A prevalence study was carried out on 847 CMD patients who had visited the Department of Oral Medicine in Pusan National University from 1990 to 1993. To obtain the same type of information, all subjects were interviewed and examined clinically using a standardized examination form, The ratio of women to men was about 3:1 and all subjects were divided into acute and chronic groups on the basis of 6 months of duration. Diagnostic groups consisted of muscle disorder, joint disorder and muscle-joint disorder. As related to gender, duration and diagnosis subjective and objective symptoms in CMD were studied. The obtained results were as follows : 1. Muscle-joint disorder had the highest percent, followed by muscle disorder and joint disorder. 2. The most common reasons for CMD treatment were pain, joint noise and limited opening, while headache and neckache were relatively often reported as associated symptoms and dizziness, ringing in the ears also reported as secondary CNS excitatory effects. 3. Pain was more ofter seen in women, acute group and muscle-related disorder groups (p<0.05, p<0.01). Noise was significantly frequent in chronic group and joint-related groups (p<0.01). 4. Analysis of contributing factors presented that macrotrauma was found frequently in men (p<0.05), and that muscle-related groups were more related to stress than joint disorder grop (p<0.05). 5. Hard end feel was seen significantly often in joint-related disorder group (p<0.05). On the other hand, soft end feel was frequent in muscle disorder. 6. Reciprocal clicks and crepitation increased with chronicity. Subjects with joint-related disorder groups significantly often reported all kinds of noises (p<0.01). 7. Tender muscles and joints were more often reported in women and chronic group. Whereas muscle-related disorder groups revealed significantly more tender muscles (p<0.01). joint-related disorder groups presented significantly more tender joints (p<0.01).
The masticatory muscle disorder is the most common problem that patients with temporomandibular disorder often complain. For such complaints, treatment is directed towards reducing hyperactivity of muscles or effects of the central nervous system. However, if nonspecific occlusal change or pain persists, it is necessary to consider that muscle weakness might be the cause of the persistence of temporomandibular disorder. Stabilization of occlusion and improvement of the pain symptoms were achieved in both cases through the chewing gum exercise. This exercise may enable masticatory movements done in normal function by using muscle engram and achieve reinforcement of the masticatory muscles with balanced, simultaneous contacts of the teeth. In addition, it may be a viable method for treating temporomandibular disorders that do not respond well to conventional mandibular stabilization therapies.
PURPOSE: The aim of this study was to determine how biofeedback training with Shaker exercise effects the activation of the cervical myocardial muscle in stroke with swallowing disorder. METHODS: From June 2018 to September 2018, 30 patients who were hospitalized at C hospital and K hospital in Daegu, and K hospital in Gumi were surveyed to meet the criteria for selection. The participants were classified into three groups, and Shaker exercise was applied three times a day, five times a week, for four weeks; visuoauditory biofeedback group (VABG), visual biofeedback group (VBG), and self-exercises group (SG). In addition, the suprahyoid muscle activity was performed three times (before intervention, after intervention, and after two weeks). RESULTS: The pre and post-test comparisons, revealed a significant difference between the three groups (p<.01); VABG had the highest suprahyoid muscle activity. The post-test and follow-up test produced similar results in, the three groups (p>.05). The mean comparison showed the smallest difference in VABG, indicating that muscle persistence was the best. CONCLUSION: Shaker exercise has an effect on suprahyoid muscle activation. Biofeedback training, which provides an input of multi-sensory information in swallowing disorder treatment is recommended because it has the greatest effect when combined with visuoauditory biofeedback.
To examine the changes in pain, the severity of the neck disorder, craniovertebral angle, and muscle activity in young adults with forward head posture. 37 "N" University students in their 20s with forward head posture, including both male and female participants. Measurement of pain, NDI (neck disorder index) craniovertebral angle, and muscle activity were taken before and after the 6 week intervention period. The pain was measured using the visual analog scale. The severity of the neck disorder was measured using the NDI The craniovertebral angle was measured by taking a photo. The muscle activity was measured using surface electromyography. Neck posture correction exercises paired with proprioceptive training is the most effective intervention for reducing pain. Both neck posture correction exercises paired with Kinesio taping or proprioceptive training are effective interventions for addressing neck disability, craniocervical angle, and muscle activity. Neck posture correction exercises paired with Kinesio taping or proprioceptive training are more effective at addressing pain, neck disorder, craniocervical angle, and muscle activity than performing the neck posture correction exercises alone.
PURPOSE: The purpose of this study was first to develop a Korean version of pediatric functional muscle testing for children with a motor developmental disorder who have a communication problem and who are aged below five years, and also to verify the content validity. METHODS: First, the preliminary study was conducted to verify the reliability of pediatric functional muscle testing, developed by Venita in Korea. Based on the results of the study, the primary evaluation items were selected and modified. Second, the first test of content validity was conducted through a panel discussion, and a second investigation of content validity was carried out by utilizing the Item Content Validity Index (I-CVI). In this study, we selected only 0.78 or more items from I-CVI. RESULTS: Based on the results of the preliminary study, 19 primary evaluation items were selected. Based on the results of the first and second content validity tests, 15 tertiary evaluation items for the Korean version of pediatric functional muscle testing were determined. CONCLUSION: This study developed a new Korean version of pediatric functional muscle testing and suggested that it will be a useful tool to measure muscle strength for Korean children with a motor developmental disorder and a communication problem, aged below five years.
Purpose : Temporomandibular joint disorder (TMJD) is often accompanied by pain and limited range of motion of the jaw joint, which affect patients' quality of life and result in hypertrophy or hyperactivity of the muscles around the jaw joint. In this study, we compared the muscle thickness and tone of the masseter and sternocleidomastoid (SCM) muscles and the jaw range of motion in individuals with and without TMJD. Correlation comparison was performed on the results of the TMJD group. Methods : This study included 40 patients; 20 patients were assigned to an experimental group (TMJD group) and 20 to a control group (non-TMJD group). Ultrasonography, myotonometry, and measurements performed with digital Vernier calipers were used to determine the changes in muscle thickness, muscle tone, and maximum jaw opening, respectively. The independent t-test was used for intergroup comparison of data, and Pearson correlation coefficients were used to compare correlations in the TMJD group results. Results : We observed a significant intergroup difference in the masseter and SCM thickness during the relaxed and clenched phases (p<.05). A significant intergroup difference was also observed in maximum jaw opening (p<.05). With regard to muscle tone, we observed a significant intergroup difference in frequency (p=.011) and stiffness (p=.011) of the masseter, as well as in the frequency (p=.009) and stiffness (p=.026) of the SCM. We observed a moderate negative correlation (r=-.524) between maximum jaw opening and the frequency of the masseter. Additionally, we observed a moderately negative correlation between jaw opening and muscle stiffness (r=-.321). Conclusion : Planning exercise programs to treat patients with TMJD who present with pain should focus on efforts to reduce muscle thickness and achieve muscle relaxation (to reduce muscle tension) for improved jaw range of motion.
Background and Objectives : Patients with so-called 'functional voice disorders' who have structurally normal larynges and demonstrate muscle misuse in the larynx, and those with several interacting causes including habitual muscle tension, are probably better defined as having a 'muscle misuse voice disorder'. The purpose of this study was to analyze the voice and effectiveness of voice therapy in patients with functional voice disorders and to provide a guide for the treatment of functional voice disorder. Materials and Method : The records of 35 patients, presenting with functional voice disorder and receiving voice therapy during October, 2001 to September, 2002, were reviewed. Prior to voice therapy, the stroboscopic examination of their larynx, aerodynamic and acoustic analysis was done. The results of voice therapy were compared according to the patient's subjective, perceptual evaluation of voice, and maximal phonation time. Results : Patient's subjective, perceptual evaluation, and maximal phonation time showed superior results after voice therapy. Conclusion : The result of this study indicates that voice therapy is an effective treatment method of patients with functional voice disorder, especially muscular tension dysphonia.
Purpose: The purpose this research was to provide with basic data in the control of the sleep disorder found in the patients with fibromyalgia by analysing the main factors that affect that. Method: At two university medical center, appointed 171 out-patients diagnosed of fibromyalgia according to the conditions by American College of Rheumatology(1990). The research instruments used in this study were VAS Scale(Anxiety, fatigue, pain, joint stiffness and depression), physical activity family support, the number of tender points and Self-efficacy scale. In data analysis, SPSS 11.0 program was utilized and data were analyzed using descriptive statistics, Pearson's correlation coefficient and multiple regression. Result: The main factors that affect the sleep disorder of patients with fibromyalgia were fatigue, physical activity, and depressions which explained 46.2% of the sleep disorder. Conclusion: It has been confirmed that the regression equation model of this research may serve as a sleep disorder prediction factors in patients with fibromyalgia.
Objectives : The purpose of this study was to investigate the relationship between masseter and cervical muscle activity and temporomandibular disorder in female office workers. Methods : Experimental group of 24 healthy subjects complained of temporomandibular joint related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Control group of 20 healthy subjects had no complaints of minimal discomfort on the day of test, and had no discomfort in the past 7 days. If they had reported discomfort in the past 12 months, it was of a short duration(<3 months) and resolved at least 3 months prior to participation. Outcomes were assessed by meridian-electromyography(MEMG), whole spin x-ray, mandibular function impairment questionnaire(MFIQ), neck disability index(NDI), visual analog scale(VAS), Beck depression inventory(BDI), stress reaction inventory(SRI) and Holmes & Rahe social readjustment rating scale(SRRS). Results : The contraction power of masseter muscle, upper trapezius, sternocleido-mastoid muscle and erector spinae by MEMG was significantly higher in the experimental group. The muscle fatigue of masseter muscle and sternodeido-mastoid muscle by MEMG was significantly higher in the experimental group. SRI was significantly higher in experimental group. There was no significant difference between two groups in the Jackson's angle, Cobb's method and cranio-cervical posture. Conclusions : The results suggest that temporomandibular disorder related mental stress but physical stress does not change cervical structure significantly.
Purpose: Brown syndrome is motility disorder of the eyeball which shows limited elevation in adduction and occurs very rarely after eye surgery. The authors have experienced a case of strabismus-like Brown syndrome combined with blepharoptosis and report this case with the review of literatures. Methods: A 28-year-old female suffered from hypotropia in the primary gaze and severe blepharoptosis with diplopia of the right eye after upper blepharoplasty. Rotation showed an inability to elevate the adducted right eye. She underwent extraocular muscle surgery about the 7 mm tucking of the right superior rectus muscle and 6 mm recession of right inferior rectus muscle. Intraoperatively, injury of the superior rectus muscle and foreign body were observed. Seven months after the extraocular surgery, the patient underwent frontalis muscle transfer on the right upper eyelid for the correction of blepharoptosis. Results: Postoperatively, the patient was orthophoric in the primary gaze, and she had improvements in the correction of blepharoptosis and eyeball movement. Conclusion: Repeated eyelid surgeries increase the risk of ocular motility disorder. Careful approach is essential for the proper treatment and successful outcome in secondary surgeries.
