Predictive Factors Affected to Forced Vital Capacity in Children with Cerebral Palsy (뇌성마비 아동에서 노력성 폐활량에 영향을 미치는 요인 분석에 관한 연구)

  • Purpose: Children with cerebral palsy generally have a high incidence of respiratory problem, resulted from poor coughing, airway clearance problem, respiratory muscle weakness, kyphoscoliosis and so forth. The purpose of this study is to investigate the possible factors that can be affected to forced vital capacity (FVC) in children with cerebral palsy. Methods: Total thirty six children with diplegic and hemiplegic cerebral palsy were recruited in this study. They were evaluated by general demographic data (i.e., age, gender, body mass index (BMI)) and variables related to respiratory functions (i.e., chest mobility, waist mobility, maximal phonation time, and maximum inspiratory/expiratory pressure (MIP/MEP)). The correlation between forced vital capacity and the rested variables were analyzed, and multiple regression with stepwise method was conducted to predict respiratory function, in terms of FVC as the dependent variable, and demographic and other respiratory variables as the independent variable. Results: FVC showed a significant correlation with waist mobility (r=0.59, p<0.01), maximal phonation time (r=0.48, p<0.05), MIP (r=0.73, p<0.01), and MEP (r=0.60, p<0.01). In addition, the multiple regression analysis model indicated that FVC could be predicted by the assessment of each waist mobility and MIP. Conclusion: These finding suggest that respiratory function is related to body size and respiratory muscle strength, and that BMI, waist mobility, and MIP can be predictable factors to affected respiratory function in term of FVC.

Immediate Breast Reconstruction with DIEP Free Flap (심부하복벽 천공지 유리피판을 이용한 즉시 유방재건술)

  • In the past decade, there has been increasing breast reconstructions after mastectomy, and the abdomen has been the gold standard for donor site. TRAM (transverse rectus abdominis myocutaneous), MSTRAM (muscle sparing transverse rectus abdominis myocutaneous), DIEP (deep inferior epigastric artery perforator), SIEA (superficial inferior epigastric artery) flap has been widely used nowadays. Among them, DIEP free flap spares the whole rectus abdominis muscle and anterior rectus sheath resulting in decreased donor site morbidity. Between March of 2006 and February of 2008, six patients had undergone immediate breast reconstructions using DIEP free flap. The mean age of patients was 48.5 years. All patients had unilateral breast reconstructions. We dissected two perforators which were included in the unilateral pedicle. Thoracodorsal artery and its venae comitantes were chosen as recipient vessels. For venous anastomosis, we used the GEM Microvascular Anastomotic Coupler System (Synovis Micro Companies Alliance, Inc., Birmingham, Ala.) in four cases. All flaps were survived completely except one who showed fatty abdomen in old age. She showed repetitive vascular spasm intraoperatively. None of the patients had abdominal hernia, bulge or weakness. We believe that DIEP free flap provides a reliable method for autologous breast reconstruction if the patients are selected appropriately and performed by a skillful surgeon.

The Immediate Effects of Functional Taping on Pain, Muscle Strength, and Range of Motion of the Shoulder After Surgery in Patients With Rotator Cuff Tears (기능적 테이핑이 어깨둘레근 수술 후 환자의 어깨관절 통증과 근력, 관절가동범위에 미치는 즉각적 영향)

  • Background: Patients after rotator cuff (RC) surgery experienced pain, weakness and limited of motion of the shoulder. Physical therapists have used heat therapy, electrotherapy, range of motion (ROM) exercise and other methods to treat patients after RC surgery. In addition, functional taping is also used to support joint movement and to increase shoulder joint stability. Objects: The purpose of this study was to determine the initial effects of functional taping using non-elastic tape on pain, strength and ROM of the shoulder following RC surgery. Methods: Forty-eight patients with who underwent RC surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, $n_1=25$) and a control group (CG, $n_2=23$). First, non-allergic tape was applied to the shoulder to prevent skin irritation. The EG applied functional taping using non-elastic tape and the CG applied sham taping using elastic tape. Assessment tools included the shoulder pain and disability index for functional activity score, visual analog scale for level of pain, shoulder muscle strength, hand grip strength and ROM testing. Results: Pain score in the both group significantly decreased (p<.05), and change in pain score of in the EG increased significantly than in the CG (p<.05). Shoulder strength and ROM in the both group significantly increased (p<.05). Especially external rotation and extension of the shoulder ROM in the EG increased significantly more than in the CG (p<.05), but the rate of change in the two groups showed no significant difference. Conclusion: These results suggest that functional taping using non-elastic tape was initially effective in decreasing pain score level in patients with RC surgery.

A Study on Comparison between Growth Indices of Weak Children Groups via Analyzing Bone Age and Body Composition (허약아와 성장지표의 상관성에 대한 연구)

  • Objectives The purpose of this study is to investigate the relationship between weak children and growth indices. Methods One hundred twenty-six of the first and the second grade elementary school students who didn't have to develop yet their secondary sexual characteristics answered the questionnaire. Their bone age and body composition were measured. Results 1. According to the Weak Children questionnaire analysis, respiratory problem was one of the biggest problems in the weak children group (35.7%), followed by digestive problems (23.0%), psycho-neurological problems (22.2%), neuromotor problems (9.5%), and urogenital problems (8.7%). 2. From the comparison between growth indices of weak children and that of normal children are as follows: 1) The group of children who had problem in their digestive system had lower growth indices than the normal average children group. The growth indices includes mean height, weight, total body water, protein mass, mineral mass, body fat, skeletal muscle mass, body fat percentage, body mass index (BMI), and basal metabolic rate (BMR). 2) The children who had urogenital weakness had lower mean height, RH (Recent Height), RH - MPH {Recent Height(%) - Mid-Parental Height(%)}, RUS (Radius, Ulna and Short bone) score, weight, protein mass, body fat, BMI, and visceral fat than the normal children group. 3. The results of the multiple comparisons of growth indices and weak children groups are as follows: 1) Digestive weak children were the lowest in total body water, protein mass, mineral mass, skeletal muscle mass, and basal metabolic rate. 2) Urogenital weak children were the lowest in height, RH, RH - MPH, RUS score, and weight. Conclusion These results showed that children's growth is strongly correlated to their own growth problems, especially to those children who have digestive and urogenital problems. Therefore, this may be an effective way for children growth treatment in Korean medicine to treat weak symptoms.

The Effects of Balloons Blowing and Kinesio Taping on Vital Capacity (풍선불기와 키네시오 테이핑이 폐활량에 미치는 영향)

  • Background : Respiratory muscle weakness has serious clinical consequences. Vital Capacity is the volume of air that is normally exchanged in a single breath. It varies widely with pulmonary health and overall fitness. Purpose : The purpose of this study was to investigate the variation of vital capacity(VC) according to the effects of Kinesio taping and balloons blowing. Methods : Twenty-one non-smokers were participated in this research and these subjects were randomly assigned into three groups; a balloons blowing group(n=7), a Kinesio taping group(n=7), and a balloons blowing and Kinesio taping group(n=7). This experiment was done from June 22, 2009 to July 5, 2009. In order to measure the VC variation effects of Kinesio taping, balloons blowing, and both intervention, an windmill type spiropet was used. The collected data were analyzed statistically by using a paired t-test and ANCOVA. Results : The results of this study were as follows; 1) In the case of balloons blowing, there was a significant difference, 3.35% increase of VC, between before and after 2 weeks experiments(p<.01). 2) In the case of Kinesio taping, there was a significant difference, 11.66% increase of VC, between before and after 2 weeks taping intervention(p<01). 3) In the case of both balloon blowing and Kinesio taping, there was a significant difference, 15.84% increase of VC, between before and after 2 weeks both interventions(p<.01). 4) After 2 weeks experiment, the improvement of VC was shown a significant difference with intergroup(p<.05). Conclusion : From these results, it was revealed that balloons blowing or Kinesio taping, or both interventions were effective to improve VC of healthy adults.

  • Objectives : Through the literatures on the effets of Bufonis Venenum, we are finding out the clinical possibility and revealing the more effective to intractable diseases. Methods : We investigated the literatures of Oriental Medicine and experimental reports about Bufonis Venenum. Results : 1. Bufonis Venenum is made of bufonidae bufo bufo gargarizans cantor or bufo melanostictus schneider of white serum which secreted from parotid gland or dermato gland, and it is dried for using. 2. In oriental medicine, Bufonis Venenum has been mainly used on the tumors, cacanthrax and dermatic disease, and then it has been clinically used on infantile athrepsia, tetanus, sore throat, toothache, and so on. 3. The pharmacological effects of Bufonis Venenum are cardiotonic, respiration stimulation, depressor or vasopressor, topical anesthcsia, hallucination, striped muscle stimulation, antiasthmatic, antibacterial, antiinflammatory, anticancer, diuretic, immuno effects, etc. 4. Bufonis Venenum is largely divided in ether binding steroid compound, hydroxyl steroid compound, carboxyl or aldehyde steroid compound, indole compound, and adrenaline, cholesterole, etc. 5. Symptoms of Bufonis Venenum poisoning in digestive system are vommitig, abdominal pain, diarrhea, dehydration, in circulatory system are palpitation, shock, bradycardia, in nervous system are vertigo, somnolentia, muscle-tendon reflex weakness, and critical conditions to tissue necrosis and heart attack. 6. Ways to treat Bufonis Venenum poisoning include gastric irrigation with $0.2~0.5\%$ potassium permanganate fluid and atropine $0.5{\sim}1.0mg$ subcutaneous injection. From the chinese book of Bon Cho Gang Moke(本草綱目), if white serum of Bufonis Venenum enter the eyes, it happens the edema and pain. And then washed the eyes by juice of Lithospermi Radix(紫草) that the edema is removed. Conclusions and Discussion : The results from above literary studies show that prescriptions and Aqua-acupuncture of Bufonis Venenum could be clinically used to sedative, anticonvulsant, antibacterial, antiinflammatory, anticancer and topical ataralgesia. However it is expected that pharmacological and side effects of Bufonis Venenum are further studied.

Revision Rotator Cuff Repair (회전근 개 봉합술 후 재수술)

  • The primary purposes of revision repair for a failed rotator cuff repair are a relief of pain and functional improvement. Therefore, revision repair is most proper in patients with the functional deficit accompanied with the shoulder weakness as well as the persistent pain. The important factor that is considered in revision repair is a quality of torn cuff. Especially, Care must be taken to ensure that the revision repair is possible, considering the size of tendon defect, atrophy of the muscle, fatty infiltration and extent of the retraction of tendon. Revision repair of a failed rotator cuff repair is more difficult, and the functional results are less satisfactory than those of primary repair, because excessive bursal scarring and tendon retraction may be exhibited, a large or massive tear is often detected, tear has usually been present for a long time, and a quality of muscle-tendon may be poor. So, we discuss our experiences related to revision repair after a failed cuff repair that has been recently introduced through the articles.

  • Eaton Lambert Syndrome(ELS)is a disorder of neuromuscular transmission. The defect of neuromuscular transmission is due to decrease in the release of acetylcholine guanta from nerve terminal. This syndrome is frequently associated with bronchogenic carcinoma. The diagnosis is established by electromyography, which characteristically shows 1) low amplitude of evoked compound muscle potential to a single supramaximal stimulus on nerve, 2) significant decremental response at low rates of stimulation 3) marked incremental response at high rates of stimulation. Our patient is 52year old man with dyspnea, coughing and muscle weakness of proximal lower limbs. He has small cell lung cancer and associated with ELS, Superior venacava syndrome and has metastatic lesion on right supraclavicular lymphnode confirmed by pathology. Metastatic mass and SVC syndrome are marked improved following chemotherapy and radiotherapy, however follow up EMG finding does not improved. We are here reporting one case which considered compatible for ELS, with a few elementary reviewed literatures.

  • In patients with myasthenia gravis, it is difficult to manufacture and maintain dentures because the muscles and nervous system in the oral cavity are not properly adjusted. In addition, excessively extended or thick dentures may cause muscle weakness by stimulating the muscles, so dentures should be manufactured in consideration of this. In this case, a denture was fabricated using the closed mouth technique in a patient with myasthenia gravis with mouth opening limitation. Using the closed mouth technique, the patient's masticatory pressure and muscle movements were reproduced and denture with good retention were manufactured.

A Case of Cervical Spinal Stenosis Improved by Combined with Chuna Manual Therapy and Exercise Treatment (추나치료 및 상지 운동치료를 통해 호전된 경추 협착증 환자 치험례)

  • This study reports the effects of Korean medicine treatments including Chuna manual therapy (CMT) and exercise treatment for a patient suffering with cervical spinal stenosis. We treated patient diagnosed with cervical spinal stenosis. The patient was treated with CMT once a day for 26 days. Manual muscle testing (MMT), ratio and degree of numbness, numerical rating scale (NRS) were used as tools for evaluating the patient's progress. MMT was increased from 3+ to 5, ratio and degree of numbness were decreased from grade (Gr.) 9 to Gr. 4, NRS was decreased from 5 to 3. This study suggests that CMT and self-exercise can be considered as effective treatment for cervical spinal stenosis.
