Comparison of Distribution of Cadmium-109 and Gallium-67 in Sarcoma-Bearing Mice (Sarcoma-bearing Mice에서 Cadmium-109과 Gallium-67의 체내 분포의 비교)

  • The search for tumor-avid agents for use in nuclear medicine imaging or therapy is a field of ongoing importance. Metallothionein (MT) is an intracellular protein that binds many metals with isotopes having imaging or radiotherapeutic potential. The purpose of the study was to determine whether uptake of radioisotopes that bind to MT is increased in tumor. We measured the uptake of Cd-109 and Ga-67 in tumor and normal tissues of sarcoma-bearing mice. Tumors were grown subcutaneously in female Balb/C mice from cultured Balb/3T3 cells transformed by the Moloney murine sarcoma virus (MMSV). When the tumors reached about 1 cm in diameter, mice were injected subcutaneously with Cd-109 and Ga-67. Eighteen and seventy-two hours later, the mice were sacrified. Organs and tissues were removed, weighed, and activity per mg tissue determined by gamma well-counting. Uptake of Cd-109 by MMSV tumors exceeded that by normal tissues examined, with the exception of liver and kidney (the organs known to be richest in MT). The tumor-to-tissue ratios of uptake for Cd-109 were far greater than those for Ga-67 for many normal tissues of great importance in terms of background activity (bone, intestine, fat, muscle, and blood). We concluded that metals that bind to MT may be useful for oncologic imaging or rediotherapy of cancer.

The Effect of Ginkgo Biloba Extract on Hypoxic Fraction of C3H Mouse Fibrosarcoma (Ginkgo Biloba Extract가 C3H 마우스 섬유육종의 저산소세포 분획에 미치는 영향)

  • Purpose : To investigate the effect of Ginkgo biloba extract (GBE) on hypoxic cell fraction and metabolic status in fibrosarcoma (FSa II) of C3H mouse. Materials and Methods : Fibrosarcoma (FSa II) 6 mm in diameter, growing in the right hindleg muscle of C3H mouse was used for estimation of hypoxic cell fraction using comparison of $TCD_{50}$. Radiation was given one hour after administration of GBE (100 mg/kg. i.p.) with or without priming dose of GBE (100 mg/kg, i.p.) given 24 hours earlier. Radiation was also given under air breathing condition or clamp hypoxia without GBE as controls. $^{31}p$ NMR spectroscopy was performed before and one hour after administration of GBE with or without priming dose of GBE. Results : $TCD_{50/120's}$ were 81.7 (77.7-86.0) Gy when irradiated under clamped hypoxia 69.6 (66.8-72.5) Gy under air breathing condition. 67.5 (64.1-71.1) Gy with a single dose of GBE (100 mg/kg) given one hour before irradiation, and 62.2 (59.1-65.5) Gy with two doses of GBE given at 25 hours and one hour before irradiation. The hypoxic cell fractions, estimated from $TCD_{50/120's}$, were $10.6{\%}$ under air breathing condition, $7.2{\%}$ after a single dose of GBE, and $2.7{\%}$ after two doses of GBE. The results of $^{31}P$ NMR spectroscopy were as follow. PCr/Pi ratio was $0.27{\pm}0.04$ and $0.40{\pm}0.04$ before and one hour after a single dose of GBE (p<0.05), respectively, without priming dose and $0.30{\pm}0.02$ and $0.71{\pm}0.04$, respectively, with priming dose (p<0.01). These findings indicate that the metabolic status is slightly improved after a single dose and markedly after repeated administrations. Conclusion : GBE decreases the hypoxic cell fraction and imprvoes the meta bolic status of tumor, probably by increasing the blood flow and delivery of oxygen and nutrients, resulting in increased radiosensitivity of tumor.

Giant Schwannoma May Mimic Soft Tissue Sarcoma (악성 연부 종양으로 오인하기 쉬운 신경 및 연부조직의 거대 신경초종)

  • Purpose: Schwannoma is a benign tumor that occurs mainly in the peripheral nerve. If the tumor is large or is in areas other than the nerves, it is likely to be mistaken for malignant soft tissue tumors. The authors reviewed 50 cases of giant schwannomas and assessed the distribution of the primary locations, clinical symptoms, radiological and pathological diagnosis, and diagnostic accuracy. Materials and Methods: Of the 214 pathologically confirmed schwannomas, 50 cases with a maximum diameter of 5 cm or more were extracted. The entire cohort was classified into three subgroups (major peripheral nerve, intramuscular, bone) according to the primary location, and the anatomical locations were specified. Results: When the entire cohort was classified according to the primary location, 14 tumors occurred in the major peripheral nerve, 31 cases in the muscle, and 5 cases in the bone. The mean size of the tumor in the entire cohort was 7.0 cm, and the intramuscular subgroup had the largest size with 8.0 cm. The radiological diagnosis revealed 33 out of 50 cases to be benign schwannoma (66.0%), 15 cases as low-grade malignancy (30.0%), and the remaining two cases (4.0%) as a suspicious tuberculosis abscess and tenosynovial giant cell tumor, respectively. On the clinical symptoms, Tinel sign was the most common in the peripheral nerve group with 78.6% (11/14), while 93.5% of the intramuscular group had palpation of the mass with a mean duration of 66.6 months. In the bone group, one out of five cases was reported as a low-grade malignancy. Two cases of postoperative complications were encountered; one was bleeding after tumor excision, which required hemostasis, and the other was peroneal nerve palsy after surgery. Conclusion: When assessing the large-sized soft tissue tumors in the muscles, the possibility of a benign schwannoma should be considered if 1) there is a long period of mass palpation and 2) non-specific findings in MRI. Preoperative pathology confirmation with a biopsy can help reduce the risk of overtreatment.

Analysis of the Effect of Entry-Level 3D Printer Materials on CT Images (보급형 3D프린터 재료가 CT 영상에 미치는 영향 분석)

  • In this study, based on PLA, we analyzed the Hounsfield Unit (HU) of materials containing 20% each of aluminum, wood, copper, carbon, and marble, and tried to analyze how they affect the image. A cylindrical phantom of 5×30×30 ㎣ (thickness×diameter×height) was fabricated using a entry-level 3D printer. The kV was changed to 80, 100 and 120, and the mAs was changed to 100 and 200 mAs, and the phantom in the center of the table was cross-scanned under a total of six conditions. A circular ROI was set using image J program and the quantification value of the material part HU and the quantification value of the peripheral part CNR were obtained. The HU average of the material part increased in the order of [PLA - wood 20%], [PLA - marble 20%], [PLA - carbon 20%], [PLA 100%], [PLA - aluminum 20%], [PLA - copper 20%] (p<0.05) a negative correlation was confirmed with the HU by increasing kV. It was confirmed that the CNR value in the peripheral area increased in the order of [PLA - marble 20%], [PLA - copper 20%], [PLA - carbon 20%], [PLA - wood 20%], [PLA - aluminum 20%], and [PLA - 100%] (p<0.05). Human organs with similar HU values for each material are [PLA - copper 20%] compact bone, [PLA - aluminum 20%] cancellous bone, [PLA 100%] coagulated blood, [PLA - carbon 20%] and [PLA - marble 20%] liver, muscle, spleen and [PLA - wood 20%] had similar values to fat. In addition, we confirmed the blur phenomenon that blurs the image around the filament with all materials, and confirmed that [PLA 100%] especially has the most blur around the filament. Therefore, it is considered desirable to reflect the HU value of the target organ and consider cloudiness around the phantom when selecting materials for medical phantom fabrication, and this research can provide basic data.
