Temporal Change in Radiological Environments on Land after the Fukushima Daiichi Nuclear Power Plant Accident

  • Massive environmental monitoring has been conducted continuously since the Fukushima Daiichi Nuclear Power accident in March of 2011 by different monitoring methods that have different features together with migration studies of radiocesium in diverse environments. These results have clarified the characteristics of radiological environments and their temporal change around the Fukushima site. At three months after the accident, multiple radionuclides including radiostrontium and plutonium were detected in many locations; and it was confirmed that radiocesium was most important from the viewpoint of long-term exposure. Radiation levels around the Fukushima site have decreased greatly over time. The decreasing trend was found to change variously according to local conditions. The air dose rates in environments related to human living have decreased faster than expected from radioactive decay by a factor of 2-3 on average; those in pure forest have decreased more closely to physical decay. The main causes of air dose rate reduction were judged to be radioactive decay, movement of radiocesium in vertical and horizontal directions, and decontamination. Land-use categories and human activities have significantly affected the reduction tendency. Difference in the air dose rate reduction trends can be explained qualitatively according to the knowledge obtained in radiocesium migration studies; whereas, the quantitative explanation for individual sites is an important future challenge. The ecological half-lives of air dose rates have been evaluated by several researchers, and a short-term half-life within 1 year was commonly observed in the studies. An empirical model for predicting air dose rate distribution was developed based on statistical analysis of an extensive car-borne survey dataset, which enabled the prediction with confidence intervals. Different types of contamination maps were integrated to better quantify the spatial data. The obtained data were used for extended studies such as for identifying the main reactor that caused the contamination of arbitrary regions and developing standard procedures for environmental measurement and sampling. Annual external exposure doses for residents who intended to return to their homes were estimated as within a few millisieverts. Different forms of environmental data and knowledge have been provided for wide spectrum of people. Diverse aspects of lessons learned from the Fukushima accident, including practical ones, must be passed on to future generations.

Qualitative Evaluation of 2D Dosimetry System for Helical Tomotherapy (2차원 토모테라피 선량측정시스템의 정성적 평가)

  • The purpose of this study is to see the feasibility of the newly developed 2D dosimetry system using phosphor screen for helical tomotherapy. The cylindrical water phantom was fabricated with phosphor screen to emit the visible light during irradiation. There are three types of virtual target, one is one spot target, another is C-shaped target, and the other is multiple targets. Each target was planned to be treated at 10 Gy by treatment planning system (TPS) of tomotherapy. The cylindrical phantom was placed on the tomotherapy table and irradiated as calculations of the TPS. Every frame which acquired by CCD camera was integrated and the doses were calculated in pixel by pixel. The dose distributions from the fluorescent images were compared with the calculated dose distribution from the TPS. The discrepancies were evaluated as gamma index for each treatment. The curve for dose rate versus pixel value was not saturated until 900 MU/min. The 2D dosimetry using the phosphor screen and the CCD camera is respected to be useful to verify the dose distribution of the tomotherapy if the linearity correction of the phosphor screen improved.

A Phase I/II Trial of $DCVac/IR^{(R)}$ Dendritic Cell Immunotherapy Combined with Irradiation in Cases of Refractory Colorectal Cancer with Multiple Liver Metastases (기존의 치료에 반응하지 않는 다발성 간전이 대장암 환자에서 방사선조사와 병합한 수지상세포 면역치료의 1, 2상 임상시험)

  • Purpose: To assess the toxicity and tumor response induced by $DCVac/IR^{(R)}$ dendritic cell(DC) immunotherapy combined with irradiation for refractory colorectal cancer patients with multiple liver metastases. Materials and Methods: Between May 2004 and November 2006, applicants from a pool of refractory colorectal cancer patients with multiple liver metastases were enrolled. The patients were registered after having signed the informed consent form, which had been approved by the Institutional Review Board from the Dong-A University and Busan National University Hospital. DCs were obtained from peripheral blood of each patient, and then cultured in vitro. A total of $6{\times}10^6$ DCs were packed into a vial($DCVac/IR^{(R)}$, 0.5 ml) at the convenience of each patient's schedule. On the day before and on the day of each vaccination, each patient received a 4 Gy radiation dose to the target tumor. On the day of vaccination, the indicated dose of autologous DCs was injected into the irradiated tumor using ultrasound-guided needle injection procedures. A total of four vaccinations were scheduled at three 2-week intervals and one 4 week interval at the Dong-A University and Busan National University Hospital. If the tumor status was deemed to be stable or responding to therapy, an additional vaccination dose or two was approved at 4 week intervals beyond the fourth immunization. A tolerance test for DCs was conducted by injecting a range of doses($3{\times}10^6\;to\;12{\times}10^6$ DCs) after the 3rd injection. Moreover, the maximal tolerable dose was applied to additional patients. Treatment safety was evaluated in all patients who had at least one injection. Treatment feasibility was evaluated by the 10th week by assessing the response of patients having at least 4 injections. For systemic toxicities, the evaluation was performed using the National Cancer Institute Common Toxicity Criteria, whereas adverse effects were recorded using common WHO toxicity criteria. Results: Of the 24 registered patients, 22 received the DCs injections. Moreover, of the 14 patients that applied for the tolerance test, only 11 patients completed it because 3 patients withdrew their testing agreement. A grade 3 or more side effect, which was possibly related to the DC injection, did not occur in additional patients. The $12{\times}10^6$ DC injection was identified as the maximum tolerable dose, and was then injected in an additional 8 patients. Patients tolerated the injection fairly well, with no fatal side effects. In order to assess the feasibility of DC immunotherapy, the response was evaluated in other hepatic lesions outside of the targeted hepatic lesion. The response evaluation was performed in 15 of the 17 patients who received at least 4 injections. Stable and progressive disease was found in 4 and 11 patients, respectively. Conclusion: The DC-based immunotherapy and radiotherapy is theoretically synergistic for the local control and systemic control. The $DCVac/IR^{(R)}$ immunotherapy combined with irradiation was tolerable and safe in the evaluated cases of refractory colorectal cancer with multiple liver metastases. Future work should include well designed a phase II clinical trials.

Radiation Absorbed Dose Measurement after I-131 Metaiodobenzylguanidine Treatment in a patient with Pheochromycytoma (갈색세포종 환자에서 Medical Internal Radiation Dose법을 이용한 I-131 Metaiodobenzylguanidine 치료 후 흡수선량 평가)

  • Purpose: The measurement of radiation absorbed dose is useful to predict the response after I-131 labeled metaiodobenzylguanidine (MIBG) therapy and determine therapy dose in patients with unresectable or malignant pheochromocytoma. We estimated the absorbed dose in tumor tissue after high dose I-131 MIBG in a patient with pheochromocytoma using a gamma camera and Medical Internal Radiation Dose (MIRD) formula. Materials and Methods: A 64-year old female patient with pheochromocytoma who had multiple metastases of mediastinum, right kidney and periaortic lymph nodes, received 74 GBq (200 mCi) of I-131 MIBG. We obtained anterior and posterior images at 0.5, 16, 24, 64 and 145 hours after treatment. Two standard sources of 37 and 74 MBq of I-131 were imaged simultaneously. Cummulated I-131 MIBG uptake in tumor tissue was calculated after the correction of background activity, attenuation, system sensitivity and count loss at a high count rate. Results: The calculated absorbed radiation dose was 32-63 Gy/ 74 GBq, which was lower than the known dose for tumor remission (150-200 Gy). follow-up studies at 1 month showed minimally reduced tumor size on computed tomography, and mildly reduced I-131 MIBG uptake. Conclusion: We estimated radiation absorbed dose after therapeutic I-131 MIBG using a gamma camera and MIRD formula, which can be peformed in a clinical nuclear medicine laboratory. Our results suggest that the measurement of radiation absorbed dose in I-131 MIBG therapy is feasible as a routine clinical practice that can guide further treatment plan. The accuracy of dose measurement and correlation with clinical outcome should be evaluated further.

The Relationship between Internet Use for Non-academic Purposes and Happiness and Stress in Adolescents (학업 목적 외의 인터넷 사용과 청소년들의 행복감 및 스트레스와의 관련성)

  • Purpose: Previous studies focused on the negative effects of Internet use such as obesity, depression, and addiction. However, few studies investigated whether the Internet use for an appropriate time has positive effects on adolescents' health. Thus, the purpose of the study was to examine the dose-response relationship between Internet use for non-academic purposes (NAP) and happiness/stress in adolescents. Methods: This study used data from the 2013 Korea Youth Risk Behavior Web-Based Survey. A total of 72,435 adolescents were included. Rao-Scott ${\chi}^2$ and multiple logistic regression were conducted. To test the dose-response relationship, this study examined how happiness/stress changed with increase in the time of Internet use for NAP. Results: After controlling for general characteristics, the odds ratios for being unhappy and stressful were lowest in adolescents using the Internet for NAP for more than 0 hour and less than an hour a day. Interestingly, students who did not use the Internet for NAP had a significantly higher risk of being unhappy/stressful than those using for more than 0 hour and less than an hour a day. Conclusion: Internet use for NAP for more than 0 hour and less than an hour a day has positive effects on happiness/stress in adolescents.

  • The damage which radiation produces in tissues such as the lungs can be discussed at the molecular, biophysical, cellular, and organ levels. The cellular effects of irradiating the lungs are related to the histologic and clinical sequelae. In the present study the right lung of rabbits were exposed to single dose of 20 Gy of X-irradiation. Animals from each group were sacrificed monthly for 6 months postexposure. Sections of lung were examined by light microscopy(LM) and by transmission electron microscopy(TEM). Multiple exudative lesions were seen at 2 months after the 20Gy irradiation,and they progressed to a proliferative and then reparative fibrotic lesion by 6 months. Changes in epithelial lining of lung components, particulary the presence of type II pneumocytes were found by both LM and TEM. Capillary endothelial damages were less pronounced. The possible implication of cellular components in radiation pneumonitis and fibrosis is discussed.

  • Cardiopulmonary depression of long-term constant rate infusion (CRI) administration of multiple analgesic drugs is important, especially in critically ill dogs. Therefore, this study was conducted to evaluate the effects of lidocaine, ketamine or combined lidocaine-ketamine combination CRI treatment on vital signs and left ventricular (LV) function in healthy dogs. Six adult Beagle dogs were administered either ketamine (initial loading dose of 0.5 mg/kg followed by $10{\mu}g/kg/min$ CRI), lidocaine (initial loading dose of 2 mg/kg followed by 0.025 mg/kg/min CRI), or combined lidocaine-ketamine intravenously. Arterial blood pressure (BP), heart rate (HR), respiratory rate (RR), body temperature (BT) and echocardiographic LV dimensions were measured before administration of medications, immediately after administration of drugs, and then every 10 min for 2 h. There were no significant changes in HR, RR, BT and BP after the administration of either lidocaine CRI, ketamine CRI, or combined lidocaine and ketamine CRI. There were also no significant changes in LV dimensions and stroke volume. The results revealed that treatment with either lidocaine, ketamine or combined lidocaine-ketamine may not cause cardiopulmonary suppression in healthy dogs.

Monte Carlo shielding evaluation of a CSNS Multi-Physics instrument

  • The Multi-Physics (MP) instrument is one of 20 neutron spectrometers planned in the China Spallation Neutron Source (CSNS). This paper presents a shielding calculation for the MP instrument using Monte Carlo codes MCNPX and FLUKA. First, the neutrons that escape from the CSNS decoupled water moderator and are delivered to the beam line of the MP instrument are calculated to use as the source term of the shielding calculation. Then, to validate the calculation method based on multiple variance reduction techniques, a cross check between MCNPX and FLUKA codes is performed by comparing the calculation results of the dose rate distribution on a simplified beam line model. Finally, a complete geometry model of the MP instrument is set up, and the primary parameters for the shielding design are obtained according to the calculated dose rate map considering different worst-case scenarios.

Efficacy of Bacteriophage Treatment in Murine Burn Wound Infection Induced by Klebsiella pneumoniae

  • In the present study, the therapeutic potential of purified and well-characterized bacteriophages was evaluated in thermally injured mice infected with Klebsiella pneumoniae B5055. The efficacy of five Klebsiella phages (Kpn5, Kpn12, Kpn13, Kpn17, and Kpn22) was evaluated on the basis of survival rate, decrease in bacterial counts in different organs of phage-treated animals, and regeneration of skin cells as observed by histopathological examination of phage-treated skin. Toxicity studies performed with all the phages showed them to be non-toxic, as no signs of morbidity and mortality were observed in phage-treated mice. The results of the study indicate that a single dose of phages, intraperitoneally (i.p.) at an MOI of 1.0, resulted in significant decrease in mortality, and this dose was found to be sufficient to completely cure K. pneumoniae infection in the burn wound model. Maximum decrease in bacterial counts in different organs was observed at 72 h post infection. Histopathological examination of skin of phage-treated mice showed complete recovery of burn infection. Kpn5 phage was found to be highly effective among all the phages and equally effective when compared with a cocktail of all the phages. From these results, it can be concluded that phage therapy may have the potential to be used as stand-alone therapy for K. pneumoniae induced burn wound infection, especially in situations where multiple antibiotic-resistant organisms are encountered.

Five Year's Follow-up of the Cardiovascular Disease Risk Factors among the Low Level Organic Solvent Exposure Workers (저농도 유기용제 노출 사업장 근로자의 심혈관질환 위험인자 변화 추적(2008~2012))

  • Purpose: The objective of the present study was to evaluate the effects of occupational exposure to low dose organic solvents on the prevalence of cardiovascular risk factors. Methods: Study design was retrospective cohort study subjected on 191 exposures and 118 controls working in a petrochemical manufacture company. The eight indicators related to CVD risk were followed up for five years from 2008 to 2012. The risk level was compared during the follow up years and subject's characteristics, and the change of risk level were analyzed with repeated measures ANOVA and multiple logistic regression analysis. Results: At the start year 2008, the rate over cutoff value (ROCV) of BS (p<.001) and mean systolic BP (p=.017) were higher in organic solvent exposure group and the others showed no difference. And by the subject's characteristics, odds ratio of the ROCV of BS were higher in organic solvent exposure group and work shift group as 2.51 and 3.07. Comparing the results in 2012 to those of 2008, cardiovascular disease risk in organic solvent exposure group was about 1.5 times higher than that of in the control group. Conclusion: Gradual increase in the CVD risk was identified in organic solvent exposure group. However, the risk might be influenced by shift work and bad behaviors rather than organic solvent exposure.
