정상발과 평발에서의 목말밑 관절가동범위와 등쪽굽힘근의 근활성도 비교 (Comparison of Subtalar Joint Range of Motion and Dorsiflexor Muscle Activity Between Normal and Pes Planus Feet)

  • PURPOSE: The imbalance of pretibial muscles can be a factor contributing to the development of pes planus. However, no study has yet compared the muscle activity of the tibials anterior (TA) to that of the extensor digitorum longus (EDL). The purpose of this study was to determine whether there are differences in the electromyographic (EMG) TA and EDL amplitude indexes (AIs) between normal and pes planus feet. METHODS: A total of 14 subjects with normal feet and 15 subjects with bilateral pes planus participated in this study. TA and EDL muscle activities were measured using a wireless EMG system and the angles of ankle dorsiflexion and eversion of the subtalar joint were measured using a universal goniometer during active ankle dorsiflexion in the prone position. AI was calculated as follows: $\text{amplitude_{TA}-amplitude_{EDL}/(amplitude_{TA}+amplitude_{EDL})}/2{\times}100$. RESULTS: The AIs of the TA and EDL were significantly lower in pes planus feet than in normal feet (p<.05). The angle of subtalar eversion was significantly greater in pes planus feet than in normal feet during active ankle dorsiflexon (p<.05). However, there was no significant difference in the angle of ankle dorsiflexion between normal feet and pes planus feet (p>.05). CONCLUSION: This study showed that TA muscle activation was lower in pes planus feet than in normal feet, resulting from greater eversion range of motion during active ankle dorsiflexion. We suggest that the imbalance of ankle dorsiflexors must be considered in pes planus management.

The Effect of Plank Exercises with Hip Abduction Using Sling on Trunk Muscle Activation in Healthy Adults

  • Purpose: The purpose of this study is to investigate the effects of healthy persons performing a plank exercise with hip abduction and use of sling on trunk muscle activity. Methods: Twenty-three subjects participated in this study. Muscle activities of the multifidus, rectus abdominis, external oblique and internal oblique were assessed with electromyography (EMG) while the subjects performed 6 various types of plank exercises in random order. Results: There was a significant difference in multifidus and internal oblique on the supported side and the multifidus, external oblique, and internal oblique on the elevated side (p<0.05), and differences in plank methods were significantly higher in the multifidus, rectus abdominis, external oblique, internal oblique of the supported side and the rectus abdominis, external oblique and internal oblique on the elevated side (p<0.05). In addition, post-hoc analysis of the various plank methods showed that muscle activity was significantly higher during the general plank exercise compared with the knee-supported-in-sling plank exercises (p<0.05), and was significantly higher with the ankle-supported-in-sling plank exercises compared with the general plank exercise. Conclusion: There was greater trunk muscle activity with plank exercises performed with the ankle-supported-in-sling and hip abduction plank exercises compared with the general or knee-supported-in-sling plank exercises. Through this study, various plank exercises have been discovered that can be selected at different intensities for core muscle training purposes based on trunk muscle strength.

Relationship of the Frontal Knee Alignment Measured by the HKA-Angle with the Relative Activation of the Quadriceps Muscles

  • Purpose: The purpose of this study was to compare Hip-Knee-Ankle(HKA) angle and muscle activation ratio between vastus medialis(VM), rectus femoris(RF), and vastus lateralis (VL), and by this, to examine their relationship. It is aimed to explore how the activation ratio among the muscles involved in patellofemoral kinetics would vary in relation with the frontal alignment of the lower extremity. Subjects and Methods: 26 healthy subjects were recruited for the study. The HKA angles were measured with radiograph. The VM, RF, VL muscle activation level were measured by surface electromyography while each participant performed 4 different types of movement (isometric knee extension, squat, ambulation, step-up) and VM/RF, VM/VL, RF/VL ratios were calculated. Pearson correlation was used to estimate the relationship between the HKA angle and the muscle ratio. Results: There was significant moderate correlation between HKA angle and VM: RF on the left side during ambulation (p<0.05). Moderate correlations were also observed during step-up and squat with less significance (p<0.1). Conclusion: The frontal alignment of the knee measured by the HKA angle was conditionally associated with muscle activation ratio between VM and RF (VM:RF); On the left, during ambulation, step-up, and squat, the more valgus knee tended to correlate with the more VM muscle activation ratio, which is expected to induce more stabilizing effect to the patella and its tracking. It suggests that the frontal alignement measured by the HKA-angle can affect PF kinetics. It also indicates a possibility that increase in valgus alignment of the knee, by the HKA measurement, may not act unfavorably to generate PFP.

골반 자세 변화에 따른 일어서기동작의 운동형상학적 분석과 근전도 연구 (Kinematic and EMG Analysis of Sit-to-Stand With Changes of Pelvic Tilting)

  • The purpose of this study was to analyze the effects of three different pelvic tilts on sit-to-stand ativities and to suggest a new therapeutic approach for movement reeducation in patients who have difficulty with sit-to-stand activities. The three different pelvic tilts were: (1) comfortable pelvic tilt sit-to-stand (CPT STS), (2) posterior pelvic tilt sit-to-stand (PPT STS) and (3) anterior pelvic tilt sit-to-stand (APT STS). To analyze the kinematic component of STS, a motion analysis system (Zebris) was applied to the ankle, knee, hip joint, and thigh-off area. Also, to determine the onset time of muscle contraction, surface electrodes were placed to the rectus femoris muscle (RF), the vastus lateralis muscle (VL), the biceps femoris muscle (BF), the tibialis anterior muscle (TA), the gastrocnemius muscle (GCM), and the soleus muscle (SOL). One-way repeated ANOVA was used for the statistical analysis. First, significant differences were found in kinematic variables for the hip, knee, ankle joint, and thigh-off among the three activities. Second, there was significant difference in muscle activation pattern in TA. VL. and BF among three activities. In conclusion, the findings of this study suggest the following evaluative and therapeutic approach for STS activity: (1) Changes in knee and ankle joints should be prioritized and recruitment order differences in VL and RF can be generated to accomplish abnormal STS activity. (2) APT STS can be introduced for movement efficiency and functional advantage when abnormal STS is treated.

인체 전경골근의 수의적 수축시 선행 동심성 근수축이 항정상태 등척성 근력에 미치는 영향 (Force Depression Following Active Muscle Shortening during Voluntary Contraction in Human Tibialis Anterior Muscle)

  • The purpose of this study was to investigate steady-state force depression following active muscle shortening in human tibialis anterior muscle during voluntary contractions. Subjects (n = 7; age $24{\sim}39$ years; 7 males) performed isometric reference contractions and isometric-shortening-isometric contractions, using maximal voluntary effort. Force depression was assessed by comparing the steady-state isometric torque produced following active muscle shortening with the purely isometric reference torque obtained at the corresponding joint angle. In order to test for effects of the shortening conditions on the steady-state force depression, the speed of shortening were changed systematically in a random order but balanced design. Ankle dorsiflexion torque and joint angle were continuously measured using a dynamometer. During voluntary contractions, muscle activation of the tibialis anterior and the medical gastrocnemius was recorded using surface electromyography. Force depression during voluntary contractions, with a constant level of muscle activation, was 12 %, on average over all subjects. Force depression was independent of the speeds of shortening ($13.8{\pm}2.9%$, $10.3{\pm}2.6%$ for 15 and 45 deg/sec over 15 deg of shortening, respectively). The results of this study suggest that steady-state force depression is a basic property of voluntarily-contracting human skeletal muscle and has functional implication to human movements.

노인 보행 시 하지 근 활동 양상과 관절의 안정성이 낙상에 미치는 영향 -전향적 연구(Prospective Study)- (Effects of Muscle Activation Pattern and Stability of the Lower Extremity's Joint on Falls in the Elderly Walking -Half a Year Prospective Study-)

  • Objective: The aim of this study was to determine the peak torques of the knee and ankle joint and local stability of the lower extremity's joints, and muscle activation patterns of the lower extremity's muscles between fallers and non-fallers in the elderly women during walking. Method: Four elderly women (age: $74.5{\pm}5.2yrs.$; height: $152.1{\pm}5.6cm$; mass: $55.3{\pm}5.4kg$; preference walking speed: $1.19{\pm}0.06m/s$) who experienced falls within six months since experiment had been conducted (falls group) and thirty-six subjects ($74.2{\pm}3.09yrs.$; height: $153.6{\pm}4.9cm$; mass: $56.7{\pm}6.4kg$; preference walking speed: $1.24{\pm}0.10m/s$) who had no experience in falls (non-falls group) within this periods participated in this study. They were measured torque peaks of the knee and ankle joint using a Human Norm and while they were walking on a treadmill at their natural pace, kinematic variables and EMG signals were collected with using a 3-D motion capture system and a wireless EMG system, respectively. Lyapunov Exponent (LyE) was determined to observe the dynamic local stability of the lower extremity's joints, and muscles activation and their co-contraction index were also analysed from EMG signals. Hypotheses between falls and non-falls group were tested using paired t-test and Mann-Whitey. Level of significance was set at p<.05. Results: Local dynamic stability in the adduction-abduction movement of the knee joint was significantly lower in falling group than non-falling group (p<.05). Conclusion: In conclusion, muscles which act on the abduction-adduction movement of the knee joint need to be strengthened to prevent from potential falls during walking. However, a small number of samples for fallers make it difficult to generalize the results of this study.

스쿼트 운동 시 발목 관절에 키네시오 테이프의 적용이 넙다리 네갈래근과 뒤넙다리근의 활성에 미치는 영향 (Effects of Kinesiology tape in ankle joint of quadriceps and hamstring muscles activation during squat exercise)

  • Background: During squat exercises, Ankle joint stability during squat movement transfers forces produced by unstable supports or various loads to the leg joints and trunk, reducing the risk of injury; therefore, a reference is needed for correct ankle joint posture during squat exercises. The purpose of this study was to examine the effect of ankle joint stability on quadriceps femoris and hamstring activation during squat exercises. Design: Quasi-experiment one group pretest-posttest design. Methods: The participants in this study were 20 volunteers who performed squat exercises with and without kinesiology tape. We measured quadriceps femoris and hamstring activation using surface electromyography. The ankle joint was stabilized with kinesiology tape using the ankle balance taping (ABT) method. A paired t-test was performed to compare differences between taping conditions. Results: Vastus medialis and vastus lateralis activation were greater following squat exercises with kinesiology tape than without; however, the difference was not significant. Medial and lateral hamstring activation was not significantly different between taping conditions. Conclusion: Although it is difficult to maintain stability using kinesiology tape alone, ankle joint stability is believed to affect quadriceps femoris muscle activity, which increases the effectiveness of the squat exercise.

The Comparison of the Gluteus Maximus Activity during Various Prone Hip Extension in Subject with Chronic Low Back Pain

  • PURPOSE: Prone hip extension (PHE) has been used for assessment of lumbo-pelvic movement and strengthening exercise for weakness of the hip joint muscles in patients with chronic low back pain (CLBP). On the other hand, few studies have examined which are the best PHE exercises to activate the gluteus maximus (GM) selectively in physical therapy practice. To aim of this study compared the muscle activity of the GM, rectus femoris (RF), biceps femoris (BF), tibialis anterior (TA) during these four different prone hip extensions, PHE, PHE with quadriceps activation (PHEQA), PHE with ankle dorsiflexion (PHEAD), and PHE with ankle plantarflexion (PHEAP), in subjects with CLBP. METHODS: Nineteen subjects with low back pain participated in this study. Subject performed four PHE exercises and surface electromyography (EMG) was used to evaluate the muscle activity. Data were analyzed by one-way repeated-measures analysis of variance (${\alpha}=.05/3=.017$) and a Bonferroni adjustment was performed if a significant difference was found. RESULTS: The muscle activities recorded by EMG showed significant among the four exercises. The muscle activity of the GM increased significantly during PHEQA than during PHEAP (P=.012). CONCLUSION: PHEQA is the most effective exercise for eliciting greater GM muscle activation among the four PHE exercises in subjects with CLBP.

훈련 시뮬레이터를 이용한 스키점프 도약 시 발생되는 EMG 패턴 분석 - 스키점프 유소년 국가대표 사례 연구 - (Analysis of EMG Patterns during Ski Jumping using Training Simulator - Case Study for Ski Jumping Youth National Athletes -)

  • Objective: The purpose of this study was to verify the effectiveness among simulating ski jumping trainings by comparing with actual ski jump. Method: Three healthy youth national athletes were recruited for this study (age: 13.70 ± 0.9 yrs, height: 169.30 ± 0.9 cm, jumping caree: 5.3 ± 0.9 yrs). Participants were asked to performed ski jumping with 3 simulating and one actual situation. A 3-dimensional motion analysis with 5 channels of EMG was performed in this study. Muscle activations of Rectus Femoris [RF], Tibialis Anterior [TA], Thoracis [TH], Gluteus maximus [GM], and Gastronemius [GL] were achieved with sampling rate of 2,000 Hz during each jump. Results: In the case of S1 in the actual jumping motion, the deviation of the muscle activity peak did not appear each trial, and the jump timing was consistent. For S2, the timing of the muscles peak activation which can maintain the posture of the upper body and ankles appeared at the beginning. In the case of S3, the part maintaining the ankle posture at the beginning appeared, but it could be expected that it would progress in the vertical direction due to the activation of GL at the time of jumping. Conclusion: The muscle activation peak before the take-off point showed a different pattern for each athlete, and individual differences were large. In addition, it was attempted to confirm the actual jump with simulation jump, and it was found that not only the difference in patterns but also the fluctuations in the timing of each muscle activation peak were large.

건강한 성인의 슬개건 반사 시 무릎 감쇠효과를 고려한 대퇴사두근의 근력 및 근활성도 예측 (Identification of Muscle Forces and Activation of Quadriceps Femoris Muscles of Healthy Adults Considering Knee Damping Effects during Patellar Tendon Reflex)

  • 인체 해석모델은 주로 인간이 의식적으로 행하는 운동을 중심으로 발전해 왔다. 의식적 운동과 달리 슬개건 반사는 뇌를 거치지 않고 일어난다. 본 연구는 건강한 성인의 슬개건 반사로 인한 대퇴부의 근력과 근활성도를 해석적으로 예측하고자 하였다. 해석 모델은 시상면에서 평면운동을 하고, 앉은 자세에서 상체와 허벅지를 고정시켜 종아리만 진자 운동이 가능하도록 모델링 하였다. 무릎은 레볼루트 조인트로 모델링 하였고, 발목관절은 고정시켜 종아리와 발을 하나의 강체로 가정하였다. 근력은 Mamizuka 의 실험 결과로부터 얻은 운동학 정보를 이용하여 역동역학 해석을 통해 구하였으며, 근활성도는 Hill-type 근육 모델을 이용하여 예측하였다. 해석 결과는 실험결과를 통해 검증되었다.
