The objective of this study was to investigate the nutritional status, biochemical parameters, lipid and electrolytes concentrations of the enteral nutrition patients according to the duration of enteral nutrition. Eighteen neurosurgery patients in the intensive care unit (ICU) at K University Hospital were subjected in this study. The duration of enteral nutrition was classified into under or over six month of period. Anthropometric, biochemical, clinical, and dietary assessments were performed. Patients' intakes of energy and protein were insufficient, from 82% to 95% of their requirements. Mid-arm muscle circumference (MAMC) and mid-am muscle area (MAMA) were significantly lower in patients over six months of enteral nutrition than those in patients under six months. The subjects were malnourished as indicated by nutrition-related parameters such as hemoglobin, albumin, total lymphocyte count (TLC), tricep skinfold thickness (TSF), mid-arm circumference (MAC), MAMC, and MAMA. Serum chloride level of the patients eve, six months of enteral nutrition was lower (94.7 $\pm$ 3.4 mmo1/1) significantly as compared to that of patients (99.3 $\pm$ 3.5 mmol/ 1) under six months. Urinary sodium and chloride levels were lower in the longer time of enteral nutrition patients than those of shorter period of enteral nutrition patients (p < .05). While serum phospholipid level was higher in the patients over six months of enteral nutrition, other blood biochemical parameters and electrolyte concentrations did not show any differences with the duration of enteral nutrition. Neurosurgery patients in the ICU undergoing long-term enteral nutrition tube-feeding were malnourished and had a variety of metabolic complications. The duration of enteral nutrition could affect the patients' nutritional status, biochemical parameters, and electrolytes balance. The patients who require nutritional support over an extended time need the continuous follow-up care and monitoring by the nutrition support team for laboratory, clinical, and nutritional assessments.
Purpose: This study was to examine the effects of 12-week Tai Chi exercise on the autonomic nervous system activity and risk factors of metabolic syndrome in patients with metabolic syndrome. Methods: A single group pre-posttest was used. Subjects were 16 participants aged 51~71 years with metabolic syndrome. All of the subjects were met the criteria of the National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III). The Tai Chi exercise consisted of 19 movements from Yang and Sun styles provided twice a week for 3 months. Metabolic syndrome risk factors including waist circumference, blood pressure, glucose, triglyceride, high density lipoprotein cholesterol (HDL-cholesterol) were measured. A heart rate variability device was used to measure the activity of autonomic nervous system. Results: After 12-week Tai Chi exercise, systolic pressure, waist circumference, and HDL-cholesterol improved significantly. Tai Chi exercise had no effect on the autonomic nervous system activity. Conclusion: The 12-week Tai Chi exercise was partially effective in managing risk factors of metabolic syndrome. A larger study with longer period to improve the autonomic nervous system activity in patients with metabolic syndrome is needed.
Objectives: Metabolic disease is strongly associated with future insulin resistance, and its prevalence is increasing worldwide. Thus, identifying early biomarkers of metabolic-related disease based on serum profiling is useful to control future metabolic disease. Our study aimed to assess the association of serum branched chain amino acids (BCAAs) and aromatic amino acids (AAAs) ratio and metabolic disease according to body mass index (BMI) status among Korean adults. Methods: This cross-sectional study included 78 adults aged 20-59 years in Korea. We compared serum amino acid (AA) levels between adults with normal-weight and adults with obesity and investigated biomarkers of metabolic disease. We examined serum AA levels, blood profile, and body composition. We also evaluated the association between serum AAs and metabolic-related disease. Results: The height, weight, BMI, waist circumference, hip circumference, waist-hip-ratio, body fat mass, body fat percent, skeletal muscle mass, systolic blood pressure, and diastolic blood pressure were higher in the group with obesity compared to normal weight group. The group with obesity showed significantly higher levels of BCAA, AAA, and BCAA and AAA ratio. Further, BCAA and AAA ratio were significantly positively correlated with triglyceride, body weight, and skeletal muscle mass. The evaluation of metabolic disease risks revealed an association between the ratios of BCAAs and AAAs, hypertension, and metabolic syndrome. Conclusions: Our study is showed the associations between BCAA and AAA ratio, obesity, and obesity-related diseases using various analytical approaches. The elevated BCAA and AAA ratio could be early biomarkers for predicting future metabolic diseases in Korean population.
Objectives : Polyneuropathies are diseases of multiple peripheral nerves. They are usually characterized by symmetrical, bilateral distal motor and sensory impairment with a graded increase in severity distally. It is generally regarded that the natural courses are poor, so we wanted to study the effects of Oriental medical treatment on a patient with polyneuropathy. Methods : We treated by conservative Oriental medical treatment a woman of 68 years who was diagnosed as a polyneuropathy and was hospitalized at Seoul Oriental Hospital, Kyungwon University, from 12th Mar. to 31st May, 2003. Changes of functional disability were checked by Amyotrophic Lateral Sclerosis Functional Rating Scale (ALSFRS), muscle atrophy was checked by measuring circumference of the thighs, calves, arms, and sensory impairment was checked by a sensory test. Results : 1. Functional disability caused by motor impairment was reduced after the Oriental medical treatment 2. Muscle atrophy was reduced after the Oriental medical treatment 3. Sensory impairment was reduced after the Oriental medical treatment Conclusion : We treated a patient who was diagnosed with polyneuropathy for over 80 days and recorded good effects of Oriental medical treatment on polyneuropathy.
Purpose In Sasang Constitution Medicine, persons are divided into four types such as Taeyangin, Taeumin, Soyangin and Soeumin according to their appearance, moral nature and characteristic of constitutional symptoms. In teenager group, diagnosis of constitution based on their appearance was so difficult that we adopted new diagnostic method which measures and compares body compositions. Methods 180 middle school students who live in Pusan were studied from 20th of March, 1999 to 20th of March, 2000. After practicing Questionnaire(I), QSCC(Questionnaire of Sasang Constitution) I , QSCCII, we have measured the height and body compositions with Inbody 3.0, five times. Results 1. The results of Sasang Constitution analysis showed 91 students were classified as Taeumin(50.3%), 75 as Soyangin(41.4%), 14 as Soeumin(7.7%), and 1 as Taeyangin(0.6%). 2. The level of total body water, muscle area, BMR(Basal Metabolic Rate), BCM(Body Cell Mass), and AMC(Arm Muscle Circumference) were increased during spring and summer. That of body weight, fat body weight, fat body weight ratio, waist hip ratio, and BMI(Body Mass Index were increased during fall and winter. 3. In Taeumin and Soyangin groups, the changed capacity of total body water, muscle area and BMR were increased with statistical significance. 4. Waist hip ratio of Soeumin group was reduced more than those of Taeumin, Soyangin groups with statistical significance.

BACKGROUND/OBJECTIVES: The prevalence of morbid obesity in Korean women has consistently been increasing, while the overall prevalence rate of obesity in Korean women seems to be stable. In addition to bariatric surgery, intragastric balloons (IGBs), as a nonsurgical therapy, have been reported to be effective in weight loss. However, the beneficial effects of IGB in Korean women with obesity have not been fully investigated. The aim of this study was to evaluate the changes in fat mass in Korean women with obesity who had undergone IGB treatment for 6 mon. SUBJECTS/METHODS: Seventy-four women with obesity (body mass index [BMI] ≥ 25.0 kg/m2) were recruited. Clinical data, including general information, comorbidities with obesity, anthropometric data, and changes in the body fat composition before and after IGB treatment, were obtained from the subjects. RESULTS: Most subjects had one or more comorbidities, such as osteoarthropathy and woman's disease, and had poor eating behaviors, including irregular mealtimes, eating quickly, and frequent overeating. Body composition measurements showed that weight, fat mass, and waist-hip circumference ratio decreased significantly at 6 mon after IGB treatment. In particular, women with morbid obesity (BMI ≥ 30 kg/m2) showed 33% excess weight loss. There was no significant difference in skeletal muscle mass and mineral contents after IGB treatment. CONCLUSIONS: This study suggested that 6 mon of IGB treatment can be a beneficial treatment for obesity without muscle mass and bone mineral loss.

The present study was designed to evaluate effects of intermittent electrical stimulation of the sciatic nerve on the atrophic response of antigravity muscles, such as the soleus (slow m.) and medial gastrocnemius (fast m.) muscles. Rats (Sprague-Dawley, 245-255g) were subjected to a hindlimb suspension and divided into three groups : one was with hindlimb suspension (MS) and another with hindlimb suspension plus intermittent electrical stimulation of the sciatic nerve (HS ES). Control group (CONT) was kept free without strain of the hindlimb. After 7 days of hindlimb suspension, the soleus and medial gastrocnemius muscles were cut at their insertion sites, and were then connected to the force transducer to observe their mechanical properties. Optimal pulse width and frequency of electrical stimulation were 0.2ms, 20Hz for the soleus muscle and 0.3ms, 40Hz for the medial gastrocnemius muscle under supramaximal stimulation. Body weight and circumference of the hindlimb were significantly decreased in HS and HS-ES groups compared with the control group. In HS-ES group, however, the weight of the soleus muscle was not different from that in the control group while the weight of the medial gastrocnemius muscle was lower than that in the control group. In HS group, mechanical properties of muscle contraction including contraction time, half relaxation time, twitch tension, tetanic tension, and fatigue index of both muscles were significantly decreased compared with the control group except for twitch tension and tetanic tension of medial gastrocnemius muscle. The degree of atrophy of the soleus muscle in HS group was more prominent than that of the medial gastrocnemius muscle. Twitch tension and fatigue index of the soleus muscle and fatigue index of the medial gastrocnemius muscle in HS-ES group were not different from those of the control group. While mechanical properties of the soleus muscle examined were all significantly increased in HS-ES group compared with HS group, only contraction time and fatigue index of the medial gastrocnemius muscle were significantly increased in HS-ES group. These data indicate that intermittent electrical stimulation may be useful in prevention of muscle atrophy.
Objectives: This study was conducted to investigate the association between sarcopenia and sarcopenic obesity and cardiovascular disease risk in Korean postmenopausal women. Methods: We analyzed data of 2,019 postmenopausal women aged 50-64 years who participated in the Korea National Health and Nutrition Examination Survey in 2008-2011 and were free of cardiovascular disease history. Blood pressure, height, and weight were measured. We analyzed the serum concentrations of glucose, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol and triglyceride levels. Waist circumference was used to measure obesity. Appendicular skeletal muscle mass was measured by dual-energy X-ray absorptiometry. Sarcopenia was defined as the appendicular skeletal muscle mass/body weight<1 standard deviation below the gender-specific means for healthy young adults. The estimated 10-year risk of cardiovascular disease risk was calculated by Pooled Cohort Equation. Subjects were classified as non-sarcopenia, sarcopenia, or sarcopenic obesity based on status of waist circumference and appendicular skeletal muscle mass. Results: The prevalence of sarcopenia and sarcopenic obesity was 16.3% (n=317) and 18.3% (n=369), respectively. The 10-year risk of cardiovascular disease risk in the sarcopenic obesity group was higher ($3.82{\pm}0.22%$) than the normal group ($2.73{\pm}0.09%$) and sarcopenia group ($3.17{\pm}0.22%$) (p < 0.000). The odd ratios (ORs) for the ${\geq}7.5%$ 10-year risk of cardiovascular disease risk were significantly higher in the sarcopenic obesity group (OR 3.609, 95% CI: 2.030-6.417) compared to the sarcopenia group (OR 2.799, 95% CI: 1.463-5.352) (p for trend < 0.000) after adjusting for independent variables (i.e., exercise, period of menopausal, alcohol use disorders identification test (AUDIT) score, income, education level, calorie intake, %fat intake and hormonal replacement therapy). Conclusions: Sarcopenia and sarcopenic obesity appear to be associated with higher risk factors predicting the 10-year risks of cardiovascular disease risk in postmenopausal women. These findings imply that maintaining normal weight and muscle mass may be important for cardiovascular disease risk prevention in postmenopausal women.
The aim of this study is to investigate the effect of pharmacopuncture combined thread implantation therapy for treating localized obesity. The study was conducted on 3 subjects with abdominal localized fat, 1 subject with thigh and 1 subject with calf localized fat. We practiced two kinds of pharmacopunture, which were cultivated wild ginseng and contacting Ephedra sinica pharmacopuncture. In addition, we practiced thread implantation to 20 points on muscle membrane layer. We performed these treatments once in 2 weeks, totally for 3 times. After treatments, abdominal circumference and waist-hip ratio were reduced by 4.6 % and 0.13, respectively. In addition, circumferences of thigh and calf were reduced by 5.8 % and 4%. We supposed that this combined therapy could be useful to deal with localized fat deposit. Long-term evaluation and further studies are required to identify effect size of this therapy.

The purpose of this study was to investigate the pre- and post-exercise performance, edema, and pain of plyometrics in water and land environments. Twelve males in their 20s were selected as subjects and performed 10 sets of squat jumps 10 times in 2 environmental conditions (water and ground). There was no significant difference in iEMG of vastus medilais according to exercise conditions and time. In MPV of CMJ, there was no significant difference according to exercise conditions and time. The thigh circumference showed a significant difference according to the exercise condition and time, and was higher in the ground condition after exercise. There was a significant difference in pain according to the exercise condition and time, and it was found to be high after exercise, 48 hours, and 72 hours in the ground condition. We believe that plyometric training in an aquatic environment will have less swelling and pain compared to plyometric training conducted in a land environment, and the pain will improve quickly, so we think that training can be conducted in a relatively shorter period than in the land environment.
