreover, SMR wave increase was closely related to the change in Alpha wave, which demonstrates that SMR wave increased due to stress alleviation. Accordingly, it may be possible to assume that aroma foot spa is effective in relieving stress.

A Study on the Relationship between Body Composition, Exercise Status, Fitness Status and Bone Mineral Density in Some Rural Residents (일개 농촌지역 주민의 체구성, 운동 및 체력상태와 골밀도와의 관계)

  • This study was performed to be used as basic data to preventive the osteoporosis of a rural community people, after finding out the bone mineral density and a state of health, that of exercise, body composition and basic physical strength of the rural community adult. the subject for 143 adults of a rural community, the relation between bone mineral density and a state of health, that of exercise, body measurement and basic physical strength was studied. The research results were as follows. 1. In survey subjects, the more the age increased(p<0.01) and the lower the educational level was, the lower bone mineral density was(p<0.05). 2. Bone mineral density was significantly lower in those who had chronic diseases than those who didn't (p<0.05). 3. Bone mineral density was significantly higher in those who had much weight, BMI, body fat mass, and fat-free mass than those who didn't(p<0.01). 4. Bone mineral density was significantly high in those who exercised in the past, whose basal metabolism was high, and whose muscle mass was much(p<0.05). 5. Bone mineral density was high in those who had much grasping power and a number of sit-ups and push-ups (p<0.01). 6. As a result of Multiple Regression Analysis in which BMD was a dependent variable, the more the age increased, the more the score of bone density decreased when they had chronic diseases. And the exercise of the past affected the increase of bone mineral density.

A Study for the Distinction between Denervation Potentials and Endplate Spikes for the Diagnosis of Neuropathy (신경질환의 진단을 위한 탈신경 전위와 종판 전위의 구별에 관한 연구)

  • During the electromyographic evaluation for the diagnosis of neuropathy, presence for site of lesion could be predicted by a detection of denervation potentials such as fibrillation potentials or positive sharp waves in a group of muscles. Unfortunately, since denervation potentials are distinguished by examiner's experience, it is possible to make mistake identifying endplate spikes as a denervation potential. The aim of the study was to extract parameters to make an algorithm for quantitative distinction between denervation potentials and endplate spikes. It will help to minimize the examiner's bias and to localize the site of lesion thus increase the reliability on EMG diagnosis. There types of signals, endplate spike, fibrillation potential and positive sharp wave were obtained from the EDB(extensor digitorum brevis) muscle using then neuropathic patients. Eight parameters such as duration, area, slope, peak-to-peak amplitude, positive peak negative peak amplitude, ratio of positive to negative peak amplitude, and number of phase were extracted and compared. As a results, peak-to-peak amplitude, positive peak amplitude, ratio of positive to negative peak amplitude showed statistically significant differences between endplate spikes and denervation potentials. It was concluded that those parameters could be used to establish an algorithm which will improve the accuracies in automated quantitative EMG diagnosis.

A study of image evaluation and exposure dose with the application of Tube Voltage and ASIR of Low dose CT Using Chest Phantom (흉부 Phantom을 이용한 Low Dose CT의 관전압과 ASIR(Adaptive Statistical Iterative Reconstruction)적용에 따른 영상평가 및 피폭선량에 관한 연구)

  • Purpose: The purpose of this study has attempted to evaluate and compare the image evaluation and exposure dose by respectively applying Filtered Back Projection(FBP), the existing test method, and Adaptive Statistical Iterative Reconstruction(ASIR) with different values of tube voltage during the Low Dose Computed Tomography(LDCT). Materials and Methods: With the image reconstruction method as basis, Chest Phantom was utilized with the FBP and ASIR set at 10%, 20% respectively, and the change of Tube Voltage (100kVp, 120kVp). For image evaluation, Back ground noise, Signal to Noise ratio(SNR) and Contrast to Noise ratio(CNR) were measured, and, for dose evaluation, CTDIvol and DLP were measured respectively. The statistical analysis was tested with SPSS(ver. 22.0), followed by ANOVA Test conducted after normality test and homogeneity test. (p<0.05). Results: In terms of image evaluation, there was no outstanding difference in Ascending Aorta(AA) SNR and Infraspinatus Muscle(IM) SNR with the different values of ASIR application(p<0.05), but a significant difference with the different amount of tube voltage(p>0.05). Also, there wasn't noticeable change in CNR with ASIR and different amount of Tube Voltage (p<0.05). However, in terms of dose evaluation, CTDIvol and DLP showed contrasting results(p<0.05). In terms of CTDIvol, the measured values with the same tube voltage of 120kVp were 2.6mGy with No-ASIR and 2.17mGy with 20%-ASIR respectively, decreased by 0.43mGy, and the values with 100kVp were 1.61mGy with No-ASIR and 1.34mGy with 20%-ASIR, decreased by 0.27mGy. In terms of DLP, the measured values with 120kVp were $103.21mGy{\cdot}cm$ with No-ASIR and $85.94mGy{\cdot}cm$ with 20%-ASIR, decreased by $17.27mGy{\cdot}cm$(about 16.7%), and the values with 100kVp were $63.84mGy{\cdot}cm$ with No-ASIR and $53.25mGy{\cdot}cm$ with 20%-ASIR, a decrease by $10.62mGy{\cdot}cm$(about 16.7%). Conclusion: At lower tube voltage, the rate of dose significantly decreased, but the negative effects on image evaluation was shown due to the increase of noise. For the future, through the result of the experiment, it is considered that the method above would be recommended for follow-up patients or those who get health checkup as long as there is no interference on the process of diagnosis due to the characteristics of Low Dose examination.

Analgesic effect of acupuncture applied to $SI_6$ in a rat model of neuropathic pain (흰쥐의 신경병증성(神經病症性) 통증(痛症) 모델에서 양로(養老) 자침(刺鍼)의 진통효과(鎭痛效果))

  • Objectives : The usage of acupuncture has gained popularity for certain chronic pain conditions. However, the efficacy of acupuncture in various diseases has not been fully established and the underlying mechanism is not clearly understood. In the present study, the effect of electroacupuncture (EA) applied to yangno$(SI_6)$ on the neuropathic pain was examined. Methods : A common source of persistent pain in human is a neuropathic pain. Neuropathic pain was induced by tight ligation of L5 spinal nerve. When rats developed pain behaviors, EA was applied for 30 min. under enflurane anesthesia with repeated train stimuli at the intensity of 10X of muscle twitch threshold. The foot withdraw latency of the hind limb was measured for an indicator of pain level after each manipulation. Results : EA increased the mechanical threshold of the foot in the rat model of neuropathic pain significantly for the duration of 1 hr. suggesting a partial alleviation of pain. EA applied to SI6 point produced a significant improvement of mechanical sensitivity of the foot lasting for at least 1 h. However, $ST_{36}$ point did not produce any significant increase of mechanical sensitivity. The improvement of mechanical threshold was interpreted as an analgesic effect. The analgesic effort was specific to the acupuncture point since the analgesic effect on the neuropathic pain model could not be mimicked by EA applied to a point, $ST_{36}$. In addition, this analgesic effect of EA is mediated by a adrenergic mechanism of descending control of spinal cord from the brain. Conclusions : The data suggest that EA produces a potent analgesic effect on the neuropathic pain model in the rat; and 2) that EA-induced analgesia is mediated by a adrenergic mechanism of descending control in a point specific manner.

AN EXPERIMENTAL STUDY OF EFFECT OF INTERMAXILLARY FIXATION AND OCCUSAL SPLINT ON PULMONARY FUNCTION (악간고정과 교합 상이 호흡기능에 미치는 영향에 관한 실험적 연구)

  • Intermaxillary fixation and occusal splint are routine procedure for maxillofacial fracture and orthognathic surgery. When these methods could obstruct oral airway the patients who kept intermaxillary fixation and occusal splint in their mouth, are very difficult to breath after surgery. Nasal bleeding and pharyngeal edema due to nasotracheal intubation, residual effect of muscle relaxants, and anesthetic agent could be contributing factor of airway obstruction. In this study, pulmonary function test was evaluated before and after intermaxillary fixation, and intermaxillary fixation with occusal splint in 22 volunteers. The results were as follows 1. FVC, %FVC, $FEV_1$, $FEV_1%$, PEF, $PEF_{50}$, MVV without intermaxillary fixtion were 4.45L, 88%, 4.03L, 90.9%, 10.26L/s, 5.53L/s, and 136.14L/min, and with intermaxillary fixation were 3.51L, 68.67%, 3.06L, 69.39L, 6.52L/s, 3.94L/s, and 69.39L/min. The results with intermaxillary fixation and occusal splint were 2.15L, 42.41%, 1.71L, 38.81%, 2.83L/s, 1.74L/s, and 37.14L/min. 2. Compared with before and after intermaxillary fixation, all values of pulmonary function test were decreased and after intermaxillary fixation and intermaixillary fixation with occulasal splint, the results were decreased. 3. MVV and PEF were decreased significantly with interaxillary fixtion and occusal splint, and FVC was less decreased. It meant that intermaxillary fixation and occluasal splint induced reduction of respiratory flow significantly, but less reduction of respiratory volume. 4. Intermaxillary fixation and occulsal splint induced increase of airway resistance, decrease of expiratory volume and air flow. So severe respiratory difficulty could be seen to all volunteers who kept intermaxillary fixtion and occusal splint. 5. In classification of respiratory difficulty, intermaxillary fixation with occulsal splint induced complex respiratory difficulty more than intermaxillary fixation only did. From the above results, doctors who care patients kept intermaxillary fixation and occusal splint should be aware of respiratory depression caused by these treatment.

Effect of 8 Weeks Complex Exercise Program on The Height, Body Composition, Physical Fitness in Elementary School Children (8주간의 복합운동프로그램이 초등학생의 신장, 체성분, 체력에 미치는 효과)

  • Objective : The purpose of this study was to investigate the effect of 8 weeks complex exercise program on height, body composition, and physical fitness in elementary school children. Method : The experimental group had 11 children (male=6, female=5) who belong to Seong-Nam Community Welfare Center in Gyeonggi-Do. All children received 50 minutes of complex exercise program 2 times a week for total of 8 weeks. In-body 520 were measured initially to serve as baseline data for height, and body composition. To determine the impact on physical fitness, Primus RS Power Track II commander Sit-up Trunk forward flexion measurement was also done. Repeat measurements of In-body520 Primus RS Power Track II Commander Sit-up Trunk forward flexion were done after 8 weeks. Results : There was significant increase in height, skeletal muscle mass, body fat percentage, lower limbs strength, and muscular endurance after the complex exercise program. There was, however, no significant difference in upper limbs strength, and flexibility after the program. Conclusions : These results indicate that the complex exercise program used in this study was very effective in producing significant benefits in height, body composition, and physical fitness in elementary school children. There would be better objective results if the control group and various environmental factors are considered in the future research.
