The passive stretching, massage, and muscle energy technique effects on range of motion, strength, and pressure pain threshold in musculoskeletal neck pain of young adults

  • Objective: Musculoskeletal neck pain have many symptoms which include decreased range of motion (ROM) and muscle strength, and increased pain. However, the management methods are controversial. The purpose of this study was to examine the effects of three interventions on ROM, strength, and pressure pain threshold (PPT) with musculoskeletal neck pain. Design: Pretest-posttest design. Methods: Thirty subjects participated in this experiment. They were randomly assigned to thefollowing groups: passive stretching (PS) group (n=10), massage (MASS) group (n=10), and muscle energy technique (MET) group (n=10). The treatment were applied bilaterally on the upper trapezius. The PS was applied 3 times for 30 seconds each time. The MASS was applied using two different techniques for 2 minutes per technique. For MET, the subjects performed 2 sets of 3 repetitions of isometric resistance exercise that was maintained for 10 seconds, followed by 10 seconds of rest. ROM, strength, and PPT parameters were measured after intervention. Results: In the MASS group, there was a significant improvement in all outcomes except for muscle strength (p<0.05). In the MET group, ROM and strength significantly improved compared to the pre-treatment results (p<0.05). As result of measuring the amount of change in each group, there was a significant difference in ROM (flexion) in the PS group compared with the MASS and MET group, a significant difference in strength in the MET group compared with the PS and MASS groups, and a significant difference in PPT in the MASS groups compared with the PS and MET groups (p<0.05). Conclusions: This study showed that PS, MASS, and MET are effective methods for improving ROM, strength, and PPT for musculoskeletal neck pain. Therefore, various therapeutic interventions for improving ROM, strength, and pain are suggested.

  • Objectives : The purpose of this study is to report the effect of muscle energy technique(MET) on idiopathic scoliosis with double curved thoracolumbar spine. Methods : Two patients with idiopathic scoliosis with double curved thoracolumbar spine were treated by MET. To investigate the outcome of the patients' improvement, we observed the standing X-ray of cervical, thoracic, lumbar spine. And Cobb's angle, correctability and visual analog scale(VAS) were used to measure changes during treatment. Results : After treatment, Cobb's angle, correctability and VAS were improved significantly. Conclusions : MET showed significant improvement to idiopathic scoliosis with double curved thoracolumbar spine. There is limitation on this study due to insufficient number of cases. Further research should be followed.

Effects of Muscle Energy Technique of Upper Trapezius and Sternocleidomastoid Muscles on Bell's Palsy

  • Background: The purpose of this study was to investigate the efficacy of muscle energy techniques (MET) of upper trapezius and sternocleidomastoid muscles on Bell's palsy. Methods: In this retrospective study, we screened the medical records of patients with Bell's palsy who had received inpatient and outpatient treatment at the Department of Acupuncture & Moxibustion, Korean Medicine Hospital Dong-Eui University between November 28, 2016 and April 30, 2017. A total of 34 out of 93 Bell's palsy patients met the inclusion criteria. The 34 patients were divided into two groups: Group A patients had undergone Korean-Western combination treatment and MET of upper trapezius and sternocleidomastoid muscles; Group B patients had undergone Korean-Western combination treatment only. MET of upper trapezius and sternocleidomastoid muscles had been performed three times a week during the inpatient period, and two to three times a week during the outpatient period. Yanagihara scores had been assessed at the first visit, and 1, 2, 3, and 4 weeks after the first visit. Results: Group A Yanagihara scores were significantly improved during each interval from the first visit to 4 weeks later. Group B Yanagihara scores were also significantly improved except during the first week. During every period, the improvements observed in Yanagihara score were significantly higher in Group A than in Group B. Conclusion: These results suggest that MET of upper trapezius and sternocleidomastoid muscles may be effective treatment for Bell's palsy.

  • PURPOSE: This study compared the effect of the muscle energy technique (MET) and stretching technique on ankle dorsiflexion passive range of motion, balance, and gait ability of stroke patients with limited ankle dorsiflexion. METHODS: Forty-four post-stroke patients participated. The participants were randomized into the MET group (METG; n = 22) and the stretching group (STG; n = 22). The METG was subjected to the MET to relax the dorsiflexion, while the STG was subjected to the dorsiflexion stretching technique. Both groups completed standard neurological physical therapy for 30 min per session. The intervention was conducted five times a week over 3 weeks for a total of 15 times. All participants underwent ankle dorsiflexion passive range of motion measurement and Berg Balance Scale score determination and completed a 10-m walking test and the timed up and go test before and after the intervention. RESULTS: After the 3-week intervention, both groups showed significant improvement after the intervention (p < .05). METG participants showed greater improvements in ankle dorsiflexion passive range of motion and 10-m walking test results compared to STG participants (p < .05). CONCLUSION: Both interventions improved ankle dorsiflexion passive range of motion, balance, and gait ability in stroke patients with limited ankle dorsiflexion. Moreover, the MET was superior to ankle dorsiflexion passive range of motion on the 10-m walking test.

목 근육에 대한 근에너지기법 적용이 전방머리자세에 미치는 영향 (The Effect of Applying the Muscle Energy Technique to Neck Muscles on the Forward Head Posture)

  • Purpose : The purpose of this study is to compare muscle activity after applying two muscle energy techniques (MET) to subjects with forward head posture to see if the post isometric relaxation (PIR) technique is more effective than the reciprocal inhibition (RI) technique. Methods : The muscle activity was measured using EMG after applying the PIR and RI techniques to 30 adults at K College. Subjects were selected for forward head posture whose ear center was 2.5 ㎝ front of the center of the shoulder. EMG equipment was used to measure muscle activity, and the measurement sites were measured in cervical flexor and extensor muscles. The experiment period was performed once a week for a total of two weeks, and after the pre-measurement was performed for 5 minutes PIR and RI exercise. In the PIR technique, the head is tilted back in a sitting position, and the experimenter applies resistance with the same force for 7~10 seconds and repeats 3-5 times after rest. In the RI technique, in a sitting position, the subject gives the force to bend the head forward, and the experimenter applies resistance with the same force for 7 to 10 seconds, and repeats 3 to 5 times after rest. Results : The result is same as the following. In the comparison of muscle activity, there was a significant decrease in both PIR and RI at 1 and 2 weeks. And there was a greater decrease in muscle activity in PIR. There was no difference in the comparison of decrease in muscle activity at 1 week and 2 week. Conclusion : Both PRI and RI can be said to be effective in improving the function of the forward head posture in the neck muscles. Therefore, the selection of the two techniques in clinical practice should be appropriately performed under the judgment of experts according to the patient's situation.

  • Purpose: This study examined whether a single application of a lumbar flexion muscle energy technique (MET) could significantly influence pain, functional status, and range of motion in chronic back pain patients with restricted active trunk flexion. Methods: Twenty volunteers were randomly assigned to either a control group or an experimental group. Ten subjects in the control group received no treatment. Ten subjects in the experimental group received five sessions with the muscle energy technique, five times per week for two weeks. Outcomes were measured before and after two weeks of treatment in both groups. Pain intensity was assessed using a visual analogue scale (VAS). Range of motion was measured using the sit-and-reach test (SRT). Functional disability levels were assessed using the Korean version of the Oswestry Disability Index (KODI). Results: The experimental group showed a significant difference in VAS, KODI, and SRT between pre- and post-treatment (p < .05), while the control group showed no significant changes. The changes in VAS, ODI, and SRT were statistically significant between the groups pre- and post-treatment (p < .05). Conclusion: This muscle energy technique is clinically effective for chronic low back pain patients.

근에너지기법과 경근간섭파요법이 요통환자의 요방형근 경근전도에 미치는 영향 (The Effects of MET and ICT in Patients with Lumbago by Meridian Muscle Electrography)

  • Objectives The purpose of this study is to compare the effects of MET (muscle energy technique) and ICT (interferential current therapy) on lumbago by meridian muscle electrography (MMEG). We performed MET and ICT on quadratus lumborum muscle. Methods This study was carried out on 30 lumbago patients. 30 patients were randomly divided into MET group and ICT group. MET group take treatment 10 minutes. ICT group take treatment 15 minutes. After performing MET and ICT twice, we compared both in terms of root mean square (RMS), special edge frequency (SEF), VAS and ODI. Results 1. Both MET group and ICT group showed significant improvement in VAS and ODI after treatment. 2. Both MET group and ICT group showed significant improvement in special edge frequency (SEF) of the left side after treatment. But SEF of the right side showed insignificant improvement. 3. Both MET group and ICT group showed insignificant improvement in root mean square (RMS) for both sides. 4. There are no significant differences between both groups. Conclusions According to above results, we found out that performing MET or ICT on quadratus lumborum muscle has effect in terms of VAS and ODI. But, in terms of SEF, we found out different result for both side. And we can't found out effect in terms of RMS.

슬관절 전치환술 후 만성기 여성 노인의 슬관절 신전근에 근에너지기법이 근력, 관절가동범위, 균형, 보행능력에 미치는 영향 (Effects of Muscle Energy Technique on Knee Extensor Muscle Strength, Knee Range of Motion, Balance, and Walking Ability in Elderly Women during the Chronic Phase after Total Knee Replacement)

  • Background: This study aimed to use the muscle energy technique (MET) with total knee replacement (TKR) during the chronic phase in a clinical setting and confirm its effects on the knee extensor strength and ROM, balance, and walking ability. Methods: A total of 20 female patients who underwent TKR 1~4 years ago were assigned to two groups (Control: Q setting exercise+general physical therapy, n=10; Exp: MET+general physical therapy, n=10). Interventions were performed three times a week for 4 weeks. The strength of the knee extensor was evaluated using an aneroid sphygmomanometer, and ROM was evaluated using degrees at the end range on active knee flexion. The main balance outcomes were evaluated using two standard scale (TSS) and timed up and go (TUG) test, whereas the walking ability was evaluated using the 10 meter walk test (10MWT). Results: Analysis showed that both groups had significant increases in strength, ROM, TSS, TUG, and 10MWT. Differences in all variables were significant between the control and Exp groups at the post-intervention evaluation (p<.05). However, no significant difference was observed in strength and TUG. Conclusion: Results of this study demonstrated that MET would help improve the strength, ROM, balance, and walking ability of patients with chronic TKR who want to enhance their abilities and performance in activities of daily living.

요통 유무에 따른 20대 성인의 넙다리뒤근에 적용한 두 가지 근에너지 기법 종류에 따른 골반 경사각 및 넙다리뒤근 길이에 미치는 즉각적 효과 비교 (Comparison of the Immediate Effects of Two Types of Muscle Energy Techniques Applied to the Hamstring of Adults in Their Twenties With or Without Low Back Pain on the Pelvic Inclination and the Length of the Hamstring)

  • Background: Lower back pain (LBP) is a major cause of disability and a common musculoskeletal disorder encountered at some point in life. Dysfunction of the lumbar vertebrae has been associated with decreased flexibility of the hamstrings, which exhibited a strong positive correlation with LBP. Hamstring tension affects lumbar pelvic rhythm. We aimed to activate pelvic stability with compression by Active Therapeutic Movement (ATM), muscle energy technique (MET) was applied to increase the flexibility of the hamstring. Objects: In this study, we aimed to investigate the effects of MET with ATM and general MET were applied to the hamstring of adults, who were in their twenties with nor without LBP, on their pelvic inclination and the length of their hamstring. Methods: A total of 32 subjects were briefed about the purpose of this study and agreed to participate voluntarily. Before the experiment, all subjects were pre-examined, and they were divided into an LBP group and a no lower back pain group accordingly. Thereafter, all subjects participated in both in a crossover manner. After at least one week, they switched to another group and participated in the same experiment. Results: The study results revealed that both groups demonstrated significant results in the modified active knee extension test (p < 0.01) and the sit and reach test (p < 0.01) performed to assess the hamstring flexibility; an interaction (p < 0.05) was noted. Moreover, a more significant difference was observed between the MET with ATM and the general MET. Although significant results were obtained for the pelvic inclination (p < 0.01), interaction was not noted. Conclusion: Conclusively, in this study, when the MET with ATM was applied to the two groups, there was a significant difference compared to the general MET for hamstring flexibility, but it was confirmed that there was no significant difference for the pelvic inclination.

정골의학적(Osteopathic) 수기요법 분석 및 경근추나와의 관련성 연구 (Analysis of Osteopathic Manipulation and Study on Relationship with Chuna Manual Therapy for Meridian Sinew System)

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.
