Tumors Involving Skin, Soft Tissue and Skeletal Muscle: Benign, Primary Malignant or Metastatic?

  • Background: Metastatic cancer with invasion of skin, soft tissue and skeletal muscle is not common. Examples presenting as soft tissue masses could sometimes lead to misdiagnosis with delayed or inappropriate management. The purpose of current study was to investigate clinical characteristics in the involvement of metastatic cancer. Materials and Methods: A total of 1,097 patients complaining of skin or soft tissue masses and/or lesions were retrospectively reviewed from January 2012 to June 2013. Tumors involving skin, soft tissue and skeletal muscle of head and neck, chest wall, abdominal wall, pelvic region, back, upper and lower extremities were included in the study. Results: Fifty-seven (5.2%) patients were recognized as having malignancies on histopathological examination. The most common involvement of malignancy was basal cell carcinoma, followed by cutaneous squamous cell carcinoma, sarcoma and melanoma. The most common anatomical location in skin and soft tissue malignancies was head and neck (52.6% of the malignancies). Four (0.36%) of the malignant group were identified as metastatic cancer with the primary cancer source from lung, liver and tonsil and the most common site was upper extremities. One of them unexpectedly expired during the operation of metastatic tumor excision at the scalp. Conclusions: Discrimination between benign and malignant soft tissue tumors is crucial. Performance of imaging study could assist in the differential diagnosis and the pre-operative risk evaluation of metastatic tumors involving skin, soft tissue and skeletal muscle.

닭 조직에 따른 Nebulin Isoform 단백질의 특이적 발현 (Tissue-Specific Expression of Nebulin Isoform Proteins in Chicken)

  • 대부분 척추동물에서 골격근 내 filament들의 길이 조절은 근 수축 기작의 구조를 이해하는데 중요한 단서가 된다. Nebulin은 thin filament의 전체에 걸쳐있는 거대한 단백질로 골격근에만 특이적으로 존재하는 것으로 알려져 왔다. 본 연구에서는 닭의 근육과 비근육 조직에서 nebulin isoform단백질들을 확인하기 위하여 전기영동과 immunoblot의 방법을 이용하였다. 각 조직의 단백질들은 soluble과 insoluble fraction으로 분리 준비하였다. 실험결과, 닭의 근육과 비근육 조직들에서 조직 특이성을 나타내는 다양한 nebulin isoform 단백질들이 확인되었다. Nebulin은 성계의 골격근에서 500 kDa 정도의 크기로 나타났고, nebulett은 계배와 성계의 심장근에서 107 kDa 정도로 발현되었다. 그리고 계배의 비근육 조직인 뇌에서 380 kDa 정도의 거대 단백질이 확인되었다. 이 단백질은 뇌 조직의 soluble fraction에서 인지되었다. Nebulin isoform 단백질들이 서로 다른 조직에서 발현되는 양상을 보아 서로 다른 독자적인 기능을 가질 것으로 추정된다.

박근 유리피판술과 피부이식술을 이용한 아킬레스건이 노출된 창상의 재건 (Reconstruction of Soft Tissue Defect, Exposing Achilles Tendon by Using Gracilis Muscle Free Flap and Skin Graft)

  • Purpose: Reconstruction of the soft tissue defect exposing Achilles tendon is a formidable challenge because of the paucity of soft tissue and relatively poor blood supply. This article describes the reconstruction of soft tissue defect exposing Achilles tendon using gracilis muscle free flap and split-thickness skin graft. Methods: From 2000 to 2005, four patients with soft tissue defect exposing Achilles tendon and infection were operated using gracilis muscle free flap and split-thickness skin graft. The defect size ranged from 3.5 to 5cm wide and 6.5 to 8cm long. The mean postoperative follow-up was twenty months. Results: All the flaps were survived without necrosis and infection. We obtained the satisfactory results with good functional and aesthetical outcomes. All cases showed good results with the characteristics of a relatively thin flap without additional debulking procedure. Conclusion: Gracilis muscle free flap with split thickness skin graft could be a good option for reconstruction of soft tissue defect of posterior ankle, exposing Achilles tendon with minimal morbidity of the donor site.

뇌졸중 환자에서 주관절 각도 변화에 따른 상완이두근의 근구조 및 탄성 변화에 관한 연구 (A Study on Muscle Architectural and Tissue Compliance of Biceps Brachii in Stroke Patient Based on Elbow Joint Angle)

  • 본 연구의 목적은 뇌졸중 환자에서 주관절 각도 변화에 따른 상완이두근(biceps brachii)의 근구조 및 탄성 변화에 관하여 알아보고자 하였다. 연구대상은 주관절 수동 ROM범위가 $10^{\circ}{\sim}90^{\circ}$ 가능하며 MAS(modified Ashworth scale) 1~3등급인 뇌졸중 환자 12명을 대상으로 하였다. 이완(resting) 상태의 건측과 환측 상완이두근의 우모각, 근섬유속길이, 근탄성의 측정은 초음파 영상 촬영장치와 근긴장도 측정기(myotonometer)를 이용하여 주관절을 $10^{\circ}{\sim}90^{\circ}$로 변화시키면서 측정하였다. 본 연구 결과, 환측 상완이두근을 건측과 비교 시 우모각은 신전위치($<40^{\circ}$)에서 큰 값을 나타냈으며(p<.05), 근섬유속길이는 굴곡위치($>20^{\circ}$)에서 짧은 길이를 나타냈으며(p<.05), 근긴장은 신전위치($<50^{\circ}$)에서 전위 값이 낮게 나타나 근긴장이 높아짐을 나타냈다(p<.05). 이렇듯 이완 시 상완이두근의 근구조 및 탄성은 주관절 각도에 의해 영향을 받는다는 것을 알 수 있었다. 본 결과를 뇌졸중 환자의 근구조 변화 및 임상치료의 연구 자료로 사용할 수 있을 것으로 생각된다.

Effects of exhaustive exercise on ER Stress of skeletal muscle and adipose tissue in rats

  • The purpose of this study was to identify the effects of gene expression of endoplasmic reticulum (ER) stress in skeletal muscle and adipose tissue on acute exhaustive exercise. Thirty-five Sparague Dawley male rats were divided into a control group (CON, n = 7) and a exhaustive exercise group (n = 28), immediately after exhaustive exercise group (n = 7), after 30 minutes exhaustive exercise group (n = 7), after 60 minutes exhaustive exercise group (n = 7), after 180 minutes exhaustive exercise group (n = 7). As a result, changes in the composition of the blood serum triglyceride concentration increased significantly in immediately after exhaustive exercise group, On the contrary, blood glucose showed a significantly decreased (p < .05). Homeostasis of energy metabolism due to exhaustive exercise as a result of the mechanism of action of skeletal muscle in the glycogenolysis and absorption, which indicates that the process of means. On the other hand, a result of examining changes in endoplasmic reticulum stress-related proteins in skeletal muscle and adipose tissue, JNK1 except in skeletal muscle BiP, ATF4, CHOP, GRP78 mRNA increased significantly immediately after exercise, and after 30 minutes returned to normal levels that could be confirmed (p < .05). BiP mRNA in adipose tissue show a similar pattern and skeletal muscle increased significantly immediately after exercise, but other changes in the specificity of the endoplasmic reticulum stress-related proteins also did not appear. In conclusion, Exercise applies and exercise training duration and exercise intensity as well as research on the interaction of the endoplasmic reticulum stress-related genes should be study continuously, to be more clear.

Reverse Latissimus Dorsi Muscle Flap for an Extensive Soft Tissue Defect Accompanied by Infectious Spondylitis

  • Spinal infection is an inflammatory process around the vertebral body, and it can extend to the epidural space, posterior elements and paravertebral soft tissues. Infectious spondylitis is a rare infectious disorder, which is often associated with significant neurologic deficits and mortality. When an extensive soft tissue defect is accompanied by infectious spondylitis, effective infection control and proper coverage of soft tissue are directly connected to successful outcomes. However, it is not simple to choose the appropriate treatment methods for infectious spondylitis accompanied by a soft tissue defect. Herein, we report a case of severe infectious spondylitis that was accompanied by an extensive soft tissue defect which was closed with a reverse latissimus muscle flap after traumatic spinal epidural hemorrhage.

Immediate Effects of Soft Tissue Massage on Posterior Shoulder Muscle Tightness: A Preliminary Study

  • Posterior shoulder muscle tightness is frequently observed in shoulder impingement syndrome because tightness in the posterior portion of the shoulder muscles can cause anterior and superior translation of the humeral head in relation to the glenoid fossa. The purpose of this study was to determine the immediate effects of soft tissue massage on acromiohumeral distance (AHD), anterior translation of the humeral head, and glenohumeral (GH) range of motion (ROM) in subjects with posterior shoulder muscle tightness. Twenty-seven subjects with greater than $10^{\circ}$ difference in the range of GH horizontal adduction between right and left sides were recruited. The range of GH horizontal adduction and internal rotation were measured by a digital inclinometer. The AHD and anterior translation of the humeral head were measured using ultrasonography. A paired t-test was used to compare AHD, anterior translation of the humeral head, and the range of GH horizontal adduction and internal rotation before and after soft tissue massage. The results showed that AHD increased significantly (p<.05) and the anterior translation of humeral head decreased slightly, but not significantly (p=.40) after the soft tissue massage. Furthermore, the ROM of horizontal adduction and internal rotation in the GH joint increased significantly after the soft tissue massage (p<.05). These findings indicate that soft tissue massage on posterior shoulder muscle tightness is an effective method to increase AHD and ROM in the horizontal adduction and internal rotation of the GH joint.

넙다리뒤근이 단축된 성인에게 시행된 근에너지기법과 도구를 이용한 연부조직가동술이 관절가동범위, 근력, 근 두께에 미치는 영향 (The Effect of Muscle Energy Technique and Instrument Assisted Soft Tissue Mobilization in Adults with Shortened Hamstring on the Range of Motion, Muscle Strength and Muscle Thickness)

  • Background: This study aimed to investigate the effects of muscle energy technique (MET) and instrument assisted soft tissue mobilization (IASTM) on knee extension ROM, knee extensor/flexor strength and muscle thickness immediately and after 24 hours. Methods: A total of 30 subjects participated in this study. The participants were assigned to either MET (n=15) or IASTM (n=15). 90-90 straight leg raise, knee extensor/flexor muscle strength, muscle thickness test were measured before, immediately after and 24 hours after the intervention. Results: Both groups significantly improved knee extension ROM on immediate (MET 10.7°, IASTM 10.21° increased) and after 24 hours assessment (MET 5.61°, IASTM 5.47° increased)(p<.05). In the MET group, knee extension and flexion muscle strength increased immediately after intervention (p<.05). In the IASTM group, knee extension muscle strength increased and knee flexor muscle strength decreased immediately after intervention (p<.05). Furthermore, both groups showed a pattern of returning to the initial strength after 24 hours. In both groups, no significant difference in muscle thickness immediately and after 24 hours was observed (p>.05). Conclusion: According to the results of the present study, MET and IASTM technique showed lasting effectiveness in flexibility of shortened hamstring immediately after and in 24 hours after the intervention. In both groups, MET increased muscle strength and increased ROM, while IASTM decreased muscle strength and increased ROM, with no change in muscle thickness.

"굴비" 제조과정중의 지방의 이동에 대한 조직학적 관찰 (MICROSCOPIC OBSERVATIONS OF FAT TRANSLOCATION IN THE TISSUE OF YELLOW CORVENIA DURING SALTING AND DRYING)

  • "굴비" 제조과정중 근섬유의 변화와 지방의 분포 및 이동상태를 조직학적으로 관찰하여 다음과 같은 결과를 얻었다. 생조기는 진피 밑에 피하지방층이 있으며, 혈합육이 지방조직으로 둘러 쌓여 있는 곳이 많다. 대부분의 지방질은 피하지방층에 축적되어 있고, 백색육에는 거의 지방이 분포하지 않았다. 또한 혈합육과 백색육간에 결체조직으로 경계가 이루어지지 않아, 어떤 부위에는 혈합육과 백색육의 경계가 뚜렷하지 않은것 등이 특징이었다. "굴비" 제조과정중 염장공정중에는 피하지방의 일부가 결체조직이나 근섬유간을 통로로하여 침투되어 들어간다. 그러나 건조공노중 침투된 지방은 결체조직을 따라 이동하여 복공을 통하여 외부로 유출한다고 볼 수 있다. 그러므로 "굴비"의 심부백색육에는 거이 지방이 분포하지 않았다 . BHA 처리 "굴비"는 지방의 이동속도가 늦어, 내부에 침투된 지방이 상당양 그대로 잔존하였다. 그리고 "굴비"의 근육조직은 각섬유가 씨로 밀착하여 경계가 뚜렷하지 않는 것을 관찰 할 수 있다. 또한 "굴비" 제품중에는 상당량 지방이 주로 피하지방층에 함유되어 있음을 알 수 있다. 끝으로 본실험에 있어, 조개표본을 제작하는데 편의를 보아주신 부대학교 의과대학 해부학교실 박 해춘하 재청 양교수 그리고 현징경 사진촬영을 도와 주신 국립수산진흥원 김 성준 연구관, 김 복원 연구교사 몇 많은 조언을 주신 부산수산대학 김장양 교수, 신필현 교수에게 심심한 사의를 표한다.

  • Muscle based face image synthesis is one of the most realistic approach to realize life-like agent in computer. Facial muscle model is composed of facial tissue elements and muscles. In this model, forces are calculated effecting facial tissue element by contraction of each muscle strength, so the combination of each muscle parameter decide a specific facial expression. Now each muscle parameter is decided on trial and error procedure comparing the sample photograph and generated image using our Muscle-Editor to generate a specific face image. In this paper, we propose the strategy of automatic estimation of facial muscle parameters from 2D marker movement using neural network. This also 3D motion estimation from 2D point or flow information in captered image under restriction of physics based face model.
